1.Exploration and thinking of online teaching of medical microbiology experiment

Medical microbiology experiment is faced with many problems in online teaching. This study adopts the teaching mode of online live broadcast + operation video + virtual experiment, and make up the operation gap to some extent through operation video and virtual experiment. The mode of assessment is subjective thinking question (closely following the operation process) + experiment design + literature review (focusing on the key technology or new technology of clinical assessment that cannot be carried out due to the limitation of conditions in traditional experiments, such as mass spectrometry, fluorescence quantitative PCR, and G-test), and it is helpful to understand students' mastery of teaching objectives, and the ability of comprehensive application and innovative thinking. The student questionnaire shows that most students hold a positive attitude towards the online experimental teaching mode, and the quality of students' homework shows that most students have a good learning effect.
2.Intrafamilial infection of Helicobacter pylori in Zhengzhou area

Objective:To investigate Helicobactor pylori (H. pylori) infection status and interfamilial transmission pattern in Zhengzhou area. Methods:A cross-sectional study was conducted from September 2020 to march 2021, among 731 individual from 266 families randomly selected from 9 communities of Zhengzhou area. H. pylori infection status was determined by serum antibody tests, and 13C-urea breath test was performed in the previously eradicated population to clarify the current infection status. The individual and familial infection rate, infection status for couples and children and adolescent were analyzed. Results:Among 731 individuals from 266 families, 397 of them were H. pylori positive. The individual infection rate was 54.31% (397/731); among infected individuals 77.83% (307/397) were infected with type Ⅰ strain, 22.67% (90/397) were infected by type Ⅱ strain. Annual household income ( χ2=0.419, 0.410, 0.213, all P>0.05), smoking history (χ 2=0.071, P>0.05), drinking history ( χ2=0.071, P>0.05), dining place ( χ2=0.009, P>0.05), gastrointestinal symptoms ( χ2=0.047, P>0.05), family history of gastric disease ( χ2=0.069, P>0.05), and history of gastric cancer ( χ2=0.004, P>0.05) had no significant differences between H. pylori-positive and -negative groups, but the infection rate in individuals with higher education level was lower ( χ2=4.449, P<0.05). The infection rate was significantly higher in≥18 age groups compared with<18 age groups ( χ2=6.531, 23.362, 20.671, 24.244, 37.948, 14.597 and 5.170, all P<0.05). The familial H. pylori infection rate was 87.59% (233/266), and in 61 families all member were infected (26.18%, 61/233). The positive rate was 23.08% (6/26) in 50 families with children under 18 years when both parents were infected. Among 231 coupled families, both couples were infected in 78 families (33.76%), one couple was infected in 113 families (48.92%), and both couples were not infected in 40 (17.32%). With the increase of marriage time, the infection rate of both spouses increased significantly ( χ2=7.775, 12.662, 15.487, all P<0.05). Conclusions:The distribution of H. pylori infection presents a family cluster pattern, and intrafamilial infection is an important transmission rout of H. pylori. The type I strain of H. pylori is the dominate strain in this area.
3.Short-term effect of robotic pancreaticoduodenectomy and laparoscopic pancreaticoduodenectomy: a meta-analysis

Objective:To evaluate the short-term effect of robotic pancreaticoduodenectomy (RPD) and laparoscopic pancreaticoduodenectomy (LPD) in the treatment of ampullary and periampullary tumor systematically.Methods:A systematic search of the PubMed, Embase and Cochrane library database using the key words "pancreaticoduodenectomy, duodenopanreatectpmy, whipple, laparoscopic, robotic, Da Vinci" . A systematic search of the Sinomed, Wangfang, VIP and CNKI databases including the key words "胰十二指肠切除" , "腹腔镜" , "机器人" , "达芬奇" . To investigate the differences of the conversion rate of laparotomy, incidence of postoperative complications, and the postoperative hospital stay between the RPD group and the LPD group. The software RevMan5.3 was used in this meta-analysis.Results:A total of 4 retrospective cohort studies and 1 001 patients were included in this meta-analysis, including 451 patients in the RPD group and 550 patients in the LPD group. The results of the meta-analysis showed that there were significant differences between RPD group and LPD group for the conversion rate laparotomy ( OR=0.35, 95% CI: 0.24-0.50, P<0.05). There were no significant differences between RPD group and LPD group for the incidence rate of overall complication( OR=1.23, 95% CI: 0.95-1.58, P>0.05), hemorrhage ( OR=0.71, 95% CI: 0.50-1.00, P>0.05), pancreatic fistula ( OR=1.09, 95% CI: 0.80-1.49, P>0.05), delayed gastric empty ( OR=0.81, 95%CI: 0.57-1.14, P>0.05) and hospital stay after surgery ( WMD=-2.87, 95% CI: -1.44-1.70, P>0.05). Conclusions:RPD is as safe as LPD, with the same complication rate. Therefor it is worthy for further application in medical institutions with relevant conditions.
4.A bionic cerebellar motion control model and its application in arm control.

How to realize the control of limb movement and apply it to intelligent robot systems at the level of cerebellar cortical neurons is a hot topic in the fields of artificial intelligence and rehabilitation medicine. At present, the cerebellar model usually used is only for the purpose of controlling the effect, borrowing from the functional mode of the cerebellum, but it ignores the structural characteristics of the cerebellum. In fact, in addition to being used for controlling purposes, the cerebellar model should also have the interpretability of the control process and be able to analyze the consequences of cerebellar lesions. Therefore, it is necessary to establish a bionic cerebellar model which could better express the characteristics of the cerebellum. In this paper, the process that the cerebellum processes external input information and then generates control instructions at the neuron level was explored. By functionally segmenting the cerebellum into homogeneous structures, a novel bionic cerebellar motion control model incorporating all major cell types and connections was established. Simulation experiments and force feedback device control experiments show that the bionic cerebellar motion control model can achieve better control effect than the currently widely used cerebellar model articulation controller, which verifies the effectiveness of the bionic cerebellar motion control model. It has laid the foundation for real brain-like artificial intelligence control.

5.Laparoscopic surgery for Mirizzi syndrome,a report of 14 cases

Objective To evaluate laparoscopic surgery for Mirizzi syndrome.Methods A retrospective study was carried out on 14 patients undergoing laparoscopic operation for Mirizzi syndrome from Feb.2013 to Nov.2018.There were 4 patients of type Ⅰ,8 patients of type Ⅱ,2 patients of type Ⅲ according to Csendes classification.Results Laparoscopic procedures were successfully completed in all patients.All 4 cases of type Ⅰ received laparoscopic cholecystectomy.4 patients of type Ⅱ were given laparoscopic cholecystectomy combined with primary suture of common bile duct;the other 4 patients of type Ⅱ underwent laparoscopic cholecystectomy plus T tube drainage.2 patients of type Ⅲ were treated with laparoscopic partial-cholecystectomy combined with repair of the duct defect and T tube drainage.All the patients recovered uneventfully.The postoperative hospitalization time was 4 to 7 d,with an average of 5.5 d.There were no major postoperative complications.14 patients were followed up for 1-8 months,and there was no biliary stricture and no stone recurrences.Conclusion For cases of type Ⅰ,type Ⅱ and type Ⅲ,laparoscopic surgery is a safe treatment.
6.Postoperative prophylactic anticoagulation in the prevention of portal venous thrombosis in patients after laparoscopic splenectomy: a Meta-analysis

Objective To study the effectiveness and safety of prophylactic anticoagulation in the prevention of portal venous thrombosis (PVST) in patients after laparoscopic splenectomy.Methods A systematic search of the PubMed,Embase,Cochrane Library,Sinomed,Wangfang,Weipu and CNKI databases was performed to identify studies which compared outcomes in patients with or without prophylactic anticoagulation after laparoscopic splenectomy.The quality of the included studies was assessed using the Cochrane collaboration tool and the Newcastle-Ottawa Scale.Heterogeneity was evaluated using the x2 and I2 tests.The primary outcome was the incidence of postoperative PVST.Results Five studies were included into this review,which involved 206 and 168 patients with or without prophylactic anticoagulation,respectively.The incidence of PVST was significantly reduced with prophylactic anticoagulation with an odds ratio (OR) of 0.32 [95% confidence interval (CI),0.13 ~0.79,P<0.05].Conclusion Prophylactic anticoagulation resulted in a significant reduced incidence of PVST after laparoscopic splenectomy.
7.The impact of combined postoperative antiviral therapy with adjuvant transarterial chemoembolization in hepatitis B-related hepatocellular carcinoma patients with high risks of recurrence

Objective To investigate the impact of combined postoperative transcatheter arterial chemoembolization (TACE) with antiviral therapy in hepatitis B-related hepatocellular carcinoma (HBV-HCC) patients with high risks of recurrence.Methods Fifty-three consecutive patients who underwent curative resection of HBV-HCC between January 2014 to February 2016 were enrolled.These patients were assigned to either the adjuvant antiviral therapy combined with TACE group (n =32),the treatment group or the no adjuvant treatment group (n =21,the control group).The recurrence-free survival (RFS) and overall survival (OS) were analyzed.Results There was no significant difference between the two groups in clinical characteristics (P>0.05).The recurrence-free survival (RFS) (mean±S.D.) was (20.1 ± 1.8) months in the treatment group and (18.7±2.4) months in the control group (P=0.752).The 1-,2-and 3-year RFS rates of the treatment group and the control group were 65.6% vs.57.1%,31.3% vs.28.6% and 15.6% vs.14.3%,respectively (P>0.05).The overall survival (OS) (mean±S.D.) was (26.8± 1.7) months in the treatment group and (21.1±2.2) months in the control group (P=0.037).The 1-,2-and 3-year RFS rates were 65.6% vs.57.1%,31.3% vs.28.6% and 15.6% vs.14.3% in the treatment group and the control group,respectively.The 1-,2-,and 3-year OS rates were 87.5% vs.66.7%,59.4% vs.38.1% and 43.8% vs.19.0% in the treatment group and the control group,respectively.Conclusion Antiviral therapy in combination with TACE did not decrease the RFS rate,but it improved the OS rate in HCC patients with high risks of recurrence.
8.Preoperative spleen-liver volume ratio predicts the risk of liver cancer recurrence after hepatectomy

Objective To explore the value of preoperative spleen-liver volume ratio (SLVR) for predicting recurrence of primary liver cancer after hepatectomy.Methods Clinical data of 86 cases of hepatocellular carcinoma undergoing hepatectomy were analyzed retrospectively from January 2009 to December 2014.According to the preoperative SLVR,these patients were divided into two groups:SLVR < 0.8 group (low SLVR group) and SLVR≥0.8 group (high SLVR group).Patients were followed-up until June 2015.Cox ratio risk pattern analysis was used for the recurrent correlative factors.Results The 1-,3-,and 5-year survival rates were 95.5%,88.6%,and 81.8% in the low SLVR group,and 73.8%,47.6%,and 40.5% in the high SLVR group,respectively.The difference were statistically significant between two groups (P <0.05).Cox regression analysis revealed that SLVR≥0.8,AFP-L3% ≥10%,the maximum diameter of the tumor > 5 cm,and hepatic or portal vein tumor thrombus were independent predictors of poor disease-free survival after hepatectomy for hepatocellular carcinomas (P < 0.05).Conclusions Preoperative SLVR≥0.8 is an independent adverse predictor of poor disease-free survival.
9.Effects ofWenyujin Essential Oil on tau Protein Phosphorylation in Mice with Aβ-induced Alzheimer Disease through PI3k/Akt Pathway

Objective To investigate the effects ofWenyujinessential oil on tau protein phosphorylation in mice with Alzheimer Disease (AD); To discuss the relevant mechanism of action.MethodsSixty Kunming mice were divided into six groups randomly: sham-operation group, model group,Wenyujin essential oil low- and high-dose group, donepezil group andShenzhiling group.β-Amyloid protein (Aβ25-35) was infused into the hippocampal of mice. Mice in each group were given relevant medicine for gavage for 15 d. After the last administration, immunohistochemistry was used to detect the expressions of tau protein phosphorylation points (Ser 404 and Thr 231). Western blot was used to detect tau protein phosphorylation points and the expression of PI3K/Akt protein. Results Compared with sham-operation group, tau protein phosphorylation points in the model group increased (P<0.05), but the phosphorylation levels of PI3K and Akt decreased significantly (P<0.05). Compared with the model group, tau protein phosphorylation (Thr231 and Ser404) inWenyujin essential oil high-dose group decreased significantly (P<0.05), and the phosphorylation levels of PI3K and Akt increased(P<0.01). ConclusionWenyujinessential oil can inhibit tau protein phosphorylation in mice. The possible mechanism may be related to the PI3K/Akt signal pathway.
10.Safety of laparoscopic surgery in radical resection for different stage rectal cancer

Objective To explore the safety of the laparoscopic surgery in radical resection for different stage rectal cancer.Methods Clinical data of 200 cases with rectal cancer underwent laparoscopic radical resection(observation group) were analyzed retrospectively,including 52 cases of stage 0-Ⅰ,148 cases of stage Ⅱ-Ⅲ.Two hundred cases patients with rectal cancer underwent open radical surgery were selected as control group,including 44 cases of stage 0-Ⅰ,156 cases of stage Ⅱ-Ⅲ.The length to distal resection margin,the number of harvested lymph nodes,the incidence of postoperative complications and the prognosis of recent situation were observed,the safety of the laparoscopic surgery was analyzed.Results Whether rectal cancer of stage 0-Ⅰ or Ⅱ-Ⅲ,there were no significant differences in terms of the length of distal resection margin((2.5±0.9) cmvs.(2.4±1.1) cm,t=0.490,P=0.625;(3.1±1.0) cm vs.(3.2±1.3) cm,t=0.749,P=0.454),the number of harvested lymph nodes((12.3±4.2) vs.(12.7±3.9),t=0.480,P=0.632;(13.9±5.4) vs.(15.1±4.9),t=2.369,P=0.118),the incidence of postoperative complications (17.3% vs.18.2%,x2 =0.012,P=0.911;27.7% vs.28.8%,x2=0.049,P=0.825),the rate of local recurrence(1.9%vs.2.3%,x2 =0.014,P =1.000;4.1% vs.3.2%,x2=0.157,P=0.692),the distant metastasis (3.8% vs.2.3%,x2=0.195,P=1.000;5.4% vs.4.5%,x2=0.137,P=0.712) and 3-year survival rate (96.2%vs.95.5%,x2=0.010,P=0.808;83.8% vs.85.3%,x2=0.132,P=0.714) between the observation group and control group (P>0.05).Conclusion For rectal cancer of stage 0-Ⅰ,laparoscopic surgery has good safety,and worthy of popularization and application.For rectal cancer of stage Ⅱ-Ⅲ,due to its operation for a long time as well as the possible risk in postoperative anastomotic fistula,the implementation of laparoscopic surgery should be more cautious.
