1.Efficacy and Mechanism of Lutongning Granules in Treatment of Trigeminal Neuralgia Induced by Injection of Talc into Infraorbital Foramen of Model Rats Based on P2X7R-mediated Neuroinflammation

ObjectiveTo evaluate the effectiveness of Lutongning granules in the treatment of trigeminal neuralgia in animal models and study its mechanism of action, so as to provide laboratory data support for the clinical application of Lutongning granules and precise treatment. MethodMale SD rats were randomly divided into normal group, model group, carbamazepine group (0.06 g·kg-1·d-1), high-dose Lutongning group (2.70 g·kg-1·d-1), and low-dose Lutongning group (1.35 g·kg-1·d-1) according to the stratified basic mechanical pain thresholds, with 10 rats in each group. A trigeminal neuralgia model of rats was prepared by injecting 30% talc suspension into the infraorbital foramen area of the rat. The drug groups were administered 10 mL·kg-1 of drugs by gavage after 2 h of modeling. The normal group and the model group were administered distilled water by gavage under the same conditions once a day for 10 consecutive days. Von Frey brushes were used to determine the mechanical pain threshold of rats. A fully automated blood and body fluid analyzer was employed to detect the blood routine of rats. Hematoxylin and eosin (HE) staining was utilized to detect the pathological changes in the trigeminal ganglion and medulla oblongata tissue. Transmission electron microscopy was used to scan the ultrastructure of the medulla oblongata tissue. Enzyme-linked immunosorbent assay (ELISA) was used to detect the levels of inflammatory factors interleukin (IL)-1, IL-6, IL-8, tumor necrosis factor (TNF)-α, neuropeptide substance P, and β-endorphins (β-EP) in the serum of rats, and Western blot was used to detect the protein expression levels of IL-1β, purinergic receptor P2X7 (P2X7R), and phosphorylated p38 mitogen-activated protein kinase (p-p38 MAPK). ResultCompared with that in the normal group, the pain threshold of rats in the model group was significantly lower (P<0.01). The absolute value of neutrophils (NEUT#) and the percentage of neutrophils (NEUT) were significantly improved, and the percentage of lymphocytes (LYMPH) was significantly reduced (P<0.01). The serum levels of IL-1, IL-6, IL-8, and TNF-α were significantly increased (P<0.01). SP content in brain tissue was significantly increased, and β-EP content was significantly decreased (P<0.01). The relative protein expression of IL-1β, P2X7R, and p-p38 MAPK was significantly increased (P<0.05). HE staining and transmission electron microscopy results of medulla oblongata tissue revealed neuronal degeneration, mild proliferation of microglial cells, reduction in the number of myelinated nerves, and obvious demyelination. The trigeminal nerve fibers of rats were disarranged, and some nerve fibers showed vacuolization. Axons were swollen, and Schwann cells proliferated. Demyelination was observed. Compared with the model group, each administration group significantly increased the pain threshold of rats (P<0.05, P<0.01), reduced NEUT# and NEUT, and elevated LYMPH (P<0.05, P<0.01). The administration group significantly decreased the levels of IL-1, IL-6, IL-8, and TNF-α in serum and SP in brain tissue (P<0.01) and increased the level of β-EP (P<0.01). They significantly down-regulated the protein expression of IL-1β, P2X7R, and p-p38 MAPK(P<0.05, P<0.01) and significantly ameliorated the pathological changes in medulla oblongata tissue and trigeminal nerves of rats. ConclusionLutongning Granules had significant therapeutic effects on trigeminal neuralgia induced by injection of talc into the infraorbital foramen of model rats, and the mechanism may be related to amelioration of P2X7R-mediated neuroinflammation and inhibition of demyelination of myelinated nerves.
2.Effect of MEIS1 expression on survival in patients after radical gastrectomy and its value in prognostic evaluation

Objective:To discuss the postoperative survival of the gastric cancer patients with different expression levels of myeloid ecotropic viral integration site 1(MEIS1),and to analyze the predictive value of MEIS1 expression in the prognosis evaluation of gastric cancer.Methods:In a gastric cancer survival cohort,215 patients who underwent radical gastrectomy were selected.Immunohistochemical staining was used to detect the expression levels of MEIS1 in both gastric cancer and adjacent normal tissues.The relationship between expression level of MEIS1 and the clinicopathological characteristics of the patients were analyzed by x2 test or Fisher's exact probability method;survival curves were plotted by Kaplan-Meier method;the differences in survival of the patients between MEIS1 high expression group and MEIS1 low expression group were compared by Log-rank test;multivariate Cox proportional hazards regression model was used to calculate the hazard ratios(HR)and 95%confidence intervals(CI)to assess the relationship between MEIS1 expression level and the survival of the gastric cancer patients.Results:The immunohistochemical staining result showed that the expression level of MEIS1 in gastric cancer tissue was decreased.The univariate analysis results showed that the patients with high MEIS1 expression had a longer overall survival than those with low expression(P=0.049),and had a better prognosis.The multivariate Cox proprotional hazards regression analysis results showed that the low MEIS1 expression and high TNM stage were the independent risk factors for poor prognosis of the patients with gastric cancer(HR=1.577,95%CI:1.011-2.460,P=0.045;HR=2.709,95%CI:1.708-4.297,P<0.001).Conclusion:The gastric cancer patients with low expression of ME1S1 have a shorter postoperative overall survival;MEIS1 is a promising biomarker for prognosis assessment of the patients after radical gastrectomy.
3.Establishment and clinical application of ibrutinib blood concentration assay

OBJECTIVE To establish a method to detect the blood concentration of ibrutinib and apply it to the clinic. METHODS Using zanubrutinib as internal standard, the concentration of ibrutinib was detected by high performance liquid chromatography (HPLC) after plasma samples were processed by solid-phase extraction. The separation was performed on an Agilent 5 TC-C18(2) column with acetonitrile-0.5% potassium dihydrogen phosphate solution (43∶57, V/V) as the mobile phase at a flow rate of 1 mL/min, a detection wavelength of 260 nm, a column temperature of 40 ℃ , a sample size of 20 μL, and a run time of 25 min. The concentration of ibrutinib was measured in the plasma of 9 patients with non-Hodgkin’s lymphoma 2 h after drug administration on the 30th day by the above method. RESULTS The linear range of the assayed mass concentration of ibrutinib was 10-500 ng/mL (R 2=0.998 9), the lower limit of quantification was 10 ng/mL, and the RSDs of the intra-batch and inter-batch precision tests were not higher than 12.77%. The recoveries of the extraction were 74.80% and 97.70%, with both RSDs<2.90%, and the RSDs of the stability tests were not higher than 7.10%. The peak plasma concentrations of 9 patients were 15.341-279.628 ng/mL. CONCLUSIONS The established HPLC method is simple and rapid, and can be used for the determination of ibrutinib concentration in plasma samples.
4.Study on the mechanism of Wenyang huazhuo tongluo formula contained serum inhibiting the proliferation of Th17 cells in scleroderma patients

OBJECTIVE To study the effects of Wenyang huazhuo tongluo formula conta ined serum on the expression and methylation of retinoic acid related nuclear orphan receptor (RORγt),and to explore the mechanism of its regulation of Th 17 cell proliferation in the treatment of scleroderma. METHODS Wistar rats were given 15,30,60 g/(kg·d)Wenyang huazhuo tongluo formula intragastrically to prepare different doses of drug-contained serum ,and peripheral blood of scleroderma patients were collected to sort Th 17 cells. Using blank serum as blank control ,methyltransferase inhibitor decitabine (DCA)as positive control , Th17 cells were treated with different concentrations of drug-contained serum ,and then the proliferation of Th 17 cells was detected by CCK- 8;mRNA expressions of RORγt and IL-17 were detected by qRT-PCR ,and the protein expression of RORγt was detected by Western blot assay ;the protein expression of IL- 17 in the cell supernatant was detected by ELISA ,and the methylation level of RORγt gene promoter was detected by methylation-specific PCR. The transcriptional activity of RORγt gene promoter was detected by dual-luciferase assay. RESULTS Compared with blank control ,Wenyang huazhuo tongluo formula contained serum and DCA could inhibit the proliferation of Th 17 cells(P<0.05),reduced the mRNA and protein expressions of RORγt and IL-17,enhanced the methylation level of RORγt gene promoter and attenuated , the transcription activity of RORγt gene promoter. Except for mRNA expression of Th 17 and the methylation level of RORγt gene promoter in drug-contained serum low-dose group ,there were statistical significance in above indexes of other groups(P<0.05). CONCLUSIONS Wenyang huazhuo tongluo formula can promote the methylation lev el of RORγt gene,and inhibit the expression of RORγt and the secretion of IL-17,so as to inhibit the proliferation of Th 17 cells.
5.Guidance of critical thinking of clinical medical students in disease-centered PBL integrated course: taking diabetes as example

Disease-centered problem-based learning (PBL) integrated course inspires the thinking of medical students in the case scenario to stimulate students' motivation of active learning. In this paper, the study of diabetes cases was taken as an example. Through the design of PBL cases, the scenario was reconstructed and information was provided step by step, so as to induce the students to discuss and learn the related knowledge of glucose metabolism and understand the predisposing factors of diabetes. Furthermore, students' critical thinking could be inspired through the information of the misdiagnose and mistreatment to recognize the clinical presentation and inducement of diabetic ketoacidosis. This teaching model is conducive to the cultivation of medical students' questioning spirit and critical thinking, laying a foundation for the cultivation of innovative medical talents.
6.Effect of psychological intervention on postoperative outcomes of adolescent patients undergoing scoliosis correction: a Meta-analysis

Objective:To evaluate the effect of psychological intervention on postoperative outcomes of adolescent idiopathic scoliosis (AIS) patients undergoing correction.Methods:A computer-based online search of PubMed, Embase, Cochrane Library, Web of Science, CINAHL, MEDLINE, PsycINFO, PQDT, China National Knowledge Infrastructure (CNKI) , Wanfang Database, VIP Chinese Journal Database, China Biology Medicine disc, controlled-trials and chictr clinical controlled trial databases from establishment to July 25, 2021 was performed for randomized controlled trials (RCTs) on the effect of psychological intervention on adolescent AIS patients undergoing correction surgery. RevMan 5.3 statistical software was used for meta-analysis.Results:Finally, 10 articles were included, with a total of 621 patients. Meta-analysis showed that psychological intervention could reduce postoperative pain ( SMD=-0.62, 95% CI: -1.15- -0.08, P=0.02) and reduce postoperative anxiety ( SMD=-0.54, 95% CI: -1.03- -0.06, P=0.03) , descriptive analysis showed that cognitive behavioral therapy could improve postoperative quality of life and mobility. Conclusions:Psychological intervention can reduce the pain and anxiety level of patients with scoliosis after surgery, improve the quality of life after surgery and improve the activity ability. However, due to the small number of studies and high heterogeneity of results, higher quality and large sample studies are needed to further validate the intervention effect and provide guidance for clinical practice.
7.Research progress of noninvasive diagnosis of male bladder outlet obstruction

Currently, invasive urodynamic testing is the " gold standard" for the diagnosis of bladder outlet obstruction (BOO). However, this test is invasive, easy to cause hematuria, urinary tract infection and other complications, the application conditions are limited.In order to solve this problem, various non-invasive methods to diagnose or predict BOO have been studied.The use of existing inspection indicators such as ultrasound measurement, penile cuff test, near infrared spectroscopy and other new non-invasive methods provide a new research direction for the non-invasive diagnosis of bladder outlet obstruction.
8.Preventive effect of puerarin on the formation of deep venous thrombosis of lower extremity after hip surgery in elderly women with osteoporosis

Objective To observe the preventive effects of puerarinon development of deep vein thrombosis (DVT) in elderly women with osteoporosis after hip replacement. Methods 100 elderly women with osteoporosis who scheduled for femoral head arthrolastybetween January 2012 and January 2014 in Cangzhou Central Hospital were selected and then randomly divided into a study group (n=50) and a control group (n=50). The control group received routine anti?inflammatory and anticoagulant therapy ,while the study group received intravenous drip of puerarinof 400 mg mixed with 5%glucose liquid 500 ml daily for 15 days as a treatment course. After the treatment, DVT incidence rate ,change of early diagnostic indexes and hemorheology were compared with their baselines. Results DVT incidence rate was significantly lowerin the study group than in the control group (8.0%vs. 24.0%, P<0.05). Ascompare with the control group,thromboxane A2/prostacyclin,plasma homocysteine and hemorheology?were significantly reduced (P < 0.05). Conclusions Puerarin has a preventive effect for deep vein thrombosis in elderly women with osteoporosis after hip replacement.
9.Circulating Tumor DNA as Biomarkers for Cancer Detection

Detection of circulating tumor DNAs (ctDNAs) in cancer patients is an important com-ponent of cancer precision medicine ctDNAs. Compared to the traditional physical and biochemical methods, blood-based ctDNA detection offers a non-invasive and easily accessible way for cancer diagnosis, prognostic determination, and guidance for treatment. While studies on this topic are currently underway, clinical translation of ctDNA detection in various types of cancers has been attracting much attention, due to the great potential of ctDNA as blood-based biomarkers for early diagnosis and treatment of cancers. ctDNAs are detected and tracked primarily based on tumor-related genetic and epigenetic alterations. In this article, we reviewed the available studies on ctDNA detection and described the representative methods. We also discussed the current understanding of ctDNAs in cancer patients and their availability as potential biomarkers for clin-ical purposes. Considering the progress made and challenges involved in accurate detection of speci-fic cell-free nucleic acids, ctDNAs hold promise to serve as biomarkers for cancer patients, and further validation is needed prior to their broad clinical use.
10.Application of ultrasound guidance in peripheral venous access:a Meta-analysis

Objective To systematically evaluate effects of ultrasound guidance in peripheral venous access. Methods A search of CNKI,CBM,VIP,WanFang database,MEDLINE,EMbase,Cochrane library was conducted to identify the random control trials (RCTs) about the peripheral veins catheter by ultrasound guidance from establisment to August 2015, RevMan 5.3 software was used for Meta-analysis. Results A number of 8 RCTs including 395 patients were involved in the study. Meta-analysis showed that ultrasound guidance could improve the success rate of the first attempt (Z=2.19,P<0.05) as well as the overall success rate (Z=3.08,P< 0.05). It reduced the numbers of the peripheral venous catheterization (Z=3.18,P< 0.05), but could not reduce the puncture time (Z=0.02,P> 0.05). Conclusions Ultrasound guidance can be promoted in clinical especially for difficult peripheral vein catheterization. Other methods should be applied for urgent and severe patients to reduce the time for the treatment.
