1.The protective effect and mechanism of Taraxasterol on Erastin induced ferroptosis in chondrocytes

Objective To investigate the role of Taraxasterol(TAR)on ferroptosis in chondrocytes induced by Erastin.Methods The C28/I2 chondrocyte line was treated with Erastin to construct the ferroptosis model of chon-drocytes in vitro and the experiments were divided into Control,Erastin,TAR,and TAR+Erastin groups.Cell via-bility was detected by the CCK-8 assay.Cytotoxicity was detected by the lactate dehydrogenase(LDH)kit and the Calcein/PI cytokinesis kit.Flow cytometry was used to detect lipid reactive oxygen species(ROS).The intracellular glutathione(GSH)content was detected by GSH kit.Mitochondrial membrane potential was detected by JC-1 stai-ning and RH123 staining.ACSL4 and GPX4 protein expression and the key indicators of ferroptosis were detected by Western blot.Results TAR restored the decreased cell viability of C28/I2 chondrocytes induced by Erastin treatment as well as reduced Erastin-induced cytotoxicity(P<0.01).Compared with the control group,the level of intracellular lipid ROS increased(P<0.01)and the content of GSH decreased(P<0.01)after treatment with Erastin,while TAR could reduce the production of lipid ROS(P<0.01)and increase the content of GSH(P<0.01).TAR restored mitochondrial membrane potential in C28/I2 chondrocytes ferroptosis,decreased ACSL4 pro-tein expression(P<0.01)and increased GPX4 protein expression(P<0.01).In addition,TAR restored the re-duced cell viability caused by IL-1 β treatment.Conclusion TAR can inhibit Erastin induced ferroptosis in C28/I2 chondrocytes,which may be related to the regulation of ACSL4 and GPX4 protein expression.
2.Application value of high frequency ultrasound in the diagnosis of shoulder joint stiffness

Objective:To explore the application value of multiple indexes of high-frequency ultrasound in the diagnosis of shoulder stiffness, and to provide data support and theoretical basis for the diagnosis and treatment of shoulder stiffness.Methods:Clinical data of 261 patients with unilateral shoulder pain with limited mobility admitted to the Orthopedic Trauma, Hebei Medical University Third Hospital from January to December 2023 were retrospectively analyzed. With the results of shoulder arthroscopy as the gold standard, the patients were divided into 127 patients with shoulder stiffness (observation group) and 134 patients with non-shoulder stiffness and shoulder pain (control group). The fluid accumulation of long head of the biceps tendon (LHBT), echo of rotator interval (RI) and blood flow signal, thickness of coracohumeral ligament (CHL), thickness of subacromial-subdeltoid bursa (SASD), fluid accumulation and blood flow signal, fluid accumulation of posterior glenohumeral joint capsule (PJ), the inferior capsule thickness (ICT) and the thickness of inferior glenohumeral ligament (IGHL) were observed and measured in the two groups in addition, the CHL difference, ICT difference and IGHL difference between the affected side and the contralateral side were calculated, and the difference between the two groups of ultrasound indicators was compared and the multivariate logistic stepwise regression analysis was performed, and the receiver operating characteristic (ROC) curve was drawn to evaluate the diagnostic value of each indicator for shoulder stiffness.Results:RI hypoecho with increased blood flow signal, SASD thickening, SASD effusion, SASD blood flow signal detection rate, IGHL thickness, IGHL difference and ICT difference in observation group were significantly higher than those in the control group (all P<0.05). Multivariate logistic stepwise regression analysis showed that SASD thickening, SASD effusion and IGHL difference were independent predictors of shoulder stiffness (all P<0.05). The area under the curve of SASD thickening, SASD effusion and IGHL difference in the diagnosis of shoulder stiffness were 0.550, 0.540 and 0.636, respectively. The diagnostic sensitivity was 89.8%, 10.2% and 60.6%, and the specificity was 20.1%, 97.8% and 67.2%, respectively. The area under the curve was 0.676, the diagnostic sensitivity was 59.8%, and the specificity was 75.4%. Conclusions:High-frequency ultrasound is an effective imaging method to assist clinical diagnosis of shoulder stiffness. SASD thickening, SASD effusion and IGHL difference may be independent predictors of shoulder stiffness. The combined application of these three indicators has higher clinical application value and provides favorable ultrasound indicators for clinical differential diagnosis of shoulder stiffness.
3.Analysis of the etiological features and clinical characteristics of herpes zoster cases under 20 years old in Beijing City from 2017 to 2021

Objective:To analyze the etiological features and clinical characteristics of herpes zoster cases under 20 years old in Beijing City from 2017 to 2021.Methods:Herpes zoster cases were collected from a surveillance system in Beijing City from December 2017 to April 2021. The cases included individuals under 20 years old from seven sentinel hospitals located in two districts (Miyun District and Changping District). The basic information, the rash date of rash onset and the location and number of lesions were investigated at the first visit to the hospital, and the lesion swab samples were collected for laboratory testing. A telephone follow-up was conducted 21 days after the onset of the rash to investigate the degree of pain, duration of the rash and duration of pain. The individuals who still experienced neuralgia were further investigated for their pain condition at 90 days after the onset of the rash, to discover cases with postherpetic neuralgia. DNA was extracted from the rash fluid, and the ORF62 gene region was amplified and sequenced to obtain the viral sequence. The wild-type strain or chickenpox vaccine strain was identified by using sequence alignment, and the clinical characteristics of cases with different varicella vaccinations were compared.Results:A total of 78 herpes zoster cases under 20 years old were investigated during 2017-2021 in Beijing City, and 61 cases completed the follow-up survey. The age range of 61 cases was 1.83 to 20.54 years with a median age of 17.50 years. There were 36 males (59.02%) and 25 females (40.98%). Among them, there were 29 cases with the chickenpox vaccine immunization history (18 cases with one dose, 5 cases with two doses and 6 cases with unknown doses), 13 cases with no vaccination history and 19 cases with unknown vaccination history. Among the 78 cases, the herpetic fluid samples of 64 cases were positive for VZV, including 62 cases identified as wild-type strains and two cases as vaccine strains. The two vaccine strain cases were both 2-year-old girls who had received one dose of varicella vaccine and developed herpes zoster 3 months and 13 months after vaccination. Among the 29 cases with chickenpox vaccine immunization history, the majority had 10 to 49 lesions, accounting for 58.62% (17 cases). The trunk was the most common site of lesions, accounting for 44.83% (13 cases). About 51.72% (15 cases) reported "no or mild" pain intensity. The median ( Q1, Q3) scores for the worst pain, duration of pain and the time to crusting of lesions in the herpes zoster cases were 3 (1.5, 5) points, 10 (1.5, 12.5) days and 10 (6.5, 13) days, respectively. There was no statistically significant difference in the constituent ratio of the location of lesions, number of lesions and pain degree among the cases with vaccination history, without vaccination history and with unknown vaccination history ( P>0.05). There was also no statistically significant difference in the distribution of pain score, duration of lesions and duration of pain across the three groups ( P>0.05). Conclusion:Wild strains are the predominant pathogens in herpes zoster cases under 20 years old in Beijing City during 2017-2021. The varicella vaccination has no significant impact on the clinical manifestations of herpes zoster cases.
4.Current situation and relevant factors of evidence-based nursing competence of oncology nurse specialists

Objective:To explore the current situation of evidence-based nursing competence of oncology nurse specialists and its influencing factors, so as to provide reference for conducting oncology nurse specialist training courses.Methods:From October 12th to 20th, 2022, convenience sampling was used to select oncology nurse specialists from Beijing as the research subject. The survey was conducted using the General Information Questionnaire, Oncology Nurse Specialists ' Cognition and Training Questionnaire on Evidence-based Nursing, Evidence-based Nursing Competence Scale, Nurse Innovative Behavior Scale, short versions of the Evidence-based Practice (EBP) Beliefs Scale, the EBP Implementation Scale, and the EBP Organizational Culture and Readiness Scale. Single factor analysis and multiple linear regression were used to explore the influencing factors of evidence-based nursing competence of oncology nurse specialists. Results:A total of 183 oncology nurse specialists were included. The scores of evidence-based nursing competence and innovative behavior of nurses were 53 (39, 65) and 30 (25, 33) respectively. The scores of the EBP Beliefs Scale, the EBP Implementation Scale, and the EBP Organizational Culture and Readiness Scale were 9 (8, 10), 9 (7, 9), and 9 (6, 9), respectively. Multiple linear regression analysis showed that the main influencing factors of evidence-based nursing competence of oncology nurse specialists were nurse innovation behavior, evidence-based practice organizational culture and readiness, frequency of reading specialized literature, and duration of evidence-based nursing training ( P<0.05), which could explain 34.6% of the total variation. Conclusions:The evidence-based nursing competence of oncology nurse specialists needs to be improved. It is recommended that hospitals or nursing managers create an excellent research atmosphere, encourage innovation, enrich evidence-based database resources, and carry out evidence-based nursing training and practice to enhance nurses ' evidence-based nursing competence.

6.Advances of quadratus lumborum block in postoperative analgesia after hip joint surgery

Hip surgery is often associated with moderate to severe postoperative pain due to exten-sive invasion.A safe and effective postoperative pain treatment can promote patients'recovering activities and facilitate the early functional recovery of the hip after the surgery.Although the routinely used intrave-nous self-controlled analgesia has an exquisite analgesic effect,it carries the risks of postoperative nausea and emesis,excessive sedation,intestinal paralysis,and even respiratory depression.As an alternative to transversus abdominis plane block(TAPB),lumbar quadratus block(QLB)has been gradually applied to the hip joint surgery.In this paper,we review the anatomical basis and characteristics of QLB and its clini-cal application in hip surgery,in order to provide the postoperative analgesia application of QLB.
7.Risk Prediction Model for Radiation-induced Dermatitis in Patients with Cervical Carcinoma Undergoing Chemoradiotherapy

Purpose:
Radiation-induced dermatitis (RD) is a common side-effect of therapeutic ionizing radiation that can severely affect patient quality of life. This study aimed to develop a risk prediction model for the occurrence of RD in patients with cervical carcinoma undergoing chemoradiotherapy using electronic medical records (EMRs).
Methods:
Using EMRs, the clinical data of patients who underwent simultaneous radiotherapy and chemotherapy at a tertiary cancer hospital between 2017 and 2022 were retrospectively collected, and the patients were divided into two groups: a training group and a validation group. A predictive model was constructed to predict the development of RD in patients who underwent concurrent radiotherapy and chemotherapy for cervical cancer. Finally, the model's efficacy was validated using a receiver operating characteristic curve.
Results:
The incidence of radiation dermatitis was 89.5% (560/626) in the entire cohort, 88.6% (388/438) in the training group, and 91.5% (172/188) in the experimental group. The nomogram was established based on the following factors: age, the days between the beginning and conclusion of radiotherapy, the serum albumin after chemoradiotherapy, the use of single or multiple drugs for concurrent chemotherapy, and the total dose of afterloading radiotherapy. Internal and external verification indicated that the model had good discriminatory ability. Overall, the model achieved an area under the receiver operating characteristic curve of .66.
Conclusions
The risk of RD in patients with cervical carcinoma undergoing chemoradiotherapy is high. A risk prediction model can be developed for RD in cervical carcinoma patients undergoing chemoradiotherapy, based on over 5 years of EMR data from a tertiary cancer hospital.
8.Automatic bone age estimation of costal cartilage CT Images based on deep learning

Objective To use the deep learning methods to extract features of the 1st to 7th adult costal cartilage CT reconstruction images to realize the automatic estimation of adult costal cartilage bone age.Methods 625 male and 625 female samples aged between 20 and 70 years old were collected retrospectively,and the corresponding VRT images were reconstructed by volume rendering technology(VRT).After image preprocessing and data augmentation,500 cases were used as the training set and 125 cases as the test set.The performance of ResNet,ResNeXt,DenseNet and GoogleNet networks was evaluated by using 5-fold cross-validation,and the average value of 5-fold cross-validation results was taken as the final estimation result.Results The ResNet50 network achieved the best results in both male and female datasets.The mean absolute error was 4.56 years and 3.91 years,the accuracy rate was 64.00%and 70.88%in the range of±5.0 years,88.96%and 94.40%in the range of±10.0 years,respectively.Conclusion Compared with traditional methods and machine learning methods,the deep learning models can avoid the influence of human factors,greatly improve the accuracy of adult costal cartilage bone age estimation,and reduce the error between predicted age and real age,which has high clinical application value.
9.Correlation between medical narrative ability, empathy ability and general self-efficacy of clinical nurses

Objective:To explore the correlation between medical narrative ability, empathy ability and general self-efficacy of clinical nurses.Methods:A total of 106 clinical nurses from Jiaozuo People's Hospital were selected as the research object by convenient sampling. The Narrative Competence Scale, Empathy Ability Scale and General Self-efficacy Scale (GSES) were used to investigate and analyze the correlation of clinical nurses' medical narrative ability, empathy ability and general self-efficacy.Results:Among 106 clinical nurses, the scores of the Narrative Competence Scale, Empathy Ability Scale and GSES were (149.38±21.03) , (108.89±16.32) , and (24.31±5.17) respectively. The medical narrative ability of clinical nurses was positively correlated with empathy ability and general self-efficacy ( r=0.627, 0.569; P<0.001) , and empathy ability was positively correlated with general self-efficacy ( r=0.518, P<0.001) . Conclusions:The medical narrative ability, empathy ability and general self-efficacy of clinical nurses are positively correlated, suggesting that the training and management of clinical nurses should be strengthened to promote the improvement of their various abilities.
10.Exploring pedagogical content knowledge of adaptive physical education for teachers using WHO Rehabilitation Competency Framework

ObjectiveTo explore the pedagogical content knowledge of adaptive physical education teachers using the World Health Organization Rehabilitation Competency Framework (RCF), to construct an adaptive physical education competency system and establish an adaptive physical education and training system and accreditation method for teachers. MethodsUsing RCF and referring to the U. S. Adapted Physical Education National Standards 3rd version (APENS-3), we examined the pedagogical content knowledge of adaptive physical education for teachers. The structure and content of the key pedagogical knowledge contents of adaptive physical education teachers were systematically studied, and the core professional competencies of adaptive physical education teachers were constructed. ResultsBased on the RCF and teaching activities, the teaching-related core competencies of the adapted physical education teachers were constructed from the knowledge and skills in the domains of practice (teaching), and learning and development (continuing education) of RCF, and the structure and content of the knowledge of adapted physical education teachers were constructed in the five domains: students, teachers, teaching, curriculum and educational evaluation. This competency framework could be applied in the fields of professional teaching competency identification, continuing education curriculum development and continuing education competency training, and performance evaluation for adaptive physical education teachers. ConclusionIn accordance with the RCF and with reference to APENS-3, the structure of pedagogical content knowledge and its core contents for adaptive physical education teachers were systematically explored. These structures and the core contents can be applied in the identification of competencie, the development of continuing education curriculum and competency training, and the assessment of performance for adaptive physical education teachers.
