1.Acupuncture Treatment Strategies for Crohn's Disease Based on the Principle of "Shaoyang as the Pivot"

The shaoyang meridian is an important pivot between the internal organs and meridians system, with the functions of regulating qi and blood, balancing yin and yang, and coordinating the ascending and descending movement of qi. Dysfunction of the shaoyang pivot can lead to spleen and kidney deficiency, impaired liver and gallbladder qi regulation, and stagnation of qi and blood. It is believed that the onset and progression of Crohn's disease are closely related to shaoyang pivot dysfunction, with the core pathogenesis characterized by shaoyang disharmony, spleen deficiency, dampness retention, and blood stasis. Based on this understanding, the treatment principle centers on harmonizing the shaoyang pivot, supplemented by methods such as warming and nourishing the spleen and stomach, tonifying shaoyang, and soothing the liver and benefiting the gallbladder. Acupuncture is employed to target key acupoints along the shaoyang meridian to restore its regulatory functions, improve spleen and stomach transformation and transportation, facilitate liver and gallbladder qi flow, and promote the circulation of qi and blood. This provides a practical therapeutic approach for acupuncture-based treatment of Crohn's disease.
2.Analysis of the particle size of traditional Chinese medicine ointment in the hospital

Objective To investigate the particle size of traditional Chinese medicine (TCM) ointments in various hospitals. Methods The powders of 7 varieties of externally-applied TCM ointments from Longhua Hospital, Shanghai University of Traditional Chinese Medicine (our hospital) were subjected to comminution processes. Each variety was separately processed by a universal grinder, a hammer mill, and a jet mill, yielding a total of 21 samples. The particle sizes of these 21 samples from our hospital, along with 6 samples obtained from 6 other hospitals, were measured by employing a Bettersize2 laser particle size analyzer and microscopic examination. Results The volume-based average particle size of 21 samples from our hospital ranged from 3.34 to 52.53 μm, while that of 6 samples from other hospitals ranged from 38.59 to 118.50 μm. Notably, the particle size of samples processed by jet milling could be reduced by 12% to 86% compared with those processed by universal mechanical crushing. Microscopic observation revealed that particles larger than 180 μm were detected in 17 samples from our hospital and all 6 samples from other hospitals. Conclusion Compared with universal grinder sample and hammer grinder, jet mill could reduce the average particle size of powder. However, the increasement in pulverization time and loss of pulverization were substantial. Furthermore, in accordance with the particle size requirements for ointments specified in the current Pharmacopoeia of the People's Republic of China (2020 Edition), particle size testing of TCM ointments remains challenging. Therefore, further data accumulation should be imperative to establish particle size standards for TCM ointments.
3.Study on the treatment of cerebral infarction with Chinese medicine Shenmajingfu granule by network pharmacology

Objective To explore the material basis and mechanism of the Chinese medicine Shenmajingfu granules in the treatment of cerebral infarction. Methods The potential active ingredients and targets of Shenmajingfu granules were retrieved through TCMSP, ETCM database and TCM Database. The related target genes of cerebral infarction were searched from OMIM database. The common targets of Shenmajingfu granules and cerebral infarction were obtained by the intersection method. Cytoscape was used to construct active components of Shenmajingfu granules-targets network. Protein-protein interaction network was constructed by STRING software. DAVID database was used for GO and KEGG enrichment analysis. Results The 183 potential active ingredients of Shenmajingfu granules were screened out. 1785 potential targets were screened in the TCMSP database, including 30 targets related to cerebral infarction. These target genes were mainly involved in the inflammatory response and apoptosis process, involving the TNF signaling pathway, HIF-1 signaling pathway and NF-κB signaling pathway. Conclusion The therapeutic effect of Shenmajingfu granules on cerebral infarction may be related to the regulation of inflammatory response, improvement of impaired neurological function and protection of cerebral ischemia-reperfusion injury.
4.Calculating Airway Resistance and Lung Compliance at Expiration End under Non-invasive Positive Pressure Ventilation:An Experimental Study

Objective Airway resistance(R)and lung compliance(C)under non-invasive positive pressure ventilation(NPPV)conditions were measured using a brief pressure release at the end of expiration,and the measurement accuracy was also evaluated.Methods An NPPV respirator was developed by programming a method for calculating R and C.An experimental platform based on the active servo lung ASL5000 was designed.By simulating a healthy adult(R=5 cmH2 O and C=50 mL/cmH2 O,1 cmH2 O=0.098 kPa),an adult patient with acute respiratory distress syndrome(R=10 cmH2 O and C=30 mL/cmH2 O),and an adult patient with chronic obstructive pulmonary disease(R=20 cmH2 O and C=50 mL/cmH2 O),a series of experiments for calculating the R and C were conducted.Results The maximum relative error of R was-12.67%,which occurred in calculating the R of an average adult.The maximum relative error of C was 17.37%,which occurred when calculating the C values of patients with acute respiratory distress syndrome.Each group of data was analyzed using a paired t-test,which showed statistically significant differences(P>0.05).Conclusions The calculation method for R and C at the end of expiration during NPPV is feasible,and its realization and application will be beneficial for achieving precise and personalized respiratory ventilation.
5.Modeling the noninvasive bi-level positive airway pressure ventilation therapy system and simulated application.

Without artificial airway though oral, nasal or airway incision, the bi-level positive airway pressure (Bi-PAP) has been widely employed for respiratory patients. In an effort to investigate the therapeutic effects and measures for the respiratory patients under the noninvasive Bi-PAP ventilation, a therapy system model was designed for virtual ventilation experiments. In this system model, it includes a sub-model of noninvasive Bi-PAP respirator, a sub-model of respiratory patient, and a sub-model of the breath circuit and mask. And based on the Matlab Simulink, a simulation platform for the noninvasive Bi-PAP therapy system was developed to conduct the virtual experiments in simulated respiratory patient with no spontaneous breathing (NSB), chronic obstructive pulmonary disease (COPD) and acute respiratory distress syndrome (ARDS). The simulated outputs such as the respiratory flows, pressures, volumes, etc, were collected and compared to the outputs which were obtained in the physical experiments with the active servo lung. By statistically analyzed with SPSS, the results demonstrated that there was no significant difference ( P > 0.1) and was in high similarity ( R > 0.7) between the data collected in simulations and physical experiments. The therapy system model of noninvasive Bi-PAP is probably applied for simulating the practical clinical experiment, and maybe conveniently applied to study the technology of noninvasive Bi-PAP for clinicians.

6.Babaodan Alleviates APAP-induced Acute Liver Injury in Mice by Inhibiting NLRP3/Caspase-1 Pathway

ObjectiveTo explore the effect of Babaodan (BBD) on the NOD-like receptor pyrin domain containing 3/cysteine aspartate-specific protease-3 (NLRP3/Caspase-1) pathway proteins in mice with acetaminophen (APAP)-induced acute liver injury. MethodC57BL/6 mice were randomly grouped, and BBD (75, 150, 300 mg·kg-1, ig) was administered twice a day for three days. After 2 hours of the last administration, the mice were treated with APAP (400 mg·kg-1, ip), and the eyeballs were removed to collect blood after 14 hours. Then they were sacrificed by cervical dislocation for sample collection. Hematoxylin-eosin (HE) staining was used to observe the morphological changes of liver tissue cells, and biochemical methods were used to detect the activities of alanine aminotransferase (ALT), aspartate aminotransferase (AST), superoxide dismutase (SOD), malondialdehyde (MDA) and myeloperoxidase (MPO) in serum of mice in each group. Real-time fluorescence quantitative polymerase chain reaction (Real-time PCR) was performed to determine the mRNA expression of tumor necrosis factor-α (TNF-α), interleukin-1β (IL-1β) and IL-6, and Western blot was performed to determine the protein expression of cyclooxygenase-2 (COX-2), inducible nitric oxide synthase (iNOS), NLRP3, Caspase-1 and IL-18 in the liver of mice. ResultCompared with the conditions in normal group, the hepatic lobule structure of mice in the model group was partially destroyed, and the hepatic sinusoids were dilated. And the expression levels of ALT and AST in serum, the protein levels of NLRP3, Caspase-1, iNOS, IL-18 and COX-2 and the mRNA levels of IL-1β, IL-6 and TNF-α were increased (P<0.05, P<0.01). Compared with the model group, the administration groups had improvement in liver cell rupture and hepatic sinusoidal compression, and a dose-dependent decrease in the levels of ALT and AST in serum as well as the protein levels of NLRP3, Caspase-1, iNOS, IL-18 and COX-2 and the the mRNA levels of IL-1β, IL-6 and TNF-α in liver tissue (P<0.05, P<0.01). ConclusionBBD can reduce APAP-induced acute liver injury in mice. The mechanism may be related to anti-oxidative stress, inhibition of NLRP3/Caspase-1 pathway, and decreased expression levels of IL-1β, IL-18, TNF-α and IL-6.
7.A consensus on the management of allergy in kindergartens and primary schools

Abstract
Allergic diseases can occur in all systems of the body, covering the whole life cycle, from children to adults and to old age, can be lifelong onset and even fatal in severe cases. Children account for the largest proportion of the victims of allergic disease, Children s allergies start from scratch, ranging from mild to severe, from less to more, from single to multiple systems and systemic performance, so the prevention and treatment of allergic diseases in children is of great importance, which can not only prevent high risk allergic conditions from developing into allergic diseases, but also further block the process of allergy. At present, there is no consensus on the management system of allergic children in kindergartens and primary schools. The "Consensus on Allergy Management and Prevention in Kindergartens and Primary Schools", which includes the organizational structure, system construction and management of allergic children, provides evidence informed recommendations for the long term comprehensive management of allergic children in kindergartens and primary schools, and provides a basis for the establishment of the prevention system for allergic children.

9.Protective effect of Shenmajing formula on ischemic brain injury in mice

Objective To observe the protective effect of Shenmajing formula on brain tissue of mice with cerebral ischemic injury and explore the possible mechanism. Methods Thirty SPF-grade C57 BL/6 male mice were randomly divided into model control group, Shenmajing group and nimodipine group, and the animal models of cerebral ischemic injury in mice were prepared by electrocoagulation. The protein expression level in endothelial progenitor cells were detected by Western blot. Results Compared with the model control group, the infarct volume of mice in the Shenmajing group was significantly reduced, and the migration, adhesion and tubule formation ability of endothelial progenitor cells were significantly improved, and the expression level of BDNF protein in endothelial progenitor cells was significantly increased. Conclusion The protective effect of Shenmajing granules on brain tissue of mice with cerebral ischemic injury could be closely related to the regulation of BDNF expression in endothelial progenitor cells and improvement of endothelial progenitor cell function of bone marrow origin.
10.Review of clinical use of Dingqing Tablets in a Tertiary A Hospital of Integrated Traditional Chinese and Western Medicine

Objective To improve the quality of prescriptions and promote the rational drug application of Dingqing Tablets by investigating the outpatient prescriptions in a tertiary A hospital. Methods A total of 4 796 prescriptions of outpatient pharmacy patients from August 1, 2020 to August 1, 2021 were extracted from the hospital information system by the hospital information software, focusing on the analysis of indications, usage and dosage, drug interaction, etc. Results 10 departments including hematology department and geriatrics department were used Dingqing Tablets, and the irrationality was mainly manifested in the superposition of drug flavors and drug interactions. Conclusion Dingqing tablets were widely used in clinic and had remarkable curative effect. However, there are certain risks in the use of Dingqing tablets. It is necessary to add medication education and supervision to promote the safe and rational use of drugs in clinic.
