Objective To investigate the current status of symptom burden and psychological distress among lung cancer patients in the diagnostic phase, and to explore the chain mediating role of social support and resilience between symptom burden and psychological distress. Methods The patients with lung cancer in the diagnostic phase who were treated in the Department of Thoracic Surgery of the Second Xiangya Hospital of Central South University from October 2022 to June 2023 were investigated by a general information questionnaire using the MD Anderson Symptom Inventory, the Social Support Rating Scale, the Connor-Davidson Resilience Scale, and the Distress Thermometer. The chain mediating role of social support and resilience between symptom burden and psychological distress was analyzed. Results A total of 413 lung cancer patients were enrolled, including 173 males and 240 females, aged (54.69±10.82) years. The detection rate of psychological distress among lung cancer patients in the diagnostic phase was 48.18%, and the average score was (3.84±2.50) points. Psychological distress was positively correlated with symptom burden (P<0.01), and negatively correlated with social support and resilience (P<0.01). The mediating effect of resilience between symptom burden and psychological distress was significant. The chain mediating effect of social support and resilience between symptom burden and psychological distress was also significant. Conclusion Lung cancer patients in the diagnostic phase have a high detection rate of psychological distress. Symptom burden can directly impact psychological distress, and can affect psychological distress through the indirect path of resilience as well as the chain mediating path between social support and resilience among lung cancer patients in the diagnostic phase.

Journal of Clinical Hepatology 2023;39(1):97-103
Objective To explore the diagnostic value of Young's modulus obtained by real-time shear wave elastography (SWE) for liver fibrosis in autoimmune hepatitis (AIH) patients. Methods A total of 75 AIH patients in the First Affiliated Hospital of Zhengzhou University from January 2013 to April 2022 were retrospectively enrolled. Scheuer scoring system was used to evaluate degrees of liver fibrosis (S0-S4). By using pathological examination of liver tissues as the golden standard, the receiver operating characteristic curve (ROC) was plotted and the area under the curve (AUC) was used to evaluate the diagnostic value of SWE for the significant fibrosis (≥S2), advanced liver fibrosis (≥S3), and liver cirrhosis (S4), respectively. Independent sample t test was used for comparison of continuous data with normal distribution between the two groups. The Kruskal-Wallis H test was used for comparison of non-normally distributed continuous data between multiple groups and Bonferroni method was used for further comparison between two groups. The Spearman correlation coefficient was used for correlation analysis. The logistic regression analysis was used to predict the impact factors in diagnosis accuracy. Results The Young's modulus measured by SWE was statistically significant different among various fibrosis groups ( H =35.186, P < 0.001) although there was no statistical significance in patients' age and platelet, alanine aminotransferase, aspartate aminotransferase, total bilirubin, alkaline phosphatase, and glutamyl transpeptidase levels (all P > 0.05). The Young's modulus measurement was positively correlated with liver fibrosis ( r =0.675, P < 0.05). The AUCs of SWE in the diagnosis of≥S2, ≥S3, and S4 were 0.839, 0.820 and 0.898, respectively and the corresponding optimum cut-off values were 9.2, 10.9, and 14.4 kPa, respectively. The overall concordance rate of the liver Young' s modulus measurements vs . fibrosis stages was 57.33%. Moreover, the alkaline phosphatase level was an independent predictor for diagnostic accuracy of SWE for stage 0-1 fibrosis ( OR =1.009, 95% CI : 1.001-1.018, P =0.029). Conclusion The SWE possessed a diagnosis value for the significant fibrosis (≥S2), advanced liver fibrosis (≥S3) and liver cirrhosis (S4), although there was a low overall concordance rate in the liver Young's modulus measurements obtained using SWE vs. fibrosis stages.

ObjectiveTo explore the risk factors of stroke, and to provide reference for the prevention and health management of stroke. MethodsFrom February 2022 to March 2022, four community residents over 60 years old with stroke in Shanghai were randomly selected as the case group (n=100), and non-stroke residents were selected as the control group (n=100). The survey was in the form of questionnaires to record and compare the age, body mass index (BMI), blood lipids, blood pressure-related indicators, family history of other diseases, living habits, mood and sleep conditions of all subjects. The value of predicting the incidence of stroke among the elderly in Shanghai community was analyzed by receiver operating characteristic (ROC), and the influencing factors of stroke were analyzed by logistic model. ResultsBMI, hypertension, heart disease, diabetes, transient ischemic attack (TIA), dyslipidemia, family history of stroke, smoking, lack of exercise or only light physical labor, SBP, DBP, TG levels were significantly higher in the case group (P<0.05). The level of HDL-C was significantly lower than that in the control group (P<0.05). BMI, SBP, DBP, TG, HDL-C predicted the incidence of stroke by ROC analysis (P<0.05). Logistic regression analysis showed that BMI≥23.820 kg·m-2, heart disease, diabetes, TIA, dyslipidemia, family history of stroke, smoking, lack of exercise or only light physical labor, SBP≥139.535 mmHg, DBP≥89.605 mmHg, TG≥1.565 mmol·L-1 and HDL-C≤1.105 mmol·L-1 were risk factors for stroke (P<0.05). ConclusionPhysical health status including blood lipids and blood pressure, family history of certain diseases, and living habits could be important risk factors for stroke in the elderly in Shanghai community. Preventive intervention measures for the above factors have important clinical significance.

Chinese Journal of Blood Transfusion 2022;35(1):114-118
Studies have shown that ABO blood group is related to the susceptibility and disease progression of SARS-CoV-2 infection, and most studies indicated that group O individuals were less likely to get infected while group A conferred a higher susceptibility to infection and propensity to severe disease. ABO blood group antigens are oligosaccharides expressed on red cells and other tissues. People with different ABO blood type have different susceptibility to a variety of pathogens, including SARS-CoV-2. There are several hypotheses to explain the differences in SARS-CoV-2 infection between ABO blood group individuals. Firstly, anti-A and/or anti-B antibodies could bind to corresponding antigens on the viral envelope and contribute to viral neutralization, thereby preventing target cell from being infected. The SARS-CoV-2 virus and SARS-CoV spike (S) proteins may be bound by anti-A isoagglutinin, which may block interactions between virus and angiotensin-converting-enzyme-2-receptor, thereby preventing entering into lung epithelial cells. Secondly, the receptor binding domain (RBD) of S protein domain can bind to antigen A expressed in respiratory epithelium and promote its infection to respiratory epithelial cells. In conclusion, most studies indicated that group O may be associated with a lower risk of SARS-CoV-2 infection while group A with a higher risk along with severe disease, and the related mechanism needs to be further studied.

Object:To observe the characteristics and predictors of alanine aminotransferase (ALT) flare in postpartum women with chronic HBV infection and high viral load who were prevented from mother-to-child transmission of HBV by anti-HBV intervention during pregnancy. Methods:Data of pregnant women seen at the Third People’s Hospital of Shenzhen liver disease outpatient clinic who met inclusion and exclusion criteria of chronic hepatitis B virus (HBV) infection were collected, including data recorded before and after antiviral therapy during pregnancy, postpartum regular 4 to 6 weeks, 12, 36, 48, 72, and 96 weeks of follow-up, results of liver function tests, HBV serological markers and HBV DNA quantitative examinations. The characteristics of normal and abnormal postnatal ALT groups were compared, and the risk factors of abnormal postnatal ALT were analyzed by logistic regression. Results:A total of 100 women with chronic HBV infection were enrolled from September 1, 2018 to December 31, 2020. All 94 women with chronic HBV infection experienced virological recurrence within 12 weeks after delivery and continued to be HBeAg positive. The rate of ALT flare (>ULN) was 60%, most of which occurred within 12-24 weeks after delivery, and most of which returned to normal without special treatment. However, ALT flare still occurred in 27% of cases within 48-96 weeks after delivery. A total of 24 patients with postnatal ALT≥5×ULN were treated with antiviral therapy again. Logistic multifactor regression analysis suggested that elevation of ALT (>ULN) after delivery and drug discontinuation was associated with ALT elevation occurred during delivery, the odds ratio (OR) was 1.089.Conclusions:ALT flare in postpartum women is common, and most of them can return to normal without special treatment. However, ALT flare still occurs after 48 weeks of delivery, and antiviral therapy is needed again. Therefore, long-term follow-up should be standardized for pregnant women with high viral load and chronic HBV infection after delivery.
