Journal of Public Health and Preventive Medicine 2025;36(1):57-60
Objective To investigate the acceptance and satisfaction of tuberculosis prevention and control personnel in Beijing with the patient management system, and to provide a basis for further improving the patient management model. Methods A survey was conducted on the current usage, satisfaction, willingness to use and system improvement opinions of the patient management system among medical staff involved in the supervision and medication management of pulmonary tuberculosis patients in Beijing. Results A total of 360 medical staff participated in the survey. “Patient management” was the function with the largest number of users, accounting for 96.94%. The proportion of users of each module who believed that the module's design met actual work needs was over 90%. About 94.44% of respondents believed that patient management systems facilitated the transfer and sharing of information between institutions. And 90.83% of respondents thought that the patient management system was easy to operate, and 89.17% of respondents believed that patient management systems reduced workload. About 97.50% of respondents were satisfied with the overall use of the patient management system. The results of the influencing factor analysis showed that those with 3 or less modules designed to meet actual work were less satisfied than those with more than 3 modules, and the difference was statistically significant (P=0.001). Respondents put forward suggestions for improvement on the optimization of operational details such as system response speed, interface design, system login and query statistics. Conclusion Medical staff involved in the follow-up management of pulmonary tuberculosis patients are highly satisfied with their work using the patient management system. During the promotion and use, it is still necessary to continuously optimize the system functions according to work needs so that the system can truly facilitate work.

Objective To explore and analyze the related factors of kinesiophobia in elderly patients with coronary heart disease (CHD) during cardiac rehabilitation exercise period, and to evaluate the effect of cognitive behavior therapy on improving kinesiophobia and promoting rehabilitation. Methods A total of 352 elderly patients with CHD admitted to the hospital were included from October 2023 to October 2024. Tampa Scale for Kinesiophobia-11 (TSK-11) was adopted to evaluate the kinesiophobia status. Patients with kinesiophobia were randomly grouped. Routine intervention (routine group, n=82) and cognitive behavior intervention (study group, n=82) were implemented respectively. The intervention effects were observed in both groups. Results Among the 352 patients, 46.59 % (164/352 ) of elderly patients with coronary heart disease had different degrees of kinesiophobia. The proportions of female, divorced/widowed, revascularization and family relationship disharmony and scores of Patient Health Questionnaire (PHQ9) and Generalized Anxiety Disorder Scale (GAD7) in patients with kinesiophobia were higher than those in patients without kinesiophobia (P<0.05) while the scores of General Self-Efficacy Scale (GSES) and Social Support Rating Scale (SSRS) were lower compared with those in patients without kinesiophobia (P<0.05). Logistic regression analysis found that female, divorced/widowed, family relationship disharmony, revascularization and scores of PHQ9, TSK-11, GAD7, GSES and SSRS were related to kinesiophobia (P<0.05). After intervention, the scores of TSK-11, PHQ9 and GAD7 in the study group were lower while the scores of GSES and SSRS, 6 min walking test distance, and cardiopulmonary exercise test peak oxygen uptake and anaerobic threshold were higher compared to the routine group (P<0.05). Conclusion The kinesiophobia in elderly patients with CHD during cardiac rehabilitation is related to gender, revascularization and psychosocial factors. Clinically, cognitive behavior intervention should be provided according to the situation and guided to carry out rehabilitation exercise regularly so as to promote improvement of cardiopulmonary function.

Modulating Tankyrases (TNKS), interactions with USP25 to promote TNKS degradation, rather than inhibiting their enzymatic activities, is emerging as an alternative/specific approach to inhibit the Wnt/β-catenin pathway. Here, we identified UAT-B, a novel neoantimycin analog isolated from Streptomyces conglobatus, as a small-molecule inhibitor of TNKS-USP25 protein-protein interaction (PPI) to overcome multi-drug resistance in colorectal cancer (CRC). The disruption of TNKS-USP25 complex formation by UAT-B led to a significant decrease in TNKS levels, triggering cell apoptosis through modulation of the Wnt/β-catenin pathway. Importantly, UAT-B successfully inhibited the CRC cells growth that harbored high TNKS levels, as demonstrated in various in vitro and in vivo studies utilizing cell line-based and patient-derived xenografts, as well as APCmin/+ spontaneous CRC models. Collectively, these findings suggest that targeting the TNKS-USP25 PPI using a small-molecule inhibitor represents a compelling therapeutic strategy for CRC treatment, and UAT-B emerges as a promising candidate for further preclinical and clinical investigations.

Osteoporosis and knee osteoarthritis often co-occur and are closely related in terms of epidemiology, clinical symptoms, pathogenesis and other aspects. Therefore, it is necessary to manage the co-morbidity and treat the two as a whole. Based on the overall relationship between wei (痿) and bi (痹) in TCM, it is believed that osteoporosis and knee osteoarthritis have marrow loss and bone atrophy as the core pathogenesis of co-morbidity, and microfractures as the central pathological link. The overall treatment is rooted in boosting kidney, supplementing marrow and strengthening the bones. According to the pathological manifestations of microfractures in the process of co-morbidity, and the different deficiency and excess characteristics of wei and bi, it can be divided into three types, "wei emerging with mild bi", "wei and bi progressing simultaneously", and "emphasis on both wei and bi", for treatment. In terms of "wei emerging with mild bi", that is the early stage of osteoporosis, the traditional Daoyin (导引) is the main therapy. For "wei and bi progressing simultaneously", it can be divided into three stages further, including the onset stage, remission stage, and recovery stage of knee pain, treated with Taohong Siwu Decoction (桃红四物汤), Bushen Huoxue Formula (补肾活血方) and self-made Bushen Qianggu Formula (补肾强骨方) as the main formula respectively. For "emphasis on both wei and bi", the proven formula, Qianggu Zhitong Formula (强骨止痛方), is taken as the main prescription.

Objective:To explore the onset characteristics and pathophysiological changes of simple liver cyst in Beijing. Methods:A retrospective cohort study was used. The physical examination data of Department of Health Management of Xuanwu Hospital of Capital Medical University for 10 years from January 1, 2012 to December 31, 2021 were analyzed. Selected clinical data of 37 389 subjects with 2 or more repeated ultrasound examinations, including 17 759 males, 19 630 females, aged (44.4±16.2) years, ranged from 22-103 years. 3 431 cases hepatic cyst were confirmed by repeated ultrasound examination, the data of the liver cyst formation after the same physical examination were the study group ( n=3 431), and the data before cyst formation were the control group ( n=3 431). The observation indicators included: (1) the epidemiological characteristics of liver cysts; (2) the age distribution of the incidence of liver cysts; (3) the gender distribution of the incidence of liver cysts; (4) the pathophysiological changes of liver cysts.Measurement data of normal distribution were expressed as mean±standard deviation ( ± s). The measurement data of skewed distribution were expressed as M ( Q1, Q3), using Wilcoxon signed rank sum test for comparing groups and chi-square test for comparing count data. The factors associated with hepatic cyst pathogenesis were summarized by multivariate Logistic regression. Results:The overall incidence of hepatic cysts was 9.18%, 9.78% in males, 8.63% in females, and the incidence of males was greater than that of females. The incidence of males over 70 to 79 years old decreased slightly, and the incidence in males and females in the other age groups increased with age. Results of multivariate Logistic analysis showed age ( OR=1.01, 95% CI: 1.01-1.02, P<0.01), waist circumference( OR=1.02, 95% CI: 1.01-1.02, P<0.01), systolic blood pressure ( OR=1.00, 95% CI: 0.99-1.00, P=0.013), ALT ( OR=0.99, 95% CI: 0.98-1.00, P<0.01), AST ( OR=1.03, 95% CI: 1.02-1.04, P<0.01), triglyceride lipids ( OR=0.90, 95% CI: 0.86-0.95, P<0.01), HDL( OR=0.71, 95% CI: 0.60-0.83, P<0.01), uric acid ( OR=1.00, 95% CI: 1.00-1.00, P<0.01), creatinine ( OR=0.99, 95% CI: 0.99-0.99, P<0.01) were the factors influencing the occurrence of liver cysts. Conclusions:The incidence of liver cysts increased linearly with age, and the incidence of males was greater than that of females. Age, waist circumference, systolic blood pressure, ALT, AST, triglycerides, HDL, uric acid, and creatinine may interact with the occurrence and development of liver cysts.

Objective:To explore the feasibility and safety of robotic-assisted living donor left lateral segmentectomy (LDLLS) in a large pediatric liver transplant program. Methods:Retrospective analysis was performed for clinical data of 45 LDLLS donors and recipients from June 2021 to September 2022.Traditional open donor liver resection (n=30) and robotic-assisted segmentectomy (n=15) were performed. Two groups were compared with regards to operative duration, intraoperative hemorrhage, postoperative healing and postoperative complications.SPSS 21.0 was utilized for statistical analysis.Independent sample T, paired sample T, Wilcoxon rank sum and Chi-square tests were performed for examining the inter-group differences. Results:Operative duration of robot-assisted surgery group was substantially longer than that of traditional open surgery group ( P<0.001). Intraoperative blood loss was less in robot-assisted surgery group was less than that in traditional open surgery group[(106.0±39.8) vs.(251.0±144.8) ml, P=0.001]. Postoperative hospital stay of robot-assisted surgery group was shorter than that of traditional open surgery group[6.0(6.0, 6.0) vs.7.0(6.0, 9.0), P<0.05]. Two cases of postoperative biliary leakage were observed in donor of traditional open surgery group.Among 2 cases of abdominal infection, one was due to biliary leakage from liver section and secondary surgery was then performed. One case of incisional infection and another case of thrombosis occurred in donor of traditional open surgery group. In robot-assisted surgery group, only one donor had amylase elevation. In traditional open surgery group, there were one case of local thrombosis in middle hepatic vein and one case of bile duct stricture. No long-term complications occurred in robot-assisted surgery group during a follow-up period of over 6 months. Finally recipient data analysis indicated that no significant inter-group differences existed in operative duration, intraoperative blood loss, postoperative hospital stay or postoperative abdominal infection ( P=0.634, P=0.180, P=0.86 and P=0.153). Conclusions:Robotic-assisted LDLLS proves to be be a safe and reliable option for living donor segmentectomy. It is superior to conventional LDLLS in terms of shorter hospital stay, less intraoperative blood loss and fewer postoperative complications.

International Journal of Surgery 2023;50(12):806-811
Multidisciplinary comprehensive treatment is the best medical model for liver cancer at present, and combined local and systemic treatment can prolong the survival period and improve the quality of life of patients. Local thermal ablation therapy represented by radiofrequency ablation has the advantages of precision and minimally invasive, easy operation, and precise efficacy, and has been increasingly widely used in the clinical application of hepatocellular carcinoma treatment. Radiofrequency ablation, as a precise and minimally invasive local thermal ablation therapy, can increase the efficacy, reduce the occurrence of complications of patients by combining with systemic therapies, such as surgery, interventional therapy, chemotherapy and targeted therapy. In this paper, the clinical application progress of radiofrequency ablation in hepatocellular carcinoma treatment is reviewed, and the combined therapeutic program of radiofrequency ablation in hepatocellular carcinoma in the era of multidisciplinary comprehensive treatment is discussed. For patients with advanced hepatocellular carcinoma, radiofrequency ablation combined with chemotherapy, targeted therapy and immunotherapy has shown good effects in the treatment, which is expected to bring more benefits to patients with advanced hepatocellular carcinoma in the future.
