1.A nationwide cross-sectional study of depressive symptoms among community-based patients with neoplasms in China

Objective:To investigate the associated factors of depressive symptoms among patients with neo-plasms.Methods:Nationwide(excluding Hong Kong,Macao,and Taiwan),30 505 residents were selected by a combination of stratified sampling and quota sampling according to the proportion of the seventh national population census.Patient Health Questionnaire-9(PHQ-9),General Anxiety Disorder-7(GAD-7),self-made questionnaire,and simplified perceived social support scale used to evaluate depressive symptoms,anxiety symptoms,behaviors,and perceived social support among patients with neoplasms.Results:Totally 359(1.2%)patients with self-repor-ted clinically diagnosed neoplasms were included,of which 151(42.1%)patients with malignant neoplasms and 208(57.9%)patients with benign neoplasms.The detection rate of depressive symptoms in patients with neo-plasms was 76.6%.Less than three days of walking for more than 10 minutes per day in the past week(OR=6.63),4-6 days of walking for more than 10 minutes per day in the past week(OR=5.00),the low(OR=4.80)or medium(OR=3.06)overall sleep quality,the lower perceived friend support(OR=4.66),and anxiety symp-toms(OR=1.74)among patients with neoplasms were risk factors for depressive symptoms.Conclusion:Patients with neoplasms generally might be at a high risk of depressive symptoms,especially for those patients with less ex-ercise,poor sleep quality,and low perceived social support.
2.Application of TfU model based on outcome-based education in the teaching of nurse interns in dermatology

Objective:To explore the application effects of a teaching for understanding (TfU) model based on outcome-based education (OBE) in clinical nursing internship in dermatology.Methods:We assigned 34 nursing interns at the department of dermatology of a grade A tertiary hospital in Chengdu in 2021 to observation group to receive instruction using an OBE-based TfU model and another 34 nursing interns in 2022 to control group to be taught using a traditional teaching model. The two groups were compared for theoretical and practical assessment scores, core competencies, clinical communication, and satisfaction with teaching. SPSS 26.0 was used to perform the t-test and chi-square test. Results:The observation group had significantly higher scores than the control group for theoretical assessment [(90.63±2.84) vs. (85.84±4.50)] and practical assessment [(93.15±1.67) vs. (91.12±2.71)]; core competencies overall [(154.38±8.05) vs. (150.18±6.96)] and lifelong learning [(20.18±1.90) vs. (18.85±1.85)], general clinical skills [(23.06±3.30) vs. (18.85±1.85)], and critical thinking and reasoning ability [(11.68±1.47) vs. (10.62±1.41)]; and communication abilities overall [(90.94±5.36) vs. (84.50±5.79)] and harmonious relationship establishment [(20.21±1.86) vs. (18.76±1.92)], patient problem confirmation [(16.91±1.69) vs. (15.26±2.09)], keen listening [(17.35±1.32) vs. (15.44±2.06)], and effective information transmission [(8.91±1.60) vs. (7.85±1.73)] compared with the control group (all P<0.05). The score of satisfaction with teaching of the observation group was significantly higher than that of the control group [(94.12±4.99) vs. (90.53±7.38), P<0.05). Conclusions:The TfU model based on OBE can effectively improve nursing students' theoretical and practical assessment scores, core competencies, communication skills, and satisfaction with internship teaching, which is conducive to their all-round development and professional practice. In addition, this study provides innovative ideas and reform directions for clinical nursing practice teaching.
3.Magnesium lithospermate B enhances the potential of human-induced pluripotent stem cell-derived cardiomyocytes for myocardial repair

Background::We previously reported that activation of the cell cycle in human-induced pluripotent stem cell-derived cardiomyocytes (hiPSC-CMs) enhances their remuscularization capacity after human cardiac muscle patch transplantation in infarcted mouse hearts. Herein, we sought to identify the effect of magnesium lithospermate B (MLB) on hiPSC-CMs during myocardial repair using a myocardial infarction (MI) mouse model.Methods::In C57BL/6 mice, MI was surgically induced by ligating the left anterior descending coronary artery. The mice were randomly divided into five groups ( n = 10 per group); a MI group (treated with phosphate-buffered saline only), a hiPSC-CMs group, a MLB group, a hiPSC-CMs + MLB group, and a Sham operation group. Cardiac function and MLB therapeutic efficacy were evaluated by echocardiography and histochemical staining 4 weeks after surgery. To identify the associated mechanism, nuclear factor (NF)-κB p65 and intercellular cell adhesion molecule-1 (ICAM1) signals, cell adhesion ability, generation of reactive oxygen species, and rates of apoptosis were detected in human umbilical vein endothelial cells (HUVECs) and hiPSC-CMs. Results::After 4 weeks of transplantation, the number of cells that engrafted in the hiPSC-CMs + MLB group was about five times higher than those in the hiPSC-CMs group. Additionally, MLB treatment significantly reduced tohoku hospital pediatrics-1 (THP-1) cell adhesion, ICAM1 expression, NF-κB nuclear translocation, reactive oxygen species production, NF-κB p65 phosphorylation, and cell apoptosis in HUVECs cultured under hypoxia. Similarly, treatment with MLB significantly inhibited the apoptosis of hiPSC-CMs via enhancing signal transducer and activator of transcription 3 (STAT3) phosphorylation and B-cell lymphoma-2 (BCL2) expression, promoting STAT3 nuclear translocation, and downregulating BCL2-Associated X, dual specificity phosphatase 2 (DUSP2), and cleaved-caspase-3 expression under hypoxia. Furthermore, MLB significantly suppressed the production of malondialdehyde and lactate dehydrogenase and the reduction in glutathione content induced by hypoxia in both HUVECs and hiPSC-CMs in vitro. Conclusions::MLB significantly enhanced the potential of hiPSC-CMs in repairing injured myocardium by improving endothelial cell function via the NF-κB/ICAM1 pathway and inhibiting hiPSC-CMs apoptosis via the DUSP2/STAT3 pathway.
4.Efficacy of thoracoscopic lobectomy versus segmentectomy for T1bN0M0 non-small cell lung cancer: A retrospective cohort study

Objective To investigate the clinical effect of thoracoscopic lobectomy versus segmentectomy in the treatment of T1bN0M0 non-small cell lung cancer (NSCLC). Methods Clinical data of 181 patients with T1bN0M0 NSCLC admitted to our hospital from 2012 to 2015 were retrospectively analyzed. They were divided into a lobectomy group and a segmentectomy group according to surgical methods. There were 117 patients in the lobectomy group (46 males and 71 females aged 61.32±8.94 years) and 64 patients in the segmentectomy group (20 males and 44 females aged 58.55±12.57 years). Perioperative indicators and prognosis were compared between the two groups. Results The segmentectomy group had longer operation time, less intraoperative blood loss, shorter postoperative hospital stay and more preservation of lung function compared with the lobectomy group (P<0.05). The lobectomy group had higher consolidation tumor ratio, bigger tumor diameter, and more lymph node sampling compared with the segmentectomy group (P<0.05). There was no statistical difference in 5-year overall survival or recurrence-free survival between the two groups (P<0.05). Conclusion For patients with T1bN0M0 NSCLC, thoracoscopic segmentectomy and lobectomy have similar prognosis, but segmentectomy has advantages with less injury and faster recovery over lobectomy.
5.Clinicopathological feature analysis of diffuse endocapillary proliferative Henoch-Schonlein purpura nephritis in children

Objective:To investigate the clinicopathological features, treatment and short-term prognosis of diffuse endocapillary proliferative Henoch-Schonlein purpura nephritis (DEP-HSPN) in children.Methods:Clinicopathological data of children with DEP-HSPN diagnosed by renal biopsy in the First Affiliated Hospital of Zhengzhou University from January 2012 to December 2019 were retrospectively analyzed.Children with HSPN with segmental endocapillary proliferation (non DEP-HSPN) and matched with the gender, age and pathological grade at the ratio of 1∶2 in the same period were recruited as controls.Results:(1) A total of 42 cases of DEP-HSPN were pathologically confirmed, accounting for 5.9% of the 712 children with HSPN during the same period.Thirty-nine newly treated cases were included, with the mean age of (8.9±3.2) years old, and the gender ratio was 1.79∶1.00.There were 21 cases of nephrotic syndrome, 14 cases of hematuria and albuminuria, 2 cases of acute glomerulonephritis, 1 case of rapid progressive nephritis and 1 case of isolated proteinuria.Pathological findings were accompanied by diffuse prolife-ration of mesangial and endocapillary.There were 13, 22 and 4 cases with pathological gradeⅡb, Ⅲb and Ⅳb, respectively.(2) Compared with non DEP-HSPN subjects, DEP-HSPN patients had a shorter course from renal symptoms to renal biopsy, and a higher incidence of nephrotic albuminuria, hypoalbuminemia, hypocomplementemia, hypertension and anemia.The main clinical type was nephrotic syndrome.The levels of D-dimer, 24-hour urinary protein (24 h UP) and urea nitrogen were significantly higher in DEP-HSPN group ( Z=-2.416, -2.595, -2.019, all P<0.05), while the red blood cells, hemoglobin, serum albumin, C 3 and glomerular filtration rate (eGFR) were significantly lower ( t=-2.499, -3.746, 2.836, -3.410, 3.236, all P<0.05). Besides, the glomerular C 3 deposition was higher than those in non DEP-HSPN subjects ( Z=-1.977, P<0.05). (3)The urinary protein remission rate in DEP-HSPN group was significantly reduced at 1 month follow-up [37.0%(10/27 cases) vs.62.5%(40/64 cases), P<0.05]. There was no significant difference between the 2 groups at 3 months, and the urinary protein remission was relieved at 6 months in both groups.There was no significant difference in hematuria remission between the 2 groups at the end of follow-up. Conclusions:Clinical manifestation of DEP-HSPN is severe, which is easy to be complicated with hypertension, anemia, hypocomplementemia C 3 and so on, and the hypercoagulable state is obvious.The degree of glomerular complement C 3 deposition was high in DEP-HSPN group.Urinary protein can be relieved slowly within 1 month after active treatment, but can be relieved at 6 months.
6.Application of yak pericardium in anti-calcification heart valves

Objective To explore the feasibility of the application of yak pericardium as anti-calcified heart valve materials in transcatheter aortic valve replacement (TAVR). Cattle pericardium and commercial Sino products were used as controls. Methods Yak and cattle pericardium was decellularized, and then the acellular extent was qualitatively and quantitatively analyzed with hematoxylin-eosin (HE) staining and DNA detection kit respectively. Two cross-linking agents, i.e. glutaraldehyde and genipin, were used to cross-link the acellular pericardium, respectively. After testing the mechanical property, thermodynamic stability and biocompatibility of the pericardium, the cross-linked pericardium was subcutaneously implanted in juvenile Wistar rats. After 2~4 weeks of the implantation, histological staining and tissue calcification analysis were conducted. Results The nuclei of the yak pericardium were almost invisible after decellularized treatment, and the DNA content decreased from (0.90±0.13)μg/mg to (0.09± 0.02)μg/mg that was significantly lower than that of Sino product (P<0.001). The mechanical strength of the acellular yak pericardium, glutaraldehyde or genipin cross-linked yak pericardium was higher than that of the cattle pericardium (all P<0.05). The maximum stress value, obtained in the glutaraldehyde cross-linked yak pericardium, was (8.44±2.61) MPa, which was higher than (7.92±1.81) MPa of the Sino product (P<0.05). The shrinkage temperature of non- and cross-linked yak pericardium was slightly higher than that of cattle pericardium, but difference was not statistically significant (all P>0.05). For the two cross-linked yak pericardium, the hemolysis rate and cell proliferation ratio was similar with that of cattle pericardium (all P>0.05), the tissue regeneration ability of subcutaneous implantations was inferior, and the calcification level was higher than that of cattle pericardium with an no statistically significant difference (all P>0.05). After 4 weeks of the subcutaneously implantation, the calcium content of the glutaraldehyde cross-linked pericardium was 32.62~65.49μg/mg. Conclusions Yak pericardium has better mechanical properties and thermodynamic stability than cattle pericardium, and the biocompatibility can meet the requirements of transcatheter aortic valve replacement.

The WeChat public platform has profoundly influenced the propagation mode of medical ethics.Through the fitting of logistic ordinary differential equation,the analysis of two-level of propagation model and path,this paper constructed the propagation mode of medical ethics based on WeChat public platform.In practice,by means of accurate positioning,pushing quality content,the integration with new media forms,online interaction and other ways,the conversion rate of WeChat push will be enhanced,which will therefore promote the transmission efficiency of medical ethics.
8.Analysis of the relation between dental arch size and upper airway morphology in patients with obstructive sleep apnea hypoventilation syndrome

Objective:To study the anatomical correlation between dental arch and the volume of upper airway in patients with obstruc-tive sleep apnea hypoventilation syndrome(OSAHS). Methods: Dental arch architecture and upper airway volume were measured by cone beam CT(CBCT) in the subjects with OSAHS(n=22) and without OSAHS(n=19). The correlation between dental arch and the supper airway volume in OSAHS patients was analyzed. Results:The length of the upper dental arch and the height of palate in OSAHS patients were larger than those of the controls(All, P<0. 05). Cross-sectional area of nasopharynx and retropalatal and the total volume of upper airway were negatively correlated with the palatal height and upper dental arch length(P<0. 05), while positively correlated with upper dental arch of molar regions(P<0. 05). Conclusion:The abnormal shape of upper dental arch is related to the airway vol-ume of nasopharynx and retropalatal region in patients with OSAHS.

10.CT appearance of thoracic lymphonodus in AIDS patients with immune reconstitution inflammatory syndrome

Objective To explore the CT appearance of thoracic lymphonodus in AIDS patients with immune reconstitution in-flammatory syndrome(IRIS)after highly active antiretroviral therapy (HAART).Methods The data of thoracic CT in 24 AIDS pa-tients after HAART with enlarged thoracic lymphonodus in IRIS were collected,and the chest CT appearance was analyzed.Results Of the 24 cases of AIDS patients with IRIS after HAART,1 9 cases were complicated with pulmonary tuberculosis,which includ-ed 5 cases with cervical tuberculous lymphadenitis,3 cases were co-infected with bacterium and fungi,1 case was infected by penicil-lium marneffei,1 case by pneumocystis carinii.The enlarged thoracic lymphonodus were primarily located in 4R region(20/24), secondly in 2R region(1 1/24)and 4L region(1 1/24),in which the density was uniform or non-uniform,edge clear or unclear,some parts of lymphonodus were fused together but not calcified.The minor axis of enlarged lymphonodus was 1 1.0-25.0 mm except X region,enhanced uniformly in 2 cases.Obstructive pneumonia and pulmonary consolidation were found in 2 cases with enlarged lym-phonodus,which were located in 10R region.Pleural effusion was found in 13 cases with greatest depth of about 22 mm,pericardial effusion was found in 5 cases with greatest depth of about 24 mm.Conclusion The enlarged thoracic lymphonodus in AIDS patients with IRIS affer HAART are mainly involved in the region of 4R,2R and 4L,with or without pleural effusion and pericardial effusion.
