Objectives:To investigate clinical characteristics, outcomes and antimicrobial resistance of community-acquired Pseudomonas aeruginosa (CAPA) infections in Chinese pediatric patients. Methods:This retrospective study was conducted at 6 tertiary hospitals in China during January 2016 to December 2018. The clinical and microbiological data of CAPA infected hospitalized children in Hainan and in other regions were collected and compared, and the antimicrobial resistance patterns, clinical characteristics and antibiotic therapy were analyzed. Between different groups were compared using the Chi-square test and Mann-Whitney U test. Results:Among 91 patients, 63 cases were males, 28 cases were females, and 74 cases were from Hainan province, 17 cases were from other regians. The age of consultation was 22.5 (5.4, 44.0) months. Twenty-four cases (26%) had underlying diseases. Fever (79 cases (87%)) and cough (64 cases (70%)) were common initial symptoms. Other concomitant symptoms included wheezing 8 cases (9%), diarrhea 3 cases (3%) and vomiting 4 cases (4%). Twenty-eight cases (31%) had organ infections, including pneumonia 22 cases (24%), skin infection 5 cases (5%), meningitis, intra-abdominal infection and upper urinary tract infection each 1 case (1%). The resistance rate of CAPA isolates to cefepime (4% (4/90)), amikacin (1% (1/90)), ciprofloxacin (2% (2/90)) and levofloxacin (1% (1/89)) was low, and to ceftazidime, piperacillin, piperacillin-azobactam, carbapenem was 12% (11/90), 3/16, 18% (10/56) and 6% (5/90), respectively. Antimicrobial combination therapy accounted for 52% (47/91) of empirical therapy and 59% (52/88) of definite therapy. Two cases (2%) were hopeless discharged, and 3 cases (3%) died during hospitalization. The worse prognosis of CAPA infection is significantly different among children in other regions and in Hainan (4/17 vs. 1% (1/74), χ2=9.74, P<0.05). Conclusions:The invasive CAPA-infection has regional difference in incidence and prognosis in China. Clinical symptoms and signs are non-specific. CAPA strains isolated from pediatric patients display low level of resistance to most of the common antipseudomonal antibiotics. The proportion of poor prognostic outcome is lower in Hainan than in other regions.

Objective:The tumor-stroma ratio(TSR)is considered an independent prognostic factor for gastric cancer.Traditionally,TSR as-sessments have relied on the visual evaluation of surgical specimens, which is a method that lacks objectivity. This study was conducted to investigate whether the TSR in preoperative biopsy specimens can be automatically quantified using deep learning methods and whether the TSR value can be used to predict the efficacy of neoadjuvant chemotherapy(NAC)in patients with gastric cancer. Methods:In total,148 preoperative biopsy slides and 43 surgical resection slides from patients with gastric cancer who underwent NAC treatment at Yunnan Can-cer Hospital between March 2013 and March 2020 were used in the study.Tumor region segmentation and epithelial-stromal segmentation models were developed. The surgical resection slides were used to trained and evaluate the model, and the biopsy slides were used to test their predictive abilities. The TSR values were determined on the basis of the intersection of predictions from both models. The postoperat-ive pathological tumor regression grade(TRG)was used to categorize patients into good responders(TRG 0-1)and poor responders(TRG 2-3).Univariate and multivariate Logistic regression analyses were conducted to determine the correlation between the TSR value and the ef-ficacy of NAC in gastric cancer. Results:The intersection over union(IOU)value was 0.94 for the tumor tissue segmentation model and 0.88 for the epithelial-stromal segmentation model.Using cutoff values of 44.93%and 70.22%,patients were classified into low, intermediate, and high TSR groups. The proportion of good responders was significantly different among these groups(P<0.05).Multivariate Logistic re-gression analysis indicated that the TSR was an independent predictor of NAC response in gastric cancer(OR=0.10,95%CI:0.03-0.32).When the TSR three-category classification was added as a predictor of treatment response alongside conventional clinical information, the area under curve(AUC)increased from 0.71 to 0.85.Conclusions:This deep learning model is capable of automatically segmenting tumor,epi-thelial,and stromal regions based on pathological slides, accurately calculating TSR value, and predicting the efficacy of NAC on the basis of the automatically computed TSR values.

Journal of Traditional Chinese Medicine 2023;64(22):2372-2376
It is believed that the basic pathogenesis of oculomotor nerve paralysis (ONP) is qi and blood depletion and channel obstruction. Taking channel pattern differentiation as the core, it is recommended to use acupuncture mainly based on location, syndrome and channel deficiency-excess differentiation. Location and channel tropism differentiation mainly follows the principle of “where the channels and collaterals pass, the main treatment can reach”, and commonly uses acupuncture points on Dumai (督脉), the hand Shaoyang (少阳) Sanjiao (三焦) channel, the hand Taiyang (太阳) small intestine channel, three yang channels of the foot, the hand Shaoyin (少阴) heart channel, the foot Jueyin (厥阴) liver channel, and others, and combines scalp acupuncture with electro-acupuncture to enhance channel stimulation. Pattern and channel tropism differentiation is to differentiate the entered channel according to clinical symptoms and zang-fu patterns. The differentiation of deficiency-excess should be payed attention to, and the principle of “supplementing the deficiency, and draining the excess” should be followed. The supplementing and draining method should be used rationally by considering the differences in the supplementing and draining acupoints themselves, as well as the actual condition of the patients. Simultaneously, it is suggested to combine manipulation and stimulation amount and effect in clinical practice.

Pregnancy in patients with Eisenmenger syndrome (ES) is associated with high maternal mortality rates of 30%‒50%, or even up to 65% in the case of a cesarean section (Yuan, 2016). Here, we report a case of term pregnancy complicated with ES and severe pulmonary artery hypertension (PAH), which was managed by a multidisciplinary team (MDT) and resulted in an uncomplicated delivery via elective cesarean section. The goal of this study is to emphasize the importance of multidisciplinary approach in the management of pregnancy with ES, which can profoundly improve maternal and infant outcomes.

ObjectiveTo investigate the effects of repetitive transcranial magnetic stimulation (rTMS) on the efficacy and cognitive function of adolescents with depression. MethodsA total of sixty adolescent depression patients who met the diagnostic criteria of the International Classification of Diseases, tenth edition (ICD-10) and were admitted to the psychological outpatient clinic of the Affiliated Hospital of North China University of Technology from September 2021 to March 2022 were selected for the study, and the random number table method was used to divide the study group (escitalopram oxalate combined with rTMS treatment) and a control group (escitalopram oxalate combined with rTMS pseudo-stimulation treatment) of 30 cases each, and both groups were treated for 4 weeks. Before treatment and at 1, 2, 3 and 4 weeks of treatment, patients' depressive symptoms were assessed using the scores and subtraction rates of the Hamilton Depression Scale-24 item (HAMD-24), and before treatment and after 4 weeks of treatment, cognitive function was assessed using the Motion Screening Task (MOT), Rule Switch Task (RST) and Delayed Matching-to-Sample (DMS) task of the Zhiyun Neuropsychological Automated Test System assessment. Adverse effects during treatment were recorded in both groups. ResultsRepeated measures ANOVA results showed statistically significant time main effects, group main effects, and the interaction between the two groups for HAMD-24 scores before treatment and after 1, 2, 3 and 4 weeks of treatment in patients (F=522.021, 39.905, 26.412, P<0.05). Individual effect analysis showed statistically significant differences in HAMD-24 scores between the two groups after 2, 3 and 4 weeks of treatment (t=-12.784~-2.776, P<0.01). After 4 weeks of treatment, the total effective rate of treatment in the study group was higher than that in the control group (93.33% vs. 73.33%, χ2=4.320, P<0.05), and the differences in each indicator of RST and DMS between the two groups were statistically significant compared with those before treatment (t=-5.616~9.135, P<0.05 or 0.01), and the differences between the two groups were statistically significant (t=-4.823~5.518, P<0.05 or 0.01). ConclusionrTMS may help improve depressive symptoms as well as cognitive function in adolescents with depression.

OBJECTIVE To study the protective effects of Dachengqi decoction (DCQD) on intestinal septic mice, and to explore the possible mechanisms from the Toll-like receptor 4(TLR4)/myeloid differentiation factor 88(MyD88) signaling pathway. METHODS The SPF male C57BL/6J mice were randomly divided into Sham group, Sham+DCQD-H group, model (CLP) group, DCQD-L group, DCQD-H group and Positive group. The model of intestinal sepsis was established by cecal ligation and puncture in CLP group, DCQD-L group, DCQD-H group and Positive group. Three days before the operation and seven days after the operation, DCQD-L group and DCQD-H group were given DCQD intragastrically at 4, 8 g/kg (calculated by crude drug), respectively. Positive group was given ulinastatin intraperitoneally 2 h before operation and 7 d after the operation (at 50 000 U/kg). In Sham group and Sham+DCQD-H group, only cecum of mice was exposed without ligation and puncture. Sham+DCQD- H group was given DCQD intragastrically (8 g/kg,calculated by crude drug) 3 days before the operation and 7 days after the operation. Both the Sham group and CLP group were given normal saline 0.2 mL intragstrically and intraperitoneally each day, for 10 consecutive days. After the operation, the severity of sepsis was assessed, and the 7 d survival rate of mice was assessed. One hour after the last medication, the levels of tumor necrosis factor-α (TNF-α) and interleukin-6 (IL-6) in serum and ileum of mice were determined; the pathological and morphological changes of mice’s liver, lung, kidney and ileum were observed; mRNA expressions of the TLR4 and MyD88 in ileum were tested. RESULTS Compared with CLP group, sepsis score, the levels of TNF-α and IL-6 in serum and ileum (except for IL-6 in ileum of DCQD-L group), damage score of the liver, lung, kidney and ileum, mRNA expressions of TLR4 and MyD88 in ileum were all decreased significantly in DCQD-L group and DCQD-H group (P<0.05 or P<0.01), while 7 d survival rate (except for DCQD-L group) was increased significantly (P<0.05). The damage to liver tissue in mice was significantly improved, and inflammation infiltration and apoptosis were reduced; lung tissue damage had been alleviated, with varying degrees of improvement in alveolar atrophy, bleeding and edema; the renal tissue damage was improved and weakened dilation of renal tubular lumen was weakened; the damage and edema of ileal tissue were significantly improved. CONCLUSIONS DCQD may exert a protective role on intestinal septic model mice. The mechanism may be related to the inhibition of systemic inflammation, the reduction of multiple organ damage, and down-regulation of TLR4/MyD88 signaling pathway.
