		                        			BACKGROUND:Exosomes play a role in all stages of wound repair,and there is currently a large body of research on exosomes in skin wound repair,which has been shown to have great potential for clinical applications. OBJECTIVE:To summarize and discuss the main mechanisms and clinical applications of exosomes in the treatment of skin wounds,in order to promote the clinical translation of exosomes. METHODS:PubMed,clinicaltrials.gov,China National Knowledge Infrastructure,Food and Drug Administration database,and Chinese Clinical Trial Register were searched from inception to March 2023.The English search terms were"exosomes,wound healing,stem cells,chronic wound,immunoregulation,inflammation,skin,therapeutic use,isolation,characterization,infections".The Chinese search terms were"exosomes,wound healing,stem cells,immunomodulation,clinical applications".A total of 79 articles were included for the summary. RESULTS AND CONCLUSION:(1)Exosomes can improve and accelerate wound healing through inflammation regulation,immune protection,angiogenesis,cell proliferation and migration,and collagen remodeling.(2)Exosomes derived from stem cells have mature preparation techniques and related mechanism research,which is currently the mainstream research direction.Non-stem cell-derived exosomes have the advantages of convenience,economy,and easy production,and can be used as a supplement for clinical applications.(3)The clinical application of exosomes is still in its infancy,but has great potential for application.Various exosome modification techniques have laid the foundation for the future development of clinically personalized services and require further research.(4)The clinical translation of exosomes faces many challenges,such as low yield,high heterogeneity,lack of unified standards for isolation,purification,and quality control,and difficulties in storage.
		                        		
		                        		
		                        		
		                        	
2.Relationship between CYP2C19, UGT1A1 gene polymorphisms and tamoxifen therapy in breast cancer patients

		                        			Objective:To investigate the relationship between CYP2C19, UGT1A1 gene polymorphisms and tamoxifen therapy in breast cancer patients.Methods:A total of 100 breast cancer patients diagnosed and treated in our hospital from Feb. 2021 to Mar. 2023 were chosen and given tamoxifen treatment after surgery. Polymerase chain reaction (PCR) was used to detect CYP2C19 and UGT1A1 gene polymorphisms, and the relationship between CYP2C19, UGT1A1 gene polymorphisms and tamoxifen treatment effect was analyzed.Results:In 100 patients with breast cancer, the distribution frequencies of CYP2C19 wild type (G/G) and mutant type (G/A, A/A) were 49.00% and 51%, respectively; The distribution frequencies of wild type and mutant UGT1A1*28 were 74.00% and 26.00%, respectively; The distribution frequencies of wild type and mutant UGT1A1*6 were 69.00% and 31%, respectively; There was no statistically significant difference between CYP2C19, UGT1A1*28 and UGT1A1*6 wild type and mutant patients in the age of onset, menstrual status, pathological type, clinical stage, estrogen receptor (ER) positive, progesterone receptor (PR) positive, hormone receptor (HR) positive ( χ2 values of CYP2C19 were 1.43, 0.38, 5.52, 0.61, 1.39, 0.01, 0.26, 1.16, χ 2 values of UGT1A1*28 were 0.63, 0.00, 2.88, 0.08, 1.94, 0.61, 2.74, 0.30, the χ2 values of UGT1A1*6 were 0.90, 0.10, 0.75, 0.09, 1.18, 0.11, 0.00, 0.37, P>0.05) ; The therapeutic effectiveness of CYP2C19, UGT1A1*28 and UGT1A1*6 wild type patients was higher than that of mutant type ( χ2=4.67, 4.86, 5.93, P<0.05) ; Logistic regression analysis showed that CYP2C19, UGT1A1*28 and UGT1A1*6 genotypes were independent risk factors for tamoxifen treatment effect ( β=0.465, 0.557, 0.568, P<0.05) . Conclusions:CYP2C19 and UGT1A1 gene polymorphisms are closely associated with tamoxifen treatment in breast cancer patients, and also lead to increased adverse reactions in patients. 3.Yishi Therapy for the Treatment of Primary Tinnitus:A Long-Term Efficacy Follow-up of 496 Cases

		                        			ObjectiveTo demonstrate the long-term effect of Yishi Therapy for primary tinnitus and analyze the factors affecting the effectiveness. MethodsWe conducted long-term follow-ups of primary tinnitus cases treated with Yishi Therapy during 1 January 2017 to 30 June 2023 by face-to-face, online and telephone between 1 July 2023 and 31 January 2024, to understand the final regression of primary tinnitus and lifestyle changes. We retrospectively analysed the basic data of patients before treatment, and statistically analysed the recovery rate and overall effective rate of tinnitus treated by the Yishi Therapy, and the factors affecting the effectiveness such as lifestyle, age, duration of disease, gender, tinnitus location, severity of tinnitus, accompanying symptoms and complications on the overall effective rate. ResultsA total of 496 cases of primary tinnitus completed follow-ups successfully, and the total Tinnitus Evaluation Questionnaire (TEQ) at follow-up was significantly lower than that before treatment (P<0.001). Among the 496 cases, 101 (20.36%) cases cured, 189 (38.10%) cases showed significant improvement, 116 (23.39%) cases were effective, 90 (18.15%) cases were ineffective, and the overall effect rate was 81.85% (406/496). At follow-up, the score of lifestyle was higher than that before treatment (P<0.001). Grouped by the score of lifestyle at follow-up, the effective rates of better lifestyle group and poor lifestyle group were 97.53% (237/243) and 66.80% (169/253), respectively, with statistical significance (P<0.001). Grouped by the mean value of the difference in lifestyle scores before and after treatment, the effective rates of the group with greater lifestyle adjustment and the group with smaller adjustment were 97.55% (239/245) and 66.53% (167/251), respectively, and the differences were statistically significant (P<0.001). Binary logistic regression showed that the better lifestyle, the greater lifestyle adjustment and the severity of tinnitus were positively correlated with the total effective rate of tinnitus (P<0.001), while older and hearing loss were negatively correlated with the total effective rate of tinnitus (P<0.05). ConclusionYishi Therapy on primary tinnitus shows good long-term effect, lifestyle, severity of tinnitus, hearing loss or not, age are the relevant factors affecting the efficacy of tinnitus, lifestyle is the main factor affecting clinical efficacy. 
		                        		
		                        		
		                        		
		                        	
4.Analysis and reflection on examination results of 2023 Beijing Nursing Association Emergency Specialist Nurse Training Course

		                        			Objective:To explore the outcomes and problems of the Beijing Nursing Association Emergency Specialist Nurse Training Course, so as to provide reference for improving the training of emergency specialist nurses.Methods:Convenience sampling was used to select 75 trainees from the 2023 Beijing Nursing Association Emergency Specialist Nurse Training Course as survey subjects. Beijing Nursing Association Emergency Specialist Nurse Training Scoring Criteria was used to score trainees from four aspects of theoretical knowledge, skill operation, case analysis, and review defense, and to summarize and analyze the existing problems.Results:A total of 75 questionnaires were distributed, and 75 valid questionnaires were collected, with a valid response rate of 100.0%. The theoretical knowledge score was (68.4±10.7), with a minimum of 38.0 and a maximum of 91.0, and the pass rate (number of passing trainees divided by the total number of trainees) was 81.3%. The score for the review defense was (78.8±5.7), with a minimum of 65.0 and a maximum of 94.0, and a pass rate of 100.0%. The skill operation score was (93.5±3.7), with a minimum of 85.0 and a maximum of 99.0, and a pass rate of 100.0%. The score for the case analysis was (90.2±5.6), with a minimum score of 80.0 and a maximum score of 100.0, and a pass rate of 100.0%. The main reasons for losing points in the theoretical knowledge were mechanical ventilation and cardiopulmonary resuscitation. The main reasons for losing points in the review defense were topic selection and paper writing. The main reasons for losing points in skill operation were preparation before operation and key points after operation. The main reasons for losing points in the case analysis were the consultation ideas and treatment measures.Conclusions:The trainees of the emergency specialist nurse training course currently have a relatively good overall professional practical skills, but there are significant differences in theoretical knowledge, weak scientific research capabilities, and room for improvement in triage decision-making. It is suggested to give full play to the advantages of teachers and set up a variety of Internet courses for trainees to learn on their own to meet the personalized needs of trainees and improve teaching homogeneity.
		                        		
		                        		
		                        		
		                        	
5.Cancer screening and its association with core knowledge of cancer in community residents

		                        			Objective  To understand the situation of cancer screening of community residents and its association with the knowledge of cancer core knowledge, and to provide reference for the formulation of secondary cancer prevention measures.  Methods  A cross-sectional survey was conducted among 2 037 residents aged 18-79 who lived in Bao 'an District, Shenzhen. Multivariate Logistic regression analysis was used to investigate the relationship between core knowledge and cancer screening behavior after adjusting for age, gender, education, marriage, occupation and smoking.  Results  The cancer screening rate of community residents was 28.10%, and the screening rate of middle-aged and elderly people aged 40-79 was 33.50 %.The proportion of “not very well understanding”, “partial understanding” and “basic understanding” of the core knowledge of cancer were 43.91%, 42.33% and 13.76%, respectively, which was higher than that of the non-participants. Multivariate analysis showed that the women, the higher the level of education of college/university or above and the higher the level of knowledge of cancer prevention core knowledge, the higher the probability of participating in cancer screening was OR (95%CI ) : 2.40 (1.86-3.12), 1.65 (1.29-2.10), 1.38 (1.18 - 1.62), respectively.  Conclusion  The proportion of cancer screening in community residents needs to be improved, and the degree of core knowledge of cancer is closely related to cancer screening behavior. It is suggested to increase the publicity and education of the core knowledge of cancer, and at the same time, carry out the health education of secondary cancer prevention for the key population such as men and people with low education level, so as to improve the proportion of “early detection, early diagnosis and early treatment” of cancer.
		                        		
		                        		
		                        		
		                        	
6.Ultrasonographic and pathologic features in fetal arthrogryposis multiplex congenita

		                        			Objective:To summarize the prenatal sonographic characteristics of affected joints of fetal arthrogryposis multiplex congenita (AMC) by comparing the ultrasonographic features and the postnatal pathological manifestations.Methods:The cases of AMC detected by antenatal ultrasound and confirmed by postnatal pathology were collected in the First Affiliated Hospital of Sun Yat-Sen University and Kaiping Central Hospital from January 2015 to June 2020. The differences between prenatal ultrasonic manifestations, types of affected joints and postnatal pathological features were analyzed. And the different involvements of joints in AMC cases with or without other system abnormalities were also explored separately.Results:A total of 31 cases of AMC were included, in which 11 cases were with other system abnormalities and 20 cases without. No significant difference was observed in number of affected joints between these two groups ( P>0.05). The prenatal sonogram features were completely consistent with the postnatal pathological manifestations in 21 (21/31, 67.7%) cases. Among 31 cases, the involvement rates of joints were: interphalangeal joints of fingers (23/31, 74.2%), knee joints (20/31, 64.5%) and ankle joints (19/31, 61.3%), temporomandibular joint (11/31, 35.5%), wrists (11/31, 35.5%), elbow joints (10/31, 32.3%), interphalangeal joints of toes (6/31, 19.4%), spinal joints (2/31, 6.5%), shoulder joint (1/31, 3.2%) and hip joint (1/31, 3.2%), respectively. The coincidence rates of prenatal ultrasound in involved joints were: interphalangeal joints of fingers (100%), ankles (100%), spines (100%), hips (100%), wrists (90.9%), knees (75.0%), elbows (70.0%), jaws (54.5%), interphalangeal joints of toes (50.0%), and shoulders (0), respectively. Conclusions:When postural abnormalities of fetal upper and lower extremities are detected by prenatal ultrasound screening, especially overlapping fingers, extended knee and club foot, AMC should be kept on alert. Simultaneously, other joints should be carefully scanned to improve the prenatal detection rate of AMC.
		                        		
		                        		
		                        		
		                        	
7.Responses to child sexual assaults and associated factors among parents of young children

		                        			To analyze the possible responses of parents of young children to Child sexual assault (CSA) and the potential influencing factors, so as to provide evidence for prevention programs of CSA.
		                        		
		                        			Methods:
		                        			Part of the data from "2016-2020 Child Injury Prevention Project" was used to analyze responses to CSA among parents of children younger than 3rd grade in primary school by chi square test and Logistic regression analysis.
		                        		
		                        			Results:
		                        			A total of 4 072 parents were included in the analysis. Among them, 86.8% of parents chose "to solve problems together", 51.0% of parents taking children to a "psychologist", 4.9% of parents "scolding children for being too careless", 2.9% of parents "letting child not disclose abuse" and 6.5% of parents chose "other" coping methods. Parents with foreign nationality [OR(95%CI)=2.58(1.25-5.15)] and lower education level (OR=0.26-0.64) tend to "blame" their children. Mothers [(OR(95%CI)=0.54(0.33-0.90)], higher parental education level (OR=0.27-0.72) were more reluctant to encourage their children to keep quiet. Mothers [OR(95%CI)=1.73(1.32-2.27)], older children (OR=1.33-1.78) and parents with higher education level (OR=1.65-2.99) tend to "accompany". Parents aged 30 or more, and from high school/technical school/technical secondary school tend to take their children to see a "psychologist" [OR(95%CI)=1.39(1.14-1.71), 1.79(1.26-2.53)].
		                        		
		                        			Conclusion
		                        			Parents universally pay attention to the physical and mental health of children after CSA, but some parents still take negative coping methods, e.g. "blame" and "not disclose". Parenting educational level, duration of parent-child communication and age of children are primary factors associated with parental responses to child sexual assault. This study suggests that prevention secondary injury following child sexual assault should be farther strengthened, while fully considering the characteristics of the educated objects.
		                        		
		                        		
		                        		
		                        	
8. Association between the time of neutrophils to the lowest and prognosis of patients with esophageal squamous cell carcinoma treated with non-operative therapy

		                        			To investigate the association between the time of neutrophils to the lowest and prognosis of patients with esophageal squamous cell carcinoma (ESCC) treated with non-operative therapy.
		                        		
		                        			Methods:
		                        			The clinical data of 325 non-operative treated ESCC patients were collected in this study. The X-title software was applied to establish optimal threshold of neutrophil reduction to the lowest value. According to the optimal threshold, the patients were divided into early group (115 cases) and late group (210 cases). The clinical features and survival time of the two groups were compared, and the factors of prognosis were analyzed by Cox regression model with univariate and multivariate analysis.
		                        		
		                        			Results:
		                        			The X-title software demonstrated the optimal cutoff values for the time of neutrophils to the lowest was 39 days. The median overall survival time was 21.0 months in the early group which was significantly higher than that in the late group (16.0 months). Multivariate Cox regression analysis showed that the treatment methods and the time of neutrophils to the lowest were independent factors for overall survival of patients with ESCC treated by non-surgical therapy. 9. Lung cancer deaths attributable to ambient PM2.5 exposure in 2016 in China

		                        			To analyze the lung cancer deaths attributable to ambient PM2.5 exposure in China in 2016.
		                        		
		                        			Methods:
		                        			All data were from the Global Burden of Disease Study 2016 (GBD 2016). Multiple-source data, including satellite observation, ground measurement, chemical migration model simulation, etc., and the data integration model for air quality (DIMAQ) were used to estimate the grid-level exposure to ambient PM2.5. Data from the vital registry and cancer registry were used to establish statistical model to estimate the lung cancer deaths by province, age and gender. The lung cancer deaths attributable to PM2.5 were calculated based on the calculation of population attributable fraction (PAF). The GBD world population age structure was adopted to calculate age-standardized rates for comparison among provinces (including 31 provinces, autonomous regions and municipalities directly under the central government, as well as Hong Kong and Macao special administrative regions, excluding Taiwan of China).

10.Novel Prognostic Nomograms for Hepatocellular Carcinoma Patients with Microvascular Invasion: Experience from a Single Center

		                        			BACKGROUND/AIMS: Microvascular invasion (MVI) is an established risk factor for hepatocellular carcinoma (HCC). However, prediction models that specifically focus on the individual prognoses of HCC patients with MVI is lacking. METHODS: A total of 385 HCC patients with MVI were randomly assigned to training and validation cohorts in a 2:1 ratio. The outcomes were disease-free survival (DFS) and overall survival (OS). Prognostic nomograms were established based on the results of multivariate analyses. The concordance index (C-index), calibration plots and Kaplan-Meier curves were employed to evaluate the accuracy, calibration and discriminatory ability of the models. RESULTS: The independent risk factors for both DFS and OS included age, tumor size, tumor number, the presence of gross vascular invasion, and the presence of Glisson's capsule invasion. The platelet-to-lymphocyte ratio was another risk factor for OS. On the basis of these predictors, two nomograms for DFS and OS were constructed. The C-index values of the nomograms for DFS and OS were 0.712 (95% confidence interval [CI], 0.679 to 0.745; p<0.001) and 0.698 (95% CI, 0.657 to 0.739; p<0.001), respectively, in the training cohort and 0.704 (95% CI, 0.650 to 0.708; p<0.001) and 0.673 (95% CI, 0.607 to 0.739; p<0.001), respectively, in the validation cohort. The calibration curves showed optimal agreement between the predicted and observed survival rates. The Kaplan-Meier curves suggested that these two nomograms had satisfactory discriminatory abilities. CONCLUSIONS: These novel predictive models have satisfactory accuracy and discriminatory abilities in predicting the prognosis of HCC patients with MVI after hepatectomy.
