1.Analysis of the 2023 national interlaboratory comparison for measurement of gross α and gross β radioactivity in water

Objective To organize a nationwide interlaboratory comparison for measurement of gross α and gross β radioactivity in water, and improve the laboratory analysis of gross α and gross β radioactivity in water. Methods A unified comparison protocol was developed by the organizers. The groundwater with high natural radioactivity was used as water sample and distributed randomly to the participating laboratories. The participating laboratories used routine analytical methods to measure the samples and provided information such as analytical results, original records, and test reports. The results were evaluated using z-score. Results A total of 76 laboratories participated in the comparison, all employing the evaporation concentration-α/β counting method. Among them, 69 laboratories achieved |z| ≤ 2 for both gross α and gross β radioactivity measurements, and 32 laboratories achieved |z| ≤ 0.50 for both gross α and gross β radioactivity measurements. There were 69 laboratories with qualified results and 30 laboratories with excellent results, yielding a qualified rate of 90.8% and an excellent rate of 39.5%. Seven laboratories showed unqualified results and the unqualified rate was 9.2%. Conclusion Most laboratories have the ability to analyze gross α and gross β radioactivity in water. The main reasons for the deviation in comparison results are calibration efficiency, errors in the total residue mass caused by improper water sample processing operations. By analyzing the main technical problems existed in unqualified laboratories, their ability for measurement of gross α and gross β radioactivity in water has been improved.

The occurrence of benign prostate hyperplasia(BPH)was related to disrupted sex steroid hormones,and metformin(Met)had a clinical response to sex steroid hormone-related gynaecological disease.How-ever,whether Met exerts an antiproliferative effect on BPH via sex steroid hormones remains unclear.Here,our clinical study showed that along with prostatic epithelial cell(PEC)proliferation,sex steroid hormones were dysregulated in the serum and prostate of BPH patients.As the major contributor to dysregulated sex steroid hormones,elevated dihydrotestosterone(DHT)had a significant positive rela-tionship with the clinical characteristics of BPH patients.Activation of adenosine 5'-monophosphate(AMP)-activated protein kinase(AMPK)by Met restored dysregulated sex steroid hormone homeostasis and exerted antiproliferative effects against DHT-induced proliferation by inhibiting the formation of androgen receptor(AR)-mediated Yes-associated protein(YAP1)-TEA domain transcription factor(TEAD4)heterodimers.Met's anti-proliferative effects were blocked by AMPK inhibitor or YAP1 over-expression in DHT-cultured BPH-1 cells.Our findings indicated that Met would be a promising clinical therapeutic approach for BPH by inhibiting dysregulated steroid hormone-induced PEC proliferation.
3.Advances in the correlation between digital eye strain and sleep quality in adolescents

Abstract
Digital eye strain can affect not only adolescents visual health, but also sleep quality. In order to provide a reference for safeguarding adolescents visual health and physical health, the paper reviews the direct correlation, feedback correlation, mediating role and the mechanisms between their digital eye strain and sleep quality, as well as proposes some strategies to reduce digital eye strain and improve sleep quality, such as screen time limits, adjusting the brightness and contrast of electronic screen devices, maintaining correct posture and viewing distance, increasing eye nutrition and protection, establishing a regular sleep routine, avoiding the use of electronic screen devices before bedtime, creating a comfortable and quiet sleep environment, and paying attention to the effects of diet and exercise.
4.The effect of two different skin flaps on replantation repair of skin defect type severed fingers and their impact on finger function recovery

Objective:To explore the effects of lateral finger flap and micro free venous flap on the replantation repair of skin defect type severed fingers and their impact on finger function recovery.Methods:A retrospective selection was conducted on the clinical data of 94 patients (94 fingers) with skin defect type finger amputation admitted to Affiliated Hospital of Jiangnan University from January 2020 to September 2022. According to the different design methods of the skin flap, there were 48 cases in the observation group (micro free venous flap) and 46 cases in the control group (finger side flap). We compared the repair effects, blood circulation, finger function recovery, and complications between two groups.Results:Two months after surgery, there was no statistically significant difference ( P>0.05) in the total effective rate of repair between the observation group [79.17%(38/48)] and the control group [76.09%(35/46)]. The blood flow velocity, skin temperature, and skin redness ratio of the observation group were higher than those of the control group, and the capillary filling time was shorter than that of the control group, with statistical significance (all P<0.05). There was no statistically significant difference (all P>0.05) in the scores of joint autonomy, daily living activities, and recovery between the two groups. The subjective sensation and appearance scores of the observation group were lower than those of the control group, and the differences were statistically significant (all P<0.05). The incidence of complications in the observation group was 10.42%(5/48), and there was no statistically significant difference ( P>0.05) compared to the incidence of complications in the control group [13.04%(6/46)]. Conclusions:For patients with severed fingers with skin defects, the lateral finger flap and mini free vein flap are effective methods for replantation. In clinical practice, reasonable skin flap repair methods can be flexibly selected based on the patient′s actual situation.
5.Clinical efficacy of vertical and oblique hollow tension screw internal fixation in patients with proximal phalangeal transverse fractures

Objective To explore the influence of longitudinal and oblique hollow lag screw internal fixation on interphalangeal range of motion,metacarpal range of motion and complications in patients with transverse fractures of proximal phalanges.Methods Retrospective analysis was performed on 102 patients with transverse fractures of proximal phalanges admitted to department of orthopedics of our hospital from June 2021 to February 2022.According to treatment methods,the patients were divided into longitudinal group and oblique group.The longitudinal group was given longitudinal hollow lag screw internal fixation while the oblique group was treated with oblique hollow lag screw internal fixation.The baseline data of the enrolled subjects were excluded from the influence of confounding factors according to the propensity matching scoring method(caliper value=0.02),and 51 patients with comparable baseline data were obtained in each group.The surgical status(surgical time and healing time),ranges of motions of interphalangeal and metacarpal joints,excellent and good rate of hand function(total active movement,TAM)and complications at 18 months after surgery were compared between the two groups.Results All patients successfully completed the surgery,and at least 18 months of follow-up data were obtained.There was no statistical significance in the surgical time or follow-up time between the two groups(P>0.05).The healing time in oblique group(6.57±1.22)weeks,was significantly shorter than that in longitudinal group(7.82±1.54)weeks(P<0.05).At 18 months after surgery,the interphalangeal range of motion was significantly higher in oblique group(82.62±3.41)° than that in longitudinal group(77.45±7.46)°(P<0.05),but there was no statistical difference in metacarpal range of motion between the two groups(P>0.05).The excellent and good rate of hand function in oblique group(94.12%)at 18 months after surgery was significantly higher than that in longitudinal group(80.39%)(P<0.05).The incidence of complications revealed no statistical significance between oblique group(1.96%)and longitudinal group(3.92%)(P>0.05).Conclusion In the hollow lag internal screw fixation treatment of patients with transverse fractures of proximal phalanges,both longitudinal and oblique screw methods have good effects,but the oblique screw method is more beneficial to the recovery of hand function.
6.Construction of a core competency indicator system for oncology advanced practice nurses

Objective:To construct a core competency indicator system for oncology advanced practice nurses.Methods:This study is a cross-sectional study. A preliminary draft of the core competency indicator system for oncology advanced practice nurses was developed through literature review and expert group coordination from June to November 2022. The core competency indicator system for oncology advanced practice nurses was established using the Delphi method for expert consultation and the analytic hierarchy process.Results:A total of 54 experts from 11 hospitals and four medical schools in 10 provinces and municipalities directly under the central government across the country were included in two rounds of expert consultation. The effective response rates of the questionnaire were all 100%, with an expert authority coefficient of 0.90, Kendall coordination coefficients of 0.089 to 0.179 and 0.101 to 0.176 ( P<0.01). The final established core competency indicator system for oncology advanced practice nurses included seven primary indicators and 69 secondary indicators. Conclusions:The core competency indicator system for oncology advanced practice nurses is comprehensive and has the characteristics of specialized oncology nursing, and the construction process is scientific and reliable, laying the foundation for future training of oncology advanced practice nurses.

Objective:To investigate the influencing factors of textbook outcomes in liver surgery (TOLS) after radical resection of gallbladder carcinoma.Methods:The retrospective case-control study was conducted. The clinicopathological data of 530 patients who underwent radical resection of gallbladder carcinoma in 15 medical centers, including the First Affiliated Hospital of Army Medical University et al, from January 2014 to January 2020 were collected. There were 209 males and 321 females, aged (61±10)years. Patients underwent radical resection of gallbladder carcinoma, including cholecystectomy, hepatectomy, invasive bile duct resection, and lymph node dissection. Observation indicators: (1) situations of TOLS; (2) influencing factors of TOLS. Measure-ment data with normal distribution were represented as Mean± SD, and comparison between groups was conducted using the independent sample t test. Measurement data with skewed distribution were represented as M( Q1, Q3), and comparison between groups was conducted using the Mann-Whitney U test. Count data were described as absolute numbers or percentages, and comparison between groups was conducted using the chi-square test. Comparison of ordinal data between groups was conducted using the Mann-Whitney U test. The univariate analysis was conducted using the corresponding statistical methods based on data type, and variables with P<0.10 were included in multivariate analysis. Multivariate analysis was conducted using the Logistic stepwise regression model. Results:(1) Situations of TOLS. All 530 patients underwent radical resection of gallbladder carcinoma, and there were 498 cases achieving R 0 resection, 508 cases without ≥grade 2 intra-operative adverse events, 456 cases without postoperative grade B and grade C biliary leakage, 513 cases without postoperative grade B and grade C liver failure, 395 cases without severe com-plications within postoperative 90 days, 501 cases did not being re-admission caused by severe com-plications within postoperative 90 days. Of the 530 patients, 54.53%(289/530) of patients achieved postoperative TOLS, while 45.47%(241/530) of patients did not achieve postoperative TOLS. (2) Influencing factors of TOLS. Results of multivariate analysis showed that American Society of Anesthesiologists classification >grade Ⅱ, preoperative jaundice, T staging as T3?T4 stage, N staging as N2 stage, liver resection as right hemi-hepatectomy, and neoadjuvant therapy were independent factors influencing TOLS in patients undergoing radical resection of gallbladder carcinoma ( odds ratio=2.65, 1.87, 5.67, 5.65, 2.55, 3.34, 95% confidence interval as 1.22?5.72, 1.18?2.95, 2.51?12.82, 2.83?11.27, 1.41?4.63, 1.88?5.92, P<0.05). Conclusion:American Society of Anesthesiologists classification >grade Ⅱ, preoperative jaundice, T staging as T3?T4 stage, N staging as N2 stage, liver resection as right hemi-hepatectomy, and neoadjuvant therapy are independent factors influencing TOLS in patients undergoing radical resection of gallbladder carcinoma.
8.Evaluation of influencing factors and uncertainty of detection of trace uranium in drinking water using ultraviolet fluorescence

Objective:To achieve rapid and accurate detection of trace uranium in drinking water by analyzing the factors influencing the accuracy of uranium measurement in drinking water using ultraviolet fluorescence method and by evaluating the uncertainty in measurement.Methods:The influence of acidity, Fe 3+ and Mn 2+ contents on the analitical result were studied to optimize the measurement conditions. The accuracy of the measurement method was verified in 7 laboratories. By studying the errors introduced in the process of standard preparation, sample pretreatment and measurement, the sources of uncertainty were analyzed and the uncertainty was synthesized. Results:At pH 1-11 in aqueous solution, the linear regression coefficient of the standard curve was greater than 0.995, which was in line with the linear measurement range of the instrument. At pH 12 or so, the linear regression coefficient was 0.761, which could not meet the measurement requirements. At pH<3 or pH>10, the increase in fluorescence count was lower, which might increase the measurement error. At Fe 3+ concentration ≥15 mg/L, a large deviation occurred in measurement value that could affect seriously measurement result. At Mn 2+ concentration ≥ 1.6 mg/L, the sample produced white precipitation, which could affect the measurement accuracy. Three spiked water samples with different concentrations were determined in 8 laboratories. Each water sample was measured six times in parallel. The relative standard uncertainty of the result were 6.42×10 -2, 4.48×10 -2 and 5.26×10 -2 μg/L, and the expanded uncertainties were 0.03, 0.06 and 0.12 μg/L( k=2), respectively. Conclusions:The optimum conditions for the determination of uranium in water using this method pH were in samples 3-10, the concentration of Fe 3+ less than 15 mg/L, and the concentration of Mn 2+ less than or equal to 1.6 mg/L. The main sources of uncertainty in the measurement of uranium in water using ultraviolet fluorescence method arise from the repeated measurement error and the volume of added standard solution.
9.Relative weights analysis on the impacts of childhood maltreatment forms on adolescent anxiety and depression

Objective:
To explore the relative effects of various forms of childhood maltreatment on adolescent anxiety and depression and to explore possible sex difference.
Methods:
By using convenient sampling method, 6 228 middle school students were selected from 2 middle schools in Chongqing. Self reported questionnaires were used to collect demographic characteristics, childhood maltreatment experiences, anxiety and depression symptoms. The relative weights analysis was conducted to analyze the relationship between childhood maltreatment forms and adolescent anxiety and depression and possible sex differences.
Results:
The prevalence of physical neglect, emotional abuse, physical abuse, emotional neglect and sexual abuse was 28.2%, 23.7%, 16.0 %, 12.4% and 10.3%, respectively. Among them, emotional abuse had the greatest impact on anxiety and depression ( R 2= 0.12 /0.15), followed by emotional neglect( R 2=0.08/0.11), physical abuse ( R 2=0.02/0.03), sexual abuse ( R 2=0.02/0.01) and physical neglect ( R 2=0.01/0.01). Compared with boys ( R 2=0.09/0.12, R 2=0.06/0.09), emotional abuse ( R 2=0.13/0.16) and emotional neglect ( R 2=0.10/0.13) had greater impacts on anxiety and depression among girls.
Conclusion
Psychological maltreatment (including emotional abuse and emotional neglect) in childhood has the greatest influence on anxiety and depression, especially for girls.
10.Study on source thickness and counting efficiency calibration for determining gross alpha activity in drinking water

Objective:To study the influence of source thickness and counting efficiency calibration on the measurement of gross alpha activity in water.Methods:241Am and natural uranium reference materials were spiked in drinking water to prepare source on a planchet with different thickness, for counting alpha activity on the planchet. Results:The effective thickness measured by spiking 241Am or uranium standard solution in water sample was consistent with the empirical value of 4 mg/cm 2. The alpha counting rate was in a linear increase trend from 2A-5A mg/cm 2 and was basically stable and no longer increase when thickness was higher than 10A mg/cm 2 (A was area of planchet). The result calculated by effective thickness method and thick source method were in good agreement when thickness was 10A mg/cm 2. Conclusions:In order to reduce the deviation of gross alpha counting rate caused by the source thickness and counting efficiency calibration, the source thickness is recommended to be 10A mg/cm 2.
