Objective:To evaluate the safety and feasibility of applying injectable cross-linked sodium hyaluronate isolation gel in radical hypofractionated radiation therapy for prostate cancer. Methods:In this prospective study, patients at Beijing Hospital who were pathologically diagnosed with clinical stage T 1-2N 0M 0 prostatic acinar adenocarcinoma by puncture and underwent radical radiation therapy were included. All patients received ultrasound-guided cross-linked sodium hyaluronate isolation gel injection and image-guided intensity-modulated radiation therapy (IG-IMRT). The prescription dose was moderately hypofractionated, with a prescription dose of 60 Gy in 20 fractions for 5 times a week, once daily, which was delivered to 95% of the planning target volume (PTV) of prostate and seminal vesicle. Analyze the prostate rectal spacing (PRS) at the baseline, on the day of injection, during the radiotherapy, 1 month and 3 months after radiotherapy, changes in rectal volume before and after injection, and incidence of rectum-related side effects. The changes in all indexes before and after injection were analyzed by using t-test. Results:A total of 13 patients were enrolled from March 2022 to February 2023. The isolation gel maintained morphologic stability without significant spatial changes during radiotherapy, and the mid-prostate had the best effect, with PRS up to 1 cm. At 3 months after radiotherapy, the isolation gel was seen to decreased in volume with a certain absorptive capacity. The irradiated volume of rectum was decreased significantly in all patients after gel injection, and the mean volumes of rectal V 60 Gy , V 50 Gy , V 30 Gy , and V 20 Gy before and after injection were 1.923% vs. 0.280%, 10.255% vs. 3.172%, 29.602% vs. 18.800%, and 49.452% vs. 40.259% (all P<0.005). The average values (range) of rectal V 60 Gy , V 50 Gy , V 30 Gy , V 20 Gy decreases were 84.9%( 29% - 100%), 69.6%(27%-100%), 36.3%(0%-75%), and 17.8%(0%-50%), respectively. No grade 3-4 side effects occurred in all patients, and there were no common grade 1-2 rectal side effects such as diarrhea, rectal bleeding, proctitis and anal pain, etc. Only one patient developed grade 1 constipation during radiotherapy. Conclusion:Injection of Chinese made cross-linked sodium hyaluronate isolation gel can significantly reduce the irradiated volume of rectum and the incidence of rectal toxicities in prostate cancer patients undergoing radical radiotherapy.

ObjectiveTo investigate the therapeutic effect of Qingmei compound on acute gouty arthritis (AGA) in rats and preliminarily clarify its mechanism. MethodForty male SD rats were randomly divided into a blank group, a model group, a colchicine group (0.3 mg·kg-1), and low- and high-dose Qingmei compound groups (200 and 400 mg·kg-1), with eight rats in each group. The AGA model was induced by injecting 50 g·L-1 monosodium urate (MSU) into the ankle joint of the rats except those in the blank group. The ankle swelling index was measured before and 6, 24, and 48 h after modeling. The pathological changes in the joint tissues of AGA rats were observed by hematoxylin-eosin (HE) staining. The expression of tumor necrosis factor-α (TNF-α) and interleukin-1β (IL-1β) in the joint tissues of rats was detected by immunohistochemistry. The protein expression of NOD-like receptor protein 3 (NLRP3) pathway and key proteins in the joint tissues of rats was detected by Western blot. ResultCompared with the blank group, the model group showed increased ankle swelling index, synovial hyperplasia, and inflammatory infiltration, and up-regulated expression of IL-1β, TNF-α, and NLRP3 proteins in the ankle joint and the ratio of Caspase-1 shear body to Caspase-1 precursor protein (Caspase-1 p20/Caspase-1) (P<0.01). Compared with the model group, the Qingmei compound groups showed reduced ankle swelling index of AGA rats, especially the low-dose Qingmei compound group (P<0.01). Meanwhile, Qingmei compound inhibited synovial hyperplasia and inflammatory infiltration (P<0.01) and reduced the levels of IL-1β, TNF-α, and NLRP3 proteins and Caspase-1 p20/Caspase-1 in joint tissues (P<0.01). ConclusionQingmei Compound can significantly alleviate the joint swelling and inflammatory infiltration of AGA, and its mechanism may be related to the inhibition of the NLRP3 signaling pathway.

Objective To evaluate the clinical effects of sensory stimulation provided by nurses and family members respectively on waking up unconscious patients with severe traumatic brain injury. Methods A total of 92 unconscious patients with severe traumatic brain injury admitted in the Intensive Care Unit, Neurosurgery Department, Affiliated Hospital of Nantong University between May 2016 and May 2017 were selected and divided into two groups according to the random number table. Patients in the control group received sensory stimulation by nurses, while patients in the observation group received sensory stimulation by family members. The Glasgow Coma Scale (GCS), Western Neuro Sensory Stimulation Profile (WNSSP), Rancho Los Amigos Scale (RLA) and electroencephalogram examination (EEG) scores were compared between the two groups before and after the intervention. Results There was no statistical difference in the scores between the two groups before the sensory stimulation (P> 0.05). Two weeks post the intervention, there were statistical differences between the control group and the observation group in GCS [(7.28±1.86) vs. (9.78±2.01)], WNSSP [(13.23±10.07) vs. (47.48±14.32)]and RLA [(2.15±0.98) vs. (3.20±1.30)]scores (P< 0.05), but there was no statistical difference in EEG between the two groups (17.39% vs. 30.43%, P> 0.05). Conclusions Sensory stimulation can enable the unconscious patients with severe traumatic brain injury to regain consciousness, cognitive and sensory functions. The sensory stimulation provided by caregivers is better than that by responsible nurses, and the former is better in accelerating the patients' rehabilitation.
