1. Mechanism and experimental validation of Zukamu granules in treatment of bronchial asthma based on network pharmacology and molecular docking

Aim To anticipate the mechanism of zuka- mu granules (ZKMG) in the treatment of bronchial asthma, and to confirm the projected outcomes through in vivo tests via using network pharmacology and molecular docking technology. Methods The database was examined for ZKMG targets, active substances, and prospective targets for bronchial asthma. The protein protein interaction network diagram (PPI) and the medication component target network were created using ZKMG and the intersection targets of bronchial asthma. The Kyoto Encyclopedia of Genes and Genomics (KEGG) and gene ontology (GO) were used for enrichment analysis, and network pharmacology findings were used for molecular docking, ovalbumin (OVA) intraperitoneal injection was used to create a bronchial asthma model, and in vivo tests were used to confirm how ZKMG affected bronchial asthma. Results There were 176 key targets for ZKMG's treatment of bronchial asthma, most of which involved biological processes like signal transduction, negative regulation of apoptotic processes, and angiogenesis. ZKMG contained 194 potentially active components, including quercetin, kaempferol, luteolin, and other important components. Via signaling pathways such TNF, vascular endothelial growth factor A (VEGFA), cancer pathway, and MAPK, they had therapeutic effects on bronchial asthma. Objective To systematically evaluate the efficacy and safety of mesenchymal stem cells(MSC)in the treatment of acute-on-chronic liver failure(ACLF).Methods This study was conducted according to PRISMA guidelines,with the PROSPERO registration number of CRD42024517851.PubMed,Embase,Wanfang Data,VIP,CNKI,CBM,and the Cochrane Library were searched for randomized controlled trials(RCT)and cohort studies on MSC in the treatment of ACLF published up to November 1,2023,and the articles were screened according to inclusion and exclusion criteria.After data extraction and quality assessment,RevMan 5.3 software was used to perform the Meta-analysis.Results A total of 11 articles involving 803 subjects were included in this meta-analysis.The results showed that for the patients with ACLF,MSC could improve 8-week survival rate(odds ratio[OR]=2.71,95%confidence interval[CI]:1.58-4.67,P=0.000 3),12-week survival rate(OR=2.24,95%CI:1.36-3.69,P=0.001),24-week survival rate(OR=2.09,95%CI:1.37-3.17,P=0.000 6),and 48-week survival rate(OR=2.09,95%CI:1.29-3.40,P=0.003)and reduce 12-week Model for End-Stage Liver Disease(MELD)score(mean difference[MD]=-3.27,95%CI:-6.07 to-0.48,P=0.02)and 24-week MELD score(MD=-2.24,95%CI:-3.16 to-1.33,P<0.000 01);it could also reduce the level of total bilirubin after 4 weeks of treatment(MD=-36.86,95%CI:-48.72 to-25.01,P<0.000 01)and increase 4-week albumin level(MD=2.11,95%CI:0.62-3.61,P=0.006)and 24-week albumin level(MD=3.54,95%CI:2.06-5.02,P<0.000 01).Adverse events were evaluated in 6 studies,with no serious adverse events.Conclusion MSC have a good safety in treatment and can improve the survival rate of patients and enhance liver function to some extent,and therefore,it holds promise for clinical application.

Pulse rate and blood oxygen levels are crucial physiological parameters that reflect physiological and pathological information within the human body.The system designs a wireless pulse wave monitoring system utilizing a flexible reflective probe and the AFE4490,which is capable of monitoring pulse wave and blood oxygen levels on the human forehead.The system is predominantly based on a reflective flexible probe,the AFE4490,a power supply module,a control microcontroller unit(MCU),and a Wi-Fi module.Post-processing by a slave computer,the collected pulse wave data is wirelessly transmitted to a smartphone.The real-time pulse waveform,pulse rate,and blood oxygen levels are displayed on an application.Following relevant tests and verifications,the system can accurately detect pulse wave signals,meet the requirements for wearable technology,and possesses significant market application potential.

Cardiopulmonary exercise testing(CPET)refers to a method of measuring various indicators of the human body under gradually increasing exercise loads to objectively evaluate cardiopulmonary reserve function and exercise endurance.Currently,CPET detection systems primarily measure subjects'ECG,respiratory flow,oxygen(O2),and carbon dioxide(CO2)parameters.This paper introduces a non-invasive multi-parameter exercise cardiopulmonary function evaluation system that incorporates impedance cardiography monitoring.The system integrates impedance cardiography monitoring with conventional CPET detection parameters and detects changes in the hemodynamic parameters of the body during exercise,aiding in the evaluation of exercise capacity.Additionally,the system features a portable design with Wi-Fi wireless transmission,which enhances its applicability.

Intestinal microorganisms and their metabolites are involved in the pathogenesis and progression of various cardiovascular diseases,especially in the progression of heart failure.This paper mainly discussed the gut microbial metabolites trimethylamine oxide(TMAO)participated in the pathological process of heart failure,and application value of TMAO in heart failure patients.This paper introduced the change characteristics of intestinal flora and its metabolites in heart failure patients,illu-minated the TMAO-mediated inflammatory response,and the related signal pathways and mechanism of myocardial hypertrophy and heart failure.High levels of TMAO are associated with poor outcomes in patients with heart failure indicating a good predictive value for the prognosis of heart failure.Regulating TMAO levels through diet,probiotics and prebiotics,antibiotics,fecal transplan-tation,and other pathways is expected to be a potential treatment for heart failure.

Objective To analyze the current situation and characteristics of risk factors in antithrombotic therapy(in-cluding antiplatelet and anticoagulant treatments)at home and abroad,and to provide a theoretical basis for the prevention and treatment of thrombosis or bleeding associated with antithrombotic therapy.Methods The literature on risk factors of an-tithrombotic therapy published in Chinese databases(China Journal Full-text Data,Wanfang Database,VIP Database)and Eng-lish databases(PubMed,Web of Science,MEDLINE)from January 2011 to November 2021 was searched and bibliometric analy-sis was performed.The visualization analysis was performed using VOS viewer software.Results A total of 595 publications were included in the analysis.The top three countries for English publications were the USA,China,and Japan.The type of stud-ies were predominantly cohort studies,with sample sizes mostly being below 1 000.Risk factors for antithrombotic therapy are cat-egorized into those affecting antiplatelet drugs,warfarin,and new oral anticoagulants.Age,gender,renal function,and combination of antithrombotic drugs are common risk factors,and different risk factors of antithrombotic drugs also have their characteristics.Conclusion While there is substantial research on risk factors in antithrombotic therapy globally,the sample size needs to be improved.Pharmacists should provide individualized medication services based on different drugs and different groups to ensure medication safety for patients.

Anti-tumor traditional Chinese medicine has a long history of clinic application, in which the star molecules have always been the hotspot of modern drug research, but they are limited by the solubility, stability, targeting, bioactivity or toxicity of the monomer components of traditional Chinese medicine anti-tumor star molecules and other pharmacokinetic problems, which hinders the traditional Chinese medicine anti-tumor star molecules for further clinical translation and application. Currently, the nanosystems prepared by supramolecular technologies such as molecular self-assembly and nanomaterial encapsulation have broader application prospects in improving the anti-tumor effect of active components of traditional Chinese medicine, which has attracted extensive attention from scholars at home and abroad. In this paper, we systematically review the research progress in preparation of supramolecular nano-systems from anti-tumor star molecule of traditional Chinese medicine, and summarize the two major categories and ten small classes of carrier-free and carrier-based supramolecular nanosystems and their research cases, and the future development direction is put forward. The purpose of this paper is to provide reference for the research and clinical transformation of using supramolecular technology to improve the clinical application of anti-tumor star molecule of traditional Chinese medicine.
9.Entity Recognition in Famous Medical Records Based on BRL Neural Network Model

ObjectiveIn order to improve the recognition accuracy of named entities in medical record texts and realize the effective mining and utilization of medical record knowledge, a Bert-Radical-Lexicon(BRL) neural network model is constructed to recognize medical record entities with respect to the characteristics of medical record texts. MethodWe selected 408 medical records related to hypertension from the the Complete Library of Famous Medical Records of Chinese Dynasties and constructed a dataset consisting of 1 672 medical records by manually labeling. Then, we randomly divided the dataset into three subsets, including the training set(1 004 cases), the testing set (334 cases) and the validation set(334 cases). Based on this dataset, we built a BRL model that fused various text features of medical records, as well as its variants BRL-B, BRL-L and BRL-R, and a baseline model Base for experiments. During the model training phase, we trained the above models using the training set to reduce the risk of overfitting. We continuously monitored the performance of each model on the validation set during training and saved the model with the best performance. Finally, we evaluated the performance of these models on the testing set. ResultCompared with other models, the BRL model had the best performance in the medical records named entity recognition task, with an overall recognition precision of 90.09%, a recall of 90.61%, and the harmonic mean of the precision and recall(F1) of 90.35% for eight types of entities, including disease, symptom, tongue manifestation, pulse condition, syndrome, method of treatment, prescription and traditional Chinese medicine(TCM). Compared with the Base model, the BRL model improved the overall F1 value of entity recognition by 5.22%, and the F1 value of pulse condition entity increased by 6.92%, which was the largest increase. ConclusionBy incorporating a variety of medical record text features in the embedding layer, the BRL neural network model has stronger named entity recognition ability, and thus extracts more accurate and reliable TCM clinical information.
10.Current Situation and Methodological Reflections on Blinding Assessment in Randomised Controlled Trials of Acupuncture

Blinding is an important means to control and reduce measurement bias in clinical research, and blinding assessment is the main method to measure the success of the blinding method. By summarizing the current situation of blinding assessment in randomized controlled trials (RCT) of acupuncture, it was found that the report rate of blinding assessment by acupuncture RCT was relatively low, and the studies reporting blinding assessment had several problems, such as incomplete assessment individuals, unreasonable assessment questionnaires, and unscientific analysis methods, and the setting of the assessment time point is controversial. Given the above problems, this paper discussed the key elements of blinding assessment individuals, assessment questionnaires, assessment time points, and analysis methods. It is suggested that blinding assessment should be carried out on all blinded participants and personnel in the study; the assessment questionnaire should be designed by direct inquiry, with responses designed using three or more categorical options that include an "unclear" option; the early stage of the trial should be taken as the mandatory time point for assessment, integrating the evaluation index of the James blinding index and the Bang blinding index, in order to standardize the application of blinding assessment in acupuncture RCT and improve the quality of acupuncture clinical research.
