1.Analysis on Quality Standard of Hedyotis Herba Dispensing Granules Based on Standard Decoction

ObjectiveTo establish the specific chromatogram and quantitative analysis of multi-components by single-marker(QAMS) based on linear calibration using two reference substances(LCTRS), explore the consistency between Hedyotis Herba dispensing granules and standard decoction, and evaluate the quality of the dispensing granules. MethodsHigh performance liquid chromatography(HPLC) specific chromatogram was established based on 15 batches of Hedyotis Herba standard decoction and 10 batches of the dispensing granules, and LCTRS was used to locate chromatographic peaks. The actual retention times of 7 characteristic peaks in the specific chromatogram was measured on 24 different types of C18 columns, taking deacetyl asperulosidic acid and asperulosidic acid as the dual standard compounds, the retention times of the other 5 characteristic peaks were predicted and validated. Based on this, QAMS was developed to determine the contents of four components(deacetyl asperulosidic acid, deacetyl asperulosidic acid methyl ester, asperulosidic acid, and p-coumaric acid). Then, the relative correction factors of deacetyl asperulosidic acid, deacetyl asperulosidic acid methyl ester and p-coumaric acid were calculated using the reference peak of asperulosidic acid in the dual standard compounds, and each component was quantified accordingly. Finally, the consistency between the dispensing granules and standard decoction was assessed by taking extract rate of the standard decoction, consistency of the specific chromatograms, contents and transfer rates of the indicator components as indexes, and the quality of the dispensing granules was evaluated. ResultsThere were 7 common peaks in the characteristic chromatogram of samples of Hedyotis Herba standard decoction and the dispensing granules, and four of them were identified by reference standards, namely deacetyl asperulosidic acid(peak 1), deacetyl asperulosidic acid methyl ester(peak 3), asperulosidic acid(peak 6) and p-coumaric acid(peak 7). The similarity between the dispensing granules and the standard decoction was >0.9. The absolute deviation in the predicted retention time for each component by LCTRS was lower than that of the relative retention time method. The extract rate of the 15 batches of Hedyotis Herba standard decoction ranged from 7.89% to 14.60%, the contents of deacetyl asperulosidic acid, deacetyl asperulosidic acid methyl ester, asperulosidic acid and p-coumaric acid were 6.62-19.70, 3.83-17.99, 1.57-6.69, 1.62-4.52 mg·g-1, and the transfer rates of these components from decoction pieces to the standard decoction were 22.89%-39.60%, 34.03%-62.24%, 24.25%-43.70%, and 40.58%-73.71%, respectively. The extract rate, index component contents and transfer rates from decoction pieces to the three batches of Hedyotis Herba dispensing granules(P1-P3), produced by manufacturer A, were similar to those of the standard decoction prepared from the same batch of decoction pieces, and all fell within the specified range. The contents of the 4 indicator components in 7 batches of the dispensing granules(P4-P10) from manufacturers B-E were all within the range of the content converted from the standard decoction based on the quantity of the dispensing granules. ConclusionThe established specific chromatogram and QAMS based on LCTRS are reasonable and reliable. Based on the evaluation indicators of standard decoction yield, consistency of specific chromatograms, contents and transfer rates of the four index components, the 10 batches of Hedyotis Herba dispensing granules from various manufacturers have exhibited good consistency with the standard decoction, indicating that the current production process is relatively reasonable.

Gastric cancer is a common malignant tumor with complex pathological mechanisms, a low early diagnosis rate, and a high mortality rate. However, surgical treatment, targeted therapy, and chemotherapy have their treatment limitations and toxic side effects. Therefore, exploring the pathogenesis and mechanism of gastric cancer and finding effective treatment methods are important. At present, researches has found that tumor epithelial cells exhibit individual differences in molecular characteristics and exhibit metabolic heterogeneity that affects cell phenotype and function. The interaction between metabolites and cytokines can inhibit the formation of the tumor immune microenvironment and promote malignant progression. Therefore, metabolic reprogramming is regarded as a key feature of tumors and plays an important role in the process of tumor occurrence and development. However, the continuous deterioration of gastric cancer may be closely related to changes in the energy metabolism of cancer cells. Gastric cancer cells may regulate the dysregulation of synthesis or decomposition pathways such as glucose metabolism, amino acid metabolism, lipid metabolism, and nucleotide metabolism and activate associated signaling pathways, key proteins, and genes, leading to proliferation, invasion, and metastasis of cancer cells. In recent years, there has been a close relationship between the effective intervention by traditional Chinese medicine in gastric cancer and the regulation of metabolic reprogramming. There has been some progress in the intervention research on effective ingredients and formulas of traditional Chinese medicine for cancer. This article summarized existing Chinese and foreign literature on how gastric cancer cells affect disease progression by regulating their related metabolic networks, such as glucose metabolism, amino acid metabolism, lipid metabolism, and nucleotide metabolism, as well as how effective ingredients and formulas of traditional Chinese medicine enhance anti-tumor effects through targeted metabolism. It reviewed metabolic reprogramming intervention in gastric cancer, providing a reference for research on metabolic reprogramming regulation by traditional Chinese medicine and new targets and strategies for the treatment and prognosis of gastric cancer.
3.Molecular Mechanism Study of Compound Dihuang Granule in Treating PD Rats with Yin-Deficiency and Dynamic Wind Syndrome by Ubiquitin-Proteasome System

OBJECTIVE To explore the molecular mechanism of Compound Dihuang Granule in treating Parkinson's disease(PD)with yin-deficiency and dynamic wind syndrome by regulating the changes of ubiquitin-proteasome system-related proteins.METHODS SPF male SD rats were randomly selected as normal control group and sham operation group,13 rats in each group.The remaining rats were treated with 6-hydroxydopamine lateral brain injection to damage the substantia nigra to establish PD yin-deficien-cy and dynamic wind syndrome rat model.The established rats were randomly divided into model group,madopar group(150 mg·kg-1),low,medium and high dose groups of Compound Dihuang Granule(1.75,3.5,7 g·kg-1),13 rats in each group.Each group was given drugs by intragastric gavage for 28 d.The general conditions and neurobehavioral manifestations of the rats were observed;the expression of ubiquitin(UB),ubiquitin activating enzyme(UBE1),ubiquitin conjugating enzyme(UBE2A)and ubiquitin carboxyl terminal hydrolase(UCH-L1)positive cells,protein and mRNA expression levels in the damaged lateral stria-tum of the brain of rats were detected by immunohistochemistry,Western blot and qPCR methods;changes of dopamine(DA)content in the damaged lateral striatum of the brain of rats were detected by ELISA.RESULTS There was no significant difference in general conditions between the normal control group and the sham operation group,and there was no significant difference in the number of ro-tations,suspension time and number of moving grids.Compared to the sham-op group,the model group rats showed poor spirit,irrita-bility,limb tremor,slow movement,dark yellow fur,less food intake,reduced body weight;the modeling increased number of rota-tions,decreased hanging time and number of moving grids(P<0.01),significantly decreased expression of UB,UBE1,UBE2A,and UCH-L1 positive cells,mRNA and protein expression levels in the damaged lateral striatum(P<0.01),and significantly decreased DA content(P<0.01).Compared with the model group,the general condition of the rats in the madopar group and Compound Di-huang Granule groups was improved,the number of rotations was reduced,the hanging time and number of moving grids were increased(P<0.05,P<0.01),the expression of UB,UBE1,UBE2A,and UCH-L1 positive cells,mRNA and protein expression levels were increased(P<0.05,P<0.01),and the DA content was increased(P<0.01);behavioral tests and the expression levels of various in-dicators were most significantly improved in the madopar group and the high-dose Compound Dihuang Granule group(P<0.05,P<0.01),and there was no statistical significance between the two groups.CONCLUSION Compound Dihuang Granule may play a role in treating PD yin-deficiency and dynamic wind syndrome by regulating the expression of ubiquitin-proteasome system-related proteins,reducing abnormal protein aggregation,and thus alleviating DA content deficiency,and the high-dose Compound Dihuang Granule has the best effect.
4.Analysis of rational drug use in the cancer hospital based on performance evaluation

Objective:To explore the impact of performance evaluation on the rational use of clinical drugs in the cancer hospital.Methods:The rational drug use performance evaluation group of Shanxi Province Cancer Hospital combined the average value of each index from 2017 to 2019 to determine the target value of each evaluation index, and assigned the corresponding performance score and established the rational drug use performance evaluation system. The proportion of national essential drug use, the intensity of antimicrobial drug use, the proportion of the amount of adjuvant drug use and the drug selection in the process of drug use, drug dosage, administration route, administration frequency, administration course, solvent or solvent dosage, overuse of drug with indication, drug use beyond the instruction, and contraindication to the dispensing of drug, and other indicators from January 2019 to December 2022 in Shanxi Province Cancer Hospital were retrospectively analyzed.Results:The national essential drug use rate in hospitalized patients increased from 20.17% in 2019 to 57.30% in 2022; the intensity of antimicrobial drug use decreased from 12.07 number of defined daily doses (DDDs) in 2019 to 12.00 DDDs in 2022, and showed a decreasing trend year by year; the percentage of the amount of adjuvant drug use decreased from 2.60% in 2019 to 0.01% in 2022. From 2019 to 2022, the unreasonable rate of all assessment indicators decreased year by year, of which the rate of inappropriate drug selection decreased from 39.20% to 5.30%, the rate of inappropriate drug dosage decreased from 8.80% to 1.10%, the rate of inappropriate administration route decreased from 4.70% to 0.10%, the rate of inappropriate administration frequency decreased from 11.70% to 1.10%, the rate of inappropriate administration course decreased from 10.10% to 0.90%, the rate of inappropriate solvent or solvent dosage decreased from 21.10% to 7.30%, the rate of overuse of drug with indication decreased from 15.30% to 0.70%, the rate of drug use beyond the instruction decreased from 2.60% to 0.10%, and the rate of contraindication to the dispensing of drug decreased from 0.20% to 0.Conclusions:The refined performance evaluation system for rational use of drugs can effectively control the drug costs in the cancer hospital and promote rational use of drugs in clinics.
5.The correlation between early recurrence and late recurrence within 48 hours after catheter ablation of atrial fibrillation

Objective:To investigate the correlation between early recurrence and late recurrence within 48 hours after catheter ablation of atrial fibrillation.Methods:A retrospective analysis was conducted on atrial fibrillation patients who underwent catheter ablation at the Heart Center of the First Affiliated Hospital of Xinjiang Medical University from April 2020 to November 2021. Preoperative examinations and baseline data of patients were collected. After surgery, the patient was given continuous electrocardiogram monitoring for 48 hours to observe whether there was early recurrence of atrial fibrillation. Follow up visits were conducted at 3, 6, 9, and 12 months after surgery to observe whether there were atrial fibrillation events. The impact of extremely early recurrence on late recurrence free atrial fibrillation was analyzed using Kaplan Meier curves, and the predictive value of extremely early recurrence and other indicators for late atrial fibrillation recurrence was evaluated using Cox proportional hazards models and receiver operating characteristic (ROC) curves.Results:A total of 96 patients who underwent catheter ablation for atrial fibrillation for the first time were included in this study, with an age of (60.2±12.6)years. Among them, 46 patients (47.92%) experienced very early recurrence within 48 hours after surgery. During the 1-year follow-up, 16 patients in the very early recurrence group experienced late recurrence, which was statistically significant compared to the very early non recurrence group ( P=0.003). The proportion of persistent atrial fibrillation was higher in patients with very early recurrence (32.6% vs 8.0%, P=0.006), and more patients with late recurrence (34.8% vs 8.0%, P=0.003). The left atrial diameter was larger [(40.28±4.72)mm vs (37.10±4.35)mm, P=0.001]. The Kaplan Meier curve showed that compared with the very early non recurrence group, the late recurrence rate of patients in the very early recurrence group was significantly higher (Long rank P=0.001 6). Cox single factor regression analysis showed that very early recurrence, age increase, increased left atrial diameter, hypertension and diabetes were risk factors for late recurrence (all P<0.2). Multivariate regression analysis suggested that extremely early recurrence was an independent risk factor for late recurrence ( HR=3.99, 95% CI: 1.22-13.06, P=0.021). The area under the ROC curve for predicting late recurrence after catheter ablation of atrial fibrillation with extremely early recurrence was 0.703, and the negative predictive value was 0.920. Conclusions:Early recurrence within 48 hours after catheter ablation of atrial fibrillation is an independent risk factor for late recurrence and has good predictive value.
6.Effects of Compound Dihuang Granules on the Expressions of PINK1,Parkin and α-syn in Striatum of Rats with Parkinson Disease Yin-deficiency and Wind Movement Syndrome

Objective To observe the effects of compound Dihuang Granules on striatum PINK1,Parkin and α-syn in rats with Parkinson disease yin-deficiency and wind movement syndrome;To explore the mechanism of compound Dihuang Granules in the treatment of Parkinson disease yin-deficiency and wind movement syndrome.Methods 6-hydroxydopamine was injected into the substantia nigra of the midbrain to prepare the model of PD yin-deficiency and wind movement syndrome.Totally 65 model rats were randomly divided into model group,Madopar(150 mg/kg)group,and TCM low-,medium-,and high-dosage(1.75,3.5,7 g/kg)groups.Another 26 rats were divided into normal control group and sham-operation group,with 13 rats in each group.Each administration group was given corresponding medication solution by gavage,the normal control group,sham-operation group and the model group were given equivalent volume of normal saline,once a day for 28 consecutive days.The behavioral changes of rats were detected,ELISA and biochemical assay kits were used to detect ATP content and COX Ⅳactivity in striatum,HE staining was used to observe the morphological changes of the striatum,immunohistochemistry staining,Western blot and RT-qPCR were used to detect the expression of PINK1,Parkin,α-syn protein and mRNA in striatal tissue.Results Compared with the normal control group and the sham-operation group,the rats in the model group showed increased number of rotations and pole climbing time,decreased swimming score(P<0.01),the ATP content and COX Ⅳ activity in striatum tissue decreased(P<0.01),with disordered arrangement and reduced number of neurons,cell swelling,partial nuclear shrinkage,formation of vacuoles,and blurred boundaries,the expressions of PINK1,Parkin protein and mRNA in striatal tissue decreased(P<0.01),and the expressions of α-syn protein and mRNA increased(P<0.01).Compared with the model group,the number of rotations and pole climbing time of rats in each medication group decreased,swimming scores increased(P<0.01),ATP content and COX Ⅳ activity in striatal tissue increased(P<0.05,P<0.01),the number of neurons increased,the arrangement tended to be neat,and the morphology improved,the expressions of PINK1,Parkin protein and mRNA in striatal tissue increased(P<0.05,P<0.01),the expressions of α-syn protein and mRNA decreased(P<0.05,P<0.01).The improvement was most significant in the Medopar group and TCM high-dosage group(P<0.05,P<0.01),and there was no significant difference between the two groups(P>0.05),and TCM groups showed dosage dependence(P<0.05,P<0.01).Conclusion Compound Dihuang Granules can improve the behavioral symptoms of Parkinson disease yin-deficiency and wind movement syndrome,possibly by regulating the expressions of PINK1,Parkin and α-syn in the striatum tissue,thereby alleviating mitochondrial dysfunction,protecting dopaminergic neurons,and playing a role in treating Parkinson disease.
7.Comparative study of white matter diffusion properties in vulnerable and resistant individuals to continuous attention after short term sleep deprivation

Objective:To investigate the differences of white matter diffusion properties between vulnerable and resistant individuals to continuous attention after sleep deprivation.Methods:According to the psychomotor vigilance test performance before and after sleep deprivation, the participants were divided into the vulnerable group( n=24) and resistant group( n=25). All participants underwent diffusion tensor imaging (DTI) scans.Tract based spatial statistics(TBSS) was used to compare fractional anisotropy(FA), mean diffusivity(MD), axial diffusivity(AD), radial diffusivity(RD) maps between the two groups.Spearman correlation analysis was conducted by SPSS 24.0 to investigate the relationships between the altered DTI metrics and PVT task performance. Results:(1) Compared with resistant group, FA value of vulnerable group decreased in the body of corpus callosum(x, y, z=-8, 9, 25, t=-7.855), right superior longitudinal fasciculus(x, y, z=-39, -7, 26, t=-6.252), bilateral anterior limb of internal capsule(x, y, z=-13, 8, 13, t=-5.235; x, y, z=12, 8, 3, t=-5.024) and right posterior thalamic radiation(x, y, z=-26, -56, 17, t=-5.469)(TFCE corrected, P<0.05, cluster size≥50 voxel). (2) Compared with resistant group, MD value of vulnerable group increased in the body of corpus callosum(x, y, z=-3, -6, 26, t=7.613), right superior longitudinal fasciculus(x, y, z=-31, -19, 38, t=5.314), bilateral anterior limb of internal capsule(x, y, z=-16, 7, 8, t=6.898; x, y, z=15, 5, 7, t=6.652), splenium of corpus callosum(x, y, z=27, -53, 17, t=6.541), and AD value increased in the right superior longitudinal fasciculus(x, y, z=-33, -19, 39, t=4.892), splenium of corpus callosum(x, y, z=-22, -49, 21, t=5.450), genu of corpus callosum(x, y, z=4, 26, 0, t=4.332), as well as RD value increased in the right superior corona radiata(x, y, z=-17, 1, 33, t=7.558), body of corpus callosum(x, y, z=4, -8, 26, t=6.699), right anterior limb of internal capsule(x, y, z=-12, 7, 3, t=5.212) (TFCE corrected, P<0.05, cluster size≥50 voxel). (3) Correlational analysis revealed that the negative correlations were found between PVT task performance and the FA value in the right superior longitudinal fasciculus( r=-0.492, P<0.001), right anterior limb of internal capsule( r=-0.510, P<0.001), right posterior thalamic radiation( r=-0.502, P<0.001) and body of corpus callosum( r=-0.464, P<0.001). The positive correlations were found between PVT task performance and the MD value in the body of corpus callosum( r=0.500, P<0.001), right superior longitudinal fasciculus( r=0.499, P<0.001), splenium of corpus callosum( r=0.462, P<0.001), right anterior limb of internal capsule( r=0.471, P<0.001), and AD value in right superior longitudinal fasciculus( r=0.643, P<0.001), as well as RD value in right superior corona radiate( r=0.498, P<0.001) (Bonferroni corrected, P<0.003). Conclusion:Differences in the microstructural characteristics of white matter fiber tracts in specific brain regions may constitute the potential neuropathological basis for the phenotypes of vulnerable and resistant individuals to continuous attention after sleep deprivation.
8.Correlation between abnormal left atrial appendage function and thrombotic events in patients with non-valvular atrial fibrillation

Objective:To investigate the association between abnormal left atrial appendage function and thrombotic events in patients with non-valvular atrial fibrillation, and the independent risk factors affecting left atrial appendage function.Methods:Patients with non-valvular atrial fibrillation, who visited the Atrial Fibrillation Center of the First Affiliated Hospital of Xinjiang Medical University from June 1, 2019 to June 1, 2021, were selected. According to left atrial appendage flow velocity (LAAFV), they were divided into normal left atrial appendage function group (297 patients with LAAFV ≥ 40 cm/s) and abnormal left atrial appendage function group (85 patients with LAAFV<40 cm/s). Baseline data and transesophageal echocardiography images were collected from all the patients. The occurrence of thrombotic events was recorded. Univariate and multivariate unconditional logistic regression analyses were conducted to investigate the correlation between abnormal left atrial appendage function and the occurrence of thrombotic events.Results:There were significant differences in gender, type of atrial fibrillation, CHA 2DS 2-VASc score, anticoagulant therapy, total cholesterol, low-density lipoprotein cholesterol, international normalized ratio (INR), left atrial diameter, proportion of patients with right atrial enlargement, left ventricular ejection fraction, inner diameter, sum of inner diameter, depth, and sum of depth of all angles of the left atrial appendage, and incidence of thrombotic events between the two groups (all P<0.05). After adjusting for confounders, multivariate unconditional logistic regression analyses showed that abnormal left atrial appendage function was closely associated with thrombotic events (β=1.168 P=0.002), and left atrial diameter ( OR=1.084, 95% CI 1.019-1.153, P=0.011) and persistent atrial fibrillation ( OR=2.323, 95% CI 1.226-4.403, P=0.010) were independent risk factors affecting left atrial appendage function. Conclusions:Abnormal left atrial appendage function is closely associated with thrombosis. The left atrial diameter and persistent atrial fibrillation were independent risk factors affecting left atrial appendage function.
9. Clinical value of dual phase ASL in evaluation of collateral circulation in patients with severe cerebral vascular stenosis

Objective:
To investigate the clinical value of dual-phase arterial spin-labeled perfusion imaging(3D-ASL) in the evaluation of collateral circulation compensation in patients with severe cerebrovascular stenosis.
Methods:
From December 2017 to August 2018, 73 patients with severe cerebral artery stenosis confirmed by MRA in Linfen Central Hospital were collected and studied.The two-phase superconducting magnetic resonance imaging was performed[Post Label Delay(PLD): 1 525ms, 2 525ms] three-dimensional quasli continuous arterial spin labeling(3D-PCASL) imaging.Bilateral phase cerebral blood flow(CBF) was measured on the affected side and mirror side respectively.The CBF values of the affected side and mirror side were compared and analyzed, and the abnormal perfusion cases were compared and analyzed.
Results:
In 73 patients with severe cerebrovascular stenosis, the CBF values of the bilateral affected side[CBF 1 525ms(18.33±6.42)mL·100g-1·min-1, CBF 2 525ms(34.81±11.68)mL·100g-1·min-1] were lower than those of the mirror side[CBF 1 525ms(41.25±16.84)mL·100g-1·min-1, CBF 2 525ms(47.74±14.90)mL·100g-1·min-1], the differences were statistically significant(

Objective To investigate the safety and efficacy of CT-guided 125I seed implantation in the treatment of metastatic lymph nodes within the abdomen.Methods Data of 55 patients who underwent CT-guided 125I seed implantation with a total of 63 metastatic lymph nodes within the abdomen were retrospectively analyzed.The efficacy was evaluated.Results 125I seeds were successfully implanted in all 55 patients,and the radiation dose reached the pre-operation plan requirements.The response rate of 1,3,6,9 and 12 months after operation was 76.19% (48/63),80.65% (50/62),71.43%(30/42),77.50%(31/40) and 78.95%(30/38),respectively.And the clinical benefit rate was 93.65%(59/63),88.71%(55/62),80.95 % (34/42),85.00%(34/40) and 84.21%(32/38),respectively.There were 42 patients combined with abdominal pain and back pain before the operation.Among them,39 patients (39/42,92.86%) relieved 1 month after operation.No severe complication occurred intraoperatively or postoperatively,such as inhibition of bone marrow,bleeding or infection.The survival rate of 1,3,6,9 and 12 months postoperatively was 100%,98.20%,61.80%,58.20% and 54.50%,respectively.Conclusion CT-guided 125I seed implantation is one of the safe and effective methods for treating metastatic lymph nodes within the abdomen.
