1.Study on the safety and pharmacological effect on improving dyspepsia of Shuangshu decoction in rats

OBJECTIVE To study the safety of Shuangshu decoction in rats and its efficacy in improving functional dyspepsia (FD) in rats. METHODS In safety test, 40 rats were divided into blank control group, Shuangshu decoction low-dose, medium- dose and high-dose groups [108, 216, 324 g/(kg·d), calculated by raw medicine, the same applies below]; they were given relevant medicine intragastrically, for continuous 14 days. The mortality and toxic reactions of rats were recorded, and the organ indexes of the liver, kidney, spleen, lung and heart of rats were calculated; the pathological morphological changes in the liver, kidney, spleen, lung, heart, stomach, duodenum, and colon were observed to evaluate the acute toxicity of Shuangshu decoction. Another 40 rats were grouped and administered in the same way for 30 consecutive days. The mortality and toxic reactions of the rats were recorded, and the corresponding organ indexes were calculated. The pathological morphological changes in the corresponding organs were observed, and blood routine and serum biochemical indicators were measured, in order to assess the subacute toxicity of Shuangshu decoction. In pharmacodynamic experiments: 50 rats were divided into blank control group, model group, and Shuangshu decoction low-, medium-, and high-dose groups (9.45, 18.9, 37.8 g/kg), with 10 rats in each group. Except for blank control group, rats in all other groups were used to establish the FD rat model by subcutaneous injection of loperamide (3.5 mg/kg). Rats in each group were administered the corresponding drug solution/normal saline intragastrically, once a day, for 14 consecutive days. After the last medication, fecal moisture content, intestinal propulsion rate, gastric emptying rate and serum level of motilin were all detected, and interstitial cell of Cajal (ICC) ultrastructure of rats was observed in colon tissue. RESULTS The safety experiments showed that no death occurred in each dose group, and no significant difference was found in organ coefficient, routine blood and serum biological index, compared to blank control group (P>0.05); no abnormality was found in organ appearance and pathological sections. The results of the pharmacodynamic experiments showed that, compared with the blank control group, the fecal moisture content, gastric emptying rate, intestinal propulsion rate, and serum motilin levels in the model group were significantly decreased (P<0.05); in the colonic tissue, the mitochondria in the ICC exhibited severe swelling with the disappearance of cristae, and the endoplasmic reticulum was dilated. Compared with model group, the rats in Shuangshu decoction high-dose group showed significant increases in the above quantitative indicators (P< 0.05); additionally, there was a large number of mitochondria in the ICC of the colonic tissue, with clear cristae and regular arrangement. CONCLUSIONS Shuangshu decoction is safe and has a beneficial improving effect on FD rats; its mechanism of action may be related to the regulation of gastrointestinal hormone expression to promote gastric emptying and intestinal propulsion, as well as the repair of mitochondrial structure in ICCs to restore gastrointestinal function.
2.Analysis of risk factors for piracetam-associated thrombocytopenia and the establishment of risk prediction model

OBJECTIVE To analyze the risk factors contributing to piracetam-associated thrombocytopenia and develop a predictive model for risk prediction. METHODS The electronic medical record information of inpatients treated with piracetam was collected retrospectively from the First Affiliated Hospital of Guangxi Medical University from January 2021 to December 2023, including gender, age, underlying diseases, combined medication, and laboratory data, etc. Patients were divided into the occurrence group and the non-occurrence group according to whether thrombocytopenia occurred, and the differences in clinical data between the two groups were compared. The independent risk factors were determined through univariate/multivariate Logistic regression analysis. A nomogram was drawn to visually present the independent risk factors, and a risk prediction model was constructed. The predictive efficacy of the model was evaluated using the receiver operating characteristic (ROC) curve, Bootstrap internal validation and calibration curve. RESULTS A total of 224 patients were included, among which 196 cases were in the non- occurrence group and 28 cases in the occurrence group. The incidence of thrombocytopenia was 12.50%. The results of the univariate Logistic regression analysis showed that the proportion of patients using three or more combined antibiotics and the level of serum creatinine in the occurrence group were significantly higher than those in the non-occurrence group, while the level of hemoglobin was significantly lower (P<0.05). The results of the multivariate Logistic regression analysis revealed that the use of three or more combined antibiotics, low hemoglobin level and high serum creatinine level were independent risk factors for piracetam-associated thrombocytopenia (P<0.05). The constructed risk prediction model was LogitP= -1.114+1.256×three or more combined antibiotics-0.017×hemoglobin level+0.009×serum creatinine level. The AUC of the ROC curve of this model was 0.757, and the optimal cut-off value was 0.474; the AUC of the ROC curve of the Bootstrap internal validation was 0.733; the apparent curve and the bias-corrected curve were close to the ideal curve. CONCLUSIONS The use of three or more antibiotics, along with low hemoglobin level and high serum creatinine level, are identified as independent risk factors for piracetam-associated thrombocytopenia. The developed risk prediction model demonstrates good predictive value.
3.Influencing factors for chronic pancreatitis complicated by pancreatogenic portal hypertension and establishment of a predictive model

Objective To investigate the influencing factors for chronic pancreatitis(CP)complicated by pancreatogenic portal hypertension(PPH),and to establish a predictive model.Methods A retrospective analysis was performed for the clinical data of 99 patients with CP complicated by PPH who were hospitalized in The First Affiliated Hospital of Kunming Medical University,Chuxiong Yi Autonomous Prefecture People's Hospital,Wenshan People's Hospital,and Puer People's Hospital from January 2017 to December 2022,and these patients were enrolled as PPH group.The incidence density sampling method was used to select 198 CP patients from databases as control group.The independent-samples t test was used for comparison of normally distributed continuous data between two groups,and the Mann-Whitney U test was used for comparison of non-normally distributed continuous data between two groups;the chi-square test or the Fisher's exact test was used for comparison of categorical data between two groups.The Least Absolute Shrinkage and Selection Operator(LASSO)regression model was used to identify the potential predictive factors for CP complicated by PPH,and the predictive factors obtained were included in the multivariate Logistic regression analysis to obtain independent risk factors,which were used to establish a nomogram prediction model.The receiver operating characteristic(ROC)curve,the calibration curve,and the Hosmer-Lemeshow goodness-of-fit test were used to perform internal validation of the model,and the clinical decision curve was used to assess the clinical practicability of the model.Results There were significant differences between the two groups in sex,history of recurrent acute pancreatitis attacks,acute exacerbation of CP,bile duct stones,peripancreatic fluid accumulation,pseudocysts,pulmonary infection,elevated C-reactive protein(CRP),elevated procalcitonin,fibrinogen(FIB),neutrophil-lymphocyte ratio(NLR),gamma-glutamyl transpeptidase,total bilirubin,direct bilirubin,low-density lipoprotein(LDL),serum amylase,D-dimer,and serum albumin(all P<0.05).The predictive variables obtained by the LASSO regression analysis included sex,recurrent acute pancreatitis attacks,bile duct stones,peripancreatic fluid accumulation,pulmonary infection,pseudocysts,CRP,NLR,FIB,and LDL.The multivariate Logistic regression analysis showed that sex(odds ratio[OR]=2.716,P<0.05),recurrent acute pancreatitis attacks(OR=2.138,P<0.05),peripancreatic fluid accumulation(OR=2.297,P<0.05),pseudocysts(OR=2.805,P<0.05),and FIB(OR=1.313,P<0.05)were independent risk factors for CP complicated by PPH.The above factors were fitted into the model,and the Bootstrap internal validation showed that the nomogram model had an area under the ROC curve of 0.787(95%confidence interval:0.730—0.844),and the calibration curve was close to the reference curve.The Hosmer-Lemeshow goodness-of-fit test showed that the model had a good degree of fitting(χ2=7.469,P=0.487).The clinical decision curve analysis showed that the prediction model had good clinical practicability.Conclusion Male sex,recurrent acute pancreatitis attacks,peripancreatic fluid accumulation,pseudocysts,and FIB are independent risk factors for CP complicated by PPH,and the nomogram model established has good discriminatory ability,calibration,and clinical practicability.
4.Characteristics and predictors of ALT flare in postpartum women with tenofovir blocking mother-to-child transmission of HBV after drug discontinuation

Object:To observe the characteristics and predictors of alanine aminotransferase (ALT) flare in postpartum women with chronic HBV infection and high viral load who were prevented from mother-to-child transmission of HBV by anti-HBV intervention during pregnancy.Methods:Data of pregnant women seen at the Third People’s Hospital of Shenzhen liver disease outpatient clinic who met inclusion and exclusion criteria of chronic hepatitis B virus (HBV) infection were collected, including data recorded before and after antiviral therapy during pregnancy, postpartum regular 4 to 6 weeks, 12, 36, 48, 72, and 96 weeks of follow-up, results of liver function tests, HBV serological markers and HBV DNA quantitative examinations. The characteristics of normal and abnormal postnatal ALT groups were compared, and the risk factors of abnormal postnatal ALT were analyzed by logistic regression.Results:A total of 100 women with chronic HBV infection were enrolled from September 1, 2018 to December 31, 2020. All 94 women with chronic HBV infection experienced virological recurrence within 12 weeks after delivery and continued to be HBeAg positive. The rate of ALT flare (>ULN) was 60%, most of which occurred within 12-24 weeks after delivery, and most of which returned to normal without special treatment. However, ALT flare still occurred in 27% of cases within 48-96 weeks after delivery. A total of 24 patients with postnatal ALT≥5×ULN were treated with antiviral therapy again. Logistic multifactor regression analysis suggested that elevation of ALT (>ULN) after delivery and drug discontinuation was associated with ALT elevation occurred during delivery, the odds ratio (OR) was 1.089.Conclusions:ALT flare in postpartum women is common, and most of them can return to normal without special treatment. However, ALT flare still occurs after 48 weeks of delivery, and antiviral therapy is needed again. Therefore, long-term follow-up should be standardized for pregnant women with high viral load and chronic HBV infection after delivery.
5.Effect of Increasing Efficacy and Decreasing Toxicity of Limax Extract on Cyclophosphamide in the Treatment of Hepatocellular Carcinoma

OBJECTIVE:To investigate the effect of increasing efficacy and decreasing toxicity of Limax extract (LE)on cyclophosphamide(CTX)in the treatment of hepatocellular carcinoma. METHODS :The mice were randomly divided into normal group,model group ,CTX group (0.02 g/kg),LE low-dose ,medium-dose and high-dose groups (LEL,LEM,LEH group ,0.6,1.2,2.4 g/kg),CTX+LE low-dose ,medium-dose and high-dose combination groups (CTX+LEL,CTX+LEM,CTX+ LEH group ,the same dose as single drug group ),with 10 huangrenbin518@163.com mice in each group. Except for normal group ,other groups were inoculated with hepatoma cells H 22 in the left ar mpit to establish tumor bearing models. After 24 h of inoculation ,normal group and model group were intragastrically given normal saline , and administration groups were intragastrically given corresponding drugs ,once a day ,for 10 days. On the second day after the last administration ,the general conditions of mice in each group were observed ;the body mass ,thymus index (LI),spleen index (SI)were measured ;the tumor inhibition rate was detected. The effect (q)of combination therapy was evaluated by King ’s formula . The counts of WBC ,RBC and PLT ,serum contents of ALT ,ALT,Scr and BUN were detected in model group ,CTX group and combination groups ,and the contents of MDA,SOD and GSH ,the levels of VEGF ,TNF-α and IL-6 in the tumor tissue were detected by colorimetry and ELISA in above groups. The protein expression of oncogenes (p53,Bcl-2 and Bax )were detected by immunohistochemical method in model group,CTX group and CTX+LEM group. RESULTS :The mice in the model group were in poor spirit and had symptoms of excessive drinking and eating ;although the body weight ,TI and SI were not significantly abnormal compared with normal group (P>0.05),WBC count and AST content were significantly increased ,ALT and BUN contents were significantly decreased (P< 0.05 or P<0.01). Compared with model group ,above symptoms of mice were all improved in administration groups. The tumor weight of administration groups ,TI and SI of CTX group and TI of combination groups were decreased significantly ,but tumor weight of LEL group and LEH group ,TI and SI of LE single groups and combination groups were significantly higher than CTX group;tumor weight of combination groups were significantly lower than CTX group (P<0.05 or P<0.01). The tumor inhibition rates of administration groups were 29.58%-72.08%. The q values of CTX+LEL group ,CTX+LEM group and CTX+LEH group were 1.03,0.97 and 0.86,respectively. Compared with model group ,WBC count ,AST and BUN contents of CTX group ,MDA contents of combination groups ,VEGF,TNF-α and IL-6 levels of administration groups ,the protein expression of Bcl- 2 in CTX group and CTX+LEM group were decreased significantly ;the activities of SOD and GSH of administration groups ,the protein expression of p 53 in CTX+LEM group and Bax in CTX group ,CTX+LEM group were increased significantly (P<0.05 or P< 0.01);WBC counts and AST contents of administration groups ,ALT content of CTX+LEM group ,SOD activity of CTX+LEH group and GSH activity of CTX+LEM group were all significantly higher than those of CTX group ;MDA content of CTX+LEH group,VEGF and TNF-α levels of CTX+LEM group and CTX+LEH group,IL-6 levels of administration groups were all significantly lower than CTX group (P<0.05 or P<0.01). CONCLUSIONS :LE combined with CTX can increase the anti-tumor effect,and LE can reduce the toxicity of CTX induced immunosuppression and bone marrow suppression in mice ,with effect of increasing efficacy and decreasing toxicity. The effect may be related to antioxidant stress ,inhibition of angiogenesis and secretion of inflammatory factors ,and regulation of apoptosis protein expression.
6.Determination of hydrazine in workplace air by gas chromatography with large bore capillary column

Objective:To determine hydrazine quantitatively in workplace air by gas chromatography with large bore capillary column.Methods:In October 2019, hydrazine in the air was adsorbed by acid silica gel tube sampling and desorped using sulfuric acid solution. After derivatization with furfural and extraction, the content of hydrazine was determined by DM-FFAP capillary column gas chromatography with flame ionization detector.Results:The linear regression equation was y=353.8 x+21.2 ( r=0.9998) between 0.1-2.0 μg/ml of target concentration. The detection limit was 0.030 μg/ml. The lower limit of quantification was 0.100 μg/ml. If 15 L air sample was collected, the minimum detection concentration was 0.004 mg/m 3 and the minimum quantitative concentration was 0.013 mg/m 3 respectively. The average desorption efficiency was 86.5%-89.4%. The recovery was 94.4%-97.1%. The relative standard deviation was 1.6%-4.9%. Hydrazine and furfural derivative was 2-furaldehyde hydrazine. Conclusion:The method has symmetrical peak shape of hydrazine derivatives chromatographic peaks, short analysis time, easy operation, and is suitable for the determination of the concentration of hydrazine in the air in the workplace.
7.The prognostic impact of diabetic mellitus and hyperglycemia during DLBCL treatment on patients with diffuse large B-cell lymphoma

Objective:This study aims to investigate the clinical features and prognostic factors of patients with diffuse large B-cell lymphoma (DLBCL) and assess the prognostic value of diabetes mellitus (DM) and hyperglycemia during DLBCL treatment in DLBCL.Methods:The clinical data of 481 newly diagnosed DLBCL patients from January 1, 2009 to December 31, 2019 at Tianjin Medical University Cancer Institute and Hospital and Sun Yat-sen University Cancer Center were retrospectively collected, focusing on their blood glucose levels before and during treatment. Cox regression method was used for univariate analysis to assess prognostic factors, and the Kaplan-Meier method was used to draw survival curves to assess the prognostic value of DM and hyperglycemia during DLBCL treatment in patients with DLBCL.Results:Eighty-two (17.0%) patients had DM before DLBCL diagnosis and treatment, and 88 (18.3%) patients had at least one blood glucose increase during DLBCL treatment. Cox univariate analysis showed that age, Ann Arbor stage, international prognostic index, and DM were associated with overall survival (OS) and progression-free survival (PFS) (all P<0.05) . The pairwise comparison between the two groups showed that the OS ( P=0.001) and PFS ( P<0.001) of patients with pre-existing DM were significantly worse than those of patients without abnormal blood glucose. Moreover, the OS ( P=0.003) and PFS ( P<0.001) of patients with hyperglycemia during DLBCL treatment were significantly worse than those of patients without abnormal blood glucose. No significant difference exists between patients with DM and patients with hyperglycemia during DLBCL treatment (OS, P=0.557; PFS, P=0.463) . Additionally, patients with adequate glycemic control during chemotherapy had a better prognosis compared with patients with poor glycemic control (OS, P=0.037; PFS, P=0.007) . Conclusion:DM is an important factor affecting the prognosis of patients with DLBCL. Moreover, hyperglycemia during treatment is related to the poor prognosis of patients with DLBCL.

9. Epidemiological and clinical characteristics analysis of 30 childhood cases with 2019 novel coronavirus infection in Shenzhen

Objectives:
To analyze the epidemiological and clinical characteristics of children with 2019 novel coronavirus (2019-nCoV) infection in Shenzhen.
Methods:
The data of 30 children diagnosed with 2019-nCoV infection in the Third People’s Hospital of Shenzhen from 16th January 2020 to 9th February 2020were collected.
Results:
Among the 30 children, 14 were boys and 16 were girls. There were 10 mild cases, 13 common cases and one severe case, and six cases with asymptomatic infection. The age ranged from 7 months to 18 years old with the median age of 7 years old. Twenty out of 30 cases (66.7%) were school children. The common clinical characteristics were fever (30.0%, 9/30) and cough (23.3%, 7/30). The body temperature waved below 37.5 ℃. Mostly the auscultations of the lungs were no rales and there was no extrapulmonary complication. A total number of one case had wheezes and hypoxia, and one case had diarrhea and vomiting. There was no critical and death case. There were 29 cases with travelling experience in Hubei province within two weeks, and 24 cases (80.0%) had relatives (parents or grandparents) diagnosed with 2019-nCoV infection. Elevated white blood cell counts (﹥12×109/L), C reaction protein level, lactate dehydrogenase level and the low proportion of T help cells occurred in three, five, five and three cases, respectively. Some cases were coinfected with human respiratory syncytial virus, mycoplasma pneumonia, human herpesvirus, influenza B virus and rubella virus. The predominant pattern of computed tomography findings of childhood patients with 2019-nCoV infection presented with patchy film and ground-glass opacities in bilateral or unilateral lung. The median time for nucleic acid to turn negative was eight days among the enrolled cases. All the cases were cured and discharged home, and the days in hospital waved from 5 - 16 days (the median time was 12 days).
Conclusions
The majority of the childhood cases are the school-age children with family cluster. Most cases present mild and common symptoms with good prognosis. Some patients may be complicated with multiple infections.
10. ECMO application of patients with Critical Corona Virus Disease 2019 and fulminant myocarditis

Objective To evaluate the clinical experience of extracorporeal membrane oxygenation (ECMO) treatment on two cases of infection with the critical Corona Virus Disease 2019 (COVID-19) complicated by fulminant myocarditis (FM) . Methods This study selects two COVID-19 cases comorbid with fulminant myocarditis and had been treated with ECMO in Shenzhen Third People's Hospital from January 2020 to February 2020. We compare the index of inflammation, immunization, D-dimer and lactic acid before and after ECMO treatment in 24 and 96 hours, cardiopulmonary function before and after ECMO treatment in 24, 48, 72, 96 hours,. We also analyze the complications and clinical outcomes of the two cases during the ECMO treatment. Results Both patients were elderly obese men with chronic cardiopulmonary disease. Comparing the laboratory test results and imaging data of the two patients, the acute lung injury score, oxygenation index, albumin level, hypersensitive C-reactive protein, lactate and lactate dehydrogenase levels in 2 patients after ECMO treatment were improved as compared with those before ECMO treatment. Finally, case 1 died of multiple organ failure and his cardiac function continued to deteriorate, while, case 2 successfully withdrew and his cardiac function gradually improved. Conclusions For critical COVID-19 patients with fulminant myocarditis, ECMO treatment can improve pulmonary function in the short term, provide valuable time for rescuing COVID-19 patients with fulminant myocarditis.
