Umbilical cord mesenchymal stem cells (UC-MSCs) have been widely used in regenerative medicine, but there is limited research on the stability of UC-MSCs formulation during production. This study aims to assess the stability of the cell stock solution and intermediate product throughout the production process, as well as the final product following reconstitution, in order to offer guidance for the manufacturing process and serve as a reference for formulation reconstitution methods. Three batches of cell formulation were produced and stored under low temperature (2-8 ℃) and room temperature (20-26 ℃) during cell stock solution and intermediate product stages. The storage time intervals for cell stock solution were 0, 2, 4, and 6 h, while for intermediate products, the intervals were 0, 1, 2, and 3 h. The evaluation items included visual inspection, viable cell concentration, cell viability, cell surface markers, lymphocyte proliferation inhibition rate, and sterility. Additionally, dilution and culture stability studies were performed after reconstitution of the cell product. The reconstitution diluents included 0.9% sodium chloride injection, 0.9% sodium chloride injection + 1% human serum albumin, and 0.9% sodium chloride injection + 2% human serum albumin, with dilution ratios of 10-fold and 40-fold. The storage time intervals after dilution were 0, 1, 2, 3, and 4 h. The reconstitution culture media included DMEM medium, DMEM + 2% platelet lysate, 0.9% sodium chloride injection, and 0.9% sodium chloride injection + 1% human serum albumin, and the culture duration was 24 h. The evaluation items were viable cell concentration and cell viability. The results showed that the cell stock solution remained stable for up to 6 h under both low temperature (2-8 ℃) and room temperature (20-26 ℃) conditions, while the intermediate product remained stable for up to 3 h under the same conditions. After formulation reconstitution, using sodium chloride injection diluted with 1% or 2% human serum albumin maintained a viability of over 80% within 4 h. It was observed that different dilution factors had an impact on cell viability. After formulation reconstitution, cultivation in medium with 2% platelet lysate resulted in a cell viability of over 80% after 24 h. In conclusion, the stability of cell stock solution within 6 h and intermediate product within 3 h meets the requirements. The addition of 1% or 2% human serum albumin in the reconstitution diluent can better protect the post-reconstitution cell viability.

Chinese Journal of Information on Traditional Chinese Medicine 2024;31(4):16-19
On the basis of inheriting the cancer toxin theory of Zhou Zhongying,a national TCM master,Professor Cheng Haibo's team proposes the theory of cancer toxin pathogenesis. This theory believes that thyroid carcinoma is caused by pathogenic surroundings, emotional injury, irregular diet and other factors, which cause the accumulation of qi stagnation, phlegm coagulation, blood stasis and cancer toxin in the neck. The main pathological factors of thyroid carcinoma are"qi,phlegm, stasis and toxin".The disease is located in front of the neck and is greatly related to liver, spleen and kidney."Liver-qi stagnation, phlegm and stasis containing toxin"is its core pathogenesis. The nature of the disease is mostly a mixture of deficiency and excess, and in the later stage of the disease,qi and yin are always depleted. The general principle of treatment in clinic is"anti-cancer and detoxification, strengthening vital energy and dispelling pathological factors".In view of the rise and fall of cancer toxin and the characteristics of different stages of the disease,it is recommended to soothe liver and regulate qi,dissolve phlegm and disperse lumps, reduce stasis and detoxify, benefit qi and nourish yin in the clinical treatment of thyroid carcinoma.Based on the theory of cancer toxin pathogenesis, the pathogenesis evolution and relevant treatment is discussed,in order to provide new ideas for the syndrome differentiation and treatment of thyroid carcinoma.

ObjectiveTo observe the clinical effect of Jieyu Qingxin formula granules combined with remote interactive cognitive behavioral therapy for insomnia (CBT-I) on chronic insomnia of liver depression and fire-turning type. MethodThis study was a prospective randomized controlled trial. 120 patients with chronic insomnia of liver depression and fire-turning type in Lu'an traditional Chinese medicine Hospital from January 2022 to June 2023 were selected as objects. They were randomly divided into two groups, with 60 cases in each group. The control group received remote interactive CBT-I. The observation group was treated with Jieyu Qingxin formula granules on the basis of the control group. Intervention treatment lasted for four weeks, and observation lasted for six weeks. Comparison of data of each group:clinical efficacy, changes in traditional Chinese medicine (TCM) syndrome score before and after treatment, changes in insomnia severity index (ISI) score,self-rating depression scale (SDS) and self-rating anxiety scale (SAS) score changes, total sleep time, wake time, sleep latency, sleep efficiency, Actigraphy sleep parameter value changes, serum neuron specific enolase (NSE) ,adenosine, dopamine (DA), 5-hydroxytryptamine (5-HT) level changes, and adverse reactions. ResultThe total effective rate in the observation group (92.45%,49/53) was higher than that in the control group(76.92%,40/52), and the difference was statistically significant(χ2=4.711 1,P<0.05). After treatment,TCM syndrome score,ISI score,SAS score, and SDS score were decreased in all groups. The total sleep time was extended, and wake time and sleep latency were shortened. The sleep efficiency was increased, but the NSE and DA levels were decreased. Adenosine and 5-HT levels were increased in all groups(P<0.05). After treatment, compared with the control group, the observation group had lower TCM syndrome score,ISI score,SAS score, and SDS score, longer total sleep time, higher sleep efficiency, shorter wake time and sleep latency, lower NSE and DA levels, and higher adenosine and 5-HT level (P<0.05). There was one case of nausea in the observation group and no adverse reaction in the control group during treatment. There was no significant difference between the two groups. ConclusionBy reducing NSE and DA and increasing the levels of 5-HT and adenosine, the anxiety (SAS score) and depression (SDS score) of patients can be improved, so as to improve their sleep and effectively treat chronic insomnia of liver depression and fire-turning type.

The Piezo protein is a non-selective mechanosensitive cation channel that exhibits sensitivity to mechanical stimuli such as pressure and shear stress. It converts mechanical signals into bioelectric activity within cells, thus triggering specific biological responses. In the digestive system, Piezo protein plays a crucial role in maintaining normal physiological activities, including digestion, absorption, metabolic regulation, and immune modulation. However, dysregulation in Piezo protein expression may lead to the occurrence of several pathological conditions, including visceral hypersensitivity, impairment of intestinal mucosal barrier function, and immune inflammation. Therefore, conducting a comprehensive review of the physiological functions and pathological roles of Piezo protein in the digestive system is of paramount importance. In this review, we systematically summarize the structural and dynamic characteristics of Piezo protein, its expression patterns, and physiological functions in the digestive system. We particularly focus on elucidating the mechanisms of action of Piezo protein in digestive system tumor diseases, inflammatory diseases, fibrotic diseases, and functional disorders. Through the integration of the latest research findings, we have observed that Piezo protein plays a crucial role in the pathogenesis of various digestive system diseases. There exist intricate interactions between Piezo protein and multiple phenotypes of digestive system tumors such as proliferation, apoptosis, and metastasis. In inflammatory diseases, Piezo protein promotes intestinal immune responses and pancreatic trypsinogen activation, contributing to the development of ulcerative colitis, Crohn’s disease, and pancreatitis. Additionally, Piezo1, through pathways involving co-action with the TRPV4 ion channel, facilitates neutrophil recruitment and suppresses HIF-1α ubiquitination, thereby mediating organ fibrosis in organs like the liver and pancreas. Moreover, Piezo protein regulation by gut microbiota or factors like age and gender can result in increased or decreased visceral sensitivity, and alterations in intestinal mucosal barrier structure and permeability, which are closely associated with functional disorders like irritable bowel sydrome (IBS) and functional consitipaction (FC). A thorough exploration of Piezo protein as a potential therapeutic target in digestive system diseases can provide a scientific basis and theoretical support for future clinical diagnosis and treatment strategies.

This study aimed to investigate the effect of iron overload on the transplant outcomes of pediatric patients with severe aplastic anemia (SAA) undergoing allogeneic hematopoietic stem cell transplantation (allo-HSCT). A retrospective analysis was conducted on the clinical data of 74 children with SAA who received allo-HSCT at the Hematology Department of Wuhan Children’s Hospital between January 2018 and August 2022. Children with iron overload (serum ferritin >1 000 μg/L) before transplantation had a longer disease course, received more red blood cell transfusions, and had a higher number of CD34 + cells infused. Moreover, iron overload significantly delayed the reconstitution of regulatory T cells after transplantation, increasing the incidence of hemorrhagic cystitis and grade Ⅲ-Ⅳ acute graft-versus-host disease after transplantation. However, iron overload did not significantly affect the overall survival and failure-free survival rates of the children.
