1.Clinical and laboratory characteristics of 202 patients with cryoglobulinemia

		                        			Objective:To analyze the clinical and laboratory characteristics of patients with cryoglobulinemia.Methods:It is a cross-sectional study. The patients diagnosed with cryoglobulinemia in our hospital were enrolled from July 2017 to March 2023. The baseline information of patients, included age, gender, qualitative, and quantitative results of serum cryoglobulins, initial clinical manifestations, etiology, serum complement 3 and 4, and the renal pathological manifestations. The clinical and laboratory characteristics of patients with different types of cryoglobulinemia were analyzed.Results:There were 62 patients (30.7%) with type Ⅰ cryoglobulinemia, 58 patients (28.7%) with type Ⅱ cryoglobulinemia, and 82 patients (40.6%) with type Ⅲ cryoglobulinemia enrolled in this study. Among these patients, 56 of primary cryoglobulinemia, 76 of autoimmune diseases, 29 of tumor-related diseases, and 52 of infectious diseases were observed. Clinical symptoms related to skin lesions (124 cases, 61.4%) and kidney damage (87 cases, 43.1%) were the most common initial clinical manifestations and arthralgia/arthritis (50 cases, 24.8%), peripheral neuropathy (33 cases, 16.3%), fatigue (28 cases, 13.9%), fever (23 cases, 11.4%) were also observed in some patients. The clinical symptoms varied in different types of cryoglobulinemia. 29.0% patients (18/62) with type Ⅰ had fatigue, which was higher than those with type Ⅱ (10.3%, 6/58) and type Ⅲ (4.9%, 4/82) ( P<0.05); Kidney damage occurred in 56.9% (33/58) patients with type Ⅱ and 52.4% (43/82) patients with type Ⅲ, which was higher than that in type Ⅰ patients (17.7%, 11/62) ( P<0.05); Only 4 patients (4.9%, 4/82) with type Ⅲ had peripheral neuropathy, which was lower than those with type Ⅰ (17.7%, 11/62) and type Ⅱ (31.0%, 18/58) ( P<0.05). The quantity of cryoglobulins in patients with type Ⅲ cryoglobulinemia [122 (82, 177) mg/L] was significantly lower than that in patients with type Ⅰ [695(229, 3 499) mg/L] ( P<0.001) and type Ⅱ cryoglobulinemia [350 (107, 1 874) mg/L] ( P<0.001). Complement 4 decreased in 49.0% (99/202) of patients and complement 3 decreased in 42.6% (86/202) of patients. Membranoproliferative glomerulonephritis (36.0%, 9/25) and endocapillary proliferative glomerulonephritis (32.0%, 8/25) were the main renal pathological manifestations of cryoglobulin nephropathy. Conclusions:The most common clinical manifestations of cryoglobulinemia are skin and kidney damage. The clinical manifestations of patients with cryoglobulinemia vary in different types of cryoglobulins. 2.The Ideas and Methods of Treatment Related to the Spleen Theory on Precancerous Lesions of Gastric Carcinoma

		                        			Precancerous lesion of gastric carcinoma(PLGC)is a critical component of the occurrence,development,and metastasis of the"inflammation-cancer transformation".Traditional Chinese medicine(TCM)is characterized by a unique mode of diagnosis and treatment which adopts a holistic concept to differentiate and treat syndromes.With this mode,it can improve symptoms in patients,enhance the quality of their lives and elevate the histopathological performance.The pathogenic factors of PLGC are mostly related to spleen,among which the weakness of spleen and stomach is the pathogenetic basis of it.Over time,dampness,phlegm and blood stasis will accumulate in the body,causing local lesions.Spleen deficiency is associated with various development mechanisms of PLGC,including the gastric mucosal inflammation,intragastric flora,gastric mucosal barrier,immune function,energy metabolism,and gastric mucosal secretion.To treat PLGC from the perspective of the spleen theory,physicians should take steps to reinforce healthy Qi to eliminate pathogenic factors,give equal consideration to deficiency and excess,and harmonize the center to restore the balance in the viscera,which will invigorate the spleen and stomach,resolve dampness,clear heat and regulate qi and blood at the same time.The differentiation of the holistic and local syndromes should be combined and the treatment with drugs should take both diseases and syndromes into consideration.
		                        		
		                        		
		                        		
		                        	
3.Research advances in FLASH radiotherapy-related clinical trials

		                        			FLASH radiotherapy (FLASH-RT) has garnered considerable attention globally in recent years. Compared to conventional radiotherapy, FLASH-RT can deliver the total radiation dose to the target volume in an extremely short time, reducing the radiation-induced damage to normal tissue while maintaining similar anti-tumor effects. FLASH-RT has been in the clinical trial stage, with several clinical research result being reported. 4.Comparison of mid-to-long term outcomes between mitral valve repair and biological valve replacement in patients over 60 with rheumatic mitral valve disease based on a propensity score matching study

		                        			Objective:To compare and discuss the mid-to-long-term outcomes of mitral valve repair (MVP) versus biological mitral valve replacement (bMVR) in patients aged 60 years and above with rheumatic mitral valve disease.Methods:This is a retrospective cohort study. A total of 765 patients aged 60 years and older, diagnosed with rheumatic mitral valve disease and who underwent MVP or bMVR at Beijing Anzhen Hospital from January 2010 to January 2023, were retrospectively included. Among them, 186 were male and 579 were female, with an age of (66.1±4.5) years (range: 60 to 82 years). Patients were divided into two groups based on the surgical method: the mitral valve repair group (MVP group, n=256) and the bioprosthetic mitral valve replacement group (bMVR group, n=509). A 1∶1 propensity score matching was performed using a caliper value of 0.2 based on preoperative data. Paired sample t-tests, χ2 tests, or Fisher′s exact tests were used for intergroup comparisons. Kaplan-Meier method was employed to plot survival curves and valve-related reoperation rate curves for both groups before and after matching, and Log-rank tests were used to compare the mid-to long-term survival rates and valve-related reoperation rates between the two groups. Results:A total of 765 patients who completed follow-up were ultimately included, with a follow-up period ( M(IQR)) of 5.1(5.0) years (range: 1.0 to 12.9 years). After matching, each group consisted of 256 patients. The incidence of early postoperative atrial fibrillation (39.1% vs. 49.2%, χ2=4.95, P=0.026) and early mortality rates (2.0% vs. 6.2%, χ2=4.97, P=0.026) were lower in the MVP group. Unadjusted Kaplan-Meier analysis showed significantly higher 5-year and 10-year survival rates for the MVP group (92.54% vs. 83.02%, 86.22% vs. 70.19%, Log-rank: P=0.001). After adjustment with propensity scores, the Kaplan-Meier analysis still indicated higher 5-year and 10-year survival rates in the MVP group compared to the bMVR group (92.54% vs. 85.89%, 86.22% vs. 74.83%, Log-rank: P=0.024). There were no significant differences in the rates of valve-related reoperation between the two groups before and after matching (5-year and 10-year reoperation rates pre-matching: 1.75% vs. 0.57%, 5.39% vs. 7.54%, Log-rank: P=0.207; post-matching: 1.75% vs. 0, 5.39% vs. 9.27%, Log-rank: P=0.157). 5.Evaluation of the anchorage effect of a new cross buccal tube in the treatment of second premolar extraction

		                        			Objective To evaluate the clinical effect of using a new cross buccal tube(XBT)to close the extraction gap of the maxillary second premolar and control the anchorage.Methods Fifteen adolescent patients with moderate to severe crowding,skeletal Class Ⅰ,missing maxillary second premolars or difficult to retain during clinical examination were selected.PASS orthodontic technique was used to reduce the number of maxillary second premolars to relieve crowding and retract the anterior teeth.Before and after treatment,head lateral images were taken and cephalometric measurements were taken.Changes of bone and dental indexes related to molar anchorage and anterior adduction were observed.Results After treatment,teeth of all patients were arranged neatly,the occlusion relationship was good and the profile was improved.Before and after treatment,there were no significant changes in bone indicators such as ∠SNA,∠SNB and ∠ANB(P>0.05).There was no significant change in vertical height ∠MP-SN(P>0.05).The angle of the upper and lower central incisors,∠U1-SN,∠U1-NA,∠ L1-MP and ∠L1-NB decreased significantly,while ∠U1-L1 increased significantly(P<0.05).The upper incisor margin point moving towards the distal(P<0.05),and there was no significant change in the position of the upper incisor root apex and first molar(P>0.05).Conclusion The application of the new cross buccal tube is beneficial for adolescent patients to control the occlusion of the extraction gap of the maxillary second premolar.

8.Discussion on the pathogenesis of "spleen deficiency and dampness accumulation" of colorectal polyps based on intestinal flora and glucose and lipid metabolism

		                        			Intestinal flora imbalance and abnormal glucose and lipid metabolism are important risk factors and pathological mechanisms of colorectal polyps. "Spleen deficiency and dampness accumulation" is the core pathogenesis of colorectal polyps. The imbalance of intestinal flora is related to spleen deficiency, and the application of Chinese herbs for invigorating spleen is helpful to the recovery of intestinal flora balance. Abnormal glucose and lipid metabolism is related to dampness accumulation, and it is effective to treat it with bitter and spicy herbs or spleen-invigorating and dampness-eliminating herbs. The interaction between intestinal flora imbalance and abnormal glucose and lipid metabolism changes intestinal microenvironment, damages intestinal epithelial cells, causes abnormal proliferation of intestinal stem cells and leads to colorectal polyps, which is consistent with the pathogenesis of spleen deficiency and dampness accumulation in Traditional Chinese Medicine. Thus, we tried to explore the biological connotation of the pathogenesis of "spleen deficiency and dampness accumulation" of colorectal polyps from the perspective of the interaction of intestinal flora and glucose and lipid metabolism, in order to provide reference for identifying high-risk population and analyzing the therapeutic mechanism of compound prescription for invigorating spleen and removing dampness.
		                        		
		                        		
		                        		
		                        	
9.Study on the pharmacodynamics and mechanism of Blumea balsamifera total flavonoids against acute myocardial infarction model rats

		                        			OBJECTIVE To study pharmacodynamics and potential mechanism of Blumea balsamifera total flavonoids against acute myocardial infarction (AMI) model rats. METHODS AMI model of SD rats was established by ligating anterior descending branch of left coronary artery. Fifty model rats were randomly divided into model group (0.8% carboxymethyl cellulose solution), positive control group (Compound danshen tablet, 300 mg/kg), B. balsamifera total flavonoids low-dose, medium-dose and high- dose groups (3, 10, 30 mg/kg), with 10 rats in each group. Other 10 rats were included in sham operation group (0.8% carboxymethyl cellulose solution). After 1 day of surgery, they were given relevant medicine 3 mL/kg intragastrically, once a day, for 4 consecutive weeks. The changes of S-T segment were recorded before and after operation, after weekly intragastric administration. The hemodynamic indexes of rats were all determined, i.e. systolic blood pressure (SBP), diastolic blood pressure (DBP), mean arterial pressure (MAP), left ventricular systolic pressure (LVSP), left ventricular end diastolic blood pressure (LVEDP), maximal left ventricular pressure rising rate (+LVdp/dtmax), maximal left ventricular pressure decreasing rate (-LVdp/ dtmax). The levels of serum myocardial enzymes [lactate dehydrogenase (LDH), creatine kinase isoenzyme-MB (CK-MB)] and inflammatory factors [tumor necrosis factor-α (TNF-α), interleukin-6 (IL-6), IL-1β] were determined. The myocardial infarction rate of rats and the phosphorylation levels of phosphoinositide 3-kinase (PI3K) and protein kinase B (Akt) proteins in myocardial tissue were determined. RESULTS Compared with model group, S-T segments of electrocardiogram were all decreased significantly in administration groups (P<0.05). SBP, DBP, MAP, LVSP, +LVdp/dtmax, -LVdp/dtmax, and ratio of p-PI3KTyr607/ PI3K, p-AktThr308/Akt, p-Aktser473/Akt were increased significantly in B. balsamifera total flavonoids medium-dose and high-dose groups (P<0.05). The levels of LVEDP, serum myocardial enzymes and inflammatory factors, myocardial infarction rate were all decreased significantly (P<0.05). CONCLUSIONS balsamifera total flavonoids can improve cardiac function of AMI model rats, the mechanism of which may be associated with inhibiting the expression of inflammatory factor and activating PI3K/Akt signaling pathway.
		                        		
		                        		
		                        		
		                        	
10.The correlation between occupational burnout and sleep quality among steelworkers

		                        			{L-End}Objective To analyze the correlation between occupational burnout and sleep quality among steelworkers. {L-End}Methods A total of 11 491 steelworkers from a large steel enterprise in Gansu Province were selected as the research subjects using convenient sampling method. The Burnout Questionnaire and the Pittsburgh Sleep Quality Index Scale were used to investigate their occupational burnout and sleep quality. Hierarchical regression analysis was used to analyze the effects of occupational burnout on the sleep quality. {L-End}Results The detection rate of occupational burnout and sleep disorder were 50.4% and 39.0%, respectively. There was a positive correlation between the level of occupational burnout and the total score of sleep quality (Spearman correlation coefficient=0.454, P<0.05). The results of hierarchical regression analysis, adjusting for confounding factors such as gender, age, marital status, education level, alcohol consumption, exercise, weekly working hours, seniority, work shift, noise exposure, dust exposure, and high-temperature work, showed that the score of occupational burnout was positively related to the score of sleep quality(P<0.01), explaining 16.0% of the variance in the score of sleep quality among these steelworkers. {L-End}Conclusion The detection rate of occupational burnout and sleep disorders are relatively high among the steelworkers in this enterprise. Higher levels of occupational burnout are associated with poorer sleep quality.
