Recent innovations in nanomaterials inspire abundant novel tumor-targeting CRISPR-based gene therapies. However, the therapeutic efficiency of traditional targeted nanotherapeutic strategies is limited by that the biomarkers vary in a spatiotemporal-dependent manner with tumor progression. Here, we propose a self-amplifying logic-gated gene editing strategy for gene/H2O2-mediated/starvation multimodal cancer therapy. In this approach, a hypoxia-degradable covalent-organic framework (COF) is synthesized to coat a-ZIF-8 in which glucose oxidase (GOx) and CRISPR system are packaged. To intensify intracellular redox dyshomeostasis, DNAzymes which can cleave catalase mRNA are loaded as well. When the nanosystem gets into the tumor, the weakly acidic and hypoxic microenvironment degrades the ZIF-8@COF to activate GOx, which amplifies intracellular H+ and hypoxia, accelerating the nanocarrier degradation to guarantee available CRISPR plasmid and GOx release in target cells. These tandem reactions deplete glucose and oxygen, leading to logic-gated-triggered gene editing as well as synergistic gene/H2O2-mediated/starvation therapy. Overall, this approach highlights the biocomputing-based CRISPR delivery and underscores the great potential of precise cancer therapy.

Objective:To analyze the effect of optical zone diameter in orthokeratology contact lenses on the efficacy of treatment. Methods:A total of 100 myopic patients (196 eyes) who wore Paragon CRT keratoplasty lenses with a diameter of 10.5 mm were retrospectively included in this study. These patients, consisting of 47 males and 53 females, were admitted to The Affiliated Hospital of Hangzhou Normal University between November 2020 and November 2022. Their ages ranged from 8 to 13 years. Their refractive errors ranged from -0.75 DS to -4.00 DS for spherical errors and from -0.25 DC to -1.50 DC for cylindrical errors. The corrected visual acuity was ≥ 0.6, and the initial axes were between 23.40 mm and 25.83 mm. The patients were grouped according to the different diameters of the optical zones of their lenses. Fifty patients (98 eyes) with an optical zone of 5.0 mm were included in the 5.0 group, and another 50 patients (98 eyes) with an optical zone of 6.0 mm were included in the 6.0 group. The general sociological and disease-related characteristics of the two groups were recorded. Logistic linear regression analysis was performed on the data with differences to obtain the therapeutic effect of different optimal zone diameters in orthokeratology contact lenses. Results:After 1 year of wearing, there was no statistically significant difference in the average non-invasive tear film break-up time between the two groups ( P > 0.05). The 5.0 group exhibited significantly lower values for the surface regularity index (0.35 ± 0.09), surface asymmetry index (0.69 ± 0.21), axial length [(0.19 ± 0.04) mm], and relative peripheral refraction [(-0.65 ± 0.20) D, (-0.84 ± 0.26) D, (-0.48 ± 0.13) D, (-0.76 ± 0.23) D, (-0.77 ± 0.21) D, (-0.56 ± 0.17) D] compared with the 6.0 group ( t = 5.94, 3.34, 4.64, 2.03, 2.07, 4.23, 2.27, 2.25, 5.55, all P < 0.05). Conversely, the uncorrected visual acuity [(0.71 ± 0.20) LogMAR] and spherical equivalent [(-0.25 ± 0.05) D] were significantly higher in the 5.0 group compared with the 6.0 group ( t = 3.31, 13.64, both P < 0.05). After 1 year of wearing, the diameter and area of the optical zone in the 5.0 group [(3.35 ± 0.28) mm and (7.50 ± 1.10) mm2] were significantly lower than those in the 6.0 group [(3.68 ± 0.38) mm and (8.50 ± 1.50) mm2, t = 6.92, 5.32, 11.16, all P < 0.05]. Furthermore, the overlapping treatment zone area to pupil area ratio (TZR) in the 5.0 group (0.53 ± 0.16) was significantly lower than that in the 6.0 group [(0.81 ± 0.19), t = 11.16, P < 0.05]. The results of logistic linear regression analysis and Pearson calculation showed that the TZR was positively correlated with the optical zone diameters of orthokeratology contact lenses ( P < 0.05). There was no significant difference in the incidence of complications between the 5.0 group [8.16% (8/98)] and the 6.0 group [12.24% (12/98), χ2 = 0.89, P > 0.05). Conclusion:The 1-year dynamic change in eye axis indicates that myopia correction is more effective and safe when the diameter of the optical zone in keratoplasty is 5.0 mm compared with when the diameter of the optical zone is 6.0 mm. Evaluating the efficacy of corneal reshaping lenses in different optical zones using statistical TZR can guarantee myopia control results. At the same time, this study demonstrates significant innovation and scientific validity, which can offer new ideas and methods for the research and application of orthokeratology contact lenses.

Objective:To compare the oncology effect and risk between hepatopplenectomy and conventional hepatectomy for hepatocellular carcinoma patients with hypersplenism, so as to provide guidance for clinical application. Methods:One hundred and eighteen patients with hepatic carcinoma with hypersplenism in the Private hospitals of Zhejiang Province Yiwu were studied retrospectively. Patients were divided into 2 groups according to different operation methods: combined hepatosplenectomy group and conventional hepatectomy group with 59 cases in each group. Blood routine, liver function, complication and tumor effect (local recurrence, distant metastasis and 1, 3, 5 years survival rate) was compared between two group. Results:There was no significant difference in operation time, intraoperative blood loss and hospitalization time between the two groups ( P>0.05). Compared with that of the control group, the clinical total effective rate of the combined group was significantly increased: 94.92% (56/59) vs. 79.66% (47/59), P<0.05; there was no significant difference in the incidence of complications between the two groups ( P>0.05); the two groups were followed up to August 1, 2019. The median survival time of the two groups was significantly higher than that of the open group (29 months vs 22 months) ( P<0.05); the local recurrence rate, distant metastasis rate, 1, 3 and 5-year survival rate between the two groups were statistically significant ( P<0.05). The platelet and white blood cells in the combined group were significantly higher than those in the control group after 7 days′ treatment: (264.48 ± 75.37) × 10 9/L vs. (153.53 ± 42.11) × 10 9/L, (7.86 ± 1.43) × 10 9/L vs. (3.48 ± 1.32) × 10 9/L, P<0.05. Compared with that of the control group, alanine aminotransferase and aspartate aminotransferase were significantly decreased: (49.42 ± 31.88) U/L vs. (84.22 ± 43.95) U/L, (36.50 ± 21.50) U/L vs. (49.98 ± 35.63) U/L, P<0.05. Conclusions:Compared with conventional hepatectomy for hepatocellular carcinoma patients with hypersplenism, combined hepatosplenectomy can effectively promote the increase of peripheral blood PLT and WBC and the recovery of liver function without increasing the risk of surgery, and the long-term follow-up results are more satisfactory, which has high clinical application value.

Objective: To study the effect of the modified preparation method with root canal flushing of different solutions on the anti-fracture properties of mechanical nickel-titanium filings. Methods: A transparent resin model of root canal was established. ProTaper Universal (PTU F1) instrument was used to prepare the root canals. The number of prepared root canals by each PTU F1 with the various solutions(n = 20) was recorded and compared among groups. Results: 6. 13 ± 3. 52 root canals were prepared in distilled water group, 6. 25 ± 1. 76 in 0. 9%saline group, 6. 27 ± 2. 07 of 0. 2% chlorhexidine group, 6. 88 ± 3. 21 in 1% sodium hypochlorite group, 4. 31 ± 2. 34 in 5% sodium hypochlorite group and 3. 26 ± 2. 08 in dry drilling group. The number between each 2 of distilled water, 0. 9%saline group, 0. 2%chlorhexidine group and 1% sodium hypochlorite group, and the number between 5% sodium hypochlorite group and dry drilling group was not statistically significant(P> 0. 05). The number of prepared root canals in the first 4 groups was more than that in the latter 2 groups(P< 0. 05). Conclusion: Distilled water, 0. 9% saline, 0. 2% chlorhexidine or 1% sodium hypochlorite for root canal irrigation can improve mechanical nickel-titanium instrument for fatigue resistance in root canal preparation, but 5% sodium hypochlorite and dry drilling can not.

Objective To explore the treatment of locally advanced pancreatic cancer(LAPC)by Nanoknife, and the demonstrations on contrast enhanced ultrasonography(CEUS)after the procedures. Methods Four patients were enrolled. Each of them was percutaneously inserted 3-5 Nanoknife electrodes to ablate the lesions with 2-3 sessions under the guidance of ultrasound. After the procedures,CEUS, contrast enhanced CT,contrast enhanced MRI or computed tomography angiography (CTA) were performed to assess the efficacy. Results All patients uneventfully were able to tolerate the high-voltage Nanoknife procedures. However, the endurable pain or discomfort in abdomen was complained. For one patient with obstructive jaundice and mild liver dysfunction, pancreatic amylase in serum increased slightly at the first day after treatment.Furthermore,C response protein in serum elevated, and biliary tract infection happened at the third day. Moreover, thromboses in portal and splenic vein were found at the seventh day.The survival time for all cases were 3-8 months.According to the findings of CEUS,a well-defined no contrast enhancement was detected at ablative zone for all the patients within one week.Yet, contrast enhancement was sporadically detected at the periphery in 3 patients 1-2 months late.Meanwhile, on contrast enhanced CT or MRI,an area of absent enhancement was also depicted at the ablation zone with heterogeneously decreased enhancement at periphery of the lesion.Base on CTA findings, local stenosis of splenic vein and superior mesenteric vein with varices of collateral vascularity were found in one patient at the fiftieth day,and superior mesenteric vein obstruction accompanied by varices of collateral vascularity was also found in another patient at the seventieth day. Conclusions For patients with LAPC,Nanoknife treatment is a safe and convenient procedure that can be percutaneously completed under the guidance of ultrasound.Along with enhanced CT,MRI or CTA,CEUS is a promising way to evaluate the efficacy of this procedure.

The evaluation of the immunosuppression state in liver transplanted recipients is vital for a correct posttransplantation management and a major step towards the personalized treatment of the immunosuppression. To date, immunological monitoring after liver transplantation relies mainly on clinical judgment and pathological examination of the graft, without a proper assessment of the actual state.Previous studies have ever identified many markers for acute rejection(AR) and immune tolerance after liver transplantation. Many markers for AR are pro-inflammatory or immunoregulatory cytokines and other proteins related to inflammation. However, many markers have been proved to be also able to predict other diseases and only a few of the markers for AR have been validated.Standard liver tests cannot be used as markers for graft rejection due to the low sensitivity and specificity. This review summarized the potential markers for AR and immune tolerance after liver transplantation based on published literatures in recent years and to provide evidence for clinical application.
