1.Effects and mechanism of rhubarb Tangluo pill on liver injury in type 2 diabetic rats

Objective To investigate the effect and mechanism of rhubarb Tangluo pill on liver injury in type 2 diabetic rats.Methods ZDF(fa/fa)rats were given high-fat diet to induce type 2 diabetes model,and were randomly divided into model group,positive control group(0.18 g·kg-1 metformin)and experimental-L,-M,-H groups(0.54,1.08 and 2.16 g·kg-1 rhubarb Tangluo pill),with 8 rats in each group.Eight ZDF(fa/+)rats were selected as control group.The control group and model group were given equal volume of pure water once a day for 12 weeks.An oral glucose tolerance test(OGTT)was performed after administration.Fasting blood glucose level,body mass and liver mass of rats were measured and liver index was calculated.The contents of glutamic-pyruvic transaminase(GPT),glutamic oxalacetic transaminase(GOT),triglyceride(TG)and total cholesterol(TC)in serum were detected.The histomorphologic changes of liver were observed by hematoxylin-eosin(HE)staining and Masson staining.The protein expression of phosphorylated insulin receptor substrate 1(p-IRS1),phosphorylated protein kinase B(p-Akt)and glucose transporter 4(GLUT4)were detected by Western blotting.Results After administration,the fasting blood glucose levels of control group,model group,positive control group and experimental-H group were(4.71±0.45),(29.9±2.97),(15.28±4.52)and(13.84±1.55)mmol·L-1,respectively;the liver index were 2.31±0.46,4.03±0.18,3.37±0.23 and 3.38±0.24;the relative expression level of p-IRS1 protein were 1.00±0.36,4.00±0.11,1.62±0.27 and 1.90±0.17,respectively;the relative expression levels of p-Akt protein were 1.00±0.25,0.21±0.04,0.73±0.15 and 0.54±0.04,respectively;GLUT4 protein relative expression levels were 1.00±0.11,0.40±0.08,0.86±0.04 and 0.70±0.06,respectively.Compared with the model group,the above indexes in the experimental-H group were statistically significant(P<0.01,P<0.05).Conclusion Rhubarb Tanglu pill can effectively improve glycolipid metabolism and liver injury in type 2 diabetes mellitus,and its mechanism may be related to the activation of IRS1/Akt signaling pathway.
2.Development of a High-throughput Sequencing Platform for Detection of Viral Encephalitis Pathogens Based on Amplicon Sequencing

Objective Viral encephalitis is an infectious disease severely affecting human health.It is caused by a wide variety of viral pathogens,including herpes viruses,flaviviruses,enteroviruses,and other viruses.The laboratory diagnosis of viral encephalitis is a worldwide challenge.Recently,high-throughput sequencing technology has provided new tools for diagnosing central nervous system infections.Thus,In this study,we established a multipathogen detection platform for viral encephalitis based on amplicon sequencing. Methods We designed nine pairs of specific polymerase chain reaction(PCR)primers for the 12 viruses by reviewing the relevant literature.The detection ability of the primers was verified by software simulation and the detection of known positive samples.Amplicon sequencing was used to validate the samples,and consistency was compared with Sanger sequencing. Results The results showed that the target sequences of various pathogens were obtained at a coverage depth level greater than 20×,and the sequence lengths were consistent with the sizes of the predicted amplicons.The sequences were verified using the National Center for Biotechnology Information BLAST,and all results were consistent with the results of Sanger sequencing. Conclusion Amplicon-based high-throughput sequencing technology is feasible as a supplementary method for the pathogenic detection of viral encephalitis.It is also a useful tool for the high-volume screening of clinical samples.
3.Simultaneous content determination of seventeen constituents in Yangxue Ruanjian Capsules by UPLC-MS/MS

AIM To establish a UPLC-MS/MS method for the simultaneous content determination of liquiritin apioside,alibiflorin,swertiamarin,methyl gallate,benzoylpaeoniflorin,sweroside,6′-O-β-D-glucosylgentiopicroside,isoliquiritigenin,loganic acid,liquiritigenin,gallic acid,paeoniflorin,oxypaeoniflorin,gentiopicroside,glycyrrhizic acid,isoliquiritoside and liquiritin in Yangxue Ruanjian Capsules.METHODS The analysis was performed on a 40℃thermostatic Waters BEH C18column(2.1 mm×100 mm,1.7 μm),with the mobile phase comprising of 2 mmol/L ammonium acetate(containing 0.1%formic acid)-acetonitrile flowing at 0.3 mL/min in a gradient elution manner,and electron spray ionization source was adopted in negative ion scanning with multiple reaction monitoring mode.RESULTS Seventeen constituents showed good linear relationships within their own ranges(r>0.999 6),whose average recoveries were 91.33%-104.03%with the RSDs of 1.58%-3.50%.CONCLUSION This rapid,accurate and stable method can be used for the quality control of Yangxue Ruanjian Capsules.
4.Epidemiological investigation of the first psittacosis death case in Hangzhou City,China

This study was aimed at performing epidemiologic investigation of the first psittacosis death case in Hangzhou City,to provide a reference for the investigation and disposal of psittacosis cases,as well as prevention and control.Epidemio-logic data were collected through field epidemiologic investigation,and close contacts and environmental samples were collected for pathogenicity testing.The first symptom in the patient was cough,which did not raise concerns at the time.Several days later,the patient developed abdominal distension and black stools,and visited two medical institutions for treatment and hospi-talization.The patient's sputum and peripheral blood were tested for Chlamydia psittaci infection by metagenomic analysis via next-generation sequencing.Samples collected from the patient's family members,close contacts,and home environment test-ed negative with real-time quantitative polymerase chain reaction.The patient later died of gastrointestinal bleeding.This article is the first report of a case of psittacosis contracted from exposure to a sick parrot in Hangzhou City,in a patient who died be-cause of an underlying disease.Operational training should be provided for medical personnel,and early diagnosis with mNGS and treatment of patients with underlying diseases should be performed as early as possible to avoid fatality.In addition,health education should be carried out to raise public awareness of the disease.
5.IL2rg-/- rats support prolonged infection of human RSV

Objective To overcome the limitations of existing human respiratory syncytial virus(hRSV)animal models,such as semi-permissiveness and short duration of infection,this study established an IL2rg gene knockout(IL2rg-/-)rat model using TALEN gene editing technology.Methods The animal model was infected with hRSV intranasally.Clinical characteristics,body weight,and temperature changes were observed over the infection period(0~35 days).The total viral loads in respiratory organs,such as the nasal tissue,trachea,and lungs,were measured at various time points(4,11,20,and 35 days post-infection).Pathological analysis was conducted on target organs at the endpoint of observation(35 days post-infection).Changes in peripheral blood T,B,NK,and NKT cells and various cytokines were assessed at various time points(4,20,and 35 days post-infection).Results(1)IL2rg/-knockout rats sustained high viral loads in the nasal cavity upon intranasal inoculation with hRSV.The average peak titer rapidly reached 1 × 1010 copies/g in nasal tissue and 1 × 107 copies/g up to 5 weeks post-infection.(2)However,no significant pathological changes were noted in nasal,tracheal,or lung tissues.(3)An increase was observed in the content of peripheral blood B cells in hRSV-infected IL2rg--rats.(4)IL-6 and MCP-1 were increased in the early stage of infection and then decreased at the end of the observation period.Conclusions This study established a new IL2rg-/-rat model using TALEN technology and found that this model effectively supported high-level replication and long-term infection of hRSV,providing a good basis for antiviral drug screening and in vivo efficacy evaluation of anti-hRSV antibodies.
6.Survey of Disease Cognition in Patients with Endometriosis and Effect Evaluation of TCM Chronic Disease Management

Objective To understand cognition level and treatment intention of patients with endometriosis(EMs);To evaluate the effects of TCM chronic disease management on EMs patients.Methods The disease cognition level and treatment intention of 1 895 EMs patients from October 2020 to December 2021 in 19 provinces,autonomous regions and municipalities were analyzed through cross-sectional investigation.The cognitive effect of chronic disease management of TCM on 801 patients with EMs from 6 Grade A hospitals in Beijing was evaluated by self-control study.Results The cognition level of EMs patients for the disease was improved with the increase of educational background.Patients generally believed that EMs was a chronic disease,requiring regular review and long-term management.The proportion of patients with high school education or below,junior college education,bachelor degree or above who knew the possibility of malignant changes in EMs was less,only 32.3%,41.6%and 47.7%,respectively.The awareness rate of patients with high school education or below for recurrence after EMs conservative surgery was 46.9%,lower than that of those with junior college education(66.8%)and bachelor degree or above(72.7%).Among the patients with dietary contraindications,the proportion of patients with high school education or below,junior college education,bachelor degree or above who knew the contraindication of the forest frog oil was less,only 20.7%,30.3%and 32.9%,respectively.In the aspect of life adjustment,the awareness of patients was generally high.EMs patients mainly recognized the disease through face-to-face communication with gynecologists.Only 34.4%of patients with high school education or below learned about EMs through WeChat public accounts,APPs and other new media,which was lower than those with junior college education(48.6%)and bachelor degree or above(55.4%).EMs patients generally tend to be treated in comprehensive hospitals,with high acceptance of TCM treatment,mainly TCM decoction,and low acceptance of TCM appropriate technology.After 1 year of TCM chronic disease management,the disease awareness of EMs patients was significantly improved compared with before management,with statistical significance(P<0.05).Conclusion EMs patients with different educational backgrounds have different cognition of the disease,and each has different emphasis.Education and popularization should be carried out according to their knowledge blind spots.Chronic disease management of TCM can improve the disease cognition level of EMs patients.

Ankylosing spondylitis (AS) combined with lower cervical fracture is often categorized into unstable fracture, with a high incidence of neurological injury and a high rate of disability and morbidity. As factors such as shoulder occlusion may affect the accuracy of X-ray imaging diagnosis, it is often easily misdiagnosed at the primary diagnosis. Non-operative treatment has complications such as bone nonunion and the possibility of secondary neurological damage, while the timing, access and choice of surgical treatment are still controversial. Currently, there are no clinical practice guidelines for the treatment of AS combined with lower cervical fracture with or without dislocation. To this end, the Spinal Trauma Group of Orthopedics Branch of Chinese Medical Doctor Association organized experts to formulate Clinical guidelines for the treatment of ankylosing spondylitis combined with lower cervical fracture in adults ( version 2024) in accordance with the principles of evidence-based medicine, scientificity and practicality, in which 11 recommendations were put forward in terms of the diagnosis, imaging evaluation, typing and treatment, etc, to provide guidance for the diagnosis and treatment of AS combined with lower cervical fracture.
8.Identification of ceRNA networks in type H and L vascular endothelial cells through integrated bioinformatics methods

Objective:Type H blood vessels are a subtype of bone-specific microvessels(CD31hiEmcnhi)that play an important regulatory role in the coupling of angiogenesis and osteogenesis.Despite reports on the distinct roles of type H and L vessels under physiological and pathological bone conditions,their genetic differences remain to be elucidated.This study aims to construct a competitive endogenous RNA(ceRNA)network of key gene for differencial expression(DE)in type H and L vascular endothelial cells(ECs)through integrated bioinformatic methods. Methods:We downloaded relevant raw data from the ArrayExpress and the Gene Expression Omnibus(GEO)database and used the Limma R-Bioconductor package to screen for DE lncRNAs,DE miRNAs,and DE mRNAs between type H and L vascular ECs.A total ceRNA network was constructed based on their interactions,followed by refinement using protein-protein interaction(PPI)networks to select upregulated and downregulated key genes.Enrichment analysis was performed on these key genes.Random validation was conducted using flow cytometry and real-time RT-PCR. Results:A total of 1 761 DE mRNAs,187 DE lncRNAs,and 159 DE miRNAs were identified,and a comprehensive ceRNA network was constructed based on their interactions.Six upregulated(Itga5,Kdr,Tjp1,Pecam1,Cdh5,and Ptk2)and 2 downregulated(Csf1r and Il10)key genes were selected via PPI network to construct a subnetwork of ceRNAs related to these key genes.Upregulated key genes were mainly enriched in negative regulation of angiogenesis and vascular apoptosis.Results from flow cytometry and real-time RT-PCR were consistent with bioinformatics analysis. Conclusion:This study proposes a ceRNA network associated with upregulated and downregulated type H and L vascular ECs based on selected key genes,providing new insights into the regulatory mechanisms of type H and L vascular ECs in bone metabolism.
9.Correlation between MDA,AOPP,Nrf2,GSH levels and acute kidney injury after laparoscopic partial nephrectomy

Objective To analyze the correlation of malondialdehyde(MDA),advanced oxidation protein products(AOPP),nuclear factor erythroid-2 related factor 2(Nrf2),glutathione(GSH)levels with postoperative acute kidney injury(AKI)among patients undergoing laparoscopic partial nephrectomy(LPN).Methods A total of 110 patients with renal cell carcinoma who were admitted to the Department of Urology,the Third Medical Center of Chinese PLA General Hospital from February to August 2022 were included in the study.Patients were divided into AKI group(n=30)and non-AKI group(n=80)based on the Kidney Disease:Improving Global Outcomes(KDIGO)criteria,and then divided into elderly AKI(>65 years old,n=14),middle-aged AKI(50-65 years old,n=16),elderly non-AKI(>65 years old,n=30),and middle-aged non-AKI(50-65 years old,n=50)four sub-groups based on age.Clinical characteristics and laboratory examination data were recorded.Venous blood was collected from the patients before the operation(T1),immediately after the surgery(T2),and 24 h after surgery(T3),and MDA,AOPP,Nrf2 and GSH levels were measured.MDA,AOPP,Nrf2 and GSH levels were compared between the four subgroups at different time points,and their correlation with the occurrence of AKI after LPN were explored.The risk factors for AKI after LPN were analyzed using the univariate and multivariate logistic regression.Results Spearman correlation analysis revealed that AKI was not associated with the MDA level at each time point(P>0.05),was positively associated with AOPP-T3 level(r=0.315,P=0.037),was negatively associated with the Nrf2-T3 level(r=-0.365,P=0.015)and GSH-T2 level(r=-0.338,P=0.025)in elderly patients after LPN.AKI was not associated with MDA,AOPP,Nrf2,and GSH levels(P>0.05)in middle-aged patients after LPN.Multivariate logistic regression analysis showed that BMI(OR=2.724,P=0.040)and surgically resected kidney volume(OR=1.309,P=0.049)were independent risk factors for AKI in elderly patients after LPN,GSH-T2(OR=0.271,P=0.042)was an independent protective factor for AKI in elderly patients after LPN.Intraoperative colloid fluid intake(OR=1.006,P=0.007)was an independent risk factor for AKI in middle-aged patients after LPN,intraoperative urine output(OR=0.104,P=0.007)was an independent protective factor for AKI in middle-aged patients after LPN.Conclusions The AKI after LPN may be related to the increase of AOPP level and the decrease of Nrf2 and GSH levels in elderly patients,and the postoperative GSH is an independent protective factor for AKI in elderly patients after LPN.The correlation of AKI after LPN is not significant with the levels of MDA,AOPP,Nrf2 and GSH in the middle-aged patients.
10.Predictive value of ScvO2 for acute kidney injury after robot-assisted partial nephrectomy in elderly patients

Objective To investigate the predictive value of central venous oxygen saturation(ScvO2)in elderly patients with acute kidney injury(AKI)after robot-assisted partial nephrectomy(RAPN).Methods Seventy-eight elderly patients who underwent RAPN in the Department of Urology,the Third Medical Center of Chinese PLA General Hospital from February to September 2022 were selected.AKI was diagnosed according to the International Nephropathy Improving Global Prognosis Criteria,and the patients were divided into AKI group(n=22)and non-AKI group(n=56)according to whether AKI occurred after surgery.Blood gas analysis of venous blood was taken after central venous puncture(T0),5 min after renal artery occlusion(T1),5 min after renal artery opening(T2),and after surgery(T3).Hemoglobin(Hb),arterial blood lactic acid(Lac),arterial oxygen partial pressure(PaO2),ScvO2 were recorded respectively.Oxygen uptake rate(O2ER)were calculated.Multivariate logistic regression analysis was used to analyze the risk factors for postoperative AKI.Receiver operating characteristic(ROC)curve was used to analyze the predictive value of ScvO2 for AKI in elderly patients after RAPN.Results Compared with non-AKI group,fasting plasma glucose(FPG)level in AKI group significantly increased at T1-T2(P<0.05).Compared with T0,FPG level in two groups obviously increased at T1-T3(P<0.01).Compared with T1,the FPG level in T3 significantly increased in non-AKI group(P<0.01).Compared with non-AKI group,ScvO2 in AKI group significantly increased at T1-T2(P<0.01),Lac level at T2 significantly increased(P<0.05).There was no significant difference in O2ER between the two groups at each time point(P>0.05).Compared with T0,O2ER in T1-T3 significantly decreased(P<0.01),and ScvO2 in both groups significantly increased(P<0.01).Compared with T1,ScvO2 in non-AKI group significantly increased at T2-T3(P<0.05 or P<0.01).Compared with T2,ScvO2 in non-AKI group significantly increased in T3(P<0.05).Multiple-factor analysis showed that T1 ScvO2(OR=1.127,95%CI 1.006-1.263,P=0.039)was an independent risk factor for AKI after RAPN.ROC curve analysis showed that T1 ScvO2 had a sensitivity of 77.3%,specificity of 71.4%,truncation value of 81%,and area under the curve(AUC)of 0.761 in predicting AKI after RAPN.Conclusion ScvO2 has certain predictive value for the occurrence of short-term AKI after RAPN.
