ObjectiveTo analyze the trend and influencing factors of low birth weight (LBW) among newborns in Chongming District of Shanghai from 2008 to 2022, so as to provide references for the development of intervention measures reducing the rate of LBW. MethodsBirth surveillance data of Chongming District of Shanghai from 2008 to 2022 were collected and organized, and the annual percentage change (APC) of LBW was calculated by using Joinpoint 5.0.2 software for trend change analysis. Logistic regression analysis was used to analyze the influencing factors of LBW. ResultsThe overall incidence of LBW was 3.71% in Chongming District, Shanghai from 2008 to 2022. Joinpoint trend analysis showed that the incidence of LBW in Chongming District had an upward trend (APC=5.49%, 95%CI: 3.31%‒7.72%, P<0.001).Multivariate logistic regression analysis showed that preterm birth, multiple births, female infants, birth defects, first pregnancy, primiparity, and a young father age (<20 years) were risk factors for LBW in Chongming District. Among the term infants, female infants, birth defects, and first pregnancy were risk factors for LBW (P<0.05). Female infants, birth defects, first pregnancy, primiparity, advanced maternal age (≥35 years), and a young father age (<20 years) were risk factors in singleton neonates. ConclusionThe incidence of LBW among newborns is on the rise in Chongming District of Shanghai. Therefore, high risk groups need to be identified, and prenatal check-ups and pregnancy care should be strengthened to reduce the risk of neonatal LBW.

Objective:To explore the effects of different graft to recipient body weight ratio (GRWR) on the efficacy of living donor liver transplantation (LDLT) in infants with biliary atresia (BA).Methods:Clinical data of 175 BA infants, including 98 males and 77 females, age at transplantation was 5.40 (4.77, 6.33) months, who underwent LDLT at the Department of Hepatobiliary Surgery, Children’s Hospital of Chongqing Medical University from May 2018 to December 2022 were retrospectively analyzed. They were divided into the routine GRWR group (2%≤GRWR≤ 4%, n=121) and high GRWR group (GRWR>4%, n=54). The preoperative general condition, intraoperative condition and postoperative recovery of children in the two groups were compared. Survival analysis was performed by using Kaplan-Meier method, and log-rank test was used to compare survival differences. Results:A total of 16 (9.14%, 16/175) children underwent unplanned surgery after LDLT. There were statistically significant differences in weight at LDLT and graft weight between children in the routine GRWR and high GRWR groups (both P<0.05). There were no statistically significant difference between the two groups in terms of operative time, intraoperative bleeding, and postoperative complication rates (all P>0.05). There were no hepatic artery thrombosis or stenosis occurred in the children of both groups after LDLT. The cumulative survival rates of the children in the routine GRWR group were 97.5% and 95.5% at 1 and 3 years after LDLT, respectively, and 96.3% and 94.2% at 1 and 3 years after LDLT in the high GRWR group, and the difference in cumulative survival rates between the two groups was not statistically significant ( P=0.692). Conclusion:The use of liver grafts with GRWR >4% is also safe and effective for LDLT in infants with BA, which supposes that may not be necessary to reduce the transplanted liver volume in children LDLT with high GRWR.

Objective:To establish a method for correction of tail vein extravasation based on PET images and to improve the accuracy of SUV.Methods:The simulation method was established by phantom on Nano PET/CT and images were reconstructed by a three-dimensional ordered-subsets exception maximum algorithm. PET images were analyzed by using the Interview Fusion 1.0 software. The optimal scanning time and the ROI delineated method were found. The accuracy of the simulation method was verified by comparing the activity of simulation method with the mice tail activity measured by the dose calibrator directly on Kunming (KM) mouse ( n=11). Using the simulation method, the impact of extravasation on SUV was proved. Independent-sample t test was used to analyze data. Results:Ten minutes was selected as the optimal scanning time and SUV max 42% threshold was selected as the ROI delineated method. The specific correction formula was as follows: actual activity=image activity/(4.48× V+ 77.05)×100 (0.3 MBq/ml≤leakage concentration<6.5 MBq/ml); actual activity=image activity/(6.65× V+ 71.10)×100 (6.5 MBq/ml

		                        			BACKGROUND:
Endocrine therapy (ET) and ET-based regimens are the preferred first-line treatment options for hormone receptor (HR)-positive and human epidermal growth factor receptor 2 (HER2)-negative metastatic breast cancer (HR+/HER2- MBC), while chemotherapy (CT) is commonly used in clinical practice. The aim of this study was to investigate the efficacy and clinical outcome of ET and CT as first-line treatment in Chinese patients with HR+/HER2- MBC.

		                        			METHODS:
Patients diagnosed with HR+/HER2-MBC between January 1st, 1996 and September 30th, 2018 were screened from the Chinese Society of Clinical Oncology Breast Cancer database. The initial and maintenance first-line treatment, progression-free survival (PFS), and overall survival (OS) were analyzed.

		                        			RESULTS:
Among the 1877 included patients, 1215 (64.7%) received CT and 662 (35.3%) received ET as initial first-line treatment. There were no statistically significant differences in PFS and OS between patients receiving ET and CT as initial first-line treatment in the total population (PFS: 12.0 vs. 11.0 months, P = 0.22; OS: 54.0 vs . 49.0 months, P =0.09) and propensity score matched population. For patients without disease progression after at least 3 months of initial therapy, maintenance ET following initial CT (CT-ET cohort, n = 449) and continuous schedule of ET (ET cohort, n = 527) had longer PFS than continuous schedule of CT (CT cohort, n = 406) in the total population (CT-ET cohort vs. CT cohort: 17.0 vs . 8.5 months; P <0.01; ET cohort vs . CT cohort: 14.0 vs . 8.5 months; P <0.01) and propensity score matched population. OS in the three cohorts yielded the same results as PFS.

		                        			CONCLUSIONS
ET was associated with similar clinical outcome to CT as initial first-line treatment. For patients without disease progression after initial CT, switching to maintenance ET showed superiority in clinical outcome over continuous schedule of CT.

Objective:To investigate the effects of different production processes on sensitization responses to human diploid cell rabies vaccines. Methods:This study randomly collected 360 serum samples in clinical trials of four rabies vaccines with different production processes. Total IgE levels at different time points were detected by ELISA. SPSS19.0 software was used for statistical analysis. Results:Total IgE test results showed that the seropositive rate was 20% (6/30) for all four vaccines. The lowest mean value of total IgE was 9 IU/ml and the highest was 210 IU/ml. Repeated measures analysis of variance showed that there was no significant difference in total IgE levels at different sampling time points ( P=0.284), and the total IgE level in people injected with multistep concentrated human diploid cell rabies vaccine was significantly below that in people immunized with Vero cell rabies vaccine ( P=0.024). Conclusions:Increasing the immune dosage of human rabies vaccine could not result in a rise in total IgE. Human diploid cell rabies vaccines had good safety as the production process could remove most of allergenic impurities.

Objective:To evaluate the correlation between endemic arsenic poisoning and abnormal electrocardiogram (ECG).Methods:PubMed, Web of Science, Embase, China National Knowledge Infrastructure (CNKI), Wanfang data, VIP and other databases were used for literature retrieval, and epidemiological literatures related to abnormal ECG of endemic arsenic poisoning published in domestic and abroad were included in the study. The time limit was from the establishment of the database to December 1, 2020. RevMan 5.3 was used for Meta-analysis of binary variables. Random effect model was selected according to the results of heterogeneity, and odds ratio ( OR) was used as the effect index. Characteristic changes were found by subgroup analysis. Bias was published by funnel plot. Results:Nine articles were included in this Meta-analysis, with 6 articles in Chinese and 3 articles in English, respectively. The abnormal ECG changes included QTc prolongation, ST-T segment change, left axis deviation and arrhythmia. Finally, 1 975 cases were included in the exposure group, including 575 cases of abnormal ECG; 750 cases of control group, including 145 cases of abnormal ECG. Meta-analysis showed that the combined OR value [95% confidence interval ( CI)] of abnormal ECG changes was 4.41 (2.83 - 6.87), with statistical significance between the two groups ( Z = 6.56, P < 0.05); the results of subgroup analysis showed that the combined OR values (95% CI) of QTc prolongation, ST-T segment change, left axis deviation and arrhythmia were 12.30 (5.91 - 25.59), 2.74 (1.39 - 5.41), 2.93 (0.89 - 9.62) and 4.13 (2.38 - 7.17), respectively. Conclusions:Endemic arsenic poisoning may cause abnormal ECG. Prolongation of QTc caused by arsenic exposure may be the characteristic change of abnormal ECG.

Dyslipidemia is an important risk factor of atherosclerotic cardiovascular disease (ASCVD). Statins delay the occurrence and development of ASCVD, and reduce the risk of cardiovascular events and death. Due to safety concerns, there exist insufficient use of lipid-lowering agents and a high withdrawal rate of the agents in the elderly. To promote the prevention and treatment of ASCVD, this expert consensus is issued and focuses on the management of dyslipidemia of Chinese elderly basing on the clinical evidence of the use of lipid-lowering drugs by the elderly, and the lipid management guidelines and expert consensus recommendations at home and abroad.
