1.The Influence of Late Pregnancy Perineal Massage Combined with Hip Training on Labor Outcomes and Psychological Elasticity of Primiparous Pregnant Women

Objective To investigate the effects of perineal massage combined with hip joint exercise on the outcome of delivery and mental resilience of primipara.Methods 90 pregnant women in the third trimester(after 36 weeks)who obtained the knowledge about perineal massage from midwife clinic were randomly divided into two groups with 45 cases each.The control group received regular antenatal examination and family self-exercise;the experimental group received perineal massage and hip joint training combined treatment.The delivery outcome,birth experience and maternal mental resilience of the two groups were compared.Results the number of vaginal delivery in the experimental group were higher than that in the control group(P<0.05);the second stage of labor was significantly shorter than that of the control group(P<0.05);the perineal integrity rate was higher than that of the control group(P<0.05);the scores of all dimensions in delivery experience questionnaire were higher than that of the control group(P<0.05);the scores of all dimensions in maternal mental resilience were higher than those of the control group(P<0.05)after intervention.Conclusion The perineal massage which conducted by midwives combined with hip movement can effectively improve the quality of delivery,relieve the negative emotions of pregnant women,improve the psychological elasticity level of pregnant women,and improve the delivery outcomes.
2.Chronic intermittent hypoxia impairs learning and memory by upregulating HMGB1 and NF-κB in rat hippocampus

Objective:To explore the effect of chronic intermittent hypoxia(CIH)on learning and memory dysfunc-tion in rats,as well as the expression of high mobility group box-1(HMGB1)and nuclear transcription factor-KB(NF-κB)in the hippocampus region.Methods:The CIH rat model was established,and forty SD rats were randomly divid-ed into four groups:normoxia group,hypoxia for 4 weeks group(CIH4 group),hypoxia for 8 weeks group(CIH8 group),and hypoxia for 12 weeks group(CIH12 group).Morris water maze was used to assess the learning memory ability of rats,and immunohistochemistry and ELISA were used to detect the expression of HMGB1 and NF-κB in the hippocampus of rats.Results:Compared with the normoxia group,the CIH12 and CIH8 groups had longer escape la-tency,the number of crossing the platform and the residence time in the quadrant of the platform were significantly shortened,but there was no significant difference in the CIH4 group.Additionally,there was no significant expression of HMGB1 and NF-κB in the hippocampal region of the normoxia group,but little expression was observed in CIH4 group,and significantly expressed in CIH8 group and CIH12 group,and the expression of CIH12 group was significantly higher than that of CIH8 group.Conclusion:CIH can lead to a decline in learning and memory function in rats,and the longer time of intermittent hypoxia led to the more significant effect on their learning and memory function.In addi-tion,CIH also leads to increased expression levels of HMGB1 and NF-κB in the hippocampus region,and the expres-sion increased more significantly after hypoxia for 12 weeks,comparing to hypoxia for 8 weeks.
3.Practice and exploration of monitoring index for the construction of national regional medical centers of Beijing Children′s Hospital

Under the leadership and high-level promotion of the central and local governments, the construction of national regional medical centers(NRMCs) in China has achieved initial results, and has received recognition and high attention from all sectors of society. The current focus of NRMCs construction has progressed from the project selection to the evaluation of construction effectiveness and the establishment of corresponding operating mechanisms. On the basis of analyzing the characteristics of NRMCs construction and reviewing the evaluation of NRMCs construction at the national level, this paper systematically introduced the monitoring index system and its application of NRMCs construction in Beijing Children′s Hospital, and put forward corresponding policy suggestions. The monitoring of the construction indexes of NRMCs could provide a basis and support for hospitals to identify problems in time, seek government support, and carry out target management, and has reference for promoting the effective construction of NRMCs.

Peking Union Medical College Hospital, as one of the most stressful medical institutions in China, is facing the problem of emergency department overcrowding. In order to effectively alleviate the emergency overcrowding, improve the medical quality and patients′ medical experience, the hospital firmly grasped the two incremental links of " throughput" and " output" factors, established a multidisciplinary and multi-department cooperation team, constructed a close medical alliance cooperation mode, and innovated and explored a harmonious emergency overcrowding relief mode with the goal of unblocking the " exit" of patients. The practice showed that the comprehensive measures could effectively alleviate the problem of emergency overcrowding, and improve the medical environment and medical quality.
5.Roles of Histidine Kinases and Histidine Phosphatases in Cancer.

Phosphorylation is the most common and important post-translational modification of proteins, which plays an important role in the regulation of cell proliferation, differentiation, development and metabolism, and is closely related to the tumorigenesis and metastasis of cancer. Protein kinases and phosphatases generally regulate protein phosphorylation levels as a pair of opposite acting enzymes. Protein phosphorylation in eukaryotes occurs mainly in serine, threonine, and tyrosine residues, and their roles in tumorigenesis and development have been extensively studied. But the roles on histidine phosphorylation is less known due to the immature mass spectrometry and enrichment techniques. In recent years, with the rapid development of related technologies and the discovery of new histidine phosphatases, researchers have paid more attention to the roles of histidine phosphorylation in tumors. Therefore, we aim to review the roles of histidine kinases and phosphatases in tumor.

6.Analysis of gonad and reproductive function of recipients after hematopoietic stem cell transplantation in severe aplastic anemia

Objective:To compare the effects of two pretreatment schemes on the efficacy, gonad and reproductive function of haploid hematopoietic stem cell transplantation recipients with severe aplastic anemia(SAA).Methods:The data of 73 patients with SAA who underwent haploid hematopoietic stem cell transplantation were analyzed retrospectively.The pretreatment scheme was divided into Fludarabine+ Cyclophosphamide+ Antithymocyte globulin group(FC lowATG group, 45 cases)and Busulfan+ Cyclophosphamide+ Antithymocyte globulin group(Bucy/ATG group, 28 cases). The changes of blood cell implantation time, follicle stimulating hormone(FSH), luteinizing hormone (LH), estradiol and testosterone were compared between the two groups. Results:there was no significant difference in blood cell implantation time between the two groups( P=0.096; P=0.133). The levels of FSH and LH in female recipients in Bucy/ATG group were higher than those in FC lowATG group, and the level of estradiol was lower than that in FC lowATG group.There were significant differences between the groups(all P<0.05). The pregnancy or fertility rate of female recipients in Bucy/ATG group was lower than that in FC lowATG group(all P<0.05). There was no significant difference in FSH, LH, testosterone and fertility between the two groups(all P>0.05). There was no significant difference in 2-year overall survival rate and failure free survival rate between the two groups( P=0.091; P=0.084). Conclusions:FC lowATG may be an effective pretreatment scheme for haploid hematopoietic stem cell transplantation in SAA with less damage to gonad and reproductive function.

8.Comparison of effects of electroacupuncture at "Dachangshu" (BL 25) or "Tianshu" (ST 25) on visceral sensitivity, c-kit and TRPV1 of irritable bowel syndrome rats.

OBJECTIVETo compare the effects of electroacupuncture (EA) at "Dachangshu" (BL 25) or "Tianshu" (ST 25) for visceral sensitivity, gene expression product c-kit of colonic Cajal interstitial cells (ICC) and capsaicin receptor 1 (TRPV1) of irritable bowel syndrome (IBS) rats, so as to investigate the effect and mechanism differences of EA at the back point and the front point of large intestine for IBS rats.
METHODSForty-two Wistar neonatal rats were randomly divided into a blank group (9 rats) and a model group (33 rats). IBS model was established with mother and child separation, acetic acid enema in young rats and colorectal dilatation method. Twenty-seven IBS rats in life were randomly divided into a model control group, a Dachangshu group and a Tianshu group, 9 rats in each group. EA (disperse-dense wave, 2 Hz/100 Hz, 0.1-0.3 mA) for 20 min was used at "Dachangshu" (BL 25) and "Tianshu" (ST 25) respectively in the Dachangshu and Tianshu groups, once every other day, totally 5 times. The rats in the model control group were fixed with soft cloth sleeve for 20 min, without acupuncture. No intervention was used in the blank group. The stool property Bristol grading score was recorded before and after intervention in each group. The visceral sensitivity was evaluated by abdominal withdrawal reflex. The latency until the first systolic wave occurred and the number of systolic wave within 90 s were observed. Immunohistochemical was used to detect the positive expressions of c-kit and TRPV1, the ICC colon specific marker.
RESULTSCompared with the blank group, the Bristol score increased,latency period shortened, systolic wave number increased, c-kit and TRPV1 positive expressions increased in the model control group (all <0.01). Compared with the model control group, the Bristol score decreased, latency period increased, systolic wave number decreased, c-kit and TRPV1 positive expressions decreased after intervention in the Dachangshu and Tianshu groups (<0.05, <0.01). Compared with the Dachangshu group, the TRPV1 positive expression decreased after intervention in the Tianshu group (<0.05).
CONCLUSIONEA at "Dachangshu" (BL 25) or "Tianshu"(ST 25) can improve the diarrhea in IBS model rats, reduce the visceral sensitivity, and its mechanism may be related to regulating the expressions of colon c-kit and TRPV1. EA at "Tianshu" (ST 25) is more apparent for TRPV1 than at "Dachangshu" (BL 25).
9.The value of prenatal ultrasound in diagnosis of fetal meningocele or encephalocele

Objective To investigate the clinical value of prenatal ultrasound in fetal meningocele and encephalocele.Methods Thirty nine fetuses with meningocele and encephalocele confirmed by prenatal ultrasound and abortion were acquired and their clinical data and sonographic features were retrospectively analyzed.Results Ultrasound screening in 111 620 cases showed 39 fetuses were with meningocele and encephalocele,accounting for 0.35%.Among them,16 cases were diagnosed with meningocele (including 1 case with two bulging parts) and 23 cases with encephalocele.Prenatal ultrasound could clearly detect the location and size fetal skull defect,and bulging features.According to their sonographic features,meningocele or encephalocele was determined.Conclusion Ultrasound could be a reliable prenatal screening method,provide an important basis for clinical intervention and have a significant clinical value in fetal meningocele and encephalocele.
10.The clinical significance of evaluating vertebral artery intracranial stenotic lesions

Objective To evaluate the correlation between intracranial vertebral artery (VA) stenotic lesions at different locations and posterior circulation ischemia (PCI) symptoms.Methods The present study included 362 cases of patients with unilateral VA severe stenosis or occlusion confirmed by carotid Doppler ultrasonography,CT angiography and (or) digital subtraction angiography.According to the relationship between the location of stenotic lesion and posterior inferior cerebellar artery (PICA),all cases were divided into pre-PICA group (n=73) and post-PICA group (n=289).According to presentation of PCI symptoms,all cases were divided into symptomatic group (n=133) and asymptomatic group (n=229).On CDFI,hemodynamic parameters were analyzed and recorded,which included the peak systolic velocity (PSV),end diastolic velocity (EDV),resistance index (RI) and VA diameter (VAD) and spectrum shape.And the correlation between location of intracranial VA stenosis / occlusion and PCI symptoms was evaluated.The hemodynamic parameters of bilateral intervertebral segments were compared in patient with unilateral intracranial VA stenosis by paired t test.The ipsilateral intervertebral segment hemodynamic changes were compared among patents with different locations of unilateral intracranial VA stenosis by using independent sample t test.Using 2 × 2 crosstables and Pearson 22 test,the correlation between the location of VA stenosis and PCI symptoms was analyszed.Results Hemodynamic parameters of VA were associated with its stenotic location to PICA.The comparison of the ipsilateral and contralateral VA showed that PSV,EDV and RI,VAD had significant difference (43.97± 1.22 vs 55.82± 1.08,6.35±0.23 vs 19.41 ±0.48,0.850±0.004 vs 0.640± 0.005,2.75 ± 0.04 vs 4.00± 0.03),difference was statistically significan (t=-7.086,-23.754,-32.603,23.842,all P < 0.001).The intervertebral segment PSV,EDV and RI of ipsilateral VA were significantly related to the stenotic location to PICA (t=-6.665,-17.459,22.143,P<0.001),but not for VAD (P>0.05).In pre-PICA group,the intervertebral segment spectrum of ipsilateral VA was unimodal-style,while in post-PICA group the spetrum was high-resistance-style.VA intracranial severe stenosis at different locations was not associated with the symptoms of PCI (r=0.023,P=0.782),while VA intracranial occlusion at different locations was associated with the symptoms of PCI significantly (r=0.792,P < 0.05).Conclusions CDU is a non-invasive,real-time and dynamic assessment tool for VA intracranial lesions in different locations,which can provide reliable information for clinical treatment and prognosis prediction.
