Ophthalmic drug-device combination products are a new method of ophthalmic disease treatment,which is characterized by high bioavailability,strong targeting and good compliance.However,it is difficult for products to be developed and regulated due to the complexity of the human eye structure,drug-device interactions,and other factors.To provide a basis for guaranteeing the safety and efficacy of products development and management,the related regulations,current research,and evaluation of the quality of products are summarized in this paper.
2.Influence of artificial liver support system therapy on platelet in treatment of hepatitis B virus-related acute-on-chronic liver failure

Objective To investigate the changing trend of platelet count (PLT) and related influencing factors in patients with hepatitis B virus-related chronic-on-acute liver failure (HBV-ACLF) after artificial liver support system (ALSS) therapy. Methods A total of 152 patients with HBV-ACLF who were hospitalized and treated in The Third Affiliated Hospital of Sun Yat-Sen University from January 2018 to November 2021 were included in the study, among whom 102 patients received plasma exchange (PE) and 50 patients received double plasma molecular absorption system combined with low-dose PE, and their clinical data and laboratory marker were measured. The independent samples t -test or the Mann-Whitney U test was used for the comparison of continuous data between two groups, and the chi-square test was used for the comparison of categorical data between two groups; a multivariate logistic regression analysis was used to investigate the risk factors for PLT > 50×10 9 /L after ALSS therapy; the receiver operating characteristic (ROC) curve was used to investigate the value of baseline PLT in predicting PLT > 50×10 9 /L after ALSS therapy. Results The patients were mostly middle-aged male adults; among the 152 patients, 70 (46.1%) had liver cirrhosis on admission, 114 (75.0%) received three sessions of ALSS therapy, and 88% had a baseline PLT count of > 50×10 9 /L. There was a significant reduction in PLT from baseline to after ALSS therapy (79.5±47.7 vs 112.5±64.1, t =4.965, P < 0.001), and at 1 week after treatment, PLT increased to the baseline level (97.2±50.7 vs 112.5±64.1, t =1.787, P =0.075). As for the change in PLT from baseline to 1 week after ALSS therapy, the liver cirrhosis group had a significantly greater reduction in PLT than the non-liver cirrhosis group ( U =1986.5, P =0.026), while there was no significant difference between different procedures of ALSS therapy and different sessions of treatment (3-5 sessions) (all P > 0.05). The multivariate logistic regression analysis showed that cirrhosis (odds ratio [ OR ]=3.097, 95% confidence interval [ CI ]: 1.255-7.645, P =0.014) and PLT > 50×10 9 /L at baseline ( OR =0.019, 95% CI : 0.002-0.154, P < 0.001) were independent risk factors for PLT > 50×10 9 /L after ALSS therapy. The ROC curve analysis of baseline PLT showed that PLT > 80.5×10 9 /L at baseline was the optimal cut-off value affecting PLT > 50×10 9 /L after treatment, with an area under the ROC curve of 0.818. Conclusion The influence of ALSS therapy on PLT is temporary, but cirrhotic patients have a weaker PLT generation ability than non-cirrhotic patients. PLT > 80.5×10 9 /L at baseline is the optimal cut-off value to reduce the risk of bleeding after ALSS therapy.
3.Qualitative research on discharge preparation needs of parents of children in Neonatal Intensive Care Unit

Objective:To understand the discharge preparation and health education needs of parents of children in Neonatal Intensive Care Unit (NICU) , and to provide the basis for taking scientific and effective health guidance programs.Methods:Using the purposive sampling method, a total of 13 parents of newborns who were hospitalized in NICU of a municipal maternal and child health hospital in Zhejiang Province from May to July 2020 were selected as the research objects. A descriptive research method was used to conduct semi-structured interviews with parents of newborns, and the data was analyzed using the Colaizzi 7-step method.Results:Three themes and five sub-themes were extracted, namely psychological state, knowledge needs (including two sub-themes of knowledge identification needs and disease-related knowledge needs) and seeking multi-party support (including three sub-themes of medical support, social support and peer support) .Conclusions:Parents have more needs during the transition period after NICU children are discharged. As medical professionals, they need to provide psychological support, professional health guidance and establish multiple ways of care and support, in order to promote the smooth transition of NICU children to home care.
4.Risk factors for readmission in patients with chronic heart failure: a Meta-analysis

Objective:To explore the relevant risk factors for readmission of patients with chronic heart failure by Meta-analysis.Methods:We searched the PubMed, Cochrane Library, Web of Science, Embase, SinoMed, China National Knowledge Infrastructure (CNKI) , Wanfang Data and VIP Database by computer, and the search time limit was from the establishment of the database to December 31, 2019. RevMan 5.3 software was used for Meta-analysis.Results:A total of 19 articles were included, including 185 545 patients and 34 related risk factors.Statistically significant risk factors included depression [ OR=3.87, 95% CI (2.87-5.23) , P<0.01], living alone [ OR=1.12, 95% CI (1.01-1.24) , P=0.03], combined with hyperlipidemia and diabetes [ OR=1.16, 95% CI (1.14-1.19) , P<0.01], high-salt diet [ OR=3.36, 95% CI (2.60-4.35) , P<0.01], decreased renal function [ OR=2.00, 95% CI (1.95-2.04) , P<0.01], heart failure with reduced ejection fraction [ OR=2.71, 95% CI (1.93-3.73) , P<0.01], anemia [ OR=1.43, 95% CI (1.27-1.60) , P<0.01]and infection [ OR=1.12, 95% CI (1.00-1.25) , P=0.04]. Conclusions:Depression, living alone, combined with hyperlipidemia and diabetes, high-salt diet, decreased renal function, heart failure with reduced ejection fraction, anemia and infection are related risk factors for readmission in patients with chronic heart failure.

6.Effects of ADRB2(rs1042713)Gene Polymorphism on Therapeutic Efficacy of Anticholinergic Drugs in the Treatment of Refractory Asthma Pediatric Patients

OBJECTIVE: To study the effects of ADRB2 (rs1042713) gene polymorphism on therapeutic efficacy of anticholinergic drug in the treatment for refractory asthma pediatric patients. METHODS: 171 children with refractory asthma were selected from outpatient department of Kunming Children’s Hospital during Nov. 2016 to Jul. 2019. The distribution of ADRB2 (rs1042713) genotype, the clinical efficacy [asthma control test (C-ACT) score, FEV1, FVC, PEF, maximal mid-expiratory flow (MMEF)] of anticholinergic drug were analyzed statistically; the response of different genotypes to the use of anticholinergic drug were also analyzed statistically. RESULTS: 148 of 171 refractory asthmatics pediatric patients were administered anticholinergic drug, among them 50 of the 71 AA genotype and 36 of the 77 GA genotype responded to anticholinergic drug treatment. Statistical analysis showed that 71 children with AA refractory asthma had improved C-ACT score, FEV1, FVC, PEF and MMEF, there was statistical significance, compared with GA genotype (P<0.05); the response rate of the AA genotype to anticholinergic drugs was 2.71 times that of the GA genotype [OR=2.71, 95%CI (1.38, 5.34), P=0.005]. CONCLUSIONS: The detection of ADRB2 (rs1042713) gene polymorphism has some guiding significance in the treatment of refractory asthma with anticholinergic drugs, and the response of AA genotype is better.
7. Comparison of two different anesthesia methods for the procedure for prolapse and hemorrhoids in elderly patients with anorectal diseases

Objective:
To compare the anesthetic effects of subarachnoid anesthesia(SA)and epidural anesthesia(EA)for the procedure for prolapse and hemorrhoids(PPH)in elderly patients with benign anorectal diseases.
Methods:
A retrospective analysis was conducted in 60 patients with benign anorectal diseases admitted to Peking University Third Hospital from March to August 2018 and undergoing PPH.According to anesthesia methods, patients were divided into the SA group and the EA group (n=30 each). The level of sensory block was tested by acupuncture, and the degree of motor block was assessed by revised Bromage score.The anesthesia effect, operation time, blood loss, adverse events and hospitalization time were compared between the two groups.

8.A bibliometric analysis of heart failure symptom clusters

Objective? To investigate and analyze the domestic and oversea studies on heart failure symptom clusters through a bibliometric analysis, and provide evidence basis for future clinical nursing research. Methods? The CNKI the Wanfang Science and Technology Journal Full-text Database, the China Biomedical Literature Database (CBM), the PubMed, and the Web of Science database were searched for the articles both in Chinese and English about heart failure symptom clusters published from database establishment dates to August 31st, 2018 and analyzed on their published years, distribution of journals, and research content. Results? A total of 21 articles complied with the standard were retrieved with the first published article dated back to 2009. Among them, there were 13 cross-sectional studies, 4 longitudinal studies, 3 reviews, and 1 tool development. Although different studies had differences in research design, sample characteristics, assessment tools, and analytical methods, the main findings were basically the same: somatic symptom cluster and emotional symptom cluster were common symptom clusters in patients with heart failure; somatic symptom cluster mainly included shortness of breath, dyspnea, fatigue, declined physical strength, dyscoimesis; emotional symptom cluster mainly included anxiety, depression, cognitive disorder; there was still controversy about the classification of edema. Conclusions? At present, the research on heart failure symptom clusters in China and abroad is still at the initial exploration stage, and the research results are still unstable. In the future, we can further develop long-term dynamic observation of heart failure symptom clusters and explore corresponding symptom management strategies.
9.Efficacy of heated humidified high-flow nasal cannula as initial treatment for preterm infants with respiratory distress: a meta-analysis

Objective To investigate the efficacy of heated humidified high-flow nasal cannula (HHHFNC) as the primary means of respiratory support for preterm infants with mild or moderate respiratory distress syndrome (RDS).Methods Randomized controlled trials (RCTs) about HHHFNC and/or nasal continuous positive airway pressure (nCPAP) in preterm infants with RDS were searched in PubMed,EMBASE,Cochrane library,Chinese Journal Full-text Database (CJFD),Wanfang Data base,VIP Database and China Biology Medicine disc (CBM).Meta-analysis was conducted with Review Manager 5.2 software to compare HHHFNC and CPAP groups in the outcomes ofpreterm infants with RDS,which included initial support failure,nasal trauma,pre-discharge mortality and incidences of bronchopulmonary dysplasia (BPD) and patent ductus arteriosus (PDA) before discharge.Results Eight randomized trials,including 1 400 preterm infants with RDS were included.Results of the meta-analysis demonstrated that no significant difference in the percentage of initial support failure was observed between the two groups (RR=1.22,95%CI:0.94-1.59,P=0.13),and HHHFNC was associated with decreased risk for nasal trauma (RR=0.35,95%CI:0.24-0.52,P<0.001).There was no significant difference in the incidence of death,BPD,PDA or gas leaking before discharge between the two groups.Conclusions HHHFNC is an efficient respiratory support for preterm infants (gestational age over 28 weeks) with mild or moderate RDS.
10.Clinical research of low-dose rituximab therapy and sequential maintenance for patients with refractory idiopathic thrombocytopenic purpura

Objective To investigate the efficacy and safety of low-dose rituximab therapy and sequential maintenance for patients with refractory idiopathic thrombocytopenic purpura. Methods Thirty-three patients with refractory idiopathic thrombocytopenic purpura received intravenous rituximab at the dose of 100 mg once a week for 4 consecutive weeks. Complete blood cell count and serum concentrations of immunoglobulin (IgG,IgM and IgA) were monitored regularly. The numbers of CD3+ and CD19+ CD20+ lymphocyte cells were assayed by flow cytometry before and after therapy. Twenty-five patients with responses(complete response and response) were divided into maintained group (12 patients) and control group (13 patients) by random digits table method. The patients in maintained group were treated with rituximab 100 mg every 6 months. The efficacy of maintenance therapy was evaluated through long-term follow-up. Results The complete response(CR) rate, response (R) rate and no response(NR) rate were 48.48%(16/33), 27.27%(9/33) and 24.24% (8/33), respectively. As a result, total effective rate was 75.76% (25/33). There were no significant changes of peripheral blood white blood cell count,hemoglobin,serum immunoglobulin and CD3+lymphocyte counts before and after treatment (P>0.05). However, CD19+ CD20+ cells were almost depleted in patients treated with rituximab: (3.71±2.64)×106/L vs. (279.33±92.78)×106/L, P<0.01. Five patients suffered from allergic response, and 1 patient developed pneumonia and respiratory failure. The relapse rates of maintained group and control group were 1/12 and 4/13, respectively. Conclusions Treatment with low-dose rituximab may be an effective and safe approach in patients with idiopathic thrombocytopenic purpura. Relapse rates can be decreased through maintenance therapy with refractory low-dose rituximab. However, the optimal therapeutic schedule need further investigation.
