1.Screening and analysis of HCV among drug users in Dehong Prefecture, Yunnan Province from 2014 to 2021

		                        			ObjectiveTo analyze the prevalence of HCV antibody positive and associated factors among drug users in Dehong Prefecture, Yunnan, and to provide scientific evidence for HCV prevention. MethodsQuestionnaire surveys and serological testing were conducted among 400 drug users continuously selected from four national AIDS sentinel surveillance in Dehong Prefecture between January and July during 2014‒2021. Results11 683 drug users were included. The prevalence of HCV antibody positive was 20.2% overall, and 14.9%, 20.1%, 22.4%, 19.8%, 22.5%, 20.6%, 24.5%, 19.0% from 2014 to 2021, respectively (trend Z=-3.78, P<0.001). Multivariable analysis indicated the following were independently associated with HCV antibody positive: that older age (OR=1.02, 95%CI: 1.02‒1.03), male (OR=1.70, 95%CI: 1.19‒2.42), unmarried (OR=1.64, 95%CI: 1.44‒1.87), divorced or widowed (OR=1.73, 95%CI: 1.48‒2.02), Jingpo ethnicity (OR=1.39, 95%CI: 1.19‒1.63), injection drug use (OR=15.46, 95%CI: 13.13‒18.12), and HIV infection(OR=4.96, 95%CI:4.12‒5.99). ConclusionThe prevalence of HCV antibody positive among drug users in Dehong Prefecture is high and increases with some fluctuations during 2014 to 2021, which highlights the need to develop interventions targeting this population. 
		                        		
		                        		
		                        		
		                        	
2.Research on three-dimensional skull repair by combining residual and informer attention.

		                        			Cranial defects may result from clinical brain tumor surgery or accidental trauma. The defect skulls require hand-designed skull implants to repair. The edge of the skull implant needs to be accurately matched to the boundary of the skull wound with various defects. For the manual design of cranial implants, it is time-consuming and technically demanding, and the accuracy is low. Therefore, an informer residual attention U-Net (IRA-Unet) for the automatic design of three-dimensional (3D) skull implants was proposed in this paper. Informer was applied from the field of natural language processing to the field of computer vision for attention extraction. Informer attention can extract attention and make the model focus more on the location of the skull defect. Informer attention can also reduce the computation and parameter count from N 2 to log( N). Furthermore,the informer residual attention is constructed. The informer attention and the residual are combined and placed in the position of the model close to the output layer. Thus, the model can select and synthesize the global receptive field and local information to improve the model accuracy and speed up the model convergence. In this paper, the open data set of the AutoImplant 2020 was used for training and testing, and the effects of direct and indirect acquisition of skull implants on the results were compared and analyzed in the experimental part. The experimental results show that the performance of the model is robust on the test set of 110 cases fromAutoImplant 2020. The Dice coefficient and Hausdorff distance are 0.940 4 and 3.686 6, respectively. The proposed model reduces the resources required to run the model while maintaining the accuracy of the cranial implant shape, and effectively assists the surgeon in automating the design of efficient cranial repair, thereby improving the quality of the patient's postoperative recovery.

3.HIV subtypes in newly reported HIV-infected cases in Dehong Prefecture of Yunnan Province during 2017 to 2019

		                        			ObjectiveTo determine the trend and influencing factors of HIV subtypes in newly reported HIV/AIDS cases in Dehong Dai and Jingpo Autonomous Prefecture (Dehong Prefecture) from 2017 to 2019. MethodsRNA extraction was conducted among newly reported HIV/AIDS cases in Dehong Prefecture from 2017 to 2019 whose plasma volume was more than 200 μL. The gag, env and pol genes were amplified by using RT-PCR and then sequenced to determine the subtypes. ResultsA total of 3 287 HIV-infected cases were newly reported in Dehong from 2017 to 2019. The HIV gag, env and pol genotypes were determined in 1 813 cases. The major subtypes were subtype C (28.4%,515/1 813), recombination form BC (22.0%,398/1 813) and CRF_01AE (18.1%,329/1 813). Furthermore, the proportion of subtype B, subtype C and CRF01_AE decreased over years, whereas 01/BC, CRF07_BC and CRF08_BC increased over years in both Chinese and Burmese patients (χ2=75.212,P<0.001). There were significant differences in gender, age, marital status, ethnicity, educational level and transmission route among Chinese and Burmese HIV-infected cases with diverse HIV genotypes (all P<0.05). ConclusionHIV subtypes in Dehong change over time, which demonstrates that the proportion of BC recombinant subtypes and unique recombinant subtypes increased significantly. 
		                        		
		                        		
		                        		
		                        	
4.Distribution of viral load before treatment in HIV cases in Dehong prefecture, Yunnan province

		                        			Objective:To determine the baseline HIV viral load and demographic characteristics of newly diagnosed HIV cases from 2015 to 2017 in Dehong prefecture and provide a scientific basis for improving HIV prevention and treatment.Methods:All newly reported HIV-infected cases from 2015 to 2017 in Dehong prefecture were included in this study. Viral load, CD 4+ T cell counts and the demographic characteristics were collected and retrospectively analyzed. Results:From 2015 to 2017, a total of 1 157 newly diagnosed HIV cases were reported in Dehong prefecture, of which 1 057 cases were tested for viral load; 64.9% were males, 59.5% aged between 25 and 49, 51.9% were married, 48.2% were Han ethnicity, 48.2% had middle school (or beyond) training and 85% were heterosexual. The median pre-treatment viral load of newly diagnosed HIV-infected individuals was 8 200 (IQR: 1 900-44 000) copies/ml. The proportion of cases with viral load between 10 3-10 5copies/ml was the highest, accounted for 65.7% (694/1 057). There were 5.2% (55/1 057) whose viral load was below the detecting limit and 24 had CD 4+ T cell counts over 500 cells/μl. Conclusions:Baseline viral load levels before treatment vary among HIV-infected individuals. In view of the impact of viral load at baseline on the effectiveness of diagnosis and treatment, it is necessary to perform viral load testing before treatment, and the result can assist in the formulation of a more personalized and effective treatment regimen.
		                        		
		                        		
		                        		
		                        	
5. Prevalence and related factors on diabetes among HIV/AIDS receiving antiretroviral therapy in Dehong Dai and Jingpo Autonomous Prefecture

		                        			 Objective:
		                        			To study the prevalence and correlates of diabetes among HIV/AIDS who were on antiretroviral therapy (ART) in Dehong Dai and Jingpo autonomous prefectures (Dehong), Yunnan province.
		                        		
		                        			Methods:
		                        			The database of HIV/AIDS receiving ART in Dehong was downloaded by using the basic information system of AIDS prevention and control in China. In this cross-sectional study, HIV/AIDS patients who were currently on ART and aged 18 years or above, were consecutively recruited, between July 2017 and June 2018, in Dehong. All the subjects underwent hemoglobin A1c (HbA1c) testing. Patient with diabetes was defined as meeting any of these indicators (HbA1c ≥6.5%, baseline FPG ≥7.0 mmol/L, FPG ≥7.0 mmol/L in the most recent visit). Both univariate and multivariate logistic regression analysis were carried on to evaluate the correlates of diabetes among the HIV/AIDS patients.
		                        		
		                        			Results:
		                        			In total of 4 376 HIV/AIDS patients were included for analysis, with the average age as (43.7±10.1) years, proportion of males as 53.8% (2 356/4 376) and the HCV positive rate as 24.1% (1 055/4 376). The mean years was (8.9±3.8) years after the HIV diagnosis was made, and the mean duration on treatment was (6.8±2.9) years. The prevalence of diabetes was 11.4% (500/4 376). Through multivariate logistic regression analysis, data showed that the risk factors of diabetes of HIV/AIDS on ART were: aged 40 years or above, being male, HCV positive, baseline body mass index ≥24.0 kg/m2, elevated TG ≥1.70 mmol/L in the most recent visit and baseline antiretroviral regimens under Efavirenz (EFV).
		                        		
		                        			Conclusions
		                        			Prevalence rate of diabetes appeared higher in HIV/AIDS patients who were on ART in Dehong. Prevention and control measures should be targeted on HIV/AIDS patients who were with risk factors of diabetes as being elderly, male, HCV positive, overweight and higher TG. 7.Tracing hepatitis C virus transmission by using high-throughput sequencing technique

		                        			Objective An approach for analysis of hepatitis C virus (HCV) quasispecies using Hiseq high-throughput sequencing (hereinafter referred to as Hiseq sequencing) technique was developed and then applied to investigate a possible case of HCV needle sharing transmission. Methods One case of HCV antibody seroconversion (P1) was found in a methadone clinic on January 15, 2015. Four HCV antibody positive injecting drug users (IDUs), P2 to P5, suspected to be involved in needle sharing transmission with P1 during the period (after March 24, 2014) that P1 may be infected with HCV were investigated, and another 28 HCV antibody positive IDUs were selected as controls (C1 to C28). These controls came from the same methadone clinic or lived in the same town with P1. The RNAs were extracted from the plasma specimens and then reverse-transcribed into cDNA. After HCV subtyping, Hiseq sequencing was performed to detect and sequence the HCV quasispecies (263 bp) in the specimens with the same subtype as P1. The frequency of quasispecies was counted and ranked. Intrapersonal and interpersonal genetic distance and phylogenetic tree were calculated. Results The HCV subtype of specimen P1 was 3b. All the other specimens with the same subtype were P2, C7, C12, C14, C15, C16, C19, C20 and C28. Hiseq sequencing was successfully performed in 9 out of these 10 specimens, and 249 753 to 1 086 333 (average 869 608) cleaned sequences representing 3 to 172 (average 48) unique HCV quasispecies were obtained. The medians (P50) of intrapersonal genetic diversities from the 9 specimens were 0.4% to 12.3%. The P50 (P25, P75) of genetic diversities between P1 and the other 8 specimens were 19.0% (18.4%, 19.8%), 10.4%(2.8%, 18.3%), 19.6% (17.8%, 21.4%),24.9% (23.8%, 26.1%), 19.8% (18.7%, 20.7%), 20.1% (18.9%, 21.2%), 20.6% (20.0%, 21.1%), 23.6% (22.4%, 24.8%). There were no significant difference between the genetic diversities of P1 and P2 and those of P1 and other 7 specimens (H=9.40, P=0.100). The genetic diversities between few HCV quasispecies from P1 and few ones from C7 were 0. Phylogenetic tree analysis indicated that there was no HCV transmission relationship between P1 and P2, but there was HCV transmission relationship between P1 and C7. 9.Curative effect of different dialysis in treatment of uremia

		                        			Objective To observe and evaluate the clinical effect of pure hemodialysis (HD﹚, hemodialysis combined with hemoperfusion (HD+HP﹚, hemodialysis combined with hemodiafiltration (HDF﹚ on onuremic patients. Methods All 90 patients with uremic in our hospital from January 2011 to May 2014 were randomly divided into HD group, (HD+HP﹚ group,HDF group according to the treatment methods, 30 cases in each group. After two months,the skin itching symptoms and blood toxin levels before and after treatment in each group of were compared. Results Before treatment,Pi, Scr and BUN levels were not significantly different between groups(P>0.05﹚. After treatment the Pi, Scr and BUN levels were significantly decreased compared with those before treatment(P<0.05﹚. No significant differences between groups(P>0.05﹚. Before treatment,three groups of PTH, β2-MG and Cys C level had no significant difference (P>0.05﹚.After treatment,three groups of PTH,β2-MG and Cys C levels were significantly decreased than before treat-ment(P<0.05﹚.Multiple comparisons between groups showed that the indexes of the level of (HD+HP﹚group was lower than that of the other two groups (P<0.05﹚.(HD+HP﹚ group,HDF group of skin pruritus symptom improvement rate were higher than those in HD group (P<0.05﹚,but (HD+HP﹚groups, HDF groups had no significant difference (P>0.05﹚. Conclusion Scavenging effects of HD,(HD+HP﹚and HDF to the small molecules are roughly the same,but scavenging effect of(HD+HP﹚and HDF for large, middle molecular substances are better than those of HD,so (HD+HP﹚ has the best effect.
		                        		
		                        		
		                        		
		                        	
10.A design of refractometer based on blur circle.

OBJECTIVEDesign a convenient and stable eye refractometer based on the theory of blur circle.
METHODSAnalyze the retinal blur circle in both Emsly reduced eye model and Liou & Brennan 1997 eye model by ZEMAX. Design the coefficients including PD (pupil diameter) and NO' (length between node point and fovea) with the purpose of improving the accuracy. At last, compare the clinical optometry data from this refractor with the data obtained from optometry hospital in Wenzhou.
RESULTSThe blur circle diameters are nearly the same in both reduced eye model and the Liou & Brennan 1997 eye model. With the PD = 4 mm and NO' = 20 mm, the refractor shows a fine accuracy in optometry. The paired t test shows that the myopia group and the astigmatism axial direction group have no statistical difference between the data from the blur circle refractor and the hospital (P > 0.05), while the astigmatism degree group has the result of P = 0.41 which may be caused by the poor cooperation of pediatric patients. 80% of the astigmatism degree data differ from the data from the hospital in less than 0.75D.
CONCLUSIONThe blur circle refractor, with the features of convenience and fine accuracy, is promised to be a new style of refractometer in the future.
