The European Society of Cardiology (ESC) released the "2024 ESC guidelines for the management of elevated blood pressure and hypertension" on August 30, 2024. This guideline updates the 2018 "Guidelines for the management of arterial hypertension." One notable update is the introduction of the concept of "elevated blood pressure" (120-139/70-89 mm Hg). Additionally, a new systolic blood pressure target range of 120-129 mm Hg has been proposed for most patients receiving antihypertensive treatment. The guideline also includes numerous additions or revisions in areas such as non-pharmacological interventions and device-based treatments for hypertension. This article interprets the guideline's recommendations on definition and classification of elevated blood pressure and hypertension, and cardiovascular disease risk assessment, diagnosing hypertension and investigating underlying causes, preventing and treating elevated blood pressure and hypertension. We provide a comparison interpretation with the 2018 "Guidelines for the management of arterial hypertension" and the "2017 ACC/AHA guideline on the prevention, detection, evaluation, and management of high blood pressure in adults."

Journal of Environmental and Occupational Medicine 2025;42(8):918-925
Background Pneumoconiosis is a prevalent and severely harmful occupational disease in China. Objective To estimate disease burden and trends of different types of pneumoconiosis among Chinese males from 1992 to 2021, and to provide insights for pneumoconiosis prevention and control as well as the development of public health policies in China. Methods Based on the latest data from the Global Burden of Disease (GBD) study (2021), this study evaluated the disease burden of pneumoconiosis among Chinese males using indicators such as mortality rate and disability-adjusted life years (DALYs). Joinpoint regression model and age-period-cohort model were applied to analyze trends in the mortality rates of pneumoconiosis among Chinese males. Results From 1992 to 2021, the age-standardized DALYs rate, age-standardized years of life lost (YLLs) rate, age-standardized years lived with disability (YLDs) rate, and age-standardized mortality rate of pneumoconiosis among Chinese men all showed a downward trend, with average annual percentage changes (AAPC) of −2.63%, −2.81%, −0.87%, and −2.56%, respectively (P<0.05). Among the four types of pneumoconiosis, the disease burden of silicosis was the highest. The age-standardized mortality rates for silicosis, asbestosis, coal worker’s pneumoconiosis, and other types of pneumoconiosis had AAPC of −2.18%, −1.24%, −3.81%, and −3.21% (P<0.05), respectively. The age effects showed mortality rates for all types increased with age, particularly in silicosis and coal worker’s pneumoconiosis. The period effects showed a general decline in mortality risk, though silicosis remained significantly higher than other types. The cohort effects indicated that later birth cohorts had progressively lower mortality risks. Conclusion From 1992 to 2021, the burden of pneumoconiosis among Chinese males has shownsome alleviation; however, pneumoconiosis remains China’s most severe occupational disease. Given that males are the primary affected group in both incidence and mortality, continued attention and targeted preventive measures are essential. Effective control strategies should be tailored to address the specific characteristics of each type of pneumoconiosis.

Amorphous solid dispersion (ASD) is one of the most effective formulation approaches to enhance the water solubility and oral bioavailability of poorly water-soluble drugs. However, maintenance of physical stability of amorphous drug is one of the main challenges in the development of ASD. Crystallization is a process of nucleation and crystal growth. The nucleation is the key factor that influences the physical stability of the ASD. However, a theoretical framework to describe the way to inhibit the nucleation of amorphous drug is not yet available. We reviewed the methods and theories of nucleation for amorphous drug. Meanwhile, we also summarized the research progress on the mechanism of additives influence on nucleation and environmental factors on nucleation. This review aims to enhance the better understanding mechanism of nucleation of amorphous drug and controlling over the crystal nucleation during the ASD formulation development.

Objective:Hepatic fibrosis is a common pathological basis for many chronic liver diseases and can progress to cirrhosis,a leading cause of mortality in liver diseases.Early identification and reversal of hepatic fibrosis are key in the treatment of chronic liver disease. This study aims to compare the expression levels of serum core fucosylated low molecular weight kininogen(LMWK-Fc)and alpha-galactosylated(α-Gal)antibodies in patients with hepatic fibrosis at different stages, and to evaluate their diagnostic efficacy for hepatic fibrosis. Methods:A retrospective analysis was conducted on 275 patients with chronic liver disease who visited the Department of Infectious Diseases at the Second Xiangya Hospital of Central South University between June 2022 and March 2023.Among these,115 patients underwent liver biopsy.Based on the extent of collagen deposition and its impact on liver structure and microcirculation, patients were staged from 0 to 4:S0(no significant collagen deposition in liver tissues;liver structure and microcirculation are normal),S1(mild collagen deposition in liver tissues, with partial disruption of lobule structure, but microcirculation remains largely normal),S2(moderate collagen deposition in liver tissues, with partial disruption of lobule structure and microcirculation),S3(extensive collagen deposition in liver tissues, with substantial disruption of lobule structure and microcirculation),and S4(development of cirrhosis, with heavy collagen deposition, complete disruption of lobule structure, and severe impairment of microcirculation).Patients were grouped as no fibrosis(S0),fibrosis(S1-S2),and significant fibrosis(S3-S4).For the 160 patients without liver biopsy, they were categorized based on liver stiffness measurement(LSM)value:no fibrosis(F0:LSM<7.3 kPa),fibrosis(F1-F2:LSM 7.3-12.4 kPa),and significant fibrosis(F3-F4:LSM>12.4 kPa).Demographic data(age, gender)and laboratory indicators(alanine transaminase, aspartate transaminase,gamma-glutamyl transferase, alkaline phosphatase,alpha-fetoprotein,platelet count)were collected to calculate the fibrosis-4 index(FIB-4)and aspartate aminotransferase-to-platelet ratio index(APRI).Serum LMWK-Fc and α-Gal antibodies were measured and compared across the groups, and their correlation with fibrosis severity was analyzed. The receiver operating characteristic(ROC)curve was used to assess the predictive value of serum LMWK-Fc and α-Gal antibody levels for hepatic fibrosis. Results:Among the 160 patients without complete liver biopsy, serum α-Gal antibody and LMWK-Fc levels increased progressively from the no fibrosis group to the significant fibrosis group, with statistically significant differences(P<0.05).Among the 115 patients with liver biopsy, serum LMWK-Fc levels were significantly higher in the fibrosis group and the significant fibrosis groups compared with the no fibrosis group, and α-Gal antibody levels were significantly higher in the significant fibrosis group compared with the no fibrosis group and the fibrosis group(P<0.001,P=0.032,respectively).Univariate and multivariate linear regression analyses showed that hepatic fibrosis was correlated with gender and LMWK-Fc levels(both P<0.05),but not with age,α-Gal antibody levels,FIB-4,or APRI(all P>0.05). Conclusion:The expression levels of serum LMWK-Fc and α-Gal antibodies vary across different stages of hepatic fibrosis, suggesting a potential association with fibrosis progression.LMWK-Fc levels have a certain predictive value for the diagnosis of hepatic fibrosis.

Aim To investigate the protective effects of dendrobium polysaccharide(DOP)against paraceta-mol(APAP)-induced liver injury in mice and eluci-date its underlying mechanism. Methods Healthy male Kunming mice were randomly assigned to the fol-lowing groups:control group,APAP model group,low,medium,high-dose DOP intervention group(225,450,900 mg·kg-1),and DOP control group. The APAP model group was given 300 mg·kg-1 per day,the DOP intervention group was given DOP for 2 h and then APAP was given, and the remaining groups received an equal volume of normal saline daily for sev-en consecutive days. After the final administration,se-rum and liver samples from the mice were collected and tested after 20 hours.Liver morphology and liver coef-ficient were examined.Liver histopathological altera-tions and apoptosis were examined using HE staining and TUNEL staining.Additionally, medium biochemi-cal indexes were assessed in serum and liver tissue u-sing kits. The levels of oxidative stress, inflammation, and apoptosis-related proteins in liver tissue were de-termined using Western blotting. Results In the APAP model group, liver coefficient increased signifi-cantly,the number of liver vacuolar necrosis and apop-tosis cells increased, and the serum ALT and AST lev-els significantly increased.Compared with the APAP group, the liver coefficient, serum ALT and AST levels were significantly reduced, and the liver pathology was improved after DOP intervention, especially in the 900 mg·kg-1 group. The levels of oxidative stress and in-flammation in the APAP group increased, and the ex-pression of apoptosis, inflammation and oxidative stress related proteins in liver was unbalanced.DOP inter-vention,especially in the 900 mg·kg-1 group, could significantly reverse the oxidative stress, apoptosis and inflammatory response induced by APAP in liver, and increase the expression levels of Nrf2 and HO-1,but reduce the expression levels of NLRP3 and HMGB1.Conclusions The hepatoprotective mechanism of DOP is mainly due to its antioxidant and anti-inflammatory response, which may be related to the activation of Nrf2/HO-1 pathway and the inhibition of HMGB1/NL-RP3 pathway by DOP.
