1.Disease burden of chronic kidney disease attributable to high BMI in China and trend prediction in 1992-2021

Objective To analyze the disease burden of chronic kidney diseases (CKD) attributed to high body mass index (BMI) in China from 1992 to 2021 and predict the disease burden for the next decade, and to provide evidence for the prevention and treatment of CKD. Methods Using the Global Burden of Disease (GBD) database and the Joinpoint model, the average annual percentage rate change (AAPC) of the mortality rate and disability-adjusted life year (DALY) rate was calculated to describe and analyze the CKD disease burden attributed to high BMI in China from 1992 to 2021. The ARIMA model was employed to predict and analyze the change trend of the CKD disease burden. Results From 1992 to 2021, the mortality rate and DALY rate attributed to high BMI-induced chronic kidney disease showed an upward trend. Compared to 1992, the attributed number of deaths increased by 324.38%, and DALYs increased by 268.56%; the mortality rate increased by 64.00%, and the DALY rate grew by 51.62%. From 1992 to 2021, the mortality rate and DALY rate for males were lower than those for females, but the growth rate for males exceeded that of females. From 1992 to 2021, the mortality rate and DALY rate of chronic kidney disease attributed to high BMI in China increased with age. The average annual change rate of chronic kidney disease attributed to high BMI in China from 1992 to 2021 (mortality rate: 1.40 per 100,000 (95% CI: 1.04–1.76), DALY rate: 1.43 per 100 000 (95% CI: 1.17–1.70)) was higher than thHuaiyin Normal University, Huai'anher social demographic index (SDI) regions. The ARIMA model predicted that the age-standardized mortality rate increased from 2.91 per 100 000 in 2022 to 3.05 per 100 000 in 2026, and the age-standardized DALY rate increased from 69.65 per 100 000 in 2022 to 73.58 per 100 000 in 2026. Conclusion Chronic kidney disease attributed to high BMI in China is on the rise, and it will continue to grow in the future. The focus of CKD prevention and control should be on males and the elderly, while active measures should be taken to reduce the occurrence and progression of chronic kidney disease.
2.Correlation analysis of functional constipation and its related factors with acute appendicitis in children

Objective:To explore the correlation between functional constipation (FC)and its related factors with acute appendicitis(AA)in children,so as to provide a reference for the prevention and clinical diagnosis and treatment of children with AA. Methods:A case-control study was conducted on 170 children diagnosed with AA in the Department of Pediatrics,the Second Affiliated Hospital of Air Force Military Medical University,from August 2022 to March 2023,and 170 non-AA children during the same period were selected as control objects.The clinical data,incidence of FC,symptoms related to FC,Bristol stool classification,past constipation history and other information were compared between two groups.Results:The incidence of FC in 170 children with AA was 22.9%(39/170),which was significantly higher than 10.6%(18/170)in the non-AA group( P<0.01);For children under four years old,the proportion of faecal retention in AA group was higher than that in non-AA group(25.6% vs. 9.3%, P<0.05);For children ≥ 4 years old,the proportion of faecal retention and dyschezia in AA group were higher than those in non-AA group(28.2% vs. 6.9%,29.0% vs. 16.4%,respectively,all P<0.05).The proportion of past constipation history in AA group was higher than that in non-AA group(29.4% vs.14.1%).The duration of constipation in AA group was longer than that in non-AA group [0.00(0.00,1.25)month vs. 0.00(0.00,0.00)month,all P<0.01].The proportion of children with low Bristol stool classification in AA group was higher than that in non-AA group( P<0.01).Multivariate Logistic regression analysis showed that faecal retention[ OR=6.186(95% CI 2.336~16.380)] and long constipation time [ OR=1.310(95% CI 1.095~1.567)]were independent risk factors for AA in children(all P<0.05). Conclusion:The incidence of faecal retention in children with AA is higher than that in children without AA,and the median duration of constipation is longer than that in children without AA.Fecal retention and long-term constipation are independent risk factors for AA in children.
3.Summary of the best evidence for management of lower urinary tract dysfunction in patients undergoing total hysterectomy

Objective:To summarize the best evidence for the management of lower urinary tract dysfunction (LUTD) in total hysterectomy patients, so as to provide evidence-based basis for clinical practice.Methods:According to the "6S" pyramid model, literature related to the management of LUTD in total hysterectomy patients was successively searched from guide websites, evidence-based websites, professional websites and comprehensive databases. The search deadline was from the establishment of the databases to March 31, 2023. Two researchers evaluated the quality of the included literature, extracted evidence and recommended the level of evidence.Results:A total of 14 articles were included, including one clinical decision, two evidence summaries, three guidelines, one expert consensus and seven systematic evaluations. A total of 25 pieces of evidence were summarized from four aspects, such as symptom assessment, urinary tract management, symptom intervention and health education.Conclusions:Medical staff should manage lower urinary tract dysfunction in patients undergoing total hysterectomy based on evidence-based evidence to prevent or reduce the occurrence of lower urinary tract dysfunction in patients.
4.Progress of immunocellular therapy in treatment of IBD

Inflammatory bowel disease(IBD)is a disease characterized by chronic,recurrent intestinal inflammation,includ-ing Crohn's disease(CD)and ulcerative colitis(UC).Etiology and pathogenesis of IBD are unknown,and the treatment is difficult.In recent years,immunocellular therapy represented by regulatory T(Treg)cell therapy and dendritic cell(DC)therapy has developed rapidly in the field of immune-related disease treatment,and some progress has also been made in the treatment of IBD.This review will introduce application and progression of Treg cell therapy and DC therapy in IBD treatment.
5.Status and influencing factors of post-traumatic growth of parents of children and adolescents with type 1 diabetes mellitus

Objective:To analyze the status quo and influencing factors of post-traumatic growth of parents of children and adolescents with type 1 diabetes mellitus (T1DM) .Methods:The convenient sampling method was used to select parents of children with T1DM who were admitted to the diabetes care consultation clinic of Beijing Children's Hospital, Capital Medical University from March 2022 to March 2023 as the research objects. The parents of children were investigated by general information questionnaire, Posttramnatic Growth Inventory, Simplified Coping Style Questionnaire and Perceived Social Support Scale.Results:A total of 200 questionnaires were sent out in this study, and 187 were effectively collected, with an effective recovery rate of 93.5% (187/200). The total score of Posttramnatic Growth Inventory for parents of T1DM children was (60.28±18.37). The positive coping score in Simplified Coping Style Questionnaire was (22.40±6.94) and the negative coping score was (8.94±4.58). The total score of Perceived Social Support Scale was (57.39±14.57). The results of multiple linear regression analysis showed that HbA1c, monthly family income, educational level, positive coping strategies and perceived social support were the influencing factors for the post-traumatic growth of parents of T1DM children, explaining 33.8% of the variation in post-traumatic growth of parents of T1DM children.Conclusions:The post-traumatic growth of parents of T1DM children is at a moderate level. Medical staff should pay more attention to parents of children with poor blood sugar control, low educational level and low monthly family income, guide them from the perspective of positive psychology, encourage parents to adopt positive and effective coping strategies and emphasize the importance of social support, so as to improve the level of post-traumatic growth of parents of children with T1DM.

7.Buyang Huanwutang Alleviates Inflammatory Injury After Cerebral Ischemia-reperfusion in Rats via Activating Adiponectin Pathway

ObjectiveTo investigate the effects of Buyang Huanwutang (BYWHT) on reducing inflammatory injury and improving neurological function after cerebral ischemia-reperfusion in rats via activating the adiponectin (APN) pathway. MethodMale SD rats were randomized into sham, model, BYHWT (16 g·kg-1, twice a day), and BYHWT + APN inhibitor (GW9662) groups. In the sham group, blood vessels were isolated. The rat model of middle cerebral artery occlusion (MCAO) was established. The rats in the BYHWT+GW9662 group was treated with subcutaneous injection of GW9662 at 4 m·kg-1 30 min before MCAO surgery and BYHWT at 16 g·kg-1 by gavage after MCAO surgery, once in the morning and once in the evening. The immunofluorescence (IF) assay was employed to observe the expression of adiponectin receptor 1 (AdipoR1) and the colocalization of AdipoR1 with neuronal nuclei (NeuN) and ionized calcium binding adaptor molecule 1 (Iba1) in the brain. Enzyme-linked immunosorbent assay (ELISA) was employed to detect the expression of APN in the serum and brain. The balance beam test was carried out to examine the balance ability, and the grasping test to assess the recovery of limb strength. The immunofluorescence assay was used to detect the expression of myeloperoxidase (MPO). Western blot was employed to determine the expression of tumor necrosis factor-alpha (TNF-α), interleukin (IL)-1β, IL-6, and IL-10 and in the brain. ResultCompared with the sham group, the modeling promoted the expression of AdipoR1 (P<0.01), lowered the APN levels in the serum and brain (P<0.05, P<0.01), increased the score in the balance beam test (P<0.01), and decreased the grasping strength of forepaw (P<0.01), which were accompanied with increased MPO, TNF-α, IL-1β, and IL-6 levels (P<0.01) and decreased IL-10 level (P<0.01). Compared with the model group, BYHWT promoted the expression of AdipoR1 (P<0.01), elevated APN levels in the serum and brain (P<0.05, P<0.01), and increased the grasping strength of forepaw (P<0.01), which were accompanied with lowered MPO, TNF-α, IL-1β, and IL-6 levels (P<0.01) and elevated IL-10 level (P<0.01). All the above effects were partially blocked by GW9662. ConclusionBYHWT can reduce inflammatory injury and improve neurological function in the rat model of cerebral ischemia-reperfusion by activating the APN pathway.
8.Ankle arthritis: joint-preserving surgery and total ankle arthroplasty.

Ankle arthritis affects approximately 1% of the adult population worldwide and represents a serious global disease burden. However, compared with hip arthritis and knee arthritis, the clinical understanding and treatment of ankle arthritis are still in their infancy. For end-stage ankle arthritis, ankle arthrodesis was considered as the "gold standard" in the past. However, ankle arthrodesis will result in loss of joint mobility, altered gait, limited daily activities, and accelerated degeneration of adjacent joints. Therefore, how to preserve the range of motion of the ankle joint while relieving pain is the key to the treatment of ankle arthritis. Currently, the surgical treatment of ankle arthritis includes arthroscopic debridement, periarticular osteotomies, osteochondral transplantation, ankle distraction arthroplasty, ankle arthrodesis, and total ankle arthroplasty. The choice of treatment should be individualized and based on various factors such as the patient's symptoms, signs, imaging performance, complaints, and financial situation. However, there are no guidelines that give clear treatment recommendations. Therefore, it is necessary to conduct extensive and in-depth discussions on the diagnosis and treatment of ankle arthritis.

10.Challenges to global measles elimination during 2020-2022

Measles incidence has increased and measles immunization coverage has stagnated in recent years, resulting in difficulties to reach measles elimination goal. Public health measures for 2019-nCoV pandemic also caused a significant drop in measles cases, but the global immunization coverage further declined, vaccine hesitancy increased and surveillance weakened during 2020-2021. This paper summarizes the latest progress in measles elimination in different areas, as well as the current difficulties to be overcome with the emphasis on the influence of 2019-nCoV pandemic on measles elimination to provide reference for accurate assessment of measles epidemic situation and effective prevention and control of measles in the context of 2019-nCoV pandemic.
