1.Hip joint biomechanical analysis of the acetabular anatomical reconstruction and nonanatomical reconstruction in to-tal hip arthroplasty for Crowe type Ⅲ developmental dysplasia of the hip by finite element method

Objective To analyze the hip joint biomechanies of the acetabular anatomical reconstruction and nonanatomi-cal reconstruction in total hip arthroplasty(THA)for Crowe type Ⅲ developmental dysplasia of the hip(DDH)by finite ele-ment method,which provided theoretical foundation and experimental basis for the anatomical acetabular reconstruction dur-ing THA in clinical practice.Methods One patient with left end-stage hip arthritis secondary to Crowe type Ⅲ DDH was se-lected in this study,who underwent total hip arthroplasty in the orthopedic department of the First Affiliated Hospital of Bengbu Medical College in April 2020.This patient was female,57 years old.The preoperative and postoperative three dimentional CT scan of the patient's pelvis were performed.Fourteen acetabular cup models with different anteversion,inclination and rotation center height were established in Mimics and 3-Matic software.The boundary and load conditions were set in Abaqus software.The Von Mises and stress distribution of the hip joint were calculated and observed.Results In the Crowe type Ⅲ DDH THA,if the hip rotation center was restored anatomically and the acetabular cup's inclination was set as 40°,the cup's anteversion var-ied from 5° to 25°,the lowest Von Mises value of acetabular cup and polyethylene liner occured in 20°anteversioin;if the hip rotation center was restored anatomically and the acetabular cup's anteversion was set as 15°,the cup's inclination varied from 35° to 55°,the lowest Von Mises value of acetabular cup and polyethylene liner occured in 35° inclination;if the acetabular cup's anteversion and inclination were set as 15°and 40°respectively,the up migration of hip rotaion center varied from 0 mm to 20 mm,the lowest Von Mises value of acetabular cup and polyethylene liner occured in 10 mm up migration.In all fourteen models,the Von Mises value of the acetabulum,acetabulum cup and polyethylene liner were lowest when the acetabular cup's anteversion and inlcination were 15°,35° respectively,as well as the rotation center was restored anatomically.Conclusion In total hip arthroplasty for Crowe type Ⅲ DDH,the anatomical restoration of hip rotation center with 15° anteversion and 35° in-clination of the acetabular cup are suggested,bone graft above the acetabular cup and additional screws are recommended si-multaneously to further reduce the Von Mises of hip joint.
2. Regulation of transient receptor potential channel protein 5 in inflammation in diabetic cardiomyopathy

Aim To investigate the effect of TRPC5 gene on the inflammation of cliabetie cardiomyopathy.Methods The biological functions of TRPC5 and the correlation between TRPC5 gene and other genes were analyzed by bioinformatics.Studies were performed in TRPC5 knockout ( TRPC5 ) and C57 mice.Mice were randomly divided into blank control and T2DM model groups, and the model was established by intraperitoneal injection of STZ (n = 10).The myocardial injury was detected by HE and Masson staining.Hie level of serum IL-1(3, IL-2, IL-6, IFN-7 and creatine kinase was examined by ELISA.Gene and protein expressions of IL-1(3, IL-2, IL-6 and TRPC5 were analysed by RT-PCR and Western blot respectively.Results By constructing the PPI network and analyses.the TRPC5 gene was identified to internet with a variety inflammatory genes and involved in immunity.The result of pathologieal section showed less myocardial damage and infiltrated immune cells in TRPC5 mice than in C57BL/6J mice.RT-PCR and serum results showed a lower expression of inflammatory factors in myocardium and serum obtained from TRPC5 model mice than in those obtained from C57BL/6J model mice.Conclusions TRPC5 participates in the development of dilated cardiomyopathy by regulating cardio- myocyte inflammation.
3.Clinical efficacy comparison of three methods for correction of moderate and severe upper eyelid skin laxity

Objective:To compare and analyze the clinical appplication and therapeutic effect of applying three methods for correction of moderate and severe upper eyelid skin laxity.Methods:From January 2017 to March 2019, retrospective analysis was performed to evaluate the therapeutic effect of applying three methods by Department of Plastic Surgery at Xijing Hospital of Air Force Medical University to correct moderate and severe upper eyelid skin laxity, such as subbrow blepharoplasty(SBB), double eyelid surgery(DES) and combination of subbrow blepharoplasty and double eyelid surgery(CSD). Patients were divided into the SBB group, DES group, and CSD group according to the different surgical methods, inclusion and exclusion criteria. The 6 month postoperative improvement of upper eyelid skin laxity, including marginal reflex distance 1(MRD1), medial corneal margin-fold distance (MCMFD), middle pupil margin-fold distance (MPMFD), lateral canthus margin-fold distance (LCMFD), the improvement of upper eyelid wrinkles and patient and surgeon visual analog scores (VAS) were compared. Measurement data were expressed as mean ± standard deviation, comparison between groups was performed by ANOVA, pairwise comparison between groups was performed by LSD- t test, P<0.05 was considered statistically significant. Results:Ninety female patients were included in this study. Thirty patients in each group, patients were between 35 and 62 years old. There was no significant difference in gender, age, and degree of upper eyelid skin laxity among the three groups ( P>0.05). All patients were followed up for 6 to 24 months. Upper eyelid skin laxity and upper eyelid wrinkles were obviously improved in all cases after operation. With the exception of 2 cases of vomiting and 1 case of scalp numbness of CSD group. The improvement of MRD1 in SBB group, DES group and CSD group was (0.14±0.09) mm, (0.34±0.11) mm, (0.43±0.15) mm, showing significant difference between three groups ( F=34.537, P <0.001). The differences between the SBB group and the DES group, the DES group and the CSD group, and the SBB group and the CSD group were statistically significant ( t=6.418, 2.824, 9.236, P <0.001, 0.008, <0.001). The improvement of MCMFD in SBB group, DES group and CSD group was (0.32±0.15) mm, (0.92±0.21) mm, (0.97±0.24) mm, showing significant difference between three groups ( F=94.082, P <0.001). The differences between the SBB group and the DES group, the SBB group and the CSD group were statistically significant ( t=11.273, 12.404, P <0.001), and there was no significant difference between the DES group and the CSD group ( t=1.132, P=0.261). The improvement of MPMFD in SBB group, DES group and CSD group was (0.34±0.13) mm, (1.07±0.24) mm, (1.37±0.23) mm, showing significant difference between three groups ( F=193.935, P<0.001). The differences between the SBB group and the DES group, the DES group and the CSD group, and the SBB group and the CSD group were statistically significant ( t=13.531, 5.628, 19.159, P <0.001). The improvement of LCMFD in SBB group, DES group and CSD group was (0.54±0.17) mm, (1.58±0.37) mm, (1.97±0.48) mm, showing significant difference between three groups ( F=121.405, P <0.001). The differences between the SBB group and the DES group, the DES group and the CSD group, and the SBB group and the CSD group were statistically significant ( t=10.971, 4.097, 15.068, P <0.001). The improvement of upper eyelid wrinkles in SBB group, DES group and CSD group was 0.70±0.47, 0.50±0.51, 1.20±0.48, showing significant difference between three groups ( F=16.471, P <0.001). There was no significant difference between the SBB group and the DES group ( t=1.592, P=0.115), while the differences between the DES group and the CSD group, and the SBB group and the CSD group were statistically significant ( t=5.571, 3.979, P <0.001). The VAS patients score in SBB group, DES group and CSD group was 2.77±0.57, 2.17±0.38, 3.90±0.31, showing significant difference between three groups ( F=124.575, P<0.001). The differences between the SBB group and the DES group, the DES group and the CSD group, and the SBB group and the CSD group were statistically significant ( t=5.379, 15.541, 10.161, P <0.001). The VAS surgeons score was 2.80±0.61, 2.27±0.58, 4.07±0.45, showing significant difference between three groups ( F=84.085, P<0.001). The differences between the SBB group and the DES group, the DES group and the CSD group, and the SBB group and the CSD group were statistically significant ( t=3.740, 12.624, 8.883, P <0.001). Conclusions:Compared with subbrow blepharoplasty and double eyelid surgery, the combination of subbrow blepharoplasty and double eyelid surgery is an effective way to excise upper eyelid skin and reduce wrinkles, which has the best results in promoting upper eyelid rejuvenation. It should be considered as optimal method for correction of moderate and severe upper eyelid skin laxity.

6.Expression of the NMDA receptor in the rat model of orchialgia.

Objective:
To explore the expression of the N-methyl-D-aspartate (NMDA) receptor in the rat model of orchialgia and its possible mechanisms.
METHODS:
According to Yoshioka's method, the male rats in the control group were injected with 0.2 ml saline, and those in the experimental group with 0.2 ml 2% acetic acid solution. Then we tested the behavioral responses of the rats and determined the expressions of the subunits NR1 and NR2B of the NMDA receptor in the dorsal root ganglion and spinal dorsal horn by Western blot, RT-qPCR and immunofluorescence staining.
RESULTS:
The withdrawal latency was decreased in the model rats, reaching the lowest value at 4 hours after modeling, significantly lower than in the controls ([4.15 ± 0.84] vs [12.32 ± 1.05], P < 0.05). Compared with the controls, the model rats showed remarkably increased mRNA and protein expressions of NR2B in the dorsal root ganglion (P < 0.05) but not in the spinal dorsal horn at 4 hours. However, no statistically significant difference was found in the expression of NR1 either in the dorsal root ganglion or in the spinal dorsal horn between the two groups (P > 0.05).
CONCLUSIONS
The NMDA receptor plays an important role in pathogenesis of orchialgia in rats. In the early stage of pain, upregulating the expression of the subunit NR2B of the NMDA receptor can mediate peripheral hyperalgesia and consequently orchialgia.
7.Prognosis of clonal chromosomal abnormalities in Philadelphia negative metaphases cells in chronic myeloid leukemia with tyrosine kinase inhibitor therapy.

Objective: To investigate the characteristics and prognosis of clonal chromosomal abnormalities appearing in Philadelphia negative metaphases (CCA/Ph(-)) cells in chronic myeloid leukemia (CML) with tyrosine kinase inhibitor (TKI) therapy. Methods: The clinical data of 30 cases with CCA/Ph(-) during TKI treatment in Henan Cancer Hospital from August 2007 to July 2017 were retrospectively analyzed. The univariate factor was analyzed by Kaplan-Meier method. Multiple-factor was analyzed by Cox proportional risk model. Results: Of the 30 cases, 19 (63.3%) were males. At the first detection of CCA/Ph(-) the median age was 44 (rang 14-68) years old and the median treatment of TKI was 13 (rang 2-94) months. The clones proportion of first detected CCA/Ph(-)≥ 50% was found in 18 (60.0%) cases. TKI treatment for 3 months with BCR-ABL(IS) less than 10% was seen in 14 (46.7%) patients. 63.3% (19/30) of CCA/Ph(-) was transient (only one time) and 36.7% (11/30) was repeated (≥2 times) . Trisomy 8 dominant accounted for 60.0% (18/30) , -7/7q- for 13.3% (4/30) , loss of chromosome Y 6.7%. With a median of follow-up 50 months, 76.7% (23/30) cases were in complete cytogenetic response (CCyR) ; 63.3% (19/30) in major molecular response (MMR) , 43.3% (13/30) in undetectable minimal residual disease (UMRD) . The median event-free survival rate of (EFS) were 44 months, and 2-year and 5-year EFS were (82.1±7.3) % and (52.4±12.8) %, respectively. The median overall survival (OS) were 50 months, and 2-year and 5-year OS rates were (92.6±5.0) % and (77.2±14.7) %, respectively. Univariate analysis shows that the 2-year EFS of who in males, more than 2 times CCA/Ph(-), BCR-ABL(IS)>10% at 3 months after TKI were significantly lower than women, transient CCA/Ph(-), and BCR-ABL(IS)≤10% (P<0.05) . The 2-year OS rate in whom the occurrence frequency of CCA/Ph(-) more than twice was significantly lower than those with transient CCA/Ph(-) (P<0.05) . Multivariate analysis showed that CCA/Ph(-) was an independent risk factor (RR=4.741, 95%CI 1.21-18.571, P=0.018) for EFS in CML patients. Conclusion: Trisomy 8, -7/7q-, and -Y were the most common CCA/Ph(-) during TKI treatment, with high clones proportion of ≥50%. CCA/Ph(-) mainly occurred transiently or was permanent occasionally. CCA/Ph(-) recurrence (≥2 times) was an independent risk factor for EFS and OS in CML with TKI.

10.Effect of nilotinib on pregnancy outcome in female patients with chronic myeloid leukemia.

Objective: To observe the pregnancy outcome among patients with chronic myeloid leukemia (CML) treated with Nilotinib (NIL) . Methods: Clinical data of pregnancy delivery in CML patients treated with NIL from March 2015 to January 2019 were retrospectively collected. Results: A total of 11 patients were recruited with median pregnancy age 28 (25-40) years. The median duration of NIL treatment before pregnancy was 34 (3-48) months. There were 12 pregnancies, included 2 planned ones and 10 (83.3%) unplanned. In the 10 unplanned patients, 9 (90.0%) received NIL 600 mg/d. The median exposure time were 4 (4-7) weeks. In eight patients with delivery outcomes, 5 cases had well-developed babies, 2 had spontaneous abortion and 1 case with an baby of syndactyly deformity, whose mother was exposed to NIL 600 mg/d for 7 weeks in the early trimester of pregnancy. Seven infants were 4 boys and 3 girls with the median height at birth 50 (41-54) cm and median weight 3.2 (3.0-4.6) kg. They all grew with a normal pattern and well developed. Now the median age is 19 (4-41) months. The disease status during 12 pregnancies included 3 cases in CMR, 2 cases in MR(4.0), 3 cases in MMR, 4 cases not acquiring MMR. The median time of drug discontinuation was 35 (15-36) weeks during pregnancy. No patient lost CHR during this period. Conclusions: Female CML patients exposed to NIL 600 mg/d for 4 weeks in early pregnancy can give birth to normal babies, but there is still a risk of spontaneous abortion and congenital malformations.
