1.Establishment of a nomogram model for predicting liver cirrhosis with esophagogastric variceal bleeding based on aspartate aminotransferase-to-platelet ratio index and platelet-albumin-bilirubin score

ObjectiveTo investigate the value of aspartate aminotransferase-to-platelet ratio index (APRI) and platelet-albumin-bilirubin (PALBI) score in predicting the risk of esophagogastric variceal bleeding in patients with liver cirrhosis. MethodsA total of 119 patients with liver cirrhosis who were admitted to The First Affiliated Hospital of Soochow University from May 2021 and June 2022 were enrolled, and clinical data, routine blood test results, serum biochemistry, and coagulation test results were collected from all patients. According to the presence or absence of esophagogastric variceal bleeding, the patients were divided into non-bleeding group with 59 patients and bleeding group with 60 patients, and a comparative analysis was performed for the two groups. The independent samples t-test was used for comparison of normally distributed continuous data between two groups, and the Mann-Whitney U test was used for comparison of non-normally distributed continuous data between two groups; the chi-squared test or the Fisher’s exact test was used for comparison of categorical data between groups. The multivariate Logistic regression analysis was used to identify the independent risk factors for esophagogastric variceal bleeding in patients with liver cirrhosis and establish a nomogram predictive model. ResultsThe male patients accounted for 75.00% in the bleeding group and 40.68% in the non-bleeding group, and there was a significant difference in sex composition between the two groups (χ2=14.384, P<0.001). Chronic hepatitis B was the main etiology in both the bleeding group and the non-bleeding group (53.33% vs 38.98%), and there was no significant difference in composition ratio between the two groups (χ2=2.464, P=0.116). Compared with the non-bleeding group, the bleeding group had a significantly higher activity of AT-IIIA (t=3.329, P=0.001) and significantly lower levels of PLT, TBil, Ca, TC, and TT (all P<0.05). There were significant differences in APRI and PALBI between the two groups (χ2=6.175 and 19.532, both P<0.05). The binary logistic regression analysis showed that APRI (odds ratio [OR]=0.309, 95% confidence interval [CI]: 0.109 — 0.881, P=0.028), PALBI (OR=7.667, 95%CI: 2.005 — 29.327, P=0.003), Ca (OR=0.001, 95%CI: 0.000 — 0.141, P=0.007), TC (OR=0.469, 95%CI: 0.226 — 0.973, P=0.042), and TT (OR=0.599, 95%CI: 0.433 — 0.830, P=0.002) were independent influencing factors for esophagogastric variceal bleeding in liver cirrhosis. A nomogram model was established based on the above factors and had an index of concordance of 0.899 and a well-fitted calibration curve. ConclusionAPRI and PALBI have a good value in predicting esophagogastric variceal bleeding in patients with liver cirrhosis, and the nomogram model established based on this study can predict the incidence rate of esophagogastric variceal bleeding in patients with liver cirrhosis.
2.A study on the animal model preparation and early histological changes of trans-sutural distraction osteo-genesis

Objective:To establish an animal model of trans-sutural distraction osteogenesis in SD rats.Methods:A self-designed V-shaped distraction device(distractor)was fabricated with the traction force(N)of 0,1.3,2.2,3.0,4.3 and 5.0 corresponding to the distraction length(mm)of 5,4,3,2,1 and 0 respectively,meeting the trans-sutural distraction osteogenesis requirements in skull of 5-week-old SD rats.The distractor was plased into the sagittal suture of 12 SD rats.Continuous sampling was conducted 1,3,5 and 7 days respectively(n=3)after operation.The tissue changes in the trans-sutural distraction area were observed by HE and Masson's trichrome staining.Inflammation levels were determined using Arg-1 immunofluorescence staining.The early angiogenesis was clarified through co-staining with CD31 and EMCN.Results:A stable trans-sutural distraction osteogenesis model was estab-lished,5 mm distraction osteogenesis width was observed completely within 7 days of distraction.Significant new bone formation was observed at 7 days after operation.Arg-1 expression increased and was concentrated at the bone margins,overlapping with the areas of new bone formation.EMCN expression gradually decreased,and by day 7 CD31 was predominant,indicating the basic maturation of blood vessels.Conclusion:This study successfully constructed a stable and effective trans-sutural distraction osteogenesis animal model,and provides an experimental basis for the investigation of its early continuous histological changes.
3.Analysis of Color and Odor Changes of Different Processed Products of Paeoniae Radix Alba Based on HS-GC-MS and Electronic Sensory Techniques

ObjectiveTo investigate the correlation between the color, odor and volatile components of Paeoniae Radix Alba(PRA) and its processed products, and to examine the effects of different processing methods on the odor and color formation of PRA. MethodThe odor and chromaticity information of PRA, honey chaff-fried PRA and honey bran-fried PRA were identified by electronic nose and colorimeter, and the volatile components in the different processed products of PRA were identified and relatively quantified by headspace gas chromatography-mass spectrometry(HS-GC-MS), and analyzed using principal component analysis(PCA) and orthogonal partial least squares-discriminant analysis(OPLS-DA), then the differential key flavor components among the three were screened according to the principles of variable importance in the projection(VIP) value>1 and relative odor activity value(ROAV)≥1. Pearson correlation analysis was used to investigate the association between the differential flavor components and the colorimetric values and electronic nose sensors, respectively. ResultAfter being fried with honey chaff and honey bran, the lightness value(L*) of PRA decreased, and red-green value(a*) and yellow-blue value(b*) increased significantly(P<0.05, P<0.01). The odor differences were mainly reflected in the S1, S2, S4, S5, S6, S8 and S11 sensors, and the results of PCA of the electronic nose indicated that the odor differences among PRA, honey chaff-fried PRA and honey bran-fried PRA were obvious, among which the overall odor intensity of honey bran-fried PRA was higher than that of honey chaff-fried PRA. A total of 47 volatile components were identified from PRA and its processed products, including 21 for PRA, 36 for honey chaff-fried PRA, and 37 for honey bran-fried PRA. Odor analysis revealed that 12, 24 and 22 volatile components may be the key flavor components in PRA, honey chaff-fried PRA and honey bran-fried PRA, respectively. Correlation analysis showed that the L* of the decoction pieces was negatively correlated with the content of the Maillard reaction products, the a* and b* were positively correlated with the content of the Maillard reaction products, the S1 and S8 sensors were negatively correlated with the content of the Maillard reaction products, and the S2, S4, S5, S6 and S11 sensors were positively correlated with the Maillard reaction products. ConclusionThe color of PRA is deepened after being stir-fried with honey chaff and honey bran, and 5-hydroxymethylfurfural, furfural and other components are generated at the same time, which is in line with the theory of burning aroma strengthens the spleen of stir-fried with honey chaff and honey bran. Honey bran-fried PRA has a stronger Maillard reaction than honey chaff-fried PRA, which makes honey bran-fried PRA with a burnt flavor and a dark yellow color, while honey chaff-fried PRA has a sweet flavor and a bright yellow color.
4.Effect of computer-assisted training on post-stroke dysarthria

Objective To compare the effect of routine speech training and computer-assisted training on post-stroke dysarthria. Methods From March,2021 to April,2023,72 patients with post-stroke dysarthria in Beijing Bo'ai Hospital were ran-domly divided into control group(n=36)and experimental group(n=36).Both groups received routine rehabili-tation,while the control group received routine speech training,and the experimental group received computer-assisted training,for four weeks.They were assessed with modified Frenchay Dysarthria Assessment(m-FDA)and Speech Intelligibility(SI)before and after intervention. Results Eight cases in the control group and one case in the experimental group dropped down.The scores of m-FDA and SI improved in both groups after treatment(|Z|>4.183,P<0.001),and there was no significant difference between two groups(|Z|<1.598,P>0.05).Noninferiority of m-FDA was found between two groups(|t|>3.656,P<0.001). Conclusion Computer-assisted training could improve the speech function of patients with post-stroke dysarthria,simi-lar to routine speech training.
5.Analysis of influencing factors and research on humanistic care strategies for falls in inpatients during the rehabilitation period of stroke

Objective:To explore the influencing factors for falls in inpatients during the rehabilitation period of stroke and research humanistic care strategies.Methods:Using a retrospective case-control study design,32 stroke patients who experienced falls in the rehabilitation department of the Second Affiliated Hospital of Xi'an Jiaotong University from January 2019 to December 2022 were selected as the observation group,and 64 stroke patients who were hospitalized in the same department during the same period and did not suffer from falls were matched 1:2 by age and gender as the control group.Logistic regression analysis was used to determine the influencing factors for falls in inpatients during the rehabilitation period of stroke.Results:The results of multivariate analysis showed that a history of falls(OR=8.688,P=0.036),a fall risk score(OR=1.615,P=0.018),sleep disorders(OR=5.378,P=0.031),malnutrition(OR=11.071,P=0.035),and osteoporosis(OR=15.831,P=0.006)were the independent risk factors for falls in inpatients during the rehabilitation period of stroke.Conclusion:There are many influencing factors for falls in inpatients during the rehabilitation period of stroke.Medical staff should take effective intervention measures based on different risk factors and implement humanistic care to reduce the incidence of falls in inpatients during the rehabilitation period of stroke.
6.Study on macroscopic anatomy and developmental model of pterygopalatine suture

Objective·To analyze the anatomical and macroscopic characteristics of the pterygopalatine suture(PPS)in Chinese population using cone-beam computed tomography(CBCT)technology,and to preliminarily investigate its developmental pattern and its association with the correction of maxillary underdevelopment.Methods·A total of 134 CBCT images,taken from July to August 2023 at Shanghai Ninth People's Hospital,Shanghai Jiao Tong University School of Medicine,were categorized into six age groups.The PPS landmarks were utilized to determine the overall transverse position(XPPS),sagittal position(YPPS),insertion angle(IAP),insertion width of the pyramidal process(IWP),and insertion depth of the pyramidal process(IDP).The pterygomaxillary junction was identified through multiplanar observations.Regression analysis was performed to assess the correlation of these parameters with age and gender,and pairwise comparisons were made to determine the stable age range for parameter changes.Paired t-tests and paired chi-squared tests were conducted to analyze the bilateral parameter differences.Results·Results showed that YPPS and IDP did not exhibit significant correlation with age,whereas XPPS,IAP,IWP,and the incidence of pterygomaxillary fusion were significantly positively correlated with age(P<0.01).Gender differences were only significant for XPPS,with males demonstrating greater values than females(P<0.01).Analysis of age-related trends indicated significant differences in XPPS between group 1(6 years≤age<9 years)and group 2(9 years≤age<12 years)(female:P=0.006,male:P=0.004);significant differences in IAP were observed between group 2 and group 3(12 years≤age<15 years)(P=0.042),with 98.5%of samples having an IAP greater than 45 degrees;IWP differences were significant between group 1 and group 3(P=0.016),and the pterygomaxillary fusion incidence was significantly different among group 1,2,and 3(group 1 vs.2:P<0.001,group 2 vs 3:P=0.037,group 1 vs 3:P<0.001),with an incidence rate exceeding 90%in adults.No significant bilateral differences were found for all parameters.Conclusion·No significant changes were observed in YPPS and IDP after the age of 6,indicating a trend towards fusion of the maxilla with the pterygoid process.The overall transverse position of the PPS tends to stabilize around 12 years of age,while the IAP and IWP continue to increase and reach stability around 15 years of age,with a sagittal insertion orientation of the pyramidal process into the pterygoid notch.
7.Melittin induces autophagy to alleviate chronic renal failure in 5/6-nephrectomized rats and angiotensin II-induced damage in podocytes

BACKGROUND/OBJECTIVES:
Chronic renal failure (CRF) is a complex pathological condition that lacks a cure. Certain Chinese medicines, such as melittin, a major component in bee venom, have shown efficacy in treating CRF patients. On the other hand, the mechanisms underlying the therapeutic effects of melittin are unclear.MATERIALS/METHODS: A 5/6 nephrectomy model (5/6 Nx) of renal failure was established on rats for in vivo assays, and mouse podocyte clone 5 (MPC5) mouse podocyte cells were treated with angiotensin II (AngII) to establish an in vitro podocyte damage model. The 24-h urine protein, serum creatinine, and blood urea nitrogen levels were evaluated after one, 2, and 4 weeks. Hematoxylin and eosin staining, Masson staining, and periodic acid-Schiff staining were used to examine the pathological changes in kidney tissues. A cell counting kit 8 assay was used to assess the cell viability. Reverse transcription polymerase chain reaction and Western blot were used to assess the mRNA and protein levels in the cells, respectively.
RESULTS:
In the rat 5/6 Nx, melittin reduced the 24-h urinary protein excretion and the serum creatinine and blood urea nitrogen levels. Furthermore, the renal pathology was improved in the melittin-treated 5/6 Nx rats. Melittin promoted podocin, nephrin, Beclin 1, and the LC3II/ LC3I ratio and inhibited phosphorylated mammalian target of rapamycin (mTOR)/mTOR in 5/6 Nx-induced rats and AngII-induced MPC5 mouse podocyte cells. Moreover, inhibiting autophagy with 3-MA weakened the effects of melittin on podocin, nephrin, and the LC3II/ LC3I ratio in podocytes.
CONCLUSION
Melittin may offer protection against kidney injury, probably by regulating podocyte autophagy. These results provide the theoretical basis for applying melittin in CRF therapy.

9.Effect of music therapy on post-stroke cognitive impairment

ObjectiveTo observe the effect of music therapy on cognitive function, motor function, and activities of daily living (ADL) of patients with post-stroke cognitive impairment. MethodsFrom December, 2020 to July, 2022, 48 patients with post-stroke cognitive impairment in Beijing Bo'ai Hospital were enrolled and randomly divided into control group (n = 24) and experimental group (n = 24). Both groups received conventional medication, nursing, and conventional rehabilitation, while the experimental group received music therapy additionally, for eight weeks. Before and after intervention, the cognitive function was assessed by Loewenstein Occupational Therapy Cognitive Assessment (LOTCA), and motor function and ADL were assessed with Fugl-Meyer Assessment (FMA) and modified Barthel Index (MBI), respectively. ResultsSix cases in the control group dropped down. The increase in the scores of LOTCA and FMA was more in the experimental group than in the control group (t > 2.665, P < 0.05), however, no significant difference was found in the increase of MBI score between two groups. ConclusionMusic therapy could improve the cognitive function and motor function of patients with post-stroke cognitive impairment.
10.Risk factors for postoperative nausea and vomiting in obese patients undergoing laparoscopic sleeve gastrectomy

Objective:To retrospectively identify the risk factors for postoperative nausea and vomiting (PONV)in the obese patients undergoing laparoscopic sleeve gastrectomy(LSG).Methods:The medical records from the obese patients who underwent elective laparoscopic sleeve gastrectomy from January 2018 to July 2022 were retrospectively collected. PONV was defined according to the use of remedial antiemetics in the nursing record sheet, and the patients were divided into PONV group and non-PONV group according to the occurrence of PONV that required treatment. The logistic regression analysis was used to identify the risk factors for PONV after LSG.Results:A total of 1 264 obese patients were included in this study, and there were 263 patients in PONV group, and the incidence of PONV was 20.81%. According to the results of multifactorial logistic regression analysis, female( OR=1.533, 95% CI 1.007-2.334, P=0.046), higher level of serum alanine aminotransferase concentrations ( OR=1.006, 95% CI 1.002-1.009, P=0.001), higher level of C-reactive protein ( OR=1.013, 95% CI 1.005-1.022, P=0.001), general anesthesia combined with nerve block (general anesthesia combined with TAPB: OR=2.737, 95% CI 1.817-4.121, P<0.001; general anesthesia combined with other nerve block: OR=1.899, 95% CI 1.249-2.889, P=0.003) and intraoperative use of sufentanil ( OR=2.114, 95% CI 1.308-3.415, P=0.002) were independent risk factors for PONV( P<0.05). However, the higher level of serum follicle-stimulating hormone concentrations ( OR=0.941, 95% CI 0.895-0.988, P=0.015), intraoperative use of dexmedetomidine ( OR=0.640, 95% CI 0.417-0.982, P=0.041), and administration of prophylactic antiemetic medication (antiemetic drugs during operation OR=0.669, 95% CI 0.469-0.955, P=0.027; antiemetic drugs after operation OR=0.303, 95% CI 0.182-0.503, P<0.001; antiemetic drugs during and after operation OR=0.215, 95% CI 0.107-0.434, P<0.001) were protective factors for PONV. Conclusions:Female, higher levels of serum alanine aminotransferase and C-reactive protein, general anesthesia combined with nerve block and intraoperative use of sufentanil are independent risk factors for PONV, while higher levels of follicle-stimulating hormone, intraoperative use of dexmedetomidine and administration of prophylactic antiemetic medication are protective factors for PONV among obese patients undergoing LSG.
