1.Quality Evaluation of Atractylodis Macrocephalae Rhizoma in Different Production Methods Based on Traditional Traits

ObjectiveBased on the experience of traditional quality evaluation, the quality of Atractylodis Macrocephalae Rhizoma(AMR) with different production methods such as direct seeding, transplanting after seedling raising, topping and non-topping, and difference in growth years was compared. MethodVernier caliper was used to measure the trait data of AMR in different production methods. Paraffin sections of AMR with different production methods were made by saffron solid green staining, and the microstructure was observed. The contents of water-soluble and alcohol-soluble extracts in AMR with different production methods were determined according to the 2020 edition of Chinese Pharmacopoeia. The content of water-soluble total polysaccharides in AMR with different production methods was detected by sulfuric acid-anthrone method. Fiber analyzer was used to detect the content of fiber components in AMR with different production methods. The contents of monosaccharides, oligosaccharides and some secondary metabolites in AMR with different production methods were detected by ultra performance liquid chromatography(UPLC), and the differences of chemical components were compared by multivariate statistical analysis methods such as principal component analysis(PCA) and partial least squares-discriminant analysis(PLS-DA). ResultIn terms of traits, the 3-year-old AMR with direct seeding and without topping was close to the high-quality AMR with "phoenix-head and crane-neck, strong sweetness and clear aroma" recorded in ancient materia medica, followed by the 3-year-old AMR with topping after transplanting, while the 2-year-old AMR with topping after transplanting with high market circulation rate was generally fat and strong with mild odor. In the microscopic aspect, the arrangement of xylem vessels and fiber bundles in the 3-year-old samples formed two obvious rings. Compared with the 2-year-old samples cultivated in Bozhou and Zhejiang, the 3-year-old samples without topping after transplanting had more wood fibers. In terms of chemical composition, the contents of 70% ethanol extract, fructose, glucose, sucrose, 1-kestose, atractylenolide Ⅰ, chlorogenic acid, neochlorogenic acid, cryptochlorogenic acid and other components in 3-year-old AMR with direct seeding and without topping were significantly higher than those in the other three samples(P<0.05). The contents of cellulose, 70% ethanol extract, sucrose, atractylenolide Ⅰ, atractylone and other components in 3-year-old AMR with topping after transplanting were significantly higher than those in the 2-year-old AMR with high market circulation rate(P<0.05), while the contents of water-soluble extract and water-soluble total polysaccharides in 2-year-old samples with topping after transplanting were significantly higher than those in the 3-year-old AMR with topping after transplanting, direct seeding and without topping(P<0.05). ConclusionUnder the current mainstream production mode, too much manual intervention makes AMR heavily enriched in polysaccharides and increased the yield, but the accumulation of sweet substances, fragrant substances and fiber substances is insufficient, which affects its quality. The current quality standard of AMR has some shortcomings in guiding the high quality production of it, it is suggested to revise the quality standard of AMR, supplement the quantitative analysis of secondary metabolites, and strengthen the production of imitation wild AMR.

3.Fasudil exerts a protective effect on down-regulation of IL-6 expression by inhibiting the NOTCH signaling pathway in EAM mice

【Objective】 To explore the effect and mechanism of Fasudil in the treatment of experimental autoimmune myocarditis (EAM) in mice so as to provide a theoretical basis for the clinical use of Fasudil in treating myocarditis. 【Methods】 Balb/c male mice were used as the research objects, and the EAM mice model was constructed using MyHC-α614-629 polypeptide. Mononuclear cells were isolated and cultured to detect the number of mononuclear cells in mouse spleen. Inflammation infiltration, fibrosis and IL-6 expression in mouse myocardial tissue were detected by HE staining, Masson staining and immunohistochemistry, respectively. The protein expressions of Notch1 and IL-6 were detected by Western blotting. qRT-PCR was used to detect the expressions of pro-inflammatory factors (IL-1α, IL-1β and IL-6) as well as key genes of TLRs and NOTCH signaling pathway. 【Results】 EAM mice showed increased HW, decreased BW, increased HW/e-BW, and increased inflammatory infiltration and fibrosis in myocardial tissue. The above-mentioned symptoms or pathological features were improved in EAM mice treated with Fasudil. The analysis showed that the pro-inflammatory factors IL-1α, IL-1β and IL-6 in the myocardial tissue of EAM mice were significantly increased, but only the expression of IL-6 was statistically different after Fasudil treatment compared with the control group. In addition, TLRs signaling pathway might also play an important role in the EAM mice treated with Fasudil. The expressions of IL-6 and Notch1 were consistent, and the expressions of the key genes of NOTCH signaling pathway (Notch1, Hes1 and Jag2) were down-regulated after Fasudil treatment. 【Conclusion】 Fasudil exerts a protective effect on down-regulation of IL-6 expression by inhibiting the NOTCH signaling pathway in EAM mice.

The hospital took such means as optimizing the rules of procedure, strengthening the pre-decision assessment, clarifying the principle of " two initiatives" , improving the " No.1 issue" system, strengthening the linkage between the Party committee and meeting the president′s office meeting, and standardizing the supervision and implementation. 5.Psychological condition and personality characteristics of patients with erectile dysfunction in various age groups

Objective:To investigate the differences of psychological condition of erectile dysfunction (ED) patients in different age groups.Methods:The emotion, personality and interpersonal relationship outpatients with ED were evaluated by the method of Symptom Checklist-90 (SCL-90), State Trait Anxiety Inventory (STAI) and Eysenck Personality Questionnaire (EPQ). A retrospective analysis was conducted and a total of 401 ED patients (aged 20-60) were divided into 4 groups [20-29(n=158), 30-39(n=182), 40-49(n=38), 50-60 (n=23)years old] from July 2018 to July 2019. The erectile function of the patients was evaluated by the international erectile function scale (IIEF-5). The symptoms of hostility, anxiety, psychotic, horrible, paranoid, obsession, somatization, interpersonal relationship and depression were evaluated by SCL-90. Furthermore, STAI was used to distinguish whether the patients had state or trait anxiety. Then EPQ was used to analyze the personality types of patients. The differences of SCL-90, STAI and EPQ between the patients and the national norm group were analyzed by the two sample t-test. The variance analysis was conducted to compare the score differences of scales among different age groups of ED patients. The chi square test was conducted to compare the distribution differences of personality types among different age groups. Results:The scores of hostility(1.64±0.67, t=4.81, P<0.001), anxiety(1.58±0.66, t=6.83, P<0.001), psychotic(1.62±0.68, t=11.87, P<0.001), paranoid(1.55±0.66, t=3.58, P=0.0004), obsession(1.95±0.70, t=9.56, P<0.001), somatization(1.43±0.58, t=2.10, P=0.036), interpersonal relationship(1.74±0.74, t=2.79, P=0.005), depression (1.66±0.74, t=4.50, P<0.001)and the total scores (1.53±0.63, t=3.07, P=0.002)of SCL-90 in the patient group were significantly higher than those of the national norm group, and there were significant differences in the scores of interpersonal relationship among different age groups(1.72±0.78, 1.65±0.69, 1.58±0.92, 1.43±0.59, F=2.84, P=0.038). The scores of state anxiety( t=7.35, P<0.001), trait anxiety ( t=6.31, P<0.001)and the total scores ( t=8.41, P<0.001)of STAI in the patient group were significantly higher than those of the national norm, and there were significant differences in the scores of state anxiety( F=5.29, P=0.001), trait anxiety ( F=5.54, P<0.001)and total scores ( F=5.66, P<0.001)among different age groups. There were significant increases in the scores of psychoticism( t=30.56, P<0.001), emotion( t=45.94, P<0.001), extraversion and introversion( t=11.72, P<0.001), concealment factors ( t=29.16, P<0.001)and total scores ( t=30.56, P<0.001)in the patient group. The proportion of depressive personality was highest in the ED patients(n=183; 45.64%), but there was no significant difference in the distribution of personality types among different age groups[20-29(depressive, choleric, mucinous and sanguine): 76, 35, 26, 21; 30-39: 87, 40, 32, 23; 40-49: 14, 10, 6, 8; 50-60: 6, 9, 2, 6; χ 2=10.65, P=0.30]. Conclusions:ED patients have a series of abnormal emotions, somatized discomfort, sensitive interpersonal relationships, introverted and unstable emotionally personality characteristics. In addition, the younger the patients are, the more serious their anxiety are, the more sensitive their interpersonal relationships are, which may be related to their introverted personality characteristics and emotional instability.
6.Construction of death early-warning model for patients with septic myocardial depression: a retrospective analysis of 129 patients

Objective To explore the death risk factors of septic myocardial depression (SMD) and their predictive effect, and to set up a death early-warning model. Methods A retrospective analysis was conducted. The patients with SMD admitted to emergency department and rescue room of Beilun Branch of the First Affiliated Hospital of Zhejiang University Medical College from January 2015 to November 2017 were enrolled. The patients were divided into survival group and non-survival group according to 28-day outcome, and the gender, age, and the initial examination parameters [white blood cell (WBC) count, neutrophil (Neut) count, activated partial thromboplastin time (APTT), procalcitonin (PCT), D-dimer, C-reactive protein (CRP), cardiac troponin I (cTnI), N-terminal pro-brain natriuretic peptide (NT-proBNP), left ventricular ejection fraction (LVEF), left ventricular end-diastolic diameter (LVEDD), and left atrium diameter (LAD)] of both groups were compared. Binary logistic regression analysis was conducted on the factors with statistically significant difference analyzed in univariate analysis, and death early-warning model was set up subsequently. For parameters in early-warning model after variable screening, receiver operating characteristic curve (ROC) was applied to evaluate the predictive effect of death. Results A total of 129 patients were enrolled, 34 patients died within 28 days with the mortality of 26.4%. Univariate analysis showed that the PCT, cTnI and NT-proBNP in non-survival group were significantly higher than those of the survival group. However, there was no statistical difference in gender, age, WBC, Neut, APTT, D-dimer, CRP, LVEF, LVEDD or LAD between the two groups. Logistic stepwise regression analysis showed that PCT and cTnI were the independent factors influencing the death of patients with SMD [PCT: odds ratio (OR) =1.495, 95% confidence interval (95%CI) = 1.192-1.876, P = 0.001; cTnI: OR = 11.154, 95%CI = 5.709-17.264, P = 0.004], and the death early-warning model was logP =-3.737+0.402×PCT+2.412×cTnI. According to the statistics of Homser-Lemeshow, the effect of this model was good (χ2= 6.258, P = 0.617). The analysis of ROC displayed that the area under ROC curve (AUC) of the combination of PCT and cTnI for predicting the prognosis of SMD patients was 0.851, and it was significantly higher than that of PCT and cTnI alone (0.738 and 0.719, respectively, both P < 0.05). When the combination of PCT and cTnI was 0.26, the sensitivity was 79.97%, the specificity was 87.01%, the positive predictive value was 71.3%, and the negative predictive value was 91.7%. Conclusions PCT and cTnI are independent factors influencing the death of SMD patients. The combination of PCT and cTnI has predictive value for the prognosis of SMD patients. The death early-warning model of SMD patients can be used to predict the prognosis of SMD patients.
7.Prenatal ultrasound in diagnosis of omphalocele-exstrophy-imperforate anus-spinal defects complex in first trimester

Objective To explore the value of prenatal ultrasound in diagnosis of omphalocele-exstrophy-imperforate anusspinal defects (OEIS) in first trimester.Methods Prenatal ultrasonic characteristics of 10 fetuses with OEIS complex in first trimester were retrospectively analyzed and compared with autopsy results.Results Cystic bulging in the lower anterior abdominal wall was observed in all 10 fetuses.Spinal scoliosis dysplasia was found in 10 fetuses,with myelomeningocele in 3 fetuses.No normal bladder was visualized in 8 fetuses.Thickened nuchal translucency was noticed in 5 fetuses,among which neck lymphatic hydrocele was found in 1 fetus.The bilateral clubbed feet and left lower mutilation was observed in 1 fetus,respectively.All 10 OEIS complex fetuses were found accompanied with short umbilical cord,while single umbilical artery and umbilical cord cyst were found in 4 and 1 fetus,respectively.Autopsy showed abdominal wall defects with exstrophy in 10 fetuses.However,no complete cystic bulging was found.Besides,autopsy also showed pubic symphysis separation and bladder exstrophy in 10 fetuses without obvious genitalia nor anus.Conclusion Cystic bulging in the lower anterior abdominal wall is the most common prenatal ultrasonic characteristic of OEIS complex in first trimester.
8.Study on the Photosynthetic Characteristics of Two Cultivars Prunus Humilis Bge. in Shandong Province

To study the photosynthetic physiological characteristics of Cultivars Prunus Humilis bge. in the region of shandong, analyze on the relationship between the quality of seeds and photosynthesis, In order to formulate reasonable cultivation measures and improve the yield quality of seed production. LI-6400 portable photosynthesis apparatus was used to determine light response curve, diurnal change of photosynthetic parameters of Huadong 1 and Huadong 2. Using vernier caliper and electronic balance to measure the quality data of two varieties of eucalypis, the quality difference was analyzed. Two kinds of Prunus Humilis bge, cultivating in Shandong, had higher photosynthetic rate and higher light saturation point, and the photosynthetic rate were "unimodal" type curve. The change of the two peaks all appeared at11: 00, and had no obvious "lunch break" phenomenon. The photosynthetic rate, the utilization of strong and weak light, and the efficiency of absorption and conversion of light energy of Huadong 2, were all higher than that of Huadong 1, which had better quality. The quality of seed of Prunus Humilis bge is related to its photosynthetic capacity. When planting Prunus Humilis bge., we should formulate reasonable cultivation measures according to its features such as photosynthesis, transpiration, water use efficiency.
9.Clinical effect of laparoscopy in the modified nerve palne-sparing radical hysterectomy of early stage cervical cancer

Objective To evaluate the clinical effect of laparoscopy in the modified nerve painesparing radical hysterectomy (NPSRH) of early stage cervical cancer.Methods From January 2014 to December 2015 in our hospital,38 patients with early cervical caucer were enrolled and randomly divided into NPSRH group and laparoscopic radical hysterectomy (LRH) group in the study.The surgery general situations of two groups were compared,and postoperative intestinal and bladder function of these patientswere also assessed.Results All of 38 patients were successfully completed the operation,without significant intraoperative or postoperative complications.There were no significant differences in the bleeding volume,pelvic lymph dissection number,cut length of main ligament and sacral ligament,cut length of anterior vaginal wall,and posterior vaginal wall between two groups (P > 0.05).The mean operation time in NPSRH group was longer than in LRH group,but the duration of postoperative hospital stay was shorter than the latter (P < 0.01).Compared to control group,the catheter retention time,residual urine volume,maximal micturition volume,and Qmax in the NPSRH group recovered better,and with lower rate of nocturia mnd dysuria (P < 0.05).The exhaust time and anal defecation time in the NPSRH group were significantly shorter than the LRH group,and lower rate of constipation additionally (P < 0.05).Conclusions The laparoscopy in the modified nerve paine-sparing radical hysterectomy is a safe and effective measurement for the treatment of early-stage cervical cancer patients.It can significantly reduce the duration of postoperative hospital stay,promote the functional recovery of bladder and rectum,and improve the patients'postoperative quality of life.
10. Clinical significance of cytogenetic monitoring in chronic myeloid leukemia

Objective:
To analyze the association of cytogenetic abnormalities with the prognosis of chronic myeloid leukemia (CML) patients in tyrosine kinase inhibitors (TKI) era.
Methods:
Karyotype analysis of chromosome G-banding was carried out in 387 newly diagnosed CML patients by short-term culture of bone marrow cells. The correlation of cytogenetic abnormalities and CML progression was explored in combination with ABL tyrosine point mutations.
Result:
Of 387 patients with positive BCR-ABL fusion gene assayed by fluorescence in situ hybridization (FISH) technique, 94.1% (364/387) patients were Ph positive and 5.9% (23/387) Ph negative; 320 patients (87.9%) had a translocation t (9;22) (q34;q11) and 5 (1.4%) a variant translocation t (v;22) . Additional cytogenetic aberrations (ACA) at diagnosis were found in 10.7% (39/387) Ph+ patients, major route ACA in 22 (56.4%) cases and minor route ACA in 15 (38.5%) cases and 2 patients (5.1%) lacked the Y chromosome (−Y) ; 23.4% (71/303) patients occurred ACA during TKI treatment and the most frequent abnormalities were abnormal chromosome numbersd, which were likely associated with high proportion of disease progression (
