1.Development and validation of a multi-modality fusion deep learning model for differentiating glioblastoma from solitary brain metastases

Objective:Glioblastoma(GBM)and brain metastases(BMs)are the two most common malignant brain tumors in adults.Magnetic resonance imaging(MRI)is a commonly used method for screening and evaluating the prognosis of brain tumors,but the specificity and sensitivity of conventional MRI sequences in differential diagnosis of GBM and BMs are limited.In recent years,deep neural network has shown great potential in the realization of diagnostic classification and the establishment of clinical decision support system.This study aims to apply the radiomics features extracted by deep learning techniques to explore the feasibility of accurate preoperative classification for newly diagnosed GBM and solitary brain metastases(SBMs),and to further explore the impact of multimodality data fusion on classification tasks. Methods:Standard protocol cranial MRI sequence data from 135 newly diagnosed GBM patients and 73 patients with SBMs confirmed by histopathologic or clinical diagnosis were retrospectively analyzed.First,structural T1-weight,T1C-weight,and T2-weight were selected as 3 inputs to the entire model,regions of interest(ROIs)were manually delineated on the registered three modal MR images,and multimodality radiomics features were obtained,dimensions were reduced using a random forest(RF)-based feature selection method,and the importance of each feature was further analyzed.Secondly,we used the method of contrast disentangled to find the shared features and complementary features between different modal features.Finally,the response of each sample to GBM and SBMs was predicted by fusing 2 features from different modalities. Results:The radiomics features using machine learning and the multi-modal fusion method had a good discriminatory ability for GBM and SBMs.Furthermore,compared with single-modal data,the multimodal fusion models using machine learning algorithms such as support vector machine(SVM),Logistic regression,RF,adaptive boosting(AdaBoost),and gradient boosting decision tree(GBDT)achieved significant improvements,with area under the curve(AUC)values of 0.974,0.978,0.943,0.938,and 0.947,respectively;our comparative disentangled multi-modal MR fusion method performs well,and the results of AUC,accuracy(ACC),sensitivity(SEN)and specificity(SPE)in the test set were 0.985,0.984,0.900,and 0.990,respectively.Compared with other multi-modal fusion methods,AUC,ACC,and SEN in this study all achieved the best performance.In the ablation experiment to verify the effects of each module component in this study,AUC,ACC,and SEN increased by 1.6%,10.9%and 15.0%,respectively after 3 loss functions were used simultaneously. Conclusion:A deep learning-based contrast disentangled multi-modal MR radiomics feature fusion technique helps to improve GBM and SBMs classification accuracy.
2.Measurement and analysis of peak skin dose for patients withinterventional cardiology procedures

Objective :
To obtain the peak skin dose (PSD) of patients with interventional cardiology procedures and toevaluate the risk of deterministic effects.
Methods :
Gafchromic XR RV3 films were used in a Level A tertiary hospital inBeijing to measure the PSD of patients who underwent interventional cardiology procedures. The measurement focused onfour common types of procedures, including coronary angiography, percutaneous transluminal coronary angioplasty,cathet-er radiofrequency ablation, and congenital heart disease. The films were scanned by EPSON EXPRESSION 10000XL andanalyzed by FILM QA ProTM 2014 software.
Results :
PSD was measured in 59 patients with interventional cardiologypro-cedures, including 23 with coronary angiography, 21 with percutaneous transluminal coronary angioplasty, 9 with catheterradiofrequency ablation, and 6 with congenital heart disease. The seven patients with PSD ≥ 2 Gy all underwentpercu-taneous transluminal coronary angioplasty, one with PSD > 3 Gy and six with PSD < 2 Gy.
Conclusion
The PSD of somepatients with interventional cardiology surgery exceeded the dose threshold of deterministic effects recommended by theICRP 118. There is a risk of deterministic effects in interventional cardiology surgery, especially in patients withpercu-taneous transluminal coronary angioplasty.
3.Practice and exploration of graduate student management by form teachers in radiation health scientific research institutes

Graduate education is critical for cultivation of high-level talents and promotion of technological innovation. Strengthening the management of graduate education is of great practical significance for ensuring and improving the quality of graduate education. The expansion of graduate education brings challenges to educational resources and management models, requiring research institutions to ensure educational quality and adapt to social needs. Based on the particularity of research institutes, this article takes the practice of radiation health graduate education as an example to explore the graduate student management elements of form teachers in research institutes. Our research provides a reference for improving the management level of graduate students in research institutes. Graduate form teachers are organizers, implementers, and mentors of graduate ideological education, academic culture, daily management, employment and entrepreneurship, and mental health education. They play an important role as a bridge and link in graduate education and cultivation. Faced with the challenges of graduate education in the new era, form teachers should continuously explore work methods that adapt to the needs of the era. They should also focus on reinforcing ideological and political theories, improving professional skills, establishing and improving management systems, and leveraging the role of student cadres to ensure the high-quality development of graduate education in research institutes.
4.Application of distance education in radiological health continuing education and training

The implementation of distance education in radiological health continuing education and training was analyzed to provide a reference for improving the quality of radiological health continuing education. The significance, measures, and advantages revealed that the application of distance education in radiological health continuing education and training was conductive to improving the quality of training and fulfilled the needs of radiological health technicians for high-quality training resources. In the future, we will continue to implement and improve distance education, promoting the high-quality development of radiological health continuing education and training.
5.Analysis of national quality control testing training on radiodiagnostic and radiotherapy equipment from 2018 to 2022

Objective To analyze the implementation of national training on quality control testing of radiodiagnostic and radiotherapy equipment from 2018 to 2022, and to summarize the effects of the training program. Methods We collected and analyzed the information on the national training program for quality control testing of radiodiagnostic and radiotherapy equipment organized by the National Institute for Radiological Protection, Chinese Center for Disease Control and Prevention from 2018 to 2022. Results Five sessions of national training on quality control testing of radiodiagnostic and radiotherapy equipment were conducted from 2018 to 2022, with each session lasting four days. The proportion of teachers with senior professional titles was 87.5% (35/40). There were a total of 1045 attendances for radiological technician training, among which 75.1% (785/1045) had intermediate or lower technical titles and 87.1% (910/1045) had a bachelor’s degree or above. The qualified rate of training was 97.7% (905/926). In terms of the content, arrangement, and other key parts of training, the degrees of satisfaction of the trainees were all above 96.0%. Conclusion The national training program for quality control testing of radiodiagnostic and radiotherapy equipment worked well, with both the qualified rate and satisfaction degree above 95.0%. The program has helped professionals better understand and master the quality control testing of radiodiagnostic and radiotherapy equipment, and improved the high-quality development of continuing radiological education.
6.Peak skin dose measurements for patients in neuro-interventional procedures using radiochromic film

Objective:To determine the peak skin dose (PSD) to patients from neuro-interventional procedures and evaluate the risk of the deterministic effect.Methods:Gafchromic XR RV3 films were used in a level A tertiary hospital in Beijing to measure the patients′ PSD from neuro-interventional procedures, mainly three common types of procedures, including vascular embolization, vascular angioplasty and vascular angiography. The films were scanned by Epson Expression 10000XL, read by ImageJ software, and analyzed by Film QA Pro?2014 software.Results:PSD was measured in 23 embolizations, 14 stentings and 12 arteriography. There were 20 patients whose PSD were equal or greater than 2 Gy, including 15 in vascular embolization and 5 in angioplasty. The PSDs to patients in cerebral arteriography were all below 2 Gy. The PSDs to some of the patients were higher than the threshold for deterministic effect recommended by ICRP Publication 118.Conclusions:There is a risk of deterministic effect in neurointerventional procedures. It is suggested that the patients be followed up to observe their radiation injury as well as to know in time the subsequent diagnosis and treatment.
7.Analysis of risk factors of early enteral nutrition intolerance in extremely severe burn patients

Objective:To explore the risk factors of early enteral nutrition intolerance in extremely severe burn patients.Methods:A retrospective case-control study was performed. From January 2018 to December 2020, seventy-six adult patients with extremely severe burns who met the inclusion criteria were admitted to Hwa Mei Hospital of University of Chinese Academy of Sciences, including 55 males and 21 females, aged (45±11) years with burns of 62% (52%, 82%) total body surface area. Depending on the patient's tolerance to early enteral nutrition, they were divided into tolerance group (47 patients) and intolerance group (29 patients), and their clinical data were statistically analyzed, including age, sex, body mass index (BMI), underlying disease, total burn area, full-thickness burn area, abbreviated burn severity index (ABSI) score, implementation of mechanical ventilation on the day of admission, stable shock state, vomiting before feeding. The following data were recorded including the onset time, duration length, and frequency of enteral nutrition intolerance of patients in intolerance group, and the number of operations, the length of hospitalization, the occurrence of sepsis within 2 weeks after injury, the outcome, as well as the serum hypersensitive C-reactive protein (hs-CRP), albumin, fasting blood glucose, alanine aminotransferase (ALT), aspartate aminotransferase (AST), and γ-glutamyl transpeptidase (γ-GT) on post burn day (PBD) 1, 5, 9, and 13 of patients in the two groups. Data were statistically analyzed with independent sample t test, Mann-Whitney U test, and chi-square test to screen the related factors of early enteral nutrition intolerance of the patients. Binary univariate and multivariate logistic regression analysis were used to analyze the independent risk factors of early enteral nutrition intolerance of the patients. Results:There were no statistically significant differences in age, sex, BMI, and percentage of underlying disease of patients between the two groups ( P>0.05). The total burn area, full-thickness burn area, ABSI score, mechanical ventilation percentage on the day of admission, percentage of unstable shock period, percentage of vomiting before feeding of patients in intolerance group were significantly higher than those in tolerance group ( Z=-4.559, -3.378, -4.067, χ 2=18.375, 23.319, 8.339, P<0.01). In intolerance group, the onset time of intolerance was (9±4) d after injury, and the duration length was 4 (2, 6) d, with a total of 46 times occurred. Compared with tolerance group, the percentage of sepsis and mortality of patients in intolerance group were significantly higher within 2 weeks after injury ( χ 2=16.571, 12.665, P<0.01). The number of operation and length of hospitalization of patients in the two groups were similar ( P>0.05); however the length of hospitalization of patients in the intolerance group was significantly more than that in tolerance group after excluding the death cases ( Z=-2.266, P<0.05). On PBD 1, the level of fasting blood glucose and AST of patients in intolerance group were significantly higher than those in tolerance group ( t=3.070, Z=-3.070, P<0.01). On PBD 5, the levels of hs-CRP, albumin, fasting blood-glucose, ALT, AST, and γ-GT of patients in the two groups were similar ( P>0.05). On PBD 9, the level of hs-CRP of patients in intolerance group was significantly higher than that in tolerance group ( t=2.836, P<0.01), and the levels of ALT and γ-GT of patients in intolerance group were significantly lower than those in tolerance group ( Z=-3.932, -2.052, P<0.05 or P<0.01). On PBD 13, the level of hs-CRP of patients in intolerance group was significantly higher than that in tolerance group ( t=3.794, P<0.01), and the levels of fasting blood glucose, ALT, and γ-GT of patients in intolerance group were significantly lower than those in tolerance group ( t=-2.176, Z=-2.945, -2.250, P<0.05 or P<0.01). Binary univariate logistic regression analysis showed that total burn area, full-thickness burn area, ABSI score, implementation of mechanical ventilation on the day of admission, unstable shock period, vomiting before feeding, and fasting blood-glucose on PBD 1 of patients were related to early enteral nutrition intolerance (odds ratio=1.086, 1.052, 1.775, 9.167, 12.797, 10.125, 1.249, 95% confidence interval=1.045-1.129, 1.019-1.085, 1.320-2.387, 3.132-26.829, 4.199-39.000, 2.003-51.172, 1.066-1.464, P<0.01). Multivariate logistic regression analysis showed that the large total burn area, unstable shock period, vomiting before feeding, and high fasting blood-glucose on PBD 1 of patients were the independent risk factors of early enteral nutrition intolerance in patients (odds ratio=1.073, 6.390, 9.004, 1.246, 95% confidence interval=1.021-1.128, 1.527-26.734, 1.134-71.496, 1.007-1.540, P<0.05 or P<0.01). Conclusions:The percentage of early enteral nutrition intolerance is very high in extremely severe burn patients, which is closely related to poor prognosis. Large total burn area, vomiting before feeding, unstable shock phase, high fasting glucose on PBD 1 of patients are the independent risk factors for early enteral nutrition intolerance in extremely severe burn patients. The benefits and risks should be carefully evaluated before starting enteral nutrition in such patients, and early enteral nutrition should not be blindly pursued.
8. Epidemiological investigation of 511 adult inpatients with gas burns

Objective:
To analyze the epidemiological characteristics of adult inpatients with gas burns in the Department of Burns of Hwa Mei Hospital of University of Chinese Academy of Sciences (hereinafter referred to as the author′s unit) , so as to provide evidence for the prevention of gas burn.
Methods:
Medical records of all inpatients with flame burns admitted to the author′s unit from January 2011 to December 2017 were collected. The percentage of adult inpatients with gas burns in total inpatients with flame burns in the same period, and their gender, age, injury season, accident place, burn severity, common compound injury, complication, population caliber, education, industry, as well as the pre-injury disease and prognosis of elderly inpatients with gas burns were retrospectively analyzed. In addition, the age, accident place, education, and industry of the floating population in the adult inpatients with gas burns were analyzed separately and compared with the total population of adult inpatients with gas burns in 7 years. Data were processed with chi-square test or Fisher′s exact probability test (Monte Carlo algorithm).
Results:
During the 7 years, 1 490 inpatients with flame burns were admitted to the author′s unit, among which 511 were adult inpatients with gas burns, accounting for 34.30%. The number of adult inpatients with gas burns increased gradually during the 7 years, but its percentage in the total inpatients with flame burns during the same period showed no significant difference (
10.Evaluation of the sensitivity and specificity of corneal visualization scheimpflug technology biomechanical index in the diagnosis of keratoconus

Objective To study the sensitivity and specificity of parameter Corvis biomechanical index (CBI)— a new biomechanical index of Corvis ST in the diagnosis of keratoconus and evaluate the role of CBI in the diagnosis of keratoconus and the change of biomechanic.Methods A diagnostic trial study was adopted,and 66 eyes from 49 keratoconic patients (keratoconus group) and 91 right eyes from 91 myopic patients (control group) from April in 2018 to August in 2018 in Henan Eye Hospital were enrolled.Pentacam and Corvis ST examinations were performed by the same operator after the basic eye examinations.Using the evaluation of diagnostic test,consistency test,and receiver operating characteristic (ROC) curve analysis,gain the outcome of the sensitivity,specificity,consistency,Youden index,and area under the ROC curve of the parameter CBI.This study protocol was approved by Ethic Committee of Henan Eye Hospital and followed the Declaration of Helsinki.Written informed consent was obtained from each patient prior to any medical examination.Results Sixty eyes were diagnosed as keratoconus by the parameter CBI of Corvis ST.Evaluation of diagnosis test:the sensitivity was 97.0%,and the specificity was 97.8%;consistency check:Kappa =0.948,P<0.05.ROC curve analysis:the sensitivity was 98.5%,the specificity was 96.8%,Youden index was 96.3%,P<0.000 1 and AUC was 0.996.Conclusions CBI can separate healthy eyes from keratoconic eyes with highly sensitivity and specificity,which was highly consistency with results of Rabinowitz keratoconus diagnostic criteria.CBI could be used as a new biomechanical indicator for the diagnosis of keratoconus.
