2.Clinical observation of virtual reality technology combined with isokinetic strength training for patients after anterior cruciate ligament reconstruction.

OBJECTIVE:
To explore application value and effectiveness of virtual reality technology combined with isokinetic muscle strength training in the rehabilitation of patients after anterior cruciate ligament (ACL) reconstruction surgery.
METHODS:
Forty patients who underwent ACL reconstruction surgery from December 2021 to January 2023 were selected and divided into control group and observation group according to treatment methods, 20 patients in each group. Control group was received routine rehabilitation training combined with isokinetic muscle strength training, including 15 males and 5 females, aged from 17 to 44 years old, with an average of (29.10±8.60) years old. Observation group was performed virtual reality technology combined with isokinetic muscle strength training, including 16 males and 4 females, aged from 17 to 45 years old with an average of (30.95±9.11) years old. Lysholm knee joint score, knee extension peak torque, and knee flexion peak torque between two groups at 12 (before training) and 16 weeks (after training) after surgery were compared.
RESULTS:
All patients were followed up for 1 to 6 months with an average of (3.30±1.42) months. There were no statistically significant difference in Lysholm knee joint score, peak knee extension peak torque, and peak knee flexion peak torque between two groups (P>0.05) before training. After training, Lysholm knee joint score, knee extension peak torque, and knee flexion peak torque of both groups were improved compared to before training (P<0.05);there were significant difference in Lysholm knee joint score, knee extension peak torque, and knee flexion peak torque between two groups(P<0.05).
CONCLUSION
The application of virtual reality technology combined with isokinetic muscle strength training could promote recovery of knee joint function and enhance muscle strength in patients after ACL reconstruction surgery in further.

OBJECTIVES:
To investigate the clinical treatment outcomes and the changes of the outcomes over time in extremely preterm twins in Guangdong Province, China.
METHODS:
A retrospective analysis was performed for 269 pairs of extremely preterm twins with a gestational age of <28 weeks who were admitted to the department of neonatology in 26 grade A tertiary hospitals in Guangdong Province from January 2008 to December 2017. According to the admission time, they were divided into two groups: 2008-2012 and 2013-2017. Besides, each pair of twins was divided into the heavier infant and the lighter infant subgroups according to birth weight. The perinatal data of mothers and hospitalization data of neonates were collected. The survival rate of twins and the incidence rate of complications were compared between the 2008-2012 and 2013-2017 groups.
RESULTS:
Compared with the 2008-2012 group, the 2013-2017 group (both the heavier infant and lighter infant subgroups) had lower incidence rates of severe asphyxia and smaller head circumference at birth (P<0.05). The mortality rates of both of the twins, the heavier infant of the twins, and the lighter infant of the twins were lower in the 2013-2017 group compared with the 2008-2012 group (P<0.05). Compared with the 2008-2012 group, the 2013-2017 group (both the heavier infant and lighter infant subgroups) had lower incidence rates of pulmonary hemorrhage, patent ductus arteriosus (PDA), periventricular-intraventricular hemorrhage (P-IVH), and neonatal respiratory distress syndrome (NRDS) and a higher incidence rate of bronchopulmonary dysplasia (P<0.05).
CONCLUSIONS
There is a significant increase in the survival rate over time in extremely preterm twins with a gestational age of <28 weeks in the 26 grade A tertiary hospitals in Guangdong Province. The incidences of severe asphyxia, pulmonary hemorrhage, PDA, P-IVH, and NRDS decrease in both the heavier and lighter infants of the twins, but the incidence of bronchopulmonary dysplasia increases. With the improvement of diagnosis and treatment, the multidisciplinary collaboration between different fields of fetal medicine including prenatal diagnosis, obstetrics, and neonatology is needed in the future to jointly develop management strategies for twin pregnancy.

4. The current situation and relevant risk factors for disability and dementia in the elderly in urban areas of Jiangsu Province

Objective:
To study the current situation and relevant risk factors for disability and dementia among the elderly in urban areas of Jiangsu province.
Methods:
A stratified cluster sampling method was used to select representative elderly people aged 60 years and over in urban areas in Jiangsu Province, and a face-to-face questionnaire survey was conducted.The activities of daily living(ADL)scale and mini-mental state examination(MMSE)were used to assess the status of disability and dementia.And the relevant risk factors for disability and dementia were analyzed by univariate and multivariate Logistic regression methods.
Results:
A total of 1 600 elderly people were investigated and 1 514 valid questionnaires were recovered.The effective recovery rate was 94.6%.The rates of disability and dementia were 17.9%(271/1 514)and 8.5%(129/1 514)respectively in the elderly in Jiangsu urban areas.Univariate analysis showed that age, marital status, education level, empty-nest, whether or not to suffer from chronic diseases, health status, medication, and whether or not to have dementia were significantly related with the disability in the elderly(

Objective: To investigate the current status and real performance of the detection of RUNX1-RUNX1T1 fusion transcript levels and WT1 transcript levels in China through interlaboratory comparison. Methods: Peking University People's Hospital (PKUPH) prepared the samples for comparison. That is, the fresh RUNX1-RUNX1T1 positive (+) bone morrow nucleated cells were serially diluted with RUNX1-RUNX1T1 negative (-) nucleated cells from different patients. Totally 23 sets with 14 different samples per set were prepared. TRIzol reagent was added in each tube and thoroughly mixed with cells for homogenization. Each laboratory simultaneously tested RUNX1-RUNX1T1 and WT1 transcript levels of one set of samples by real-time quantitative PCR method. All transcript levels were reported as the percentage of RUNX1-RUNX1T1 or WT1 transcript copies/ABL copies. Spearman correlation coefficient between the reported transcript levels of each participated laboratory and those of PKUPH was calculated. Results: ①RUNX1-RUNX1T1 comparison: 9 samples were (+) and 5 were (-) , the false negative and positive rates of the 20 participated laboratories were 0 (0/180) and 5% (5/100) , respectively. The reported transcript levels of all 9 positive samples were different among laboratories. The median reported transcript levels of 9 positive samples were from 0.060% to 176.7%, which covered 3.5-log. The ratios of each sample's highest to the lowest reported transcript levels were from 5.5 to 12.3 (one result which obviously deviated from other laboratories' results was not included) , 85% (17/20) of the laboratories had correlation coefficient ≥0.98. ②WT1 comparison: The median reported transcript levels of all 14 samples were from 0.17% to 67.6%, which covered 2.6-log. The ratios of each sample's highest to the lowest reported transcript levels were from 5.3-13.7, 62% (13/21) of the laboratories had correlation coefficient ≥0.98. ③ The relative relationship of the reported RUNX1-RUNX1T1 transcript levels between the participants and PKUPH was not always consistent with that of WT1 transcript levels. Both RUNX1-RUNX1T1 and WT1 transcript levels from 2 and 7 laboratories were individually lower than and higher than those of PKUPH, whereas for the rest 11 laboratories, one transcript level was higher than and the other was lower than that of PKUPH. Conclusion: The reported RUNX1-RUNX1T1 and WT1 transcript levels were different among laboratories for the same sample. Most of the participated laboratories reported highly consistent result with that of PKUPH. The relationship between laboratories of the different transcript levels may not be the same.

6.Health status and healthcare service needs for the rural elderly population in Northern Jiangsu

Objective To study the health status ,the current health service needs and the influencing factors concerning the rural elderly population in Northern Jiangsu. Methods Multi-stage stratified cluster sampling was conducted in elderly people over 60 in Xuzhou ,Lianyungang ,and Suqian.Participants were face-to-face interviewed with questionnaires.Information on health status and health service needs was collected and analyzed. Results A total of 2655 elderly people were enrolled in this study ,including 1252 males (47.2%)and 1403 (52.8%)females with a mean age of (70.9 ± 6.7)years.Self-assessed health was generally poor ,with the two-week morbidity at 26.6%and the prevalence of chronic diseases at 60.1%;46.5% of the participants regularly took medications ;4.1% of them had physical disability ;8.2% had impaired daily activities ;The two-week hospital visit rate was 29.4%;The one-year hospitalization rate was 18.5%;The rate of non-hospitalization for people in need of hospitalization was 27.6%.Multivariate analysis showed that old age(OR=5.51) ,adequate income(OR=2.29)and short journey to a medical institution (OR=7.90) were associated with high two-week hospital visit rates ;old age(OR = 1.76) ,female gender(OR =1.36) ,empty-nest(OR= 1.37 ) ,family harmony (OR= 1.91 ) ,having chronic diseases (OR= 3.49 ) , and high frequency in taking medications (OR=1.69)were correlated with high hospitalization rates. Conclusions The elderly population in rural areas of Northern Jiangsu shows poor health ,has high needs for health services ,and lacks sufficient service utilization.
7.Effect of Qingguang'an Ⅱ on expression of PAX6, Ngn1 and Ngn2 mRNA of rats with chronic high intraocular pressure

AIM:To remark the effect of Qingguang'an Ⅱ on expression of PAX6, Ngn1, and Ngn2 mRNA of rats with chronic high intraocular pressure.METHODS:Totally 40 male SD rats were randomly divided into 6 groups, that was:A:blank group, B:model group, C:Qingguang'an Ⅱ low dose group, D:Qingguang'an Ⅱ moderate dose group, E:Qingguang'an Ⅱ high dose group, F:Yimaikang disket group.B, C, D, E, F groups of experimental rats were established the model of chronic high intraocular pressure (IOP) by cauterizing of superficial scleral vein.Animal model was established successfully by using monitoring IOP consistently keep above 25mmHg for 8wk as cut-off criterion.Tissues of Eyes were obtained after intragastric administration for 2wk and 4wk.The expressions of PAX6, Ngn1, and Ngn2 mRNA were investigated by Real-time PCR.RESULTS:At the time-point of 2wk, PAX6, Ngn1, and Ngn2 mRNA in group B were statistically expressed in lower level comparing with other groups (P<0.05).Moreover, at the time-point of 4wk, PAX6, Ngn1, and Ngn2 mRNA in group C, D and E were statistically expressed in higher level comparing with group F (P<0.05).Besides, PAX6, Ngn1, and Ngn2 mRNA in group C and D were statistically expressed in lower level comparing with group E (P<0.05).PAX6, Ngn1, and Ngn2 mRNA in group C and D were expressed in similar level(P>0.05).CONCLUSION:In summar, Qingguang'an Ⅱ and Yimaikang disket can remarkably increase the expressions of PAX6, Ngn1, and Ngn2, which suggest protecting the optic nerve of rats caused by chronic high IOP.What's more, this study indicated that, in the protection of optic nerve of rats with chronic high IOP, the high dose of Qingguang'an Ⅱ at the time-point of 4wk was the better choice.
8.Establishment and application of a RT-PCR detection method for bovine viral diarrhea virus

Objective To establish a dual RT-PCR detection method for bovine viral diarrhea virus(BVDV)in bovine-derived samples. Methods The primers were designed and synthesized according to the published BVDV1 and BVDV2 genes containing highly conservative sequences in the 5' untranslated regions(5' UTR)to establish the dual RT-PCR method. The specificity,sensitivity,stability of this method were evaluated. Then 41 bovine-derived samples and 64 bovine plasma samples including bovine calf serum, deproteinized calf serum extract and one lienal polypeptide injection were detected with this method. Results There was no cross reaction with bovine parainfluenza virus type 3(BPIV3), classical swine fever virus(CSFV)and Japanese encephalitis virus(JEV)when samples were detected with the established dual RT-PCR method. The lowest concentration of template DNA for detection of BVDV1 and BVDV2 was 8.87 × 102copies and 6.31 × 102copies per microliter,respectively. Electrophoresis bands of about 151 bp and 303 bp were still amplified and detected after the BVDV1 and BVDV2 cDNA was stored at -30℃ for 12 months. The BVDV positive rate of 41 bovine-derived samples and 64 bovine plasma samples detected with this dual RT-PCR method was 14.6% and 29.7%, respectively. Conclusions The established dual RT-PCR method has the advantages of high efficiency, specificity,sensitivity and stability,and can be used for the detection of BVDV in bovine-derived samples.
9.Detection of periodontal pathogens from saliva of type 2 diabetic patients in urban area of Beijing

Objective To investigate the prevalence of periodontal pathogens from saliva of patients with type 2 diabetes mellitus (T2DM),and to characterize the association between the glucose status and periodontal pathogens in oral cavity.Methods All the subjects were hypertension patients under regular care at Beijing hypertension prevention and management institute.Whole unstimulated saliva samples were collected from 45 non-diabetic subjects (non-DM group),80 well-controlled diabetic patients (DM-well group) and 100 poor-controlled diabetic patients (DM-poor group).DNA was extracted from the salivary deposition,Porphyromonas gingivalis (Pg),Tannerella forsythia (Tf) and Treponema denticola (Td) were detected by polymerase chain reaction (PCR) method based on 16SrRNA.Prevalence and quantity of the pathogens under different glucose states were compared and logistic regression model was set to analyze the factors related to each bacterium.Results The prevalence of Tf in DM-well group and DM-poor group was significantly lower than that of non-DM group [81% (65/80),80% (80/100) vs 91% (41/45),P =0.048],meanwhile the quantity of Tf was also lower than that of non-DM group[1.9 (2.6),2.1(5.3) vs 3.4 (6.4)] (P > 0.05).With the worsening of glucose control,the quantity of Tf was declining (P =0.032).However,the prevalence and the quantity of Pg,Td in 3 groups had no statistical differences(P >0.05).After adjusting age,gender,number of missing teeth and other periodontal parameters,OR of having Tf in saliva from DM-well group and DM-poor group was 0.58 and 0.53,respectively.Conclusions Abnormal blood glucose state may affect the colonization of Tf in oral cavity.
10.Deepgoing study on intrathoracic tuberculous lymphadenitis in adults using multidetector CT.

BACKGROUNDStudies on intrathoracic tuberculous lymphadenitis in adults are confined to the preliminary CT findings with ordinary CT and ordinary spiral CT. There has been no deepgoing study of multidetector CT to date. Multidetector CT could contribute to better imaging of intrathoracic tuberculous lymphadenitis in adults. The purpose of this study was to explore the multidetector CT features of intrathoracic tuberculous lymphadenitis in adults, and the correlation with clinical symptoms and pathologic changes.
METHODSMultidetector CT findings from 42 consecutive adult patients with intrathoracic tuberculous lymphadenitis were analyzed retrospectively with regard to locations, sizes, numbers, shapes, margins, and densities reviewing precontrast and enhanced images. CT results were correlated with clinical symptoms and pathologic results (n = 37).
RESULTSOne hundred and eighty-five intrathoracic lymph nodes that had tuberculous lymphadenitis in 42 patients were distributed mainly in regions 4R (n = 37), 2R (n = 33), 7 (n = 31) and 10R (n = 21), more than 2 regions were implicated in 34 patients. One hundred and twenty-two (72.2%) of the tuberculous lymphadenitis without confluence were oval or round with clear margins. On precontrast scanning, 78.4% of tuberculous lymphadenitis had a homogeneous density. Seven enhancement patterns were demonstrated in 169 tuberculous lymphadenitis from 37 patients with pathologic results: homogeneous enhancement with no clinical symptom (n = 12), corresponded pathologically to tuberculous hyperplasia without caseous necrosis; heterogeneous enhancement with a small central no enhancement area, slight clinical symptoms (n = 22), tuberculous granulomas with a little caseous necroses; peripheral irregular thick wall enhancement with a central area with no enhancement, slight clinical symptoms (n = 52), tuberculous granulomas with some caseous necroses in the center; peripheral thin rim enhancement with a central area having no enhancement, moderate clinical symptoms (n = 36), a few tuberculous granulomas with a great quantity of caseous necroses in the center; peripheral irregular enhancement without central enhancement, extending outside the capsule, severe clinical symptoms (n = 4), caseous necroses ruptured from capsule; peripheral irregular rim enhancement with central separate enhancement, severe clinical symptoms (n = 40), multiple lymph nodes with liquefaction of caseous necroses were adherent and confluent, rim and separation were tuberculous granulomas; no obvious enhancement, severe clinical symptoms (n = 3). Caseous necrosis was usually associated with little tuberculous granulomas.
CONCLUSIONSThe main multidetector CT features of intrathoracic tuberculous lymphadenitis in adults are involvement of multiregional lymph nodes with oval or round shape and clear margins, a basically homogeneous density on precontrast scanning, multiple enhancement patterns, and they correlate closely with clinical symptoms. Multidetector CT could reveal pathological changes of intrathoracic tuberculous lymphadenitis in adults.
