1.Customized maxillary incisor position relative to dentoskeletal and soft tissue patterns in Chinese women: A retrospective study

Objective:
To provide reliable prediction models based on dentoskeletal and soft tissue variables for customizing maxillary incisor positions and to optimize digitalized orthodontic treatment planning.
Methods:
This study included 244 Chinese women (age, 18–40 years old) with esthetic profiles after orthodontic treatment with fixed appliances (133 in group I: 1° ≤ The angle between the nasion [N]-A point [A] plane and the N-B point [B] plane [ANB] ≤ 4°; 111 in group II: 4° < ANB ≤ 7°). Dental, skeletal, and soft tissue measurements were performed on lateral cephalograms of the participants. Correlation and multiple linear regression analyses were used to determine the influence of dentoskeletal and soft tissue variables on maxillary incisor position.
Results:
The ideal anteroposterior position of the maxillary incisor varied between sagittal skeletal patterns. The position of the maxillary incisor correlated with the sagittal discrepancy between the maxilla and the mandible (ANB), protrusion of the midface, nasal tip projection, development of the chin, and inclination of both the maxillary and mandibular incisors. Distance from the maxillary central incisor to nasion-pogonion plane predicted using multiple linear regression analysis was accurate and could be a practical measurement in orthodontic treatment planning.
Conclusions
Instead of using an average value or norm, orthodontists should customize a patient’s ideal maxillary incisor position using dentoskeletal and soft tissue evaluations.
2.Application of neural network model and logistic regression in the prediction of chronic obstructive pulmonary disease

Objective To establish a mathematical prediction model for chronic obstructive pulmonary disease (COPD) by applying an artificial neural network (ANN) and logistic regression analysis method. Methods A cross-sectional survey was conducted in 2015 to collect epidemiological data of COPD of 2 400 residents from Hubei Province. Subjects were randomized into training group and test group at a ratio of 7:3. The prediction models of COPD were established using ANN and logistic multiple regression. The predictive performance of the two models was compared. Results Information from a total of 1 569 subjects was valid and analyzed, including 1,099 cases in the training group and 470 cases in the test group. The area under curve (AUC) of ANN for training group and test group was 0.80 and 0.78, respectively. The AUC of logistic regression for training group and test group was 0.75 and 0.74, respectively. Conclusion It is feasible to apply ANN and logistic regression models to predict COPD, which can provide scientific evidence for COPD prevention and treatment.
3.Systematic Review of Cost-utility Analysis Methodology for PD- 1/PD-L1 Inhibitors in the Treatment of Non- small Cell Lung Cancer

OBJECTIVE:To provide reference for impro ving the quality of programmed cell death protein 1 (PD-1)/ programmed cell death 1 ligand(PD-L1)inhibitors in the treatment of non-small cell lung cancer related pharmacoeconomic studies in China. METHODS :Retrieved from Embase ,PubMed,Medline,Cochrane Library ,CNKI,Wanfang database ,VIP and other Chinese and English database ,cost-utility studies about PD- 1/PD-L1 inhibitors in the treatment of non-small cell lung cancer published during Jan. 2016-Jan. 2021 were collected. The data of the included studies were extracted. After the quality of the included studies was evaluated by using the Consolidated Health Economic Evaluation Reporting Standards list ,the relevant data were summarized and compared from the aspects of model framework ,model parameters and uncertainty analysis. RESULTS & CONCLUSIONS:A total of 17 studies were finally included ,the overall quality of them was high but the differences in methodology were great. Markov model or partition survival model based on three states was adopted for 16 studies. The time horizon ranged from 5 years to lifetime ;the cycle length ranged from 1 week to 6 weeks. A total of 8 studies used the standard parameter distribution method for parameter fitting ,and 7 studies additionally adopted other parameters estimation methods as KM curves or spline models. Eleven studies performed the validation of model extrapolation. All studies considered the direct medical costs and reported the incremental cost-effectiveness ratio using quality-adjusted life years as the health outcome. Sixteen studies conducted the deterministic sensitivity analysis and probabilistic sensitivity analysis to improve the stability of the model. It is suggested that studies should keep the integrity of the report ; format,choose the appropriate positive comparators ,selectthe health economic model and construct reasonable assumptions according to the available data format , use Cholesky decomposition to explore the uncertainty of the parameter fitting , perform the validation of extrapolation combined with external data and use the appropriate indirect comparison in the absence of the head-to-head clinical trials to improve the quality of related pharmacoeconomic studies in China.
4.Preliminary study of superselective lymph node dissection in regional lateral cervical lymph node metastasis of papillary thyroid carcinoma

Objective:To explore the applied value of super-selective cervical lymph node dissection in papillary thyroid carcinoma (PTC) patients with clinically suspicious lateral lymph node metastasis (LNM).Methods:We retrospectively analyzed the clinical data of 232 cN1b PTC patients who underwent surgery from September 2013 to May 2018 in the Department of Head and Neck Surgical Oncology, National Cancer Center. Among them, 90 cases received super-selective neck dissection (level Ⅲ and IV) and 142 cases underwent selective neck dissection (level Ⅱ-Ⅳ). The LNM of two groups were analyzed.Results:Postoperative pathological results showed that 173 cases had LNM in the central compartment. The LNM cases of level Ⅱ-Ⅳ were 47, 147 and 130, respectively. Eight patients of super-selective neck dissection and 6 of selective neck dissection had postoperative lymphatic fistulas ( P=0.146). No patients in super-selective neck dissection group while 9 patients in the selective lymph node dissection group had postoperatively permanent impairment of shoulder mobility, the difference was statistically significant ( P=0.015). In the super-selective neck dissection group, 2 patients had long-term postoperative incision discomfort, and 5 cases had obvious cicatrix after surgery. In the patients with selective neck dissection, 27 cases experienced long-term incision discomfort after surgery, and 26 patients had apparent scar tissue, the differences were statistically significant ( P<0.005). There was no recurrence during the follow-up. Conclusions:Super-selective neck dissection is a feasible, safe and effective treatment for cN1b PTC patients. It can improve the quality of postoperative life and avoid the over treatment for patients.

We investigated 42 patients with hypopharyngeal squamous cell carcinoma underwent laryngeal preservation surgery and 38 patients underwent total laryngectomy by QOL questionnaire and followed up their survival.Results:The somatic function dimension, psychological function dimension, and social function dimension of patients underwent laryngeal preservation surgery were (92.46±15.71), (80.56±22.67) and (90.08±19.50), respectively, which were higher than (79.39±32.75), (68.42±25.05) and (61.84±29.55) of the total laryngectomy group ( P<0.05), while the economic dimension was not significantly different between the two groups ( P>0.05). The social function dimension (including social support and socialization, family relationship) of laryngeal preservation surgery group were (89.04±25.47) for postoperative time < 70 months and (90.94±13.28) for postoperative time ≥70 months, which were higher than (65.48±29.14) and (57.35±30.32) of the total laryngectomy group ( P<0.01). Conclusions:The somatic function dimension, psychological function and social function of patients with hypopharyngeal squamous cell carcinoma underwent laryngeal preservation surgery obtain a better QOL than patients underwent total laryngectomy. Therefore, we should improve the laryngeal function and QOL of patients under the premise of ensuring the survival rate.

8. Cognitive behavior group intervention on the depression of patients with acute leukemia

Objective:
To investigate the effect of cognitive behavioural therapy (CBT), which is dominated by nurses, on depression in patients with acute leukemia.
Methods:
Patients with acute leukemia admitted to the Department of Hematology, Ruijin Hospital, Shanghai Jiao Tong University School of Medicine were evaluated with a rapid depressive symptom (QIDS-SR16) self-rating scale. Patients with a score higher than 11 were psychiatrists from the Shanghai Mental Health Center. Interviews and Hamilton Depression Rating (HAMD-17) assessment and clinical stereotype examination (SCID-I) diagnostic screening were performed. Patients with significant depressive symptoms who met the inclusion criteria were included (without considering DSM-5 diagnosis for depressive disorder). The computer random number method was assigned to the control group (group C), the therapist-led group CBT group (group T), the group CBT group with the nurse as the main body and therapists as the supervisors (group N). The group C was given the conventional method and the group T and N was given the standardized group CBT once a week for 8 weeks. Interviews were performed at baseline (before intervention), 4 weeks, and 8 weekends to assess the HAMD scale score.
Results:
Finally, 90 patients were enrolled, 30 in each group. At the end of the 4th week, the HAMD scale score (22.60±2.71) in the group T was lower than that before the intervention (25.77±2.93), and lower than the group N (24.57±2.82) and the group C (25.90± 3.88), the difference was statistically significant (
9.Optimization of Ultrafine Pulverization Technology in Chaige Tuire Powder

OBJECTIVE:To optimize the ultrafine pulverization technology in Chaige tuire powder;to compare the content and microcharacteristics between ultrafine powder and ordinary powder. METHODS:Using contents of 3 active ingredients(puera-rin,glycyrrhizin and baicalin)and powder d(0.5)as main indexes,bulk density,angle of repose and microcharacteristic as refer-ence indexes,orthogonal test was designed to optimize the initial particle size,moisture and pulverized frequency in ultrafine pul-verization technology. Contents of 3 active ingredients of ultrafine powder and ordinary powder(over 65/80 mesh sieve)and obser-vation results of calcium oxalate crystal under microscope were compared. RESULTS:The optimized technology was as follow as over 65 mesh sieve,moisture of 2.5%preliminary powder in 60 Hz of frequency for pulverization. In verification test,d(0.5)aver-age value of 3 ultrafine powder samples was 31.5 μm(RSD=0.45%,n=3);contents of puerarin,glycyrrhizin and baicalin were 0.232 mg/g(RSD=1.31%,n=3),0.212 mg/g(RSD=1.62%,n=3),8.962 mg/g(RSD=0.89%,n=3),respectively,which were increased about 30%-40% than in ordinary powder(0.158,0.15669,6.140 mg/g). There were no or little bundles of calci-um oxalate crystal that is common in ordinary powder. CONCLUSIONS:Optimized ultrafine pulverization technology is stable and feasible;contents of 3 active ingredients in Chaige tuire ultrafine powder are higher and calcium oxalate crystal are litter than ordi-nary powder,which possibly reduces the adverse reactions in clinical application.
10.In Vitro Study on Morusin Regulating Cell Cycle Arrest and Inhibiting Cell Proliferation of ;HCT116 Cells in Colorectal Cancer

Objective To investigate the effects of Morusin on cell line proliferation, cell cycle and cell apoptosis of colorectal cancer HCT116 cell;To discuss its mechanism. Methods HCT116 cells were treated with different concentrations of Morusin for 72 h. Cell proliferation was detected by CCK-8 assay, and cell growth inhibition rate and IC50 value were calculated. HCT116 cells were treated with 25.4, 50.8 μmol/L Morusin for 24 h, 48 h and 72 h. Cell morphology was observed under the microscope, and cell proliferation was detected by CCK-8 assay. After HCT116 cell line was treated with 25.4μmol/L Morusin for 24 h, cell cycle phase distribution was detected by flow cytometry and the cell apoptosis was detected by TUNEL assay under the fluorescence microscope. Post-intervention protein expressions were detected by Western Blot. Results The inhibitory effects of Morusin on the proliferation of HCT116 cell line was concentration/time dependent and IC50 value at 72 h was 25.4μmol/L;Morusin induced the cell cycle arrest at G0/G1 phase and G2/M phase, but there was no induction of cell apoptosis;Morusin significantly decreased the expression ofβ-catenin and its target c-Myc, and downregulated the expressions of cyclinD1 and cyclinB1, which were involved in cell cycle regulation. Conclusion Morusin can inhibit HCT116 cell cycle and the proliferation of colorectal cancer cells. Its mechanism might be realized by suppressing the activity ofβ-catenin pathway and reducing the protein expressions of cyclinD1 and cyclinB1.
