1.Clinical Characteristics and Influencing Factors of Rheumatoid Arthritis in Patients with Cold Dampness Obstruction Syndrome

ObjectiveTo study the clinical characteristics and influencing factors of rheumatoid arthritis (RA) in the patients with cold dampness obstruction syndrome. MethodsThe RA patients treated in the Department of Traditional Chinese Medicine and Rheumatology of the China-Japan Friendship Hospital from August 2022 to June 2024 were selected. The demographic information, clinical data, laboratory test results, and traditional Chinese medicine (TCM) symptom information were collected for syndrome differentiation, on the basis of which the characteristics and influencing factors of cold dampness obstruction syndrome were analyzed. ResultsA total of 258 RA patients were selected in this study, including 88 (34.1%) patients with cold dampness obstruction syndrome, 53 (20.5%) patients with dampness and heat obstruction syndrome, 31 (12.0%) patients with wind dampness obstruction syndrome, 29 (11.2%) patients with liver-kidney deficiency syndrome, 19 (7.4%) patients with Qi-blood deficiency syndrome, 14 (5.4%) patients with phlegm-stasis obstruction syndrome, 15 (5.8%) patients with stasis obstructing collateral syndrome and 9 (3.5%) patients with Qi-Yin deficiency syndrome. The patients were assigned into two groups of cold dampness obstruction syndrome and other syndromes. The group of cold dampness obstruction syndrome had lower joint fever, 28-tender joint count (TJC28), and 28-joint disease activity score (DAS28)-C-reactive protein (CRP) and higher central sensitization, cold feeling of joints, fear of wind and cold, cold limbs, and abdominal distention than the group of other syndromes (P<0.05). The binary logistic regression analysis showed that central sensitization (OR 5.749, 95%CI 2.116-15.616, P<0.001) and DAS28-CRP (OR 0.600, 95% CI 0.418-0.862, P=0.006) were the independent factors influencing cold dampness obstruction syndrome in RA. ConclusionCold dampness obstruction syndrome is a common syndrome in RA patients. It is associated with central sensitization, cold feeling of joints, abdominal distension and may be a clinical syndrome associated with central sensitization.
2.Promoting Effect of Naringin on Airway Inflammatory Cell Apoptosis in Asthmatic Mice and Its Mechanism

Objective To investigate the regulation of naringin on apoptosis of airway inflammatory cells in asthmatic mice and its relationship with Tas2rs.Methods 36 BALB/c mice were randomly divided into 6 groups(n=6):blank control group,ovalbumin model group(OVA),dexamethasone group(10 mg·kg-1,positive control drug),naringin low,medium and high dose groups(10,20,40 mg·kg-1).Airway asthma was induced by OVA in all groups except the blank control group.After continuous administration of solvent,dexamethasone or naringin for 6 days,the respiratory function and the number of cells in alveolar lavage fluid of mice were measured,the lung and airway morphology of mice were observed,respiratory resistance(Rrs),lung elastic resistance(Ers)and respiratory compliance(Crs)were measured,and mRNA expression levels of lung tissue-related pro-apoptotic factors were measured.The mRNA expression levels of Tas2rs and its downstream genes were determined.Results Compared with OVA model group,naringin could dose-dependent decrease Rrs and Ers,increase Crs,and decrease the number of leukocytes,eosinophils,lymphocytes and neutrophils.Compared with OVA model group,the infiltration of inflammatory cells in lung tissue was significantly reduced in high-dose naringin group,and the mRNA expression levels of Tas2r108,Tas2r135,Tas2r143 and their downstream target genes α-gust and Trpm5 in lung and airway tissue were significantly decreased.The mRNA expression levels of proapoptotic factors P53,Bax and Casp3 were increased,while the mRNA expression levels of apoptosis inhibitor Bcl2 were decreased.Conclusion Naringin prophylactic administration can promote the activation of airway Tas2rs signal,decrease the number of airway inflammatory cells and alleviate airway injury in asthmatic mice.Naringin as a Tas2rs agonist can be developed as a potential anti-asthmatic agent.

It is widely used in medicine, health-care food, biocatalysis and other fields. Natto is rich in many pharmacological active substances and has significant medicinal research value. This paper summarizes the pharmacological activities and applications of natto in and outside China, so as to provide references for further research and development of natto.
4.Clinical study of Yiqi Huoxue decoction on improving cancer pain and cancer-related fatigue in patients with malignant tumors

[摘 要] 目的:探究益气活血汤对恶性肿瘤患者癌痛及癌因性疲乏(CRF)的疗效。方法: 选取2020年1月至2022年12月间安徽省中医药大学第二附属医院收治的82例确诊发生CRF的恶性肿瘤患者(气血亏虚证),采用随机数字表余数分组法将其分为对照组与观察组,每组各41例。对照组患者采用常规止痛、止吐、化痰等对症治疗及健康、心理指导,观察组患者在对照组干预的基础上联合益气活血汤治疗,4周为1个疗程。治疗前及治疗4周后,对两组患者进行中医证候积分评估,以积分变化评估中医临床疗效;采用修订版Piper疲乏量表(RPFS)评估CRF的改善情况;采用数字疼痛分级法(NRS)评分比较癌痛情况;检测患者外周血纤维蛋白原(FIB)、D-二聚体(D-D)评价凝血功能差异,检测患者肝、肾功能指标以评估益气活血汤治疗的安全性。结果:治疗前,两组患者在中医证候积分、RPFS评分、NRS评分及外周血FIB、D-D方面的差异均无显著统计学意义(均P>0.05)。治疗4周后,两组患者在神疲乏力、面色淡白或萎黄、自汗、失眠健忘、手足麻木的证候评分及总积分均较治疗前明显降低(均P<0.05),且观察组各项评分均低于对照组(均P<0.05),中医临床疗效明显高于对照组(P<0.05);两组患者RPFS各维度评分及总分均较治疗前降低(均P<0.05),观察组行为、情感、感觉维度RPFS评分及总分均低于对照组(均P<0.05),观察组CRF的改善明显优于对照组(P<0.05);两组患者NRS评分及外周血FIB、D-D指标均较治疗前降低(均P<0.05),且观察组均低于对照组(均P<0.05)。两组患者治疗期间均未发生肝功能、肾功能等明显异常,说明益气活血汤安全性良好。结论:益气活血汤可纠正气血亏虚之证,改善机体凝血功能,促进恶性肿瘤患者CRF及癌痛的减轻,临床应用价值较高。
5.Analysis of the predictive value of serum Nesfatin-1 combined with the Status Epilepticus Severity Score in the short-term prognosis of children with status epilepticus

Objective:To analyze the predictive value of serum Nesfatin-1 combined with the Status Epilepticus Severity Scale (STESS) score on the short-term prognosis of children with status epilepticus (SE).Methods:A clinical data of 145 children with SE who were admitted to the Children′s Hospital Affiliated to Zhengzhou University, Henan Children′s Hospital, Zhengzhou Children′s Hospital, from January 2016 to January 2020 were analyzed retrospectively.After admission, the serum levels of Nesfatin-1 and the STESS score were measured.According to the Glasgow Outcome Scale (GOS) score at discharge, children with SE were divided into poor prognosis group (<5 scores) and good prognosis group (5 scores). Univariate and multivariate Logisitc regression analyses were performed to analyze influence of the serum Nesfatin-1 level and STESS score on the short-term prognosis of children with SE.Receiver operating characteristic (ROC) curve was depicted to evaluate the predictive value of serum Nesfatin-1 level combined with STESS score in the short-term prognosis of children with SE. Results:Twenty-five cases out of 145 (17.24%) children with SE were discharged with a GOS score of <5 (poor prognosis group), 120 cases were in the good prognosis group.In the poor prognosis group, the overall attack (88.00% vs.66.67%), attack time of SE > 1 h (76.00% vs.27.50%), admission to child intensive care unit(PICU) (76.00% vs.37.50%), implementation of endotracheal intubation (16.00% vs.5.00%), abnormal electroencephalogram(EEG) results (73.91% vs.41.03%), abnormal proportion of head imaging results (82.61% vs.29.49%), serum Nesfatin-1 level[(3.65±1.45) μg/L vs.(2.20±0.77) μg/L] and STESS score[(3.01±0.75) points vs.(1.80±0.60) points] were significantly higher than those in the good prognosis group (all P<0.05). Logistic regression analysis showed that the attack time of SE > 1 h, admission to PICU, abnormal EEG, abnormal proportion of head imaging results, serum Nesfatin-1 level and STESS score were independent risk factors for the poor short-term prognosis of children with SE ( OR=4.217, 3.456, 2.626, 4.109, 3.040 and 2.012, respectively, all P<0.001). The cut-off value of serum Nesfatin-1 level and STESS score was 3.01 μg/L and 2.38 points, respectively.The Youden index and AUC of the combination of serum Nesfatin-1 level and STESS scores were 0.736 and 0.921 (95% CI: 0.861-0.959), respectively, which were better than those of single detection of either serum Nesfatin-1 level [Youden index 0.447; AUC 0.795(95% CI: 0.720-0.858)] or STESS scores [Youden index 0.562; AUC 0.859(95% CI: 0.792-0.911)]. Conclusions:The abnormal increases in serum Nesfatin-1 level and STESS score are risk factors for poor prognosis of SE in children, and their combination has a high predictive value for the poor short-term prognosis.
6.Clinical application of non-incision removal of tunneled cuffed catheter

Objective:To explore the effectiveness and complications of non-incision removal of tunneled cuffed catheter (TCC).Methods:The clinical characteristics, surgical plans and complications of patients with TCC removal in the Renal Division of Peking University First Hospital from January 1, 2015 to December 31, 2020 were collected and analyzed retrospectively. The patients were divided into non-incision removal group and traditional incision removal group. The clinical characteristics, procedure success rate, procedural duration and complications were compared between the two groups.Results:A total of 349 patients were included in this study, for whom 368 catheter removal procedures were performed, including 286 procedures in the non-incision removal group, 75 procedures in the traditional incision removal group, and 7 procedures without records of surgical plans. There was no significant difference in age, sex, basic kidney diseases and catheter remaining time and location between the two groups (all P>0.05). Two procedures in the non-incision removal group and 1 procedure in the traditional incision removal group failed respectively, and there was no significant difference in the procedure success rate between the two groups (99.3% vs 98.7%, χ2=0.290, P=0.590). The procedural duration in the non-incision removal group was lower than that in the traditional incision removal group [(5.36±1.70) min vs (17.55±3.28) min, t=44.198, P<0.001]. Among the patients who needed TCC exchange, there was no significant difference in the selection of new catheter position between the two groups ( P=0.330). In terms of complications, there were 2 procedures of local hematoma in the non-incision removal group and 1 procedure of infection in the traditional incision removal group, and there was no severe complication in both groups. Conclusions:There was no significant difference in the procedural success rate and complications between non-incision removal group and traditional incision removal group, and non-incision procedure may be superior in reducing the procedure duration and harm less to the patients. Non-incision procedure is a safe and effective method to remove TCC.
7.Lingguizhugan Decoction, a Chinese herbal formula, improves insulin resistance in overweight/obese subjects with non-alcoholic fatty liver disease: a translational approach.

Lingguizhugan Decoction (LGZG) has been investigated in basic studies, with satisfactory effects on insulin resistance in non-alcoholic fatty liver disease (NAFLD). This translational approach aimed to explore the effect and underlying mechanism of LGZG in clinical setting. A randomized, double-blinded, placebo-controlled trial was performed. A total of 243 eligible participants with NAFLD were equally allocated to receive LGZG (two groups: standard dose and low dose) or placebo for 12 weeks on the basis of lifestyle modifications. The primary efficacy variable was homeostasis model assessment of insulin resistance (HOMA-IR). Analyses were performed in two populations in accordance with body mass index (BMI; overweight/obese, BMI ⩾ 24 kg/m2; lean, BMI < 24 kg/m2). For overweight/obese participants, low-dose LGZG significantly decreased their HOMA-IR level compared with placebo (-0.19 (1.47) versus 0.08 (1.99), P = 0.038). For lean subjects, neither dose of LGZG showed a superior effect compared with placebo. Methylated DNA immunoprecipitation sequencing and real-time qPCR found that the DNA N6-methyladenine modification levels of protein phosphatase 1 regulatory subunit 3A (PPP1R3A) and autophagy related 3 (ATG3) significantly increased after LGZG intervention in overweight/obese population. Low-dose LGZG effectively improved insulin resistance in overweight/obese subjects with NAFLD. The underlying mechanism may be related to the regulation of DNA N6-methyladenine modification of PPP1R3A and ATG3. Lean subjects may not be a targeted population for LGZG.

8.Investigation of AIDS cognition and testing willingness among freshmen in Fengtai District, Beijing

Objective:
To examine college students awareness of AIDS (acquired immunodeficiency syndrome) and HIV (human immunodeficiency virus), as well as their willingness to undergo testing, and to provide guidance for further education targeted towards AIDS prevention.
Methods:
The respondents were selected from two companies of military training camps in 4 universities in Fengtai District of Beijing using cluster sampling, and a questionnaire was used to obtain relevant information among 1 248 college freshmen. The content of the questionnaire included basic information about the students, awareness of AIDS, and willingness to undergo testing.
Results:
A total of 87.18% students were familiar with AIDS related knowledge, and 62.98% students intended to have HIV tests in the future. Willingness to be tested for HIV was higher among not local students (67.39%) than among local students(55.65%)(χ 2=17.32, P<0.05). The willingness to get HIV testing was higher among students who had an understanding of AIDS (65.26%) than among those who lacked an awareness(47.50%)(χ 2=18.87, P<0.05). In terms of the willingness to be tested for HIV, the main concerns focused on personal privacy (23.24%) and the cost (18.59%). Multivariate regression analysis showed that improving students awareness of five of the items related to a basic knowledge of AIDS may increase their willingness to get HIV testing(P<0.05). Most students indicated a preference to get HIV testing at a hospital (68.51%) or at the Centers for Disease Control and Prevention(42.79%).
Conclusion
The willingness to get HIV testing can be increased by launching an AIDS health education program that targets weak knowledge points with respect to AIDS awareness.
9. The efficacy and safety of Rivaroxaban for elderly patients with thrombotic diseases

Objective:
To investigate the efficacy and safety of Rivaroxaban for elderly patients with thrombotic diseases.
Methods:
This was a retrospective study.A total of 301 elderly patients taking Rivaroxaban from October 2012 to November 2017 at the Second Medical Center of the Chinese PLA General Hospital were consecutively selected.The ages ranged from 60 to 102 years, with an average age of(86.5±8.4)years.Anticoagulation regimens were developed based on comprehensive evaluation of indications, creatinine clearance, ischemia and bleeding risk.Patients were divided into a Rivaroxaban 2.5-5.0 mg/d group(n=72), a 10.0 mg/d group(n=205), and a 15.0-20.0 mg/d group(n=24). Hepatic function, renal function, and coagulation indexes were measured before and after the administration of Rivaroxaban.Fatal bleeding, cardiovascular deaths, all-cause deaths, non-fatal bleeding and thromboembolic events were recorded during the follow-up period.
Results:
The average dose of Rivaroxaban was(9.3±3.0)mg/d, and the minimum dose was 2.5 mg/d.The average follow-up time was(14.9± 13.9)months and the longest follow-up time was 48 months.One patient had intracranial bleeding.Twenty patients(6.6%)died with a cumulative incidence of 25.2%, three(1.0%)died of cardiac events, and 55.0% died of pneumonia and multiple organ failure.Forty patients(13.3%)had non-fatal hemorrhagic events with a cumulative incidence of 42.4%.Seven patients(2.3%)had thromboembolic events with a cumulative incidence of 16.0%, including 2 cases of non-fatal myocardial infarction, 3 cases of cerebral infarction and 2 cases of deep vein thrombosis.After treatment, levels of prothrombin time and fibrinogen significantly increased while levels of D-dimer significantly deceased(
10.Influence of OSAHS on nocturnal blood pressure and angina pectoris in patients undergoing cardiac artery bypass graft

Objective:To explore the influence of obstructive sleep apnea hypopnea syndrome (OSAHS) on nocturnal blood pressure and angina pectoris in patients undergoing cardiac artery bypass graft (CABG) .Methods:From January to December 2018, we selected 76 patients with simple CABG at Beijing Anzhen Hospital affiliated to Capital Medical University by purposive sampling. Before surgery, patients were treated with the portable sleep respiratory monitoring and bedside blood pressure monitoring. Patients were divided into group of mild or no OSAHS ( n=35) and group of moderate or severe OSAHS ( n=41) based on apnea hypopnea index (AHI) . We compared the differences in the baseline data, sleep respiratory examination indexes, nocturnal angina pectoris, blood pressure at 21∶00, 3∶00 and 6∶00 among patients between two groups. Results:Among CABG patients with moderate or severe OSAHS, the systolic pressure at 3∶00 and 6∶00 was (115.8±12.5) mmHg and (156.8±17.5) mmHg respectively all higher than those among CABG patients with mild or no OSAHS with statistical differences ( P<0.05) ; the incidence of nocturnal angina pectoris was 29.27% (12/41) higher than that [2.86% (1/35) ]among CABG patients with mild or no OSAHS with a statistical difference ( P<0.05) . Logistic regression analysis showed that the systolic pressure at 3∶00 could affect the occurrence of nocturnal angina pectoris among CABG patients with OSAHS ( OR=1.050, 95% CI 1.003-1.100, P=0.039) ; the decrease of average oxygen saturation could also affect the occurrence of nocturnal angina pectoris among those patients ( OR=0.628, 95% CI 0.397-0.993, P=0.046) ; the differences were all statistical. Conclusions:Early morning systolic pressure of CABG patients with OSAHS raises, and OSAHS may increase the incidence of nocturnal angina pectoris among CABG patients.
