1.Effects of air pressure, humidity, wind and sunshine on the incidence of cardiovascular and cerebrovascular diseases in Guiyang

Objective To explore the effects of air pressure, humidity, wind, and sunshine on the incidence of cardiovascular and cerebrovascular diseases (CVD) in Guiyang, and to provide reference for the prevention of CVD. Methods Using CVD incidence data from September 2021 to August 2022 in Guiyang City and meteorological data including average air pressure, average humidity, wind, and sunshine during the same period, the effects of meteorological factors on CVD incidence were explored and the importance of each factor was analyzed. Results When air pressure was below 868 hPa, above 887 hPa, or between 877 and 883 hPa, and when air pressure dropped less than 5.3 hPa within 24 hours, there was a higher risk of CVD. When the humidity was above 81%, the wind speed was small (<1.2 m/s) or high (>4m/s), and there was less sunlight (less than 3 hours), the risk of CVD was higher. Low humidity (<60%) was not conducive to the onset of CVD. There were highest risks at lag 5~10 days and 4-25 days for high pressure and low sunlight, respectively. When the relative humidity was saturated, there was an immediate effect. When the wind speed was low and high, the immediate effect and hysteresis effects were significant. Among the above meteorological factors, the impact of 24-hour variation of pressure and high or low atmospheric pressure on the incidence of CVD was the most significant, while the impact of sunlight and humidity was the weakest. The impact of diurnal variations in wind and atmospheric pressure was not clear. Conclusion The impact of air pressure on the incidence of CVD does not exhibit a simple linear relationship. The risk of CVD is high in high humidity, low light, and moderate or strong winds. It is necessary to fully consider changes in meteorological factors for CVD prevention and control.
2.Association of menopausal time and menopausal age with nonalcoholic fatty liver disease: An analysis based on a restricted cubic spline model

ObjectiveTo investigate the association of menopausal time and menopausal age with the risk of nonalcoholic fatty liver disease (NAFLD), and to provide a basis for the early prevention and treatment of NAFLD in clinical practice. MethodsRelated data were collected from 373 postmenopausal women who attended the outpatient service of Department of Spleen, Stomach, Liver and Gallbladder Diseases, The First Affiliated Hospital of Henan University of Chinese Medicine, from January 2017 to December 2021, including general information, menopausal age, menopausal time, and presence or absence of NAFLD. The chi-square test was used for comparison of categorical data; the independent-samples t test was used for comparison of normally distributed continuous data between groups, and the Wilcoxon rank-sum test was used for comparison of non-normally distributed continuous data between groups. A Logistic regression analysis was used to calculate the association intensity and 95% confidence interval (95%CI) of menopausal time and menopausal age for the risk of NAFLD, and the restricted cubic spline (RCS) method was used to investigate the dose-response relationship between menopausal time/age and the risk of NAFLD. ResultsCompared with the women with normal menopause or late menopause, the women with early menopause had a higher prevalence rate of NAFLD and a higher degree of steatosis and fibrosis (all P<0.05). After adjustment for the confounding factors such as age and age of menarche, the risk of NAFLD in women with a menopausal time of >3 years was 4.80 (95%CI: 1.93 — 11.95, P=0.001) times that in women with a menopausal time of ≤3 years, and the risk of NAFLD in women with early or late menopause was 8.14 times (95%CI: 1.77 — 37.58, P=0.007) and 0.09 times (95%CI: 0.03 — 0.32, P<0.001), respectively, that in those with a normal menopausal age. There is a dose-response relationship between menopausal time/age and the risk of NAFLD. Menopausal time is positively correlated with the association intensity of NAFLD, while menopausal age is negatively correlated with the association intensity of NAFLD. ConclusionThe longer the menopause time and the earlier the menopause age, the ligher the risk of NAFLD.
3.Risk factors for liver cancer in 504 patients with hepatitis B virus associated cirrhosis logistic regression analysis

Objective Logistic regression model was used to analyze the risk factors of liver cancer in patients with hepatitis B virus-related cirrhosis. Methods A retrospective analysis was performed on 504 patients with hepatitis B cirrhosis who were treated in a hospital from April 2021 to April 2024. The occurrence of liver cancer was counted. The risk factors of liver cancer in patients with HBV-related cirrhosis were analyzed by logistic regression analysis. Results Among the 504 patients with hepatitis B cirrhosis, 101 patients developed liver cancer and 403 patients did not develop liver cancer, which were included in the liver cancer group (n=101) and the non-liver cancer group (n=403).. Among hepatitis B cirrhosis, the incidence rate of liver cancer was 20.04%. Compared with the non-liver cancer group, the proportion of patients with long-term drinking history, family history of liver cancer, history of diabetes mellitus, antiviral therapy, and HBV-DNA load>104 were higher in the liver cancer group (P<0.05). logistic regression analysis found that long-term drinking history (OR=3.077, 95%CI: 1.130-8.378, P=0.028), history of diabetes mellitus (OR=3.747, 95%CI: 1.765-7.954, P=0.001), no antiviral therapy (OR=3.466, 95%CI: 1.337-8.985, P=0.011) and HBV-DNA load>104 (OR=3.149, 95%CI: 1.353-7.328, P=0.008) could independently affect the occurrence of liver cancer in patients with hepatitis B cirrhosis. Conclusion According to logistic regression analysis, long-term drinking history, history of diabetes mellitus, no antiviral therapy, and HBV-DNA load>104 are risk factors for liver cancer in patients with HBV-related cirrhosis.
4.Preventive suggestions and development trajectories of symptom clusters in 286 patients with acute pancreatitis

Objective To explore the occurrence and development trajectories of symptoms at different time points in patients with acute pancreatitis (AP), and to analyze the influencing factors and preventive measures of development trajectories of AP symptom clusters. Methods A convenient sampling method was used to select AP who were admitted from January 2023 to December 2023 were selected and included in the study. The symptoms at different time points were recorded. The severities of symptom clusters in AP patients were explored, and the development trajectories of main symptom clusters were analyzed. Univariate and multivariate logistic regression analyses were used to analyze the influencing factors of development trajectories of symptom clusters in AP patients. Results The incidence rates of abdominal pain, dry mouth, abdominal distension and lack of energy were higher in AP patients during hospitalization. The incidence rates of lack of energy, anxiety, abdominal pain and sleep disturbance were higher on the 1st month after discharge. The incidence rates of abdominal distension, abdominal pain, sleep disturbance and anxiety were higher on the 3rd month after discharge. The incidence rates of anxiety, abdominal pain and irritability were higher on the 6th month after discharge. The fatigue symptom cluster, psychological symptom cluster and gastrointestinal symptom cluster were extracted during hospitalization and on the 1st month and the 3rd month after discharge, and the psychological symptom cluster and gastrointestinal symptom cluster were extracted on the 6th month. The severity scores of symptom clusters at each time point were statistically different (P<0.05). The development of gastrointestinal symptom cluster in AP patients was mainly low decline. The development of psychological symptom cluster was mainly high decline. Drinking history and diabetes mellitus were the influencing factors of development trajectory of gastrointestinal symptom cluster in AP patients (P<0.05). High disease severity, drinking history and biliary tract disease were the influencing factors of development trajectory of psychological symptom cluster in AP patients (P<0.05). Conclusion The symptom clusters of AP patients changes over time, with digestive, fatigue, and psychological symptoms being the main groups in the early stage, and psychological and digestive symptoms persisting in the later stage. Early identification and intervention are crucial for improving the prognosis of AP patients.

6.Three-dimensional finite element analysis of distal proximal occlusal defect of short crown molar restored with endocrown

BACKGROUND:For teeth with normal dental crown height,pulp cavity retention crown restoration with different depths of the pulp cavity and different repair materials affects the stress and flexural strength of tooth tissue.For short crown molar defects,the research on pulp cavity repair mainly focuses on clinical observation and in vitro flexural strength experiments. OBJECTIVE:To establish a three-dimensional finite element model for short crown molar restored by the endocrown after root canal treatment to analyze the effects of different pulp cavity retention depths and different repair materials on the distribution and size of dentin equivalent stress. METHODS:Based on establishing the complete model of the short crown mandible first molar,a three-dimensional finite element model was established for repairing the distal adjacent defect of the short crown molar with different pulp cavity retention depths(h=2,3,4 mm)and different repair materials(zirconia,lithium disilicate).Under the oblique loading,the equivalent stress distribution was observed.The peak value of dentin equivalent stress and the mean value of equivalent stress near the bottom of the mesial pulp cavity wall were calculated. RESULTS AND CONCLUSION:(1)Equivalent stress concentration areas:The stress of complete short crown molar and restored models mainly concentrated in the mesial root mesial neck and mesial root lingual neck.The stress concentration area was found in the mesial pulp cavity wall corresponding to the bottom layer of restored models,and the stress concentration was obvious in the 4 mm retention depth group.(2)Under the same repair material,the peak value of dentin equivalent stress was the lowest at 3 mm for all models after repair.The average value of equivalent stress near the bottom of the mesial pulp cavity wall was lowest at 3 mm.(3)Under the same retention depth,there was no significant difference between the two materials in the dentin equivalent stress peak and the mean value near the bottom of the mesial pulp cavity.(4)The results showed that under the conditions of this experiment,the endocrown was used to repair the defect of the short crown molar and the retention depth was 3 mm,which was more beneficial to protect the remaining dental tissue.The selection of zirconia or lithium disilicate as the repair material had little effect on the dentin stress.
7.Predicting the risk of cardiovascular and cerebrovascular diseases based on meteorological factors and the analysis of its correlation with incidence rates

Objective Based on meteorological factors, the prediction of the risk of cardiovascular and cerebrovascular diseases(CVD) and the analysis of its correlation with the incidence rate. Methods The research utilizes six years of data on CVD incidence from Xingyi, from 2017 to 2022, as the subject of study. The incidence risk levels are categorized based on the 25%, 50%, 75%, and 95% quantiles of the cumulative probability of the number of cases. The study is conducted based on the relationship between meteorological factors and the risk of incidence of CVD. Results The incidence of CVD shows a significant correlation with temperature, vapor pressure, atmospheric pressure, and relative humidity. It is negatively correlated with temperature and vapor pressure, among which the correlation with the daily minimum temperature is the highest at -0.504 (P < 0.05), and positively correlated with atmospheric pressure and relative humidity. Meteorological factors that have a significant correlation with the incidence rate are selected as input factors for the machine prediction model. It was found that the random forest model performs best in predicting the risk of incidence of CVD, with a comprehensive score of 0.851. Analysis of Relative Risk (RR) values found that there is a lagged association between exposure levels to meteorological factors and the incidence of cardiovascular and cerebrovascular diseases. A temperature drop of more than 11°C and an increase in atmospheric pressure of more than 8 hPa can significantly increase the risk of incidence. Conclusion The study revealed significant correlations between the incidence of CVD and meteorological parameters including temperature, water vapor pressure, atmospheric pressure, and relative humidity. Utilizing machine learning models, the research effectively predicted the risk of these diseases, uncovering that extreme weather conditions significantly elevate the risk of incidence. These findings provide a basis for meteorological risk assessment in public health interventions, emphasizing the importance of taking preventative measures in the context of extreme climate changes.
8.Efficacy and Safety of Combined Bedaquiline and Delamanid Use among Patients with Multidrug-Resistant Tuberculosis in Beijing,China

Objectives The combined use of bedaquiline and delamanid(BDQ-DLM)is limited by an increased risk of prolonging the QTc interval.We retrospectively evaluated patients who received DLM/BDQ-containing regimens at a TB-specialized hospital.We aimed to present clinical efficacy and safety data for Chinese patients. Methods This case-control study included patients with multidrug-resistant tuberculosis(MDR-TB)treated with BDQ alone or BDQ plus DLM. Results A total of 96 patients were included in this analysis:64 in the BDQ group and 32 in the BDQ+DLM group.Among the 96 patients with positive sputum culture at the initiation of BDQ alone or BDQ combined with DLM,46 patients(71.9%)in the BDQ group and 29(90.6%)in the BDQ-DLM group achieved sputum culture conversion during treatment.The rate of sputum culture conversion did not differ between the two groups.The time to sputum culture conversion was significantly shorter in the BDQ-DLM group than in the BDQ group.The most frequent adverse event was QTc interval prolongation;however,the frequency of adverse events did not differ between the groups. Conclusion In conclusion,our results demonstrate that the combined use of BDQ and DLM is efficacious and tolerable in Chinese patients infected with MDR-TB.Patients in the BDQ-DLM group achieved sputum culture conversion sooner than those in the BDQ group.
9.Exploration on the Intervention Effect and Mechanism of Guiqi Yiyuan Ointment Combined with Cisplatin on Lewis Lung Cancer Mice Based on lncRNA H19/miR-19b3p/FTH1 Axis

Objective To explore the inhibitory effect of Guiqi Yiyuan Ointment combined with cisplatin on Lewis lung cancer mice and its mechanism based on the lncRNA H19/miR-19b3p/FTH1 axis.Methods Totally 60 SPF grade male C57BL/6 mice were randomly selected as blank group,and Lewis lung cancer mouse model was replicated in the remaining 50 mice.The modeled mice were randomly divided into model group,cisplatin group,and TCM low-,medium-and high-dosage combined with cisplatin group,with 10 mice in each group.The cisplatin group was given intraperitoneal injection of cisplatin 5 mg/kg,while the TCM low-,medium-and high-dosage combined with cisplatin group were given Guiqi Yiyuan Ointment 1.75,3.5,7.0 g/kg by gavage combined with intraperitoneal injection of cisplatin 5 mg/kg,the blank group and model group were given equal volume of normal saline by gavage for 14 consecutive days.The general condition of mice was observed,the tumor mass of mice was measured,and the tumor inhibition rate,spleen index,and thymus index were calculated,HE staining was used to observe the pathological changes of tumor tissue,and the contents of malondialdehyde(MDA),reduced glutathione(GSH),hydrogen peroxide(H2O2),and ferrous ions(Fe2+)were detected in the tumor tissue,immunohistochemistry and Western blot were used to detect the expression of nuclear factor E2 related factor 2(Nrf2),glutathione peroxidase 4(GPX4),ferritin heavy chain 1(FTH1)and solute carrier family 7 member 11(SLC7A11)protein in tumor tissue,real time fluorescence quantitative PCR was used to detect the expression of lncRNA H19 and miR-19b3p mRNA in tumor tissue.Results Compared with the blank group,the body mass,spleen index and thymus index of mice in the model group were decreased(P<0.05).Compared with the model group,the mental state and tumor histopathological morphology of mice in each administration group were improved to different degrees,the tumor mass of mice was reduced(P<0.05),the spleen index and the thymus index of mice increased(P<0.05),tumor cell lysis,rupture,and decrease in quantity,the contents of MDA,H2O2,Fe2+in tumor tissue increased(P<0.05),while GSH content decreased(P<0.05),the expression of Nrf2,GPX4,FTH1,SLC7A11 protein in tumor tissue decreased(P<0.05),the lncRNA H19 mRNA expression decreased(P<0.05),miR-19b3p mRNA expression increased(P<0.05).Compared with the cisplatin group,the tumor mass of mice in TCM high-dosage combined with cisplatin group reduced(P<0.05),the body mass,spleen index and thymus index increased(P<0.05),the contents of MDA,H2O2,Fe2+in tumor tissue of mice increased(P<0.05),the content of GSH decreased(P<0.05),the expression of Nrf2,FTH1,SLC7A11,GPX4 protein expression decreased(P<0.05),lncRNA H19 mRNA expression decreased(P<0.05),miR-19b3p mRNA expression increased(P<0.05).Conclusion Guiqi Yiyuan Ointment combined with cisplatin can significantly inhibit the tumor of Lewis lung cancer mice,and its mechanism may be related to regulating the expression of ferroptosis molecules related to the lncRNA H19/miR-19b3p/FTH1 axis.
10.Bibliometric analysis of studies on prone position ventilation in critically ill patients

Objective:To analyze the current status, hotspots, and development trends of prone position ventilation in critically ill patients.Methods:The literature on prone position ventilation in critically ill patients was electronically retrieved in the Web of Science core collection and China National Knowledge Infrastructure, with a search period from January 1, 1998 to July 1, 2023. CiteSpace 6.2.R4 was used to analyze articles.Results:A total of 585 articles on prone position ventilation in critically ill patients were included, consisting of 162 in Chinese and 423 in English. In recent years, the number of research articles on prone position ventilation for critically ill patients, both domestically and internationally, showed a progressive upward trend. Foreign research hotspots included acute respiratory distress syndrome, pressure injury, infants, critically ill patients, electrical impedance imaging, epidemiology, enteral nutrition, volume responsiveness, and pulmonary recruitment. Domestic research hotspots involved mechanical ventilation, respiration, pressure injury, mortality, and respiratory failure.Conclusions:Research on prone position ventilation in critically ill patients is receiving increasing attention. Chinese researchers can further explore based on previous research and strengthen communication and cooperation across fields and institutions.
