1.Analysis of the burden of diabetes attributed to metabolic factors from 1990 to 2019

ObjectiveTo analyze the long-term trends of the disease burden of diabetes attributed to metabolic factors in China from 1990 to 2019, and provide scientific recommendations for diabetes prevention and control in China. MethodsDescriptive analysis was conducted on the disease burden data of diabetes attributed to metabolic factors in China from 1990 to 2019, obtained from GBD 2019, encompassing death form diabetes, disability-adjusted life years (DALY), years of life lost (YLL), and years lived with disability (YLD). Joinpoint regression models were employed to analyze the long-term trends in mortality and DALY rates. Furthermore, the study examined the impact of two metabolic risk factors, high fasting plasma glucose (FPG) levels and high body mass index (BMI) levels, on the disease burden of diabetes. ResultsFrom 1990 to 2019, the overall standardized mortality and DALY rates attributed to metabolic factors for diabetes in the general population in China showed an upward trend, with both average annual percent changes (AAPCs) of 0.1% in the total population. The trend was upward in males with AAPCs of 0.9% and 0.6%, while it was downward in females with AAPCs of -0.4% and -0.3%. As age increased, the disease burden of diabetes attributed to metabolic factors showed an upward trend, with high FPG and high BMI ranking as the top two attributing risk factors. The disease burden of diabetes attributed to metabolic factors was higher in Chinese males than females. ConclusionThe disease burden of diabetes attributed to metabolic factors is increasing among the overall population and particularly among males, while the burden for female is declining. There is a need to increase intervention efforts for males aged 65 and above, provide scientific guidance on residents’ diet and lifestyle habits, and control blood glucose and body weight.
2.Factors associated with glycemic variability in children with type 1 diabetes mellitus based on flash glucose monitoring system.

OBJECTIVES:
Patients with classical type 1 diabetes mellitus (T1DM) require lifelong dependence on exogenous insulin therapy due to pancreatic beta-cell destruction and absolute insulin deficiency. T1DM accounts for about 90% of children with diabetes in China, with a rapid increase in incidence and a younger-age trend. Epidemiological studies have shown that the overall glycated haemoglobin (HbA1c) and compliance rate are low in Chinese children with T1DM. Optimal glucose control is the key for diabetes treatment, and maintaining blood glucose within the target range can prevent or delay chronic vascular complications in patients with T1DM. Therefore, this study aims to investigate the glycemic control of children with T1DM from Hunan and Henan Province with flash glucose monitoring system (FGMS), and to explore factors associated with glycemic variability.
METHODS:
A total of 215 children with T1DM under 14 years old were enrolled continuously in 16 hospitals from August 2017 to August 2020. All subjects wore a FGMS device to collect glucose data. Correlation of HbA1c, duration of diabetes, or glucose scan rates with glycemic variability was analyzed. Glucose variability was compared according to the duration of diabetes, HbA1c, glucose scan rates and insulin schema.
RESULTS:
HbA1c and duration of diabetes were positively correlated with mean blood glucose, standard deviation of glucose, mean amplitude of glucose excursions (MAGE), and coefficient of variation (CV) of glucose (all P<0.01). The glucose scan rates during FGMS wearing was significantly positively correlated with time in range (TIR) (P=0.001) and negatively correlated with MAGE and mean duration of hypoglycemia (all P<0.01). Children with duration ≤1 year had lower time below range (TBR) and MAGE when compared with those with duration >1 year (all P<0.05). TIR and TBR in patients with HbA1c ≤7.5% were higher (TIR: 65% vs 45%, TBR: 5% vs 4%, P<0.05), MAGE was lower (7.0 mmol/L vs 9.4 mmol/L, P<0.001) than those in HbA1c >7.5% group. Compared to the multiple daily insulin injections group, TIR was higher (60% vs 52%, P=0.006), MAGE was lower (P=0.006) in the continuous subcutaneous insulin infusion group. HbA1c was lower in the high scan rates (≥14 times/d) group (7.4% vs 8.0%, P=0.046), TIR was significantly higher (58% vs 47%, P<0.001), and MAGE was lower (P<0.001) than those in the low scan rate (<14 times/d) group.
CONCLUSIONS
The overall glycemic control of T1DM patients under 14 years old in Hunan and Henan Province is under a high risk of hypoglycemia and great glycemic variability. Shorter duration of diabetes, targeted HbA1c, higher glucose scan rates, and CSII are associated with less glycemic variability.

4.Effect of application of hierarchical and special quality control in the quality control management of an ultrasound department

Objective:To explore the effect of the application of hierarchical and special quality control in the quality control management of an ultrasound department.Methods:The hierarchical and special quality control method was used to control ultrasonic reports that were related to four quality control areas: basic quality control, deep quality control, obstetrical quality control, and critical value quality control. Changes in the qualified reporting and comprehensive clinical satisfaction rates before and after the implementation of hierarchical and special quality control were examined.Results:After the implementation of hierarchical and special quality control, the qualified rates of basic quality control, deep quality control, obstetrical quality control, and critical value quality control increased significantly. The comprehensive clinical satisfaction score was (90.58±1.79) points, which was higher than before the model was applied [(80.37±2.72) points], and the difference was statistically significant ( P<0.01). Conclusion:The implementation of hierarchical and special quality control measure can effectively improve the quality of ultrasonic diagnoses and comprehensive clinical satisfaction.
5.RASSF6 plays a role in suppressing cancer and improving the sensitivity of oxaliplatin in gastric cancer cells

【Objective】 To investigate the effects of RASSF6 gene on gastric cancer cells’ proliferation, autophagy, apoptosis, and sensitivity to oxaliplatin chemotherapy. 【Methods】 Gastric cancer BGC823 cells were cultured in vitro and divided into experimental control group (control group), RASSF6 overexpression group (Oe group), RASSF6 interference group, and lentivirus control group according to the expression effect of lentivirus gene. The changes in cell proliferation, cell cycle distribution, cell migration, autophagy, apoptosis and sensitivity to oxaliplatin in each group were detected, and the number of autophagy bodies in each group was detected by electron microscopy. Real-time PCR (qRT-PCR) and Western blotting were used to detect the expression levels of apoptosis- and autophagy-related genes in each group. 【Results】 Studies on the biological behavior of gastric cancer BGC823 cells induced by RASSF6 gene expression showed that compared with the control group, the percentage of G0/G1 phase cells in the Oe group increased, while the percentage of G2 and S phase cells decreased, with statistical significance (P<0.05). The apoptosis rate was significantly increased (P<0.05). The cell scratch assay showed that the scratch healing rate was significantly decreased (P<0.05). Studies on the sensitivity of RASSF6 gene expression to oxaliplatin showed that compared with the drug group (L-OHP group), the apoptosis rate of Oe+L-OHP group was increased significantly (P<0.05). In the Oe+L-OHP group, the expression of anti-apoptotic protein Bcl-2 decreased, the expressions of Bax and Caspase-3 were increased; the expression of autophagosomes was increased; the expressions of Beclin-1 and P62 and the ratio of LC3-Ⅱ/LC3 were all increased (P<0.05). 【Conclusion】 The RASSF6 gene plays a role in suppressing gastric cancer cell BGC823, which can increase the sensitivity to oxaliplatin chemotherapy by promoting apoptosis and autophagy.
6.Effect of Chronic Unpredictable Mild Stress and Sertraline on Vitamin D Metabolism in Rat Prefrontal Cortex

Objective To explore the relationship between vitamin D and depression and the effects of chronic unpredictable mild stress (CUMS) plus sertraline on vitamin D metabolism in rat brain.Methods Male SD rats were randomly divided into normal control group,normal control + sertraline group,CUMS group and CUMS+sertraline group.CUMS was used to build the animal model of depression.After chronic exposure to CUMS or sertraline (8 mg·kg-1·d-1) for 6 weeks,behavioral response (sucrose preference test) and vitamin D metabolism in the prefrontal cortex were analyzed.Results Compared with normal control group,6 weeks of CUMS procedure induced the rats to a depression-like state,decreased weight and sucrose preference,and increased the expression of enzymes involved in vitamin D activation and catabolism (CYP27B1 and CYP24A1,respectively) and vitamin D receptor (VDR) in rat prefrontal cortex.As compared with CUMS group,CYP27B1,CYP24A1 and VDR were significantly decreased in CUMS+sertraline group (P<0.01 or P<0.05).Conclusion CUMS activates vitamin D signaling in the brain of the animal models of depression,while sertraline alleviates depressive behavior and relieves stress-induced activation of vitamin D signaling,indicating that vitamin D signaling may be involved in the stress-induced depression.
7.Short-term Therapeutic Effect of Recombinant Human Endostatin Combined with Cis-platinum on Malignant Pleural Effusions: A Meta-analysis

Objective To evaluate the efficacy and safety of recombinant human endostatin (rh-endostatin) combined with cis-platinum for patients with malignant pleural effusions.Methods A computer-based online search was performed through Elsevier, PubMed, Medline, Cochrane Library, China National Knowledge Infrastructure (CNKI), Chinese BioMedical Literature Database and Wanfang Database.According to the inclusion and exclusion criteria, the randomized controlled trials about short-term therapeutic effect of rh-endostatin combined with cis-platinum on malignant pleural effusions published until December 2015 were selected.Quality of the studies was assessed using modified Jadad scale.After data extraction, a Meta-analysis was performed by RevMan 5.3 software.Relative risk (RR) and its 95% confidence interval (CI) were calculated.The Egger test was performed by Stata 12.0 software.Results Fourteen eligible randomized controlled trials were included in this Meta-analysis involving 1 330 patients,665 in cis-platinum alone group(control group), and 665 in rh-endostatin combined with cis-platinum group(treatment group).The results showed that there were significant improvements in overall response rate (ORR) [73.53% vs 45.41%,RR=1.62,95%CI(1.47,1.78),P<0.000 01] and the rate of quality of life improvement [71.65% vs 46.94%,RR=1.52,95%CI(1.38,1.68),P<0.000 01] in the treatment group, as compared with those of the control group.Meanwhile, there were no statistically significant differences in the rate of cardio toxicity [10.38% vs 5.77%,RR=1.73,95%CI(0.99,3.03),P=0.06].Conclusion This Meta-analysis indicated that in comparison with cis-platinum alone, rh-endostatin combined with cis-platinum has a better therapeutic effect on malignant pleural effusions.
8.Pharmaceutical Care for a Pneumonia Patient after Lung Transplantation

Objective:To participate in the anti-infection regimen adjustment and perform pharmaceutical care for one pneumonia patient after lung transplantation by clinical pharmacist to provide experience for individualized medication. Methods:Clinical pharma-cist adjusted the treatment plan and doses of immunosuppressive agents according to the patient' s conditions. Results: The infection was effectively controlled through optimizing the treatment plan. Conclusion:Pharmaceutical care for patients after lung transplantation performed by clinical pharmacists is helpful to both the recovery of patients and future appropriate drug use.
9.Advances in metabolic engineering of Escherichia coli for isoprene biosynthesis.

As an important industrial chemical, isoprene is mainly used as a precursor for synthetic rubbers. In addition, it also has wide applications in the field of pharmaceutical and chemical intermediates, food, adhesives and aviation fuel. Compared with conventional petrochemical routes, production of isoprene in microbial systems has been the research focus considering environment friendly and sustainable development features. This article summarizes the metabolic pathways and key enzymes of isoprene biosynthesis, reviews current methods and strategies in improving isoprene production of Escherichia coli, and also gives some basic ideas and expectation.

10.Evaluation of the Quality Control System of Clinical Using of Antibiotics

Objective To evaluate the quality control standard and check system of clinical using of antibiotics based on the change of the constituent ratio of antibiotics. Methods In this retrospective study, we compared consumption of the ten mostly prescribed antibiotics, constituent ratio of national essential drugs, antibiotics utilization ratio and antibiotics use density between the third quarter of 2012 and that of 2013. Results Compared with 2012, the usage of antibiotics was reduced by 39.91%, and the usage of national essential drugs rose by 71.46%, whereas the consumptions of cefoxitin, aztreonam, cefminox were reduced by 84.56%, 89.99%, and 56.52%, respectively, in 2013.The consumptions of cefazolin, cefuroxime, ceftriaxone rose by 36. 39%, 363. 50% and 24. 06%, respectively. The amount of used antibiotics was reduced by 35. 41%, and the application frequency of antibiotics was reduced by 36.62%. Conclusion The establishment, running and implement of the quality control standard and check system of clinical using of antibiotics make constituent ratio of antibiotics used in the first people's hospital of Jining City more reasonable, which will help to promote the rational use of antibiotics in clinic.
