1.Effects of Chaihu Shugan Powder (柴胡疏肝散) on Duodenal Mucosal Barrier and Intestinal Flora in Functional Dyspepsia Model Rats

ObjectiveTo study the possible mechanism of Chaihu Shugan Powder (柴胡疏肝散, CSP) in the treatment of functional dyspepsia (FD). MethodsTwenty-four SD rats were randomly divided into a normal group, a model group, a CSP group and a probiotic group, with six rats in each group.The tail-clamping provocation method was used in all groups except for the normal group to replicate the FD rat model. Simultaneously, the normal group and the model group were given 10 ml/(kg·d) of saline by gavage, while the CSP group and the probiotic group were given 9.6 g/(kg·d) of CSP aqueous decoction and 0.945 g/(kg·d) of probiotic aqueous solution by gavage, respectively, twice daily for four weeks. After four weeks, the gastric emptying and small intestinal propulsion rates were detected in each group of rats. Hematoxylin-eosin (HE) staining was used to observe the histopathological changes in the gastric sinusoids and duodenum of the rats. The changes in the intestinal flora were analyzed by 16s rDNA high-throughput gene sequencing, and the expressions of the duodenal zona occludin 1 (ZO-1) and Occludin were detected by immunohistochemistry and western blotting. Pearson correlation analysis was performed on intestinal flora and ZO-1 and Occludin protein expression. ResultsThe gastric antrum tissue structure was clear in all groups, and the gland structure was regular, with smooth gastric tissue mucosa and no pathological changes such as erosion and ulcer. Compared to those in the normal group, the intestinal villi in the duodenal tissue in the model group were significantly reduced or atrophied, and the goblet cells were arranged in disorder, with eosinophilic infiltration; the gastric emptying rate and small intestinal propulsion rate, as well as ZO-1 and Occludin protein expression in duodenal tissue significantly decreased (P<0.01). Compared to those in the model group, the duodenal tissue structure was clear, and the length intestinal villi was longer, with goblet cells neatly arranged in the CSP group and the probiotic group; no obvious eosinophil infiltration was found, and the gastric emptying rate and small intestinal propulsion rate as well as ZO-1 and Occludin protein expression significantly increased in the CSP group; a small amount of eosinophil infiltration was found, and the gastric emptying rate and Occludin protein expression significantly increased in the probiotic group (P<0.05 or P<0.01). Beta diversity analysis of intestinal flora showed that the overall structure of intestinal flora in the model group changed significantly compared to that in the normal group (P<0.01). The overall structure of the intestinal flora in the CSP group and the probiotic group was closer to the normal group than the model group. Species composition analysis showed that the relative abundance of the Firmicutes decreased, while the relative abundance of the Bacteroidetes and norank_f_Muribaculaceae increased, and the Bacteroidetes/Firmicutes value increased in the model group than those in the normal group (P<0.05 or P<0.01). Compared to those in the model group, the relative abundance of the Firmicutes increased, while the relative abundance of the Bacteroidetes and norank_f_Muribaculaceae, as well as the Bacteroidetes/Firmicutes value decreased in the CSP group and the probiotic group (P<0.05 or P<0.01). There was no statistically significant difference in each indicator between the probiotic group and the CSP group (P>0.05). 3.The investigation of vaccination in patients with systemic lupus erythematosus

Objective:To investigate the prevalence of influenza, pneumococcal, hepatitis B virus (HBV), human papillomavirus (HPV), and varicella zoster virus (VZV) vaccination in patients with systemic lupus erythematosus (SLE), and to analyze the factors related to vaccination.Methods:Data were obtained from 1 203 patients with SLE, via a multi-center web-based survey using an online questionnaire. Data about their social conditions, clinical presentations, willingness for being vaccinated, vaccination within 5 years were collected. Demographic data were shown by descriptive analysis. Chi-square and logistic regression analysis were used to assess the power of related indexes as predictors of vaccination.Results:The vaccination rates of influenza, pneumococcal, HBV, HPV, and VZV were 5.49% (66/1 203), 0.66% (8/1 203), 2.08% (25/1 203), 3.82% (46/1 203), and 0.17% (2/1 203), respectively. Data analysis showed that higher education ( χ2=30.94, P<0.001) and higher income ( χ2=10.70, P=0.001) had greater effects on influenza vaccination. There was a relationship between HPV vaccination and higher education ( χ2=20.96, P<0.001), higher income ( χ2=20.56, P<0.001), younger age ( χ2=8.54, P=0.001), and single ( χ2=5.63, P=0.018). Male ( χ2=10.27, P=0.001) and higher education ( χ2=4.52, P=0.034) were associated with HBV vaccination. The multivariate logistic regression analysis revealed that higher education [ OR (95% CI)=2.14 (1.10, 4.18), P=0.026], having children under 18 years-old [ OR(95% CI)=1.802(1.02, 3.18), P=0.042], and hydroxychloroquine usage [ OR(95% CI)=2.55(1.06, 6.15), P=0.037], had a positive correlation with influenza vaccination. Male [ OR(95% CI)=4.24(1.37, 13.08), P=0.012], had an impact on HBV vaccination. The factors related to HPV vaccination included age <45 [ OR (95% CI)=0.93(0.89, 0.97), P=0.001], higher education [ OR(95% CI)=2.28(1.11, 4.65), P=0.024], higher income [ OR(95% CI)=2.68(1.32, 3.41), P=0.006] and the usage of immunosuppressive agents [ OR(95% CI)=1.92(1.03, 3.59), P=0.041]. Conclusion:The prevalence of vaccination in patients with SLE is low. Patients with higher education and income are more likely to being vaccinated.
4.Characteristics of hepatitis C virus infection and genotype distribution among entry travelers at Tengchong Port

Objective To investigate the characteristics of epidemic and genotype/subtype distribution of hepatitis C virus (HCV) among entry travelers at Tengchong port,to provide references for HCV prophylaxis and treatment.Methods A total of 54 serum samples were collected from anti-HCV positive travelers at Tengchong port from June 2009 to June 2016.HCV NS5B gene was amplified using reverse transcription polyonerase chain reation (RT-PCR) and subsequently sequenced.Based on the obtained sequences and retrieved reference sequences,phylogenetic analysis was conducted to determine HCV genotype/subtype.Results HCV infection rate among entry travelers at Tengchong ports was 0.45 % (54/12 059).Forty five samples were successfully genotyped.Phylogenetically,HCV genotype 3b was revealed to be the predominant subtype (28.89 %,13/45) in this population,followed by genotype 6n (20.0%,9/45),genotype 1b (17.78%,8/45),genotype 3a (13.33%,6/45),genotype 2a (11.11%,5/45),genotype 1a (2.22%,1/45) and genotype 6a (2.22%,1/45).The major genotype in Myanmar travelers was genotype 6,while in Chinese population,genotype 1 predominated.Genotype 6 in the population showed close phylogenetic relationship with strains prevalent in China and Southeast Asia.Genotype 3 was closely clustered with strains prevalent in China.Conclusions The distribution of HCV genotypes among entry travelers at Tengchong port is impacted by HCV epidemic strains both in Yunnan province and neighboring regions.This population serves as a transmitting media which may influence the epidemiological characteristics of HCV in Tengchong and neighboring areas.
5.Value of hysteroscopy and dilatation and curettage in diagnosis of endometrial cancer

Objective To evaluated the value of hysteroscopy and dilatation and curettage (DC) in diagnosis of endometrial cancer. Methods This retrospective analysis included clinical pathologic data of 3 676 patients with endometrial cancer from Jan. 1, 2000 to Dec. 31, 2010 in hospitals of endometrial cancer prevention projects in Guangdong Province. Results A total of 3 676 patients with endometrial cancer were divided into DC group (3 211 patients) and hysteroscopy group (465 patients). Compared to the results of pathological diagnosis, the accuracy rate between DC group and in hysteroscopy group were no statistically difference was 91.00%(2 922/3 211) vs 90.75%(422/465;χ2=0.030, P=0.862). The accuracy rate, sensitivity, specificity, positive predictive value and negative predictive value of cervical involvement between DC group and hysteroscopy group were 81.28%vs 86.45%(P<0.01), 24.78%vs 23.68%(P>0.05), 93.76%vs 98.71%(P<0.01),46.75%vs 78.26%(P<0.01) and 84.95%vs 86.88%(P>0.05), respectively. Rate of positive peritoneal cytology in DC group was 4.76%(153/3 211), and the rate was 3.23%(15/465) in hysteroscopy group, which were no statistically difference (χ2=2.206, P=0.137). There were no statistically difference in 5-year overall survival (91.02% vs 92.03%;χ2=0.033, P=0.856) and 5-year progression-free survival (89.81%vs 91.83%;χ2=1.508, P=0.219) between DC group and hysteroscopy group. Conclusions Hysteroscopy and dilatation and curettage is an effective method in diagnosis of endometrial cancer, especially hysteroscopy is better in diagnosis of cervical involvement. Hysteroscopy don′t improve risks of positive peritoneal cytology and don′t affect the prognosis of patients with endometrial cancer.
6.Application Evaluation of Clinical Pathway-based Teaching Methods in Clinical Teaching of Hematology

Objective To investigate the effects of clinical pathway-based teaching methods in clinical teaching of hematology. Methods Interns, which studied in department of hematology, were classified non-randomized into two groups. The interns in experimental group received idiopathic thrombocytopenic purpura (ITP) clinical pathway-based teaching. And the interns in the control group received the seventh edition textbook of medicine-based traditional teaching. All interns were oral tested when they finished two weeks period clinical studied of hematology. Meanwhile, a questionnaire designed for clinical pathway-based teaching was included in this test only for the experimental group. Results There were 32 interns recruited in the group of clinical pathway-based teaching and 38 interns in the control group of traditional teaching. There were statistically significant differences between the two groups in the fields of the key points about history of diseases ( = 0.0017), assessment of diagnostic criteria and differential diagnosis ( =0.0074), selection of laboratory examination items ( <0.0001) for ITP. The group of clinical pathway-based teaching achieved higher scores than the control group. However, there was no statistically significant difference between the two groups in the field of selection of first-line treatment measure of ITP ( = 0.1155) . Moreover, the control group achieved higher scores than the group of clinical pathway-based teaching in the field of how to treat ITP patients with first-line treatment failure ( =0.0003) . In addition,there were 93.8%interns in the experimental group accepted the new clinical pathway-based teaching tool in the survey. The open-ended question survey showed the clinical pathway teaching method was more simple, intuitive and standardizing than the traditional one. Conclusion The ITP clinical pathway, as an interns teaching tool of hematology, is helpful for understanding more clearly the diagnosis and treatment of ITP. However, an in-depth explanation is necessary combination with textbook of medicine study in the clinical teaching of ITP.
7.Relationship between human papillomavirus gene types and cervi-cal diseases

Objective: To investigate the relationship between human papilloma virus (HPV) gene types and cervical diseases. Methods:The subjects included women with HPV who were diagnosed in the Outpatient Department of the Third Affiliated Hospital of Sun Yat-sen University (Guangzhou, China) from March 1, 2010 to January 31, 2011. A total of 21 HPV DNA types were found in the women using DNA flow-though hybridization genotyping technique, and a questionnaire-type survey was conducted on these pa-tients. In this study, 256 cases of normal or inflamed cervix, 34 cases of cervical intraepithelial neoplasia I (CINⅠ), 61 cases of CINⅡor CINⅢ, and 9 cases of cervical cancer were analyzed. Results:The HPV infection rates were 18.4%in the normal cervical or inflam-mation group, 67.6%in the CINⅠgroup, 96.7%in the CINⅡor CINⅢgroup, and 100%in the cervical cancer group. The HPV in-fection rate was higher in the CIN I group than in the normal HPV infection or inflammation group (P<0.001) and was higher in the CIN Ⅱ or CIN Ⅲ groups than in the CIN I group (P<0.001). The main types of infection were HPV16 (44.2%), HPV58 (24.2%), HPV52 (11.6%), and HPV33 (8.4%) in the CIN groups. The main type of infection was HPV16 in the cervical cancer group (88.9%). Conclusion: Cases of cervical lesions are increasing. With this prevalence of HPV, considerable attention should be focused on HPV16-, 58-, 52-, and 33-related infections and cervical lesions. Among them, HPV16 infection is the primary cause of disease progres-sion and carcinogenesis.
8.Clinical value of neoadjuvant chemotherapy in the treatment of local advanced cervical cancer

Objectives To evaluate the clinical value of neoadjuvant chemotherapy (NACT) in the treatment of local advanced cervical cancer. Methods We searched the clinical trials on the treatment of local advanced cervical cancer with NACT followed by surgery versus initial surgery on English and Chinese published literatures from the main medical data resources (MEDLINE, PUBMED, ELSEVIER ScienceDirect, CNKI). The data about positive pelvic nodes, interstitial infiltration, vascular invasion, positive surgical margin, 3-year overall survival (OS), 3-year disease-free survival (DFS), 5-year OS, 5-year DFS were extracted from these papers, and a meta-analysis was applied. Result The hazard ratio (HR) of positive pelvic nodes on NACT group versus control group was 0. 54(95% CI 0. 33 ~0. 86), HR of interstitial infiltration was 0. 45(95% CI 0. 24 ~0. 86), HR of vascular invasion was 0. 25(95% CI 0. 16 ~0. 38), all differences were statistically significant. And HR of positive surgical margin was 0. 45 ( 95% CI 0. 21 ~0. 99), P = 0. 05, which indicated the difference was not statistically significant. And there were also no significant difference on the HR of 3-year OS, 3-year DFS, 5-year OS and 5-year DFS, and their RR were 1.18(95% CI 0. 84 ~ 1.66) ,1.31 (95% CI 0. 96 ~ 1.78) ,0. 89(95% CI 0. 68 ~ 1.15) ,and 0. 99(95% CI 0. 71 ~ 1.93) respectively. Conclusion For local advanced cervical cancer, NACT could reduce pathological risk factors but it did not improve the survival.
9.Evaluation of Different Screening Methods in Diagnosis of Cervical Cancer

Objective To retrospectivly analyze the value of Pap smear,liquid-based cells, colposcopy and colposcopic cervical biopsy in the diagnosis of cervical cancer. Methods 116 cases of cervical cancer were retrospectively analyzed, and the value of Pap smear,liquid-based cells ,colposcopy,and colposcopic cervical biopsy in the diagnosis of cervical cancer were investigated. Result 116 patients were pathologically diagnosed with cervical cancer before and after surgery. Pap smear was performed for 30 patients (25.9%) ,and 10 patients (33.3%) were detected with HSIL. Liquid-based cells was performed for47 patients (40.5%) ,and 25 patients (53.1%) were detected with HSIL. Colposcopy was performed for 99 patients (85.3%), and 75 patients (75.8 % ) were detected with HSIL or invasive cancer. Preoperative colposcopic biopsy was performed for 99 patients, and the pathological coincidence rate was 94.9%. There was no significant difference statistically between two cytology screening ( P > 0.05 ). There was significant difference statistically between liquid-based cells and colposcopy ( P < 0.01 ). There were significant differences statistically among liquid-based cells,colposcopy ,and colposcopic cervical biopsy ( P <0.01 ). Conclusion Cytology, colposcopy, colposcopic cervical biopsy could not be individually more complete and accurate detection of cervical cancer. Colposcopy,colposcopic biopsy than cytology had higher accurate rate of diagnosis of cervical cancer. A reasonable combination of multiple detection methods could improve the diagnostic accuracy of cervical cancer.
10.Inhibitory effects of recombinant human Mullerian inhibiting substance on cell proliferation in two human ovarian carcinoma cell lines

AIM:To investigate the inhibitory effects of recombinant human Mullerian inhibiting substance on cell proliferation in human ovarian carcinoma cells (OVCAR8 and SKOV3 cell lines). METHODS:The expression of MISIIR protein and the localization of MISIIR protein were analyzed by Western blotting and confocal spectral microscopy,respectively. Cell apoptosis and cell cycle were detected by flow cytometry (FCM). Cell viability was determined via MTT method. Clone formation test was used to detect oncogenicity in vitro.RESULTS:The MISIIR protein expression in OVCAR8 cells but not in SKOV3 cells was observed. MISIIR expression was seen on the OVCAR8 cell surface and in the cytoplasm with both antibodies. After treated with rhMIS for 48 h,the cell viability was significantly decreased in OVCAR8 cells. rhMIS inhibited the oncogenicity of OVCAR8 cells greatly. The cell apoptosis of OVCAR8 cell exposed to 10 mg/L rhMIS was (31.3±2.1)%,and OVCAR8 cells in the G_1 phase were increased by (70.4±3.0)%. Compared to SKOV3 cells the differences were significant (P<0.01). CONCLUSION:Recombinant human Mullerian inhibiting substance suppresses the growth of MISIIR-positive ovarian cancer cells by inducing apoptosis and cell cycle arrest. We predict that rhMIS might be a new target to treat human ovarian malignancies.
