1.Analysis on the application of magnetic resonance enterography in children in Tongji Hospital, Tongji Medical College, Huazhong University of Science and Technology from 2012 to 2023

Objective:To review the application of pediatric magnetic resonance enterography (MRE) at Tongji Hospital, Tongji Medical College, Huazhong University of Science and Technology from 2012 to 2023, and to provide referenceable data for MRE use in complex pediatric clinical scenarios.Methods:This study was a cross-sectional study. The clinical and imaging data from children aged≤18 years who underwent MRE at Tongji Hospital between December 2012 and December 2023 were retrospectively analyzed. Out of 186 children who intended to receive the MRE examination, 12 failed, remainder 174 participants (mean age 14±4 years, range 1 month to 18 years) were included. Participants were categorized into an inflammatory bowel disease (IBD) group (118 cases) and a non-IBD group (56 cases), and further divided by age into infants (0-6 years, 8 cases), children (7-12 years, 37 cases), and adolescents (13-18 years, 129 cases). The overall image quality and the intestinal filling quality were scored using a 5-point scale. Statistical analyses included χ2 tests for age distribution, history of intestinal reconstruction surgery, and MRE success rates between IBD and non-IBD groups. Mann-Whitney U test was employed to compare subjective image quality scores between the abovementioned two groups. The Kruskal-Wallis test was used to detect the differences among the three age groups. Results:The success rate of MRE significantly increased with age: 66.7% (8/12) in infants, 88.1% (37/42) in children, and 97.7% (129/132) in adolescents ( χ2=15.39, P<0.001). A statistically significant difference in age distribution was observed between the IBD and non-IBD groups ( χ2=17.94, P<0.001). The proportion of infants in the non-IBD group was 14.3% (8/56), which was higher than that of the IBD group. The majority of the IBD group were adolescents, accounting for 78.8% (93/118). There was a statistically significant difference in the intestinal reconstruction surgery history ( χ2=2.83, P=0.005). The non-IBD group had a higher incidence of intestinal reconstructive surgery (21.4%, 12/56), compared to the IBD group (6.8%, 8/118). MRE intestinal filling quality or overall image quality scores between the IBD and non-IBD groups or among different age sub-groups were not statistically significant ( P>0.05). Conclusion:Juvenilization of non-IBD children and intestinal reconstructive surgery history could make the inspection more complex. High-quality imaging can still be achieved by adhering to technical specifications.
2.Meta-analysis of Efficacy and Safety of Bifid Triple Viable Capsule Combined With Quadruple Therapy for Helicobacter pylori Infection

Background:With the increasing incidence of antibiotic resistance and adverse reactions to Helicobacter pylori(Hp),it is difficult for proton pump inhibitor-based quadruple therapy to meet the actual clinical treatment needs,so it is necessary to seek another effective combination therapy program on this basis.In recent years,the application of microecological preparations represented by Bifid triple viable capsule has been widely valued in clinical practice with its unique advantages.Aims:To evaluate the efficacy and safety of Bifid triple viable capsule combined with quadruple therapy in the treatment of Hp infection.Methods:The databases of CNKI,Wanfang,VIP,CBM,PubMed,Web of Science from January 2012 to April 2023 were searched by computer,to collect the relevant literature published at home and abroad on the treatment of Hp infection with Bifid triple viable capsule combined with quadruple therapy,and to screen the included literature for quality evaluation using the Cochrane risk bias tool,and to extract relevant data.Data were Meta-analyzed with RevMan 5.4.Results:Ten randomized controlled trials were included,including 1 828 patients with or without underlying disease who were Hp positive.Meta-analysis showed that the eradication rate of Hp in Bifid triple viable capsule combined with quadruple therapy was significantly higher than that in quadruple therapy group(RR=1.23,95%CI:1.19-1.29,P<0.000 01),while the incidence of adverse reactions was significantly decreased(RR=0.37,95%CI:0.28-0.48,P<0.000 01).Conclusions:Bifid triple viable capsule combined with quadruple therapy can significantly increase the eradication rate of Hp and reduce the adverse drug reactions,which has practical significance to guide clinical application.
3.The recognition significance of pointing gestures in children with autism spectrum disorder and its relationship with functional development

Objective:To understand the identification value of pointing gestures in children with autism spectrum disorder(ASD) and its relationship with functional development.Methods:From December 2020 to November 2021, 1 099 children from Children’s Health Care Center of Beijing Children’s Hospital were tested by pointing gestures test, including 942 ASD children and 157 typical developed children.And the data of children's neuropsychological development scale from 800 children aged 1.0-5.9 were collected.SPSS 20.0 was used for statistical analysis. Trend test was used to analyze the distribution of pointing gestures test sensitivity in autistic children, and ANOVA was used to analyze the relationship between pointing gestures test scores and functional development fields.Results:The sensitivity of pointing gestures was 83.5% in children aged 1.0-10.0 years, 76.3%-93.1% in children aged 1.0-4.9 years, and 93.1%-95.1% in children aged 1.0-2.9.With the increase of age, the sensitivity of pointing gestures in autistic children (linear-by-linear association =164.889, P<0.001) and the Yoden index had a decreasing trend. The positive predictive value (91.53%-100.00%) and negative predictive value (75.36%-91.84%) were found in the children aged 1.0-10.0 years.The sensitivity of pointing gestures test was 44.9% in children with mild autism aged 1.0-10.0 years and 46.5%-65.9% in children with mild autism aged from 1.0-3.9 years. The sensitivity of pointing gestures test was 81.5% in children with moderate autism aged from 1.0-10.0 years and 87.3%-97.8% in children with moderate autism aged 1.0-3.9 years. The sensitivity of the pointing gestures test was 97.2% in children with severe autism aged 1.0-10.0 years, and 100.0% in children with severe autism aged 1.0-3.9 years. The sensitivity of the pointing gestures in mild, moderate and severe autism children decreased with age (linear-by-linear association values were 16.725, 64.232, 66.732 respectively, all P<0.001). The children with severe autism mainly scored 2 points (80.3%, 419/522) on the pointing gestures test , and children with moderate autism mainly scored 1 point(64.2%, 170/265) on the pointing gestures test. There were significant differences in functional development among different pointing gestures test groups.Functional development score in the autism children with 0 score of pointing gestures test was significantly higher than those with 1 score and 2 scores of pointing gestures test (all P<0.05). Conclusion:Pointing gestures has good sensitivity in children with autism (especially 1.0-4.9 years of age), and may serve as an objectively observable screening method. The better children with autism score on the pointing gestures, the better their functional development.
4.Correlation analysis of survival time in patients with advanced breast cancer and the scores of Care Evaluation Scale in cancer patients

Objective:To explore the value of Care Evaluation Scale (CES) in cancer patients for predicting survival time in patients with advanced breast cancer.Methods:From November 2018 to June 2020, the convenient sampling was used to select 140 patients with advanced breast cancer who were treated in Qingdao Central Hospital Affiliated to Qingdao University and 140 home caregivers as the research objects. Researchers contacted home caregivers of patients with advanced breast cancer to set up a time and place to conduct one-on-one surveys of research objects using the Cancer Patient CES. Pearson correlation was used to analyze the correlation between survival time and CES score in patients with advanced breast cancer. Multiple stepwise regression analysis was used to analyze the influencing factors of CES scores in patients with advanced breast cancer. A total of 140 questionnaires were distributed, of which 133 were valid, and the effective recovery rate was 95.00% (133/140) .Results:Among the 133 patients with advanced breast cancer, 76 were Luminal A, 19 were Luminal B, 12 were ERBB 2+ and 26 were Basal-like. Luminal type A advanced breast cancer patients had the longest survival time, and the differences were statistically significant compared with those of ERBB 2+ and Basal-like advanced breast cancer patients ( P<0.05) . There was no statistically significant difference in survival time between Luminal B type advanced breast cancer and Luminal A, ERBB 2+ and Basal-like advanced breast cancer patients ( P>0.05) . Luminal type A advanced breast cancer patients had the highest CES score, and the differences were statistically significant compared with those of Luminal type B, ERBB 2+ and Basal-like advanced breast cancer patients ( P<0.05) . There was no statistically significant difference in the CES score between Luminal B advanced breast cancer and Luminal A, ERBB 2+ and Basal-like advanced breast cancer ( P>0.05) . Pearson correlation analysis found that the survival time of patients with advanced breast cancer was positively correlated with the CES score ( r=0.892, P<0.05) . The influencing factors of CES empowerment in patients with advanced breast cancer included physical care provided by doctors, mental health care, environment, explanation or instructions by doctors to family members, collaboration and persistence, consideration for family members, availability, cost, explanation or instructions by doctors to patients and physical care provided by nurses. Conclusions:CES in cancer patients can effectively predict the survival time of patients with advanced breast cancer. The molecular types of patients with advanced breast cancer are different and the effect of predicting survival time is also different.
5.The status of thyroid volume of children aged 8-10 years and influencing factors in Hefei City

Objective To investigate the status and the influencing factors of thyroid volume of children aged 8-10 years in Hefei City.Methods In 2016,according to the requirements of "National Iodine Deficiency Disorders Surveillance Plan (the 2016 Edition)",the method of multistage stratified sampling was used in 9 counties (cities,districts) of Hefei.Five primary schools were randomly selected from each county (city,district),with one in each direction (the east,west,south,north and center),then 40 children aged 8-10 years (male ∶ female =1 ∶ 1,with similar age distribution) were randomly chosen in each primary school.The thyroid volume,height and weight were measured.In addition,a questionnaire with the items including personality,learning pressure,dietary habits,related disease history and contact history was designed and used to collect the data of the subjects.Results Totally 1 800 children aged 8-10 years were selected from 45 primary schools in 9 counties (cities,districts).The mean thyroid volume was (3.009 ± 1.197) ml.Totally 71 people were found with thyroid enlargement and the goiter rate was 3.9%,which was in the range described in the elimination iodine deficiency disorders (IDD) standard (< 5%).Univariate analysis showed that for children in different age groups,seafood consumption frequency,radiological examination,height,and body mass,the thyroid volumes were significantly different (P< 0.05);Multivariate linear regression showed that body weight and height were main factors affecting the thyroid volume of children aged 8-10 years in Hefei City,the standard partial regression coefficient are 0.288 and 0.140,respectively.Conclusions The thyroid volume of the 8-10 years old children is generally in the normal range in Hefei.Goiter rate is lower than the standard for elimination of iodine deficiency The findings have showed that not only age,but also height,body mass and other factors should be considered while evaluating the influencing factors of the thyroid volume of school-aged children.
6.Prevalence of multiple types of human adenovirus infection among children with severe acute respiratory infection in Shanghai

Objective To investigate the typing profiles of human adenovirus infection among children with severe acute respiratory infection ( SARI ) in Shanghai. Methods A total of 441 nasopharyngeal aspirates ( NPAs) were collected from hospitalized children with SARI in Shanghai Pediatrics Hospital Affiliated Fudan University from June 2013 to March 2014.Human adenovirus was detected by nested PCR followed molecular typing by sequencing.Then the phylogenetic and epidemiological analysis was conducted.Results From 441 hospitalized children with SARI, 51 of samples ( 11.6%, 95%CI:8.6%~14.6%) were detected as HAdV infection.Most of the SARI children with HAdV infection were distributed between 0.5 to 5 years old.Typing data shown that HAdV-B (17 of HAdV-3, 16 of HAdV-7) was most dominant as 64.7%(33/51),followed by HAdV-C as 27.5%(14/51);either HAdV-D (HAdV-8) or HAdV-E ( HAdV-4 ) contained only one case, and HAdV-F ( HAdV-41 ) found in two cases. Conclusion Multiple species( B-F) of HAdVs were circulated among children with SARI in Shanghai, and HAdV-B (HAdV-3, HAdV-7) was the most predominant species of circulation.
7.Construction of nsdAmgh gene disruption mutant in Strempomyces roseoflavus Men-myco-93-63.

Insertional mutagenesis is a widely used method to determine the function(s) of a gene. To study the function(s) of the gene nsdAmgh in Streptomyces roseoflavus, a homologous recombination vector pSRNA2500 was structured in this paper. The recombination donor vector was then transformed into Strempomyces roseoflavus strain Men-myco-93-63 by conjugative transfer. The transformants were subjected to selection under the pressure of high temperature and appropriate antibiotics. As a result, several disrupted mutants of nsdAmgh gene, with a phenotype of Am(s)Km(r), were isolated and verified using PCR and Dot-blotting and Southern blotting hybridization methods. Functional analysis showed that the disrupted mutants of nsdAmgh had a two-fold higher inhibition against Verticillium dahlia Kleb than that of the wild strain Men-myco-93-63, which all will provide a new study route for future research about positive and negative regulator in Men-myco-93-63.

8.Pathogenic analysis in different types of orthopedic implant infections.

BACKGROUNDDiversity of orthopedic infections with various local environments affects the pattern and prevalence of pathogens. It is not well-characterized whether different pathogens have different propensity to cause different types of orthopedic infections. We aimed to investigate the frequency of different pathogens derived from orthopedic infections, and determine the relationship between the prevalence of clinical isolates and the type of orthopedic implants, especially focusing on staphylococci.
METHODSFrom January 2006 to December 2011, orthopedic infections were identified retrospectively from clinical microbiology laboratory and orthopedic medical records. The sources of orthopedic infections were divided into two main groups: those associated with implants and those not associated with implants. Implants-associated infections were further subdivided into five subgroups: arthroplasty, internal fixation, external fixation, internal and external fixation, and others. We analyzed microbiological spectrum in different groups and subgroups. Antibiotic susceptibility of staphylococci was analyzed.
RESULTSOnly coagulase-negative staphylococci (CoNS) was significantly more likely to be associated with implants-associated infections (P = 0.029). The overall pathogens prevalence of arthroplasty was significantly different from other subgroups (P < 0.05). 65% isolates from external fixation was Gram-negative bacteria. Some percentage (55%) of S. aureus and (83%) CoNS were resistant to methicillin. No resistance to glycopeptide was seen in all of staphylococci.
CONCLUSIONSStaphylococcus aureus was the most frequent isolates in orthopedic infections but was not associated with the presence or absence of implants. Only CoNS was implants-associated, especially for arthroplasty infection. Cefazolin alone is not enough for orthopedic surgery prophylaxis in settings with a high prevalence of methicillin-resistant staphylococci.

9.Effects of propofol on apoptosis and invasiveness of human lung cancer cell line A549 cells

Objective To evaluate the effects of propofol on apoptosis and invasiveness of human lung cancer cell line A549 cells.Methods Human lung cancer cell line A549 were seeded onto 96-well plates (100 μl/well) and 6-well plates (2 000 μl/well) at a density of 2× 105 cells/ml,and cultured for24 h at 37 ℃ in 5% CO2.The cells were randomly divided into 2 groups (n =60 each) using a random number table:dimethyl sulfoxide (DMSO) group and propofol group (group P).In group P,propofol with the final concentration of 100 μmoYL was added.In group DMSO,0.5% DMSO with the final concentration of 0.5% was added.At 24 h of incubation with drugs,caspase-3 expression was detected by high content screening (HCS); the expression of matrix metalloproteinase (MMP-2) was detected by Western blot analysis.At 0.5,1 and 5 h of incubation,ERK1/2 expression was also measured using Western blot analysis.Results Compared with group DMSO,the expression of caspase-3 was up-regulated,the expression of MMP-2 was down-regulated,ERK1/2 expression was up-regulated at 0.5 of incubation and down-regulated at 1 h of incubation,and no significant change was found in ERK1/2 expression at 5 h of incubation in group P.Conclusion Propofol can promote apoptosis in A549 cells and inhibit invasiveness of human lung cancer cell line A549 cells.
10.Study on Diuretic Effect ofMori Cortex Aqueous Extracts and Chemical Split Fractions

This study was aimed to investigate the diuretic effect ofMori Cortex and to identify effective fractions inM. Cortex. Metabolic cages were used to firstly observe the diuretic effect ofM. Cortex aqueous extracts on rats. Medications were continuously given for 5 days to screen for the best dosage of diuretic effect. Picric acid assays were used to detect creatinine levels in serum and urine after 5-day medication. Then, the diuretic effect of each chemical split fraction inM. Cortex was studied in order to indentify the effective parts. The results showed that after administration ofM. Cortex aqueous extracts for 3 to 5 days, compared with the control group, there was a significant diuretic effect on rats (P<0.05 orP<0.01). And the medium-dose ofM. Cortex aqueous extracts showed the best effect (P<0.01). However,M. Cortex aqueous extracts had no significant effect on creatinine levels in serum and urine. Assays of diuretic effect of chemical split fractions ofM.Cortex indicated that compared with the control group, 30% ethanol fraction and fatty oil fraction had the best diuretic effect (P<0.01). It was concluded thatM.Cortex aqueous extracts had a significant diuretic effect. And the chemical fractions contributed mostly to this effect were mainly existed in the 30% ethanol fraction and fatty oil fraction.
