1.Clinical Value of Translocator Protein Gene in Evaluating the Efficacy of FLT3-ITD/DNMT3A R882 Double-Mutated Acute Myeloid Leukemia.

		                        			OBJECTIVE:
		                        			To observe the clinical significance of translocator proteins (TSPO) gene in the treatment of FLT3-ITD/DNMT3A R882 double-mutated acute myeloid leukemia (AML).
		                        		
		                        			METHODS:
		                        			Seventy-six patients with AML hospitalized in the Department of Hematology of the Affiliated People's Hospital of Ningbo University from June 2018 to June 2020 were selected, including 34 patients with FLT3-ITD mutation, 27 patients with DNMT3A R882 mutation, 15 patients with FLT3-ITD/DNMT3A R882 double mutation, as well as 19 patients with immune thrombocytopenia (ITP) hospitalized during the same period as control group. RNA was routinely extracted from 3 ml bone marrow retained during bone puncture, and TSPO gene expression was detected by transcriptome sequencing (using 2-deltadeltaCt calculation).
		                        		
		                        			RESULTS:
		                        			The expression of TSPO gene in FLT3-ITD group and DNMT3A R882 group at first diagnosis was 2.02±1.04 and 1.85±0.76, respectively, which were both higher than 1.00±0.06 in control group, but the differences were not statistically significant (P=0.671, P=0.821). The expression of TSPO gene in the FLT3-ITD/DNMT3A R882 group was 3.98±1.07, wich was significantly higher than that in the FLT3-ITD group and DNMT3A R882 group, the differences were statistically significant (P=0.032, P=0.021). The expression of TSPO gene in patients who achieved complete response after chemotherapy in the FLT3-ITD/DNMT3A R882 group was 1.19±0.87, which was significantly lower than that at first diagnosis, and the difference was statistically significant (P=0.011).
		                        		
		                        			CONCLUSION
		                        			TSPO gene may be used as an indicator of efficacy in FLT3-ITD /DNMT3A R882 double-mutated AML.

2.Abnormal cortical surface-based spontaneous and functional connectivity in the whole brain in lifelong premature ejaculation patients.

		                        			Recent research has highlighted structural and functional abnormalities in the cerebral cortex of patients with premature ejaculation (PE). These anomalies could play a pivotal role in the physiological mechanisms underlying PE. This study leveraged functional magnetic resonance imaging (fMRI), a noninvasive technique, to explore these neural mechanisms. We conducted resting-state fMRI scans on 36 PE patients and 22 healthy controls (HC), and collected data on Premature Ejaculation Diagnostic Tool (PEDT) scores and intravaginal ejaculation latency time (IELT). Employing a surface-based regional homogeneity (ReHo) approach, we analyzed local neural synchronous spontaneous activity, diverging from previous studies that utilized a volume-based ReHo method. Areas with significant ReHo differences between PE and HC groups underwent surface-based functional connectivity (FC) analysis. Significant discrepancies in ReHo and FC across the cortical surface were observed in the PE cohort. Notably, PE patients exhibited decreased ReHo in the left triangular inferior frontal gyrus and enhanced ReHo in the right middle frontal gyrus. The latter showed heightened connectivity with the left lingual gyrus and the right orbital superior frontal gyrus. Furthermore, a correlation between ReHo and FC values with PEDT scores and IELT was found in the PE group. Our findings, derived from surface-based fMRI data, underscore specific brain regions linked to the neurobiological underpinnings of PE.

3.Effect of Suanzaoren Tang on Learning, Memory and Expression of Proteins Related to Hippocampal Synaptic Plasticity in PCPA-induced Insomnia Rat Model

		                        			ObjectiveTo observe the effects of Suanzaoren Tang on the behavior, growth-associated protein-43 (GAP-43), postsynaptic density protein-95 (PSD-95), and synaptophysin Ⅰ (SynⅠ) of insomniac rats induced by p-chlorophenylalanine (PCPA), and to investigate the mechanism of Suanzaoren Tang in improving the behavior of the insomniac rats. MethodSeventy-two SD rats were randomly assigned into 6 groups (12 rats in each group): control group (normal saline), PCPA (0.35 g·kg-1·d-1) group, estazolam (2.7×10-4 g·kg-1·d-1) group, and low-, medium-, and high-dose (3.25, 7.5, 15 g·kg-1·d-1, respectively) Suanzaoren Tang groups. The rat model of insomnia was established by intraperitoneal injection of PCPA and then the rats were administrated with corresponding drugs for 7 continuous days. The Morris water maze and Y-maze were used to test the learning and memory functions, and the open field to test anxiety. Histopathological changes in the hippocampus were observed via hematoxylin-eosin (HE) staining. The mRNA and protein levels of GAP-43, PSD-95, and SynⅠ in hippocampus were determined by real-time fluorescence quantitative polymerase chain reaction (Real-time PCR), Western blot, and immunohistochemistry (IHC). ResultCompared with the control group, the PCPA group showcased long escape latency, shortened time in the quadrants, and decreased times of crossing the platform in Morris water maze, decreased alternation correct rate was significantly Y-maze, and increased distance, mean velocity, and time in center of the open field test (P<0.01). Furthermore, the PCPA-treated rats showed obvious pathological damage in the hippocampus and down-regulated mRNA and protein levels of GAP-43, PSD-95, and SynⅠ in hippocampus (P<0.01). Compared with the PCPA group, the treatments with estazolam and different doses of Suanzaoren Tang improved the rat performance in Morris water maze, Y-maze, and open field test (P<0.05, P<0.01), alleviated the hippocampal damage, and up-regulated the mRNA and protein levels of GAP-43, PSD-95, and SynⅠ (P<0.05, P<0.01). ConclusionSuanzaoren Tang may alleviate the learning and memory disorders and anxiety in PCPA-induced insomnia rat model by up-regulating the mRNA and protein levels of hippocampal synaptic plasticity-associated proteins GAP-43, PSD-95, and SynⅠ. 
		                        		
		                        		
		                        		
		                        	
4.New eudesmane sesquiterpenoids from Atractylodis Macrocephalae Rhizoma and their inhibitory activities against SREBPs.

		                        			Three sesquiterpenoids were isolated and purified from the 95% ethanol extract of Atractylodis Macrocephalae Rhizoma by column chromatography on silica gel, Sephadex LH-20, ODS, and high-performance liquid chromatography(HPLC). Their chemical structures were identified on the basis of spectroscopic analysis and physiochemical properties as(7Z)-8β,13-diacetoxy-eudesma-4(15),7(11)-diene(1), 7-oxo-7,8-secoeudesma-4(15),11-dien-8-oic acid(2), and guai-10(14)-en-11-ol(3). Compounds 1 and 2 are new compounds and compound 3 was obtained from Compositae family for the first time. 5.Ultrasound-Guided Continuous Thoracic Paravertebral Block Improves Patient's Quality of Recovery After Open Hepatectomy: A Randomized, Double-Blind, Placebo-Controlled Trial.

		                        			Background Ultrasound-guided continuous thoracic paravertebral block can provide pain-relieving and opioid-sparing effects in patients receiving open hepatectomy. We hypothesize that these effects may improve the quality of recovery (QoR) after open hepatectomy. Methods Seventy-six patients undergoing open hepatectomy were randomized to receive a continuous thoracic paravertebral block with ropivacaine (CTPVB group) or normal saline (control group). All patients received patient-controlled intravenous analgesia with morphine postoperatively for 48 hours. The primary outcome was the global Chinese 15-item Quality of Recovery score on postoperative day 7, which was statistically analyzed using Student's t-test. Results Thirty-six patients in the CTPVB group and 37 in the control group completed the study. Compared to the control group, the CTPVB group had significantly increased global Chinese 15-item Quality of Recovery scores (133.14 ± 12.97 vs. 122.62 ± 14.89, P = 0.002) on postoperative day 7. Postoperative pain scores and cumulative morphine consumption were significantly lower for up to 8 and 48 hours (P < 0.05; P = 0.002), respectively, in the CTPVB group. Conclusion Perioperative CTPVB markably promotes patient's QoR after open hepatectomy with a profound analgesic effect in the early postoperative period.

6.Single center experience of transcatheter aortic valve replacement with a simplified operative protocol.

		                        			Objective: To summarize the single center experience of transcatheter aortic valve replacement (TAVR) with a simplified operative protocol. Methods: Consecutive patients who underwent transfemoral TAVR (TF-TAVR) from July 2020 to December 2020 in Fuwai Hospital were retrospectively analyzed. We compared the baseline characteristic, procedure information, 30-day follow-up outcomes of the patients who underwent TF-TAVR without the simplified operative protocol (routine group) or with the simplified operative protocol (simplified protocol group). Results: 93 patients were collected, 42 patients belonging to routine group, 51 patients belonging to simplified protocol group. In simplified protocol group, there were 51 patients planned to use ultrasound-guided femoral access puncture, procedure was successful in all 51 patients (100%). There were 49 patients planned to use the radial artery as the secondary access, procedure was successful in 45 patients (92%). There were 48 patients planned to use the strategy of avoidance of urinary catheter, this strategy was achieved in 35 patients (73%). There were 12 patients planned to use the left ventricular guidewire to pace, procedure was successful in 11 patients (92%). There were no differences in baseline characteristics, major clinical endpoints and 30-day follow-up outcomes between the two groups. Meanwhile, the procedure time ((62.5±17.9)min vs. (78.3±16.7)min, P<0.001), operation room time ((133.7±25.1)min vs. (159.2±42.6)min, P<0.001), X-ray exposure time ((17.2±6.5)min vs. (20.2±7.7)min, P=0.027) were significantly shorten in simplified protocol group compared with the routine group. Conclusion: Our study results indicate that the simplified operative protocol of TF-TAVR is as effective and safe as the routine operative protocol, meanwhile using the simplified operative protocol can significantly increase the operative efficiency of TF-TAVR.

		                        			OBJECTIVE:
		                        			To explore the efficacy difference of electroacupuncture at lower -sea point and -sea matching front- points for the treatment of gastroparesis.
		                        		
		                        			METHODS:
		                        			A total of 63 patients with gastroparesis were randomly divided into a lower point group (group A, 32 cases, 2 cases dropped off) and a matching points group (group B, 31 cases, 1 case dropped off). The group A was treated with electroacupuncture at Zusanli (ST 36), and the group B was treated with electroacupuncture at Zusanli (ST 36) and Zhongwan (CV 12). Both groups were treated with continuous wave (2 Hz in frequency) for 30 min, once a day, 5 times a week for 3 weeks. The gastroparesis cardinal symptom index (GCSI) score, gastric half-emptying time (T) and the 180 min gastric residual rate of the two groups before and after treatment were observed, and the clinical effective rate was compared.
		                        		
		                        			RESULTS:
		                        			After treatment, the total GCSI scores, T and the 180 min gastric residual rates in both groups were lower than those before treatment (<0.01), and the 180 min gastric residual rate and T in the group A were lower than those in the group B (<0.05). The total effective rate was 93.3% (28/30) in the group A, which was superior to 70.0% (21/30) in the group B (<0.05).
		                        		
		                        			CONCLUSION
		                        			Electroacupuncture at lower -sea point and -sea matching front- points can both be used to treat gastroparesis, but electroacupuncture at Zusanli (ST 36) has a better effect. 9.Associations of body mass index, waist circumference, waist height ratio and hypertension in the elderly

		                        			 Objective To explore the associations of body mass index (BMI), waist circumference (WC), waist height ratio (WHtR) and the prevalence of hypertension in elderly residents over 60 years in Baodi district, Tianjin. Methods Residents over 60 who underwent medical examinations in the Koudong Health Center, Baodi district, Tianjin, were all invited to participate in the study from April to May, 2018. Participants were asked to fill out structured questionnaires and undergo physical examinations. Stratified analysis and logistic regression analysis were applied to examine joint effects and interactions of BMI and WC (or WHtR) on the risk of hypertension. Results A total of 1 417 residents (83.75%) out of 1 692 residents participated in the study. The prevalence of hypertension in the participants was 46.36%. 66.50% of the participants were BMI overweight or obese. Participants with central obesity accounted for 74.66% (measured by the WC) and 75.38% (by the WHtR). Compared to the normal weight measured by the BMI or the WC, BMI overweight (OR=1.65, 95%CI: 1.19-2.30) or obesity (OR=3.41, 95%CI: 2.23-5.20) and WC central obesity (OR: 1.49, 95%CI: 1.00-2.23) were associated with increased risk of hypertension. The joint effects of BMI and WC (OR=2.49, 95%CI: 1.78-3.46), or BMI and WHtR (WHtR overweight: OR=2.05, 95%CI: 1.41-2.99; WHtR obesity: OR=2.37, 95%CI: 1.50-3.76) were greater than the single effect of the latter (WC overweight/obesity: OR=1.39, 95%CI: 0.90-2.15; WHtR overweight: OR=1.02, 95%CI: 0.62-1.66; WHtR obesity: OR=1.44, 95%CI:0.55-3.81). Conclusions  Of the three indices, BMI is strongly correlated with the risk of hypertension. BMI overweight or obesity has enhanced the association of WC or WHtR and the risk of hypertension, suggesting that weight control in the normal range, especially measured by the BMI index, may prevent and control hypertension. 
		                        		
		                        		
		                        		
		                        	
10. Effect of Panax Notoginseng Saponins on EMT of Rat Renal Proximal Tubular Epithelial Cells Induced by TGF-β1

		                        			 Objective:To investigate the intervention effect of panax notoginseng saponins (PNS) on epithelial-mesenchymal transition (EMT) of rat renal tubular epithelial cells (NRK-52E) induced by transforming growth factor-β1(TGF-β1), and analyze the mechanism based on the silent information regulation 2 homolog 1(SIRT1)/TGF-β1/Smad signaling pathway. Method:NRK-52E were cultured in DMEM medium with 10% fetal bovine serum, and divided into normal control group, TGF-β1 group (5 μg·L-1), resveratrol (RSV) group (50 mg·L-1), EX527 group (10 μmol·L-1), Panax notoginseng saponins (PNS) group (100 mg·L-1), and EX527+ PNS group (10 μmol·L-1+100 mg·L-1). Then cells were collected after drug intervention for 48 h. The expressions of α-SMA,E-cadherin,SIRT1,TGF-β1,Smad3,Smad4 mRNA in each group were detected by Real-time PCR. The protein expressions of α-SMA, E-cadherin,SIRT1 and TGF-β1 were detected by Western blot. Result:Compared with normal group, mRNA and protein expressions of α-SMA increased obviously(P<0.05), but E-cadherin decreased significantly(P<0.01)in TGF-β1 group. Compared with TGF-β1 group, mRNA and protein expressions of α-SMA decreased significantly(P<0.01), while E-cadherin increased(P<0.01)in resveratrol and PNS groups, and EMT was inhibited. Meanwhile, mRNA and protein expressions of SIRT1 increased significantly(P<0.01), while mRNA expressions of TGF-β1,Smad3,and Smad4 decreased(P<0.01). Under the intervention of SIRT1 blocker EX527, PNS could not play a significant inhibitory effect on the cells. Conclusion:PNS can prevent the occurrence of EMT of renal tubular epithelial cells induced by TGF-β1, and the mechanism may be related to active SIRT1 to inhibit TGF-β1/Smad pathway.
