1.Effect of topical steroid treatment on children with severe phimosis

		                        			Objective To investigate the clinical efficacy of 0.1% mometasone furoate cream in the treatment of phimosis in children.Methods A prospective study was carried out over two years period on an outpatient which basis on two groups of patients with severe phimosis.598 children with severe phimosis (Kikiros classification 4-5) aged from 2 years old to 11 years old and 8 months were selected.311 cases in the observation group and 287 cases in the control group.The observation group applied a steroid cream the foreskin twice a day for 4 weeks,and the control group used local handling of the foreskin twice a day for 4 weeks.The effects of the two groups after 2 and 4 weeks of treatment were compared.Results 29 cases in the observation group and 47 cases in the control group were loss of follow up.In the steroids group which including 282 patients,68.8% of patients (194 cases) showed a complete response (full retraction of the foreskin) to the therapy.The total efficiency rate of the 4 stage phimosis group is higher than the 5 grade phimosis group.Patients who had a history of balanoposthitis or urinary tract infection showed poorer improvement in preputial retraction.A total of 28 out of 240 patients (11.7%) in the control group showed a complete response to the therapy.The total efficiency rate of the observation group was significantly higher than the control group (x2 =173.121,P < 0.01).There were 4 cases of discomfort in the observation group and 6 cases of foreskin injured in the control group.Conclusions Topical application of 0.1% mometasone furoate cream in the treatment of severe phimosis in children is an effective,safe and simple non-invasive treatment with less adverse reactions.
		                        		
		                        		
		                        		
		                        	
2.Clinical features of 68 cases of complicated upper urinary tract infection in children

		                        			Objective To study the clinical features of complicated upper urinary tract infection in children,aiming to provide a reference for clinical diagnosis and treatment of the disease.Methods The clinical data of 68 cases with complicated upper urinary tract infection hospitalized at the Department of Urinary Surgery NO.1,Children's Hospital of Shenzhen between January 2013 and August 2015 were retrospectively analyzed.Results A total of 68 cases,in which 44 were male,24 were female,and repeated infections were found in 15 cases.Fever was the most common clinical manifestation(48 cases,70.59%),while frequent urination and odynuria were rare(9 cases,13.24%).A total of 57 strains had been cultured form the submitted specimens which were obtained from the 68 cases,including 41 strains of gram-negative bacteria,15 strains of gram-positive bacteria and 1 strain of fungus.Two different strains were cultured in 3 children.Fourteen strains of Escherichia coli had 11 extended spectrum beta lactamases (ESBLs)-positive strains.In the 11 strains of klebsiella pneumoniae,8 trains were ESBLs-positive.The drug resistant rates of gram-negative bacteria to Ampicillin and Cefuroxime sodium were both more than 90%.The sensitive rates to Piperacillin/Tazobactam was more than 90%.Thirty-one cases of complicated upper urinary tract infection were cured by administering Piperacillin/Tazobactam,while 15 cases were cured by changing Cefuroxime sodium to Piperacillin/Tazobactam according to the drug sensitivity results.Conclusions Clinical manifestations of complicated upper urinary tract infection are untypical,and fever is the most common symptom.Repeated infection is common.The gram-negative bacteria is the dominant pathogen causing the complicated upper urinary tract infection.ESBLs-positive bacteria accounts for high proportion.The drug resistance rate to penicillin and the first,second generation of the cephalosporin is high.The drug sensitive rate of piperacillin tazobactam is high,with good prognosis.
		                        		
		                        		
		                        		
		                        	
3.The expression of p53 in breast cancer and its clinical significance

		                        			Objective To investigate the expression of p53 in breast cancer and its clinical significance .Methods The clinical pathological data of 214 cases of breast cancer patients were collected ,and retrospective analyzed of p53 protein expression and its correlation with clinical pathological features .Results The positive rate of p53 was 52 .3% .The expression p53 was no correlation with age ,the size of the lesion(P> 0 .05) ,it was correlation with lymph node metastasis ,histological classification and molecular subtype(r=0 .396 ,0 .309 ,0 .167 ,P=0 .000 ,0 .000 ,0 .014) ,and also have negative correlation with the expression rate of ER ,PR (r= -0 .561 ,-0 .315 ,P=0 .000 ,0 .000) ,positive correlation with HER2 and Ki67(r=0 .374 ,0 .153 ,P=0 .000 ,0 .026) .In the group of triple negative breast cancer (TNBC) and the group of non-TNBC ,the difference was statistically significant of expression of p53 (P<0 .05) .Conclusion p53 protein has closely related with the development ,relapse and metastases of breast cancer ,the detection of p53 can be used to guide the individualized therapy and as a reference index to evaluate the prognosis of breast cancer .
		                        		
		                        		
		                        		
		                        	
4.Involvement of signal transducer and activator of transcription 3 in delayed xenograft rejection in mice-to-rats cardiac transplantation as a regulator

		                        			Objective To evaluate the roles of signal transducer and activator of transcription 3(STAT3) played in delayed xenograft rejection.Method Mice-to-rats cardiac xenograft model was established and recipients were administrated by inhibitor of phosphorylation of STAT3,5,15-Diphenylporphyrin(DPP),and immunosuppressive agent,ciclosporin A (CsA) alone or both.The survival of graft was analyzed.Immunohistochemistry and immunoblotting were utilized to detect the phosphorylated STAT3 (p-STAT3),and real-time polymerase chain reaction was used to assess STAT3-targeted genes in grafts.Result The expression of p-STAT3 was increased significantly with the prolonged survival in grafts (P < 0.05).Administration of DPP and CsA both significantly prolonged survival of the grafts (P<0.05),and decreased the expressions of STAT3-targeted genes including Bcl-xl,Bcl-2,Cyclin D1,C-myc and VEGF (P<0.05).DPP and CsA exerted the synergistic effects.Conclusion STAT3,which is persistently activated in cardiac xenografts,probably up-regulates downstream genes to promote proliferation and to inhibit apoptosis of endothelial cells,leading to the activation of endothelial cells and delayed xenograft rejection.
		                        		
		                        		
		                        		
		                        	
5.Diagnosis and treatment of glucagonoma: report of one case.

		                        			glucagonoma is a rare islet alpha-cell tumor. We report a case of glucagonoma in a 55-year-old male patient with such clinical findings of necrolytic migratory erythema, diabetes mellitus, body weight loss, and anemia. CT examination found a space-occupying lesion in the pancreas, and an elevated serum glucagon level indicate the diagnosis of glucagonoma, which was confirmed postoperatively by pathological examination of the tumor tissue. A definite diagnosis of glucagonoma relies on pathological report, and so far no standard treatment strategy has been available for this tumor. 6.Mammotome biopsy of breast calcified lesions under the real time guidance of mammography

		                        			Objective To discuss the value of mammotome biopsy of breast cakified lesions under the real time guidance of mammography.Methods The data of 42 patients with 44 mammary gland calcification(2 patients with bilateral lesions)who underwent mammotome biopsy under the real time guidance of a mammography from Jan.2011 to Feb.2012 in the First Affiliated Hospital of Chongqing Medical University were retrospective analyzed.Rapid paraffin pathology was conducted.Results Among the 44 lesions,34 were benign(27 hyperplasia,2 fibroadenoma,1 lipoma,1 cyst,3 others)and 10 were breast cancer(a benign lesion diagnosed as breast cancer by postoperative biopsy was included).Among the 42 patients,32 patients had benign lesions,accounting for 76.2% (32/42),10 patients had breast cancer,accounting for 23.8% (10/42).All lesions were completely resected during the operation.Malignant lesions were given treatment such as modified radical mastectomy,breast conservation surgergery,chemotherapy and so on.A single lesion resection took about 20 minutes.During the follow-up of 3 months,no significant event happened.Conclusions Through the qualitative diagnosis of mammotome biopsy under the real time guidance of mammography,we can get early diagnosis of breast cancer.Early diagnosis and early treatment can improve the life quality and survival time.It is more important to detect early breast cancer which was considered benign lesions clinically.Mammotome biopsy under the real time guidance of mammography has several advantages including easy to operate,accurate positioning and less complications,making it the preferred approach in managing mammary gland calcification especially suspicious malignant calcification.
		                        		
		                        		
		                        		
		                        	
7.The significance of axillary sentinel lymph node biopsy after neoadjuvant chemotherapy

		                        			Objective Comparing the influence of neoadjuvant chemotherapy on sentinel lymph node biopsy and axillary lymph nodal staging. Methods From June 2005 to June 2007 one hundred and sixty five breast cancer patients of stage Ⅰ,Ⅱa,Ⅱb and Ⅲ underwent sentinel lymph node biopsy(SLNB) using 99mTc combining methylene blue dye as tracer to help axillary lymph node dissection(AND) were retrospectively reviewed. Sixty five patients staged Ⅱb or Ⅲ were treated with three circles neoadjuvant chemotherapy prior to SLNB and AND , another one hundred patients staged ⅠorⅡa had SLNB and AND directly without neoadjuvant chemotherapy.Results Average 14.60 axillary lymph nodes were retrieved in patients had neoadjuvant chemotherapy and 14.74 lymph nodes in patients did not have neoadjuvant chemotherapy(P>0.05),a mean number of 1.46 sentinel lymph nodes in neoadjuvant chemotherapy group and 1.5 sentinel lymph nodes in non-neoadjuvant chemotherapy(P>0.05), sentinel lymph node identification rate 96.9% in neoadjuvant chemotherapy group and 97% in non-neoadjuvant chemotherapy(P>0.05), the false negative rate was 4.6% in neoadjuvant chemotherapy group and 4% in non-neoadjuvant chemotherapy group(P>0.05). Conclusions The mean number of lymph nodes,sentinel lymph nodes,SLN identification rate and false-negative rates after neoadjuvant chemotherapy are similar to those seen in patients without neoadjuvant chemotherapy.The SLNB can accurately predict lymph node status of axillary lymph node following neoadjuvant chemotherapy.
		                        		
		                        		
		                        		
		                        	
8.The Effects of PDTC plus Leflunomide and Cyclosporine on the NF-кB Signaling Pathway in Mouse-to-rat Cardiac Xenografts

		                        			In this study,the effects of pirrolidine dithiocarbamate (PDTC) plus leflunomide (Lef) models were investigated.NIH mice and Wistar rats served as donors and recipients respectively.Mouse-to-rat cardiac xenotransplantation was performed.The recipients were divided into 5 groups:group A (the control group),group B (PDTC group),group C (PDTC plus CsA group),group D (PDTC plus Lef group) and group E (PDTC plus Lef and CsA group).The expressions of IKKα/β,by immunohistochemistry and Western blot as well as electrophoretic mobility shift assay (EMSA).The median survival time of cardiac xenografts in the control group,PDTC group,PDTC plus CsA group,PDTC plus Lef group and PDTC plus Lef and CsA group was (2.17±0.41),(2.33±0.52),(4.67±1.21),(7.00±1.79) and (9.00±1.41) days respectively.The survival time of xenografts in the PDTC plus Lef and CsA group was significantly longer than that in other four groups (P<0.05 for all),that in the PDTC plus Lef group longer than that in the control group,PDTC group and PDTC plus CsA group (P<0.05 for all),that in PDTC plus CsA group longer than the control group and PDTC binding activity were notably increased in mouse-to-rat cardiac xenografts.The expressions were decreased in the control group,PDTC group,PDTC plus CsA group,PDTC plus Lef and PDTC plus Lef and CsA group in turn.It was concluded that PDTC plus Lef and CsA can significantly suppress prolonging the survival of the cardiac xenografts.
		                        		
		                        		
		                        		
		                        	
9.Advances on novel estrogen receptor GPR30

		                        			G protein-coupled receptor 30(GPR30) was a novel estrogen receptor identified as membrane associated receptor in the late 1990s.This new member of estrogen receptors was independent of the classic nuclear estrogen receptor ? and ? due to the low homology and signifi cant difference between them.It was reported that GPR30 localized endoplasmic reticulum predominantly,which was expressed in diverse cancer cells and a wide range of systems throughout the body.The rapid non-genetic response,partially at least,transcription regulation of estrogenic effects were mediated by the novel receptor via transactivation of epidermal growth factor receptor and modulation of second messengers such as cyclic adenosine monophosphate(cAMP) and Ca2+.These pathways,possibly coordinate with ER?,were involved in various physiological,physiopathological and carcinogenesis process.Theoretically,GPR30 would be a novel therapeutic target in estrogen-related diseases such as breast carcinoma.
