1.Long-term Outcomes of Endoscopic Radiofrequency Ablation versus Endoscopic Submucosal Dissection for Widespread Superficial Esophageal Squamous Cell Neoplasia

Background/Aims:
Endoscopic radiofrequency ablation (ERFA) is a treatment option for superficial esophageal squamous cell neoplasia (ESCN), with a relatively low risk of stenosis; however, the long-term outcomes remain unclear. We aimed to compare the long-term outcomes of patients with widespread superficial ESCN who underwent endoscopic submucosal dissection (ESD) or ERFA.
Methods:
We retrospectively analyzed the clinical data of patients with superficial ESCN who underwent ESD or ERFA between January 2015 and December 2021. The primary outcome measure was recurrence-free survival.
Results:
Ninety-two and 33 patients with superficial ESCN underwent ESD and ERFA, respectively. The en bloc, R0, and curative resection rates for ESD were 100.0%, 90.2%, and 76.1%, respectively. At 12 months, the complete response rate was comparable between the two groups (94.6% vs 90.9%, p=0.748). During a median follow-up of 66 months, recurrence-free survival was significantly longer in the ESD group than in the ERFA group (p=0.004), while no significant differences in overall survival (p=0.845) and disease-specific survival (p=0.494) were observed.Preoperative diagnosis of intramucosal cancer (adjusted hazard ratio, 5.55; vs high-grade intraepithelial neoplasia) was an independent predictor of recurrence. Significantly fewer patients in the ERFA group experienced stenosis compare to ESD group (15.2% vs 38.0%, p=0.016).
Conclusions
The risk of recurrence was higher for ERFA than ESD for ESCN but overall survival was not affected. The risk of esophageal stenosis was significantly lower for patients who underwent ERFA.

5.Evaluation of perioperative anxiety state and analysis of influencing factors in burn patients

Objective To evaluate the perioperative anxiety state and analyze the influencing factors of burned patients.Methods A total of 110 burned patients undergoing selective surgery under general anesthesia were included in the Fourth Medical Center of Chinese PLA General Hospital from February to August 2022.All patients were evaluated with self-rating anxiety scale(SAS),visual analogue scale-anxiety(VAS-a),visual analogue scale-pain(VAS-p),mini-mental state examination(MMSE),and Ramsay sedation score 1-day before and after operation.The patients'parameters were recorded including mean arterial pressure(MAP)and heart rate(HR)at admission(T0),before anesthesia induction(T1),2 min after intubation(T2),15 min after surgery(T3),during surgery(T4),at surgery end(T5),and immediately after leaving the operating room(T6).The occurrence and the influencing factors of perioperative anxiety in burn patients were analyzed using the univariate and multivariate logistic regression.Results The incidence of preoperative and postoperative anxiety in burn patients was 29.1%and 22.3%respectively.Univariate logistic analysis showed that gender(P=0.002),burn time(P=0.046),burn area(P=0.005),burn site(P=0.035),and degree of preoperative pain(P=0.001)were related with preoperative anxiety status in burn patients;while burn time(P=0.030),burn area(P=0.001),burn site(P=0.016),degree of preoperative pain(P=0.021),and preoperative anxiety status(P<0.001)were related with postoperative anxiety state in burn patients.Multivariate logistic regression analysis showed that gender and degree of preoperative pain were the independent influencing factors of preoperative anxiety status in burn patients(P=0.002,0.022),and preoperative anxiety status was the independent influencing factor of postoperative anxiety status in burn patients(P<0.001).Compared with the preoperative non-anxious patients(n=73),preoperative anxious patients(n=30)showed no significant difference in MAP at each time point(P>0.05),but HR was accelerated(P<0.05),and the dosage of sufentanil,remifentanil and propofol increased significantly during the operation(P<0.05).Conclusions The anxiety state of burn patients was significantly higher before operation than that after operation,and their consumption of anesthetic drugs during operation was higher,and there was no significant correlation with the type and number of operation.Gender,degree of preoperative pain and anxiety state were the independent influencing factors of perioperative anxiety state in burn patients.Early intervention against relevant factors will help patients recover quickly.
6.The inhibitory effect of GSDME on tumor progression and its underlying mechanisms in mice

This study aims to explore the role of mouse GSDME in tumor progression and its mechanisms.Wide type and GSDME knockout mice were used to establish tumor models and the tumor growth was monitored;CRISPR-Cas9 was employed to knockout GSDME in MC38 cells.Bone marrow-derived macrophage and dendritic cells were culture in vitro and adoptively transferred into tumor-bearing mice,respectively.Western blotting was conducted to detect protein expression in cells;flow cytometry was used to analysis the immune cell number,CD8+T cell function,and Cleaved Caspase-3 expression in TAM and TADC cells.According to the above experiments,we found that GSDME deficiency promoted tumor progression in mice.GSDME knockout reduced the number of CD8+T cells and decreased IFN-γ and Perforin in CD8+T cells in tumors.GSDME was mainly expressed in macrophages and dendritic cells,and adoptive transferring GSDME deficient macrophages or dendritic cells did not affect tumor progression.However,GSDME deficient mice reduced the ratio of Cleaved Caspase-3 positive macrophages and dendritic cells in tumors,and Cleaved Caspase-3 positive macrophages expressed more iNOS and MHCII,while Cleaved Caspase-3 positive dendritic cells expressed more CD80 and CD86.In general,GSDME increases Cleaved Caspase-3 positive macrophages and dendritic cells that promote CD8+T cell function,thereby inhibiting tumor progression.
7.Determination of Flucloxacillin Concentration in Newborn Plasma by HPLC-MS/MS and the Study on the Interaction with Ambroxol

Abstract
OBJECTIVE To establish an efficient and simple HPLC-MS/MS method for determination of flucloxacillin in newborn plasma, and to investigate the interaction between ambroxol and flucloxacillin in newborns. METHODS The samples were analyzed by API4000 HPLC-MS/MS. Ultimate XB-C18 column(2.1 mm×100 mm, 5 μm) were carried out. The mobile phase was composed of water-0.1% formic acid(A) and acetonitrile-0.1% formic acid(B). The quantitative analysis of the ion transitions were monitored at m/z 452.6→284.2 for flucloxacillin and m/z 821.4→397.3 for rifampicin(internal standard). RESULTS The linear range of flucloxacillin under this analysis method was 0.20-80 ng·mL-1, and the lower limit of quantification was 0.20 ng·mL-1; The intra-day and inter-day precision of flucloxacillin were both less than 8.23%; The extraction recovery was in the range of 85.3%-89.2%, and the matrix effect was between 89.3%-92.3%; The stability of plasma samples was good under conditions of 12 h at room temperature, 4 h at room temperature after treatment, repeated freeze-thaw for 3 times, and -20 ℃ freezing for 30 d. The results of clinical samples indicated that the combination of ambroxol could significantly increase the blood concentration of flucloxacillin. CONCLUSION The established HPLC-MS/MS method is accurate, sensitive and can be used for the determination of flucloxacillin concentration in neonatal plasma. The results of clinical samples indicate that ambroxol can significantly increase the blood concentration of flucloxacillin. There are drug interactions between ambroxol and flucloxacillin.
8. Study on the effect of Zizyphus jujube kernel freeze-dried powder on sleep awakening state and energy metabolism rate in rats

Objective To explore the effeet of Zizyphus jujubekernelfreeze - dried powder on sleep and wakefulness and energy metabolism rate in rats under 24 h eontinuous darkness and its mechanism. Method U- sing the eortieal eleetroeneephalography method and the experimental animal monitoring system (CLAMS) and taking the time of wakefulness and sleep as well as the number of activities, food intake, and caloric pro- duetion as indicators to explore the funetion of it on improving energy metabolism rate. Theenzyme - linked immunoassay was used to determine the changes in the eontent of LEP, POMC, and NPY involved in energy regulation in the hypothalamus, so as to study the energy change mechanism of it. Result Compared with the DD group, the MAT group had redueed daytime awakening time, increased sleep time, deereased aetivities, no signifieant differenee in food intake, and inereased caloric production. There was no significant difference in night awakening and sleep time, no significant difference in the number of activities, reduced food intake , and increased caloric production. The weight gain of rats decreased; Compared with the DD group, the LEP content in the hypothalamus of the MAT group increased, the POMC content increased and the NPY content decreased. Conclusion By increasing LEP, POMC content and decreasing NPY content in hypothalamus , Zizyphus jujubekemelfreeze - dried powder collected the disturbance of sleep and wakefulness state and abnormal energy metabolism rate of rats caused by 24 h continuous dark condition.
9. Effect of state anxiety on sleep-wake cycle in mice

Aim To observe the effect of state anxiety on the sleep-wake cycle of mice in the Triple test.Methods Eighteen male ICR mice were selected, and the electroencephalogram(EEG)and electromyogram(EMG)electrodes were embedded in the cortex and neck muscles of the mice under the conditions of anesthesia.There was a continuous 24 h tracing of mice under normal conditions after the recovery period.The model replication and behavioral evaluation of the state anxiety took place in the Triple test and the mouse sleep for 24 h was recorded for 7 consecutive days after the daily test.Results The behavioral evaluation showed that compared with 1st d, the time spent in central and% time in OF on 3 ?4 ?5 ?7 d of modeling was significantly reduced(P<0.05; P<0.01); the number of entries and% time of 4-7 d in EPM signifi-cantly increased(P<0.01); 2nd and 4th day of time spent in dark compartment of LDB increased significantly(P<0.05; P<0.01).The results of EEG analysis showed that compared with the sleep-wake cycle under normal conditions, the sleep and wake cycles had undergone significant changes, and the awakening time and its proportion were significantly prolonged(P<0.05; P<0.01), NREM sleep and its proportion markedly decreased on the 1-5 and 7 d(P<0.05; P<0.01), and a significant increase occurred in REM sleep and its proportion on 1st, 2nd, and 4th day(P<0.05).Conclusion The sleep-wake cycle of state anxiety mice replicated with the Triple test changes, and the impact on daytime sleep is more obvious.
10.A new sesquiterpene from Chloranthus henryi.

Eight sesquiterpenes were isolated and purified from the ethanol extract of Chloranthus henryi by column chromatographies over silica gel, ODS and Sephadex LH-20,and preparative HPLC. Their chemical structures were established by spectral data and physiochemical properties as(1S,6S,8S,10R)-8-ethoxy-10-methoxychlomultin C(1),tianmushanol(2),multistalide A(3),myrrhterpenoid N(4),1α,9α-dihydroxy-8,12-expoxy-eudesma-4,7,11-trien-6-one(5),4β,10α-aromadendranediol(6),oplopanone(7),10α-hydroxycadinan-4-en-3-one(8). Among them, compound(1) was a new compound, and compounds 2-8 were isolated from Chloranthus henryi for the first time.
