1.Exploration of correlation between multiple subjective and objective evaluation indicators in early stage with recovery time in combination of acupuncture and medication for treating facial neuritis

Objective To analyze the correlation between the multiple evaluation indicators in the early stage and the cure time(needed time from treatment to cure)of the patients with facial neuritis treated by ac-upuncture combined with medication,and to optimize the subjective and objective indicators enable predicting the cure time of facial neuritis in early stage.Methods All patients were treated by acupuncture and medica-tion combination.The research subjects were 64 patients with facial neuritis from the outpatient of cupuncture and moxibustion department of this hospital.The correlation between the grade of facial nerve paralysis,sur-face electromyography related data,scores of self-made symptom scoring scale,Sunnybrook Facial Grading System(SFGS)score,Facial Disability Index-Physical(FDIP)score,Facial Disability Index-Social(FDIS)score on 7 d of onset with the cure time was analyzed.Results The cure time was positively correlated with the grade of facial nerve paralysis and FDIS score on 7 d of onset(P<0.01),amd negatively correlated with the scores of self-made symptom scoring scale,FDIP score,SFGS score and the affected side to healthy ratio of CMAP amplitude of buccal temporal branch of facial nerve(P<0.01 or P<0.05);the cure time had no sig-nificant correlation with the ratio of affected side and healthy side of CMAP amplitude in zygomatic branch of facial nerve,the ratio of affected side and healthy side of CMAP latent period of temporal branch,buccal branch and zygomatic branch of facial nerve and F wave output rate(P>0.05).Conclusion In the early stage subjective indicators of the acupuncture combined with medication for treating facial neuritis,grade of facial nerve paralysis,self-made symptom scoring scale,scores of self-made symptom scoring scale,FDIP and FDIS scores and the ratio of affected side to healthy side of CMAP amplitude of the buccal branch,temporal branch of the facial nerve in sEMG in the objective indicators could be used to predict the cure time,better guide the treatment and have more effective and accurate comunication with the patients.

By various chromatographic techniques and extensive spectroscopic methods, 17 abietane diterpenoids were isolated from the dichloromethane fraction of the 95% ethanol cold-soak extracts of the seeds of Pseudolarix amabilis, namely pseudoamaol A(1), 12α-hydroxyabietic acid(2), 12-methoxy-7,13-abietadien-18-oic acid(3), 13-hydroxy-8,11,13-podocarpatrien-18-oic acid(4), 15-hydroxy-7,13-abietadien-12-on-18-oic acid(5), 8(14)-podocarpen-13-on-18-oic acid(6), holophyllin K(7), metaglyptin B(8), 7α-hydroxydehydroabietinsaure-methylester(9), 7-oxodehydroabietic acid(10), 15-hydroxy-7-oxodehydroabietinsaure-methy-lester(11), 15-methoxydidehydroabietic acid(12), 7-oxo-15-hydroxy-dehydroabietic acid(13), 15-hydroxydehydroabietic acid(14), 8,11,13-abietatriene-15,18-diol(15), 8,11,13-abietatriene-15-hydroxy-18-succinic acid(16), and 7β-hydroxydehydroabie-tic acid(17). Compound 1 was a new compound. The isolated compounds were evaluated for their antitumor activities(HepG2, SH-SY5Y, K562), and compounds 8 and 17 showed potential cytotoxic activity against K562 cells, with IC_(50) values of 26.77 and 37.35 μmol·L~(-1), respectively.

3.Association between semen collection time and semen parameters: an observational study.

The process of semen collection plays a key role in the quality of semen specimens. However, the association between semen collection time and semen quality is still unclear. In this study, ejaculates by masturbation from 746 subfertile men or healthy men who underwent semen analysis were examined. The median (interquartile range) semen collection time for all participants was 7.0 (5.0-11.0) min, and the median time taken for semen collection was lower in healthy men than that in subfertile men (6.0 min vs 7.0 min). An increase in the time required to produce semen samples was associated with poorer semen quality. Among those undergoing assisted reproductive technology (ART), the miscarriage rate was positively correlated with the semen collection time. After adjusting for confounders, the highest quartile (Q4) of collection time was negatively associated with semen volume and sperm concentration. A longer time to produce semen samples (Q3 and Q4) was negatively correlated with progressive and total sperm motility. In addition, there was a significant negative linear association between the semen collection time and the sperm morphology. Higher risks of asthenozoospermia (adjusted odds ratio [OR] = 2.06, 95% confidence interval [CI]: 1.31-3.25, P = 0.002) and teratozoospermia (adjusted OR = 1.98, 95% CI: 1.10-3.55, P = 0.02) were observed in Q3 than those in Q1. Our results indicate that a higher risk of abnormal semen parameter values was associated with an increase in time for semen collection, which may be related to male fertility through its association with semen quality.

4.Arthroscopic treatment of ankle instability combined with anteromedial impingement syndrome.

OBJECTIVE:
To investigate the surgical skills and clinical curative results of arthroscopic treatment of ankle instability combined with anteromedial impingement syndrome.
METHODS:
From February 2019 to August 2020, 13 patients with ankle instability combined with anteromedial impingement were retrospectively analyzed. There were 10 males and 3 females with age of (40.0±15.1) years old. The course of disease was(44.1±33.2) months. All patients had history of ankle sprain. MRI showed the injury of anterior talofibular ligament. All patients had anteromedial pain and pressing pain when ankle dorsiflexion. All patients were treated with ankle debridement and Brostr?m-Gould surgery under ankle arthroscopic. Postoperative results were evaluated by VAS(visual analogue scale) and AOFAS-AH(American Orthopaedic Foot and Ankle Society Ankle-Hindfoot scale, AOFAS-AH).
RESULTS:
All 13 patients completed the surgery successfully with an operative time of 60 to 90 minutes. All the surgical incisions healed by first intention, and no complications such as incision infection, skin necrosis and neurovascular injury. Follow-up time was (18.1±4.7) months. At the latest follow-up, the VAS score was 1.2±1.1, which was significantly lower than the preoperative score 4.8±1.5 (P<0.05);the AOFAS-AH score 94.2±5.1 was significantly higher than the preoperative score 65.5±11.5 (P<0.05). The AOFAS-AH score at the final follow-up ranged from 84 to 100. All patients walked with normal gait without ankle instability or impingement recurrence.
CONCLUSION
Ankle anteromedial impingement syndrome combined with ankle instability is easy to be ignored clinically. Such kind of anteromedial impingement syndrome is mostly related to osteophyte at dorsal medial talar neck. Arthroscopic treatment of ankle instability combined with anteromedial impingement syndrome has satisfactory curative effect with safety and minimal injury.

5.Early efficacy analysis of minimally invasive Chevron-Akin osteotomy for the treatment of mild to moderate hallus valgus.

OBJECTIVE:
To explore early efficacy of minimally invasive Chevron Akin(MICA) osteotomy for the treatment of mild to moderate hallux valgus.
METHODS:
From June 2019 to April 2021, a total of 26 patients (29 feet) with mild to moderate hallux valgus, including 1 male and 25 females aged from 19 to 78 years old with an average of(38.3±19.5) years old, were treated with MICA. Preoperative and postoperative hallux valgus angle(HVA), intermetatarsal angle(IMA) and shortening of the first metatarsal were observed and compared. American Orthopedic Foot and Ankle Society (AOFAS) forefoot scoring system and visual analogue scale (VAS) were applied to evaluate clinical outcome at the final follow-up, and complications were also recorded.
RESULTS:
All patients obtained followed up from 12 to 33 months with an average of(19.6±5.1) months. HVA and IMA was improved from (32.3±6.6)° and (11.7±3.2)° pre-operatively to (13.0±5.3)° and (6.1±3.2)° post-operatively, respectively, which had a significant difference (P<0.01). The average shortening of the first metatarsal was (2.7±1.1) mm. AOFAS and VAS was improved from (55.7±7.4) and (6.5±1.5) preoperatively and to (88.5±7.9) and (0.7±0.4) respectively at the final follow-up, which also had a significant difference(P<0.01). According to AOFAS score, 15 feet achieved an excellent result, 11 good and 3 moderate.
CONCLUSION
MICA osteotomy is a safe and reliable surgical technique for mild to moderate hallux valgus with advantages of minimally invasive, rapid recovery, low complication rate and an effect improvement of hallux valgus deformity.

tree model was used to analyze the risk factors related to ESRD,and the high-risk population was screened by node gain
analysis.【Results】Four important explanatory variables were screened out by the classification tree model from the candi⁃
date variables related to early renal damage,including apolipoprotein B(ApoB),gender,diabetic retinopathy,systemic blood
pressure(SBP). ApoB level was an important factor for DKD progression. For DKD patients with the chronic kidney disease
(CKD)3~4 stageswith ApoB> 1.14 mmol/L,theprobabilityofprogression toESRDfor 3 yearswas 75.0 %,and ifat the same
time with diabetic retinopathy,the probability was 79.7 %.【Conclusion】The classification tree model can analyze the risk
factors of progression to ESRD in DKD patients effectively,to identify the characteristics of high-risk populations.
7. Effect of Tianlong Tongxin Tablet on Myocardial Ischemia Reperfusion Injury and Hemorheology

Objective:To explore the protective effect of Tianlong Tongxin tablet on myocardial ischemia reperfusion injury in rats, and observe its effect on thrombosis, blood viscosity and platelet aggregation in rabbits. Method:Totally 56 Wistar rats were collected. Except for the sham operation group, all of the remaining rats were involved in the establishment of the rat myocardial ischemia reperfusion injury model. The successfully established model was divided model group, Hexinshuang group, compound Danshen tablet group and Tianlong Tongxin tablet groups (4, 2, 1 g·kg-1). Nitrotetrazolium blue (N-BT) method was used to observe the alleviation of myocardial infarction. Colorimetry was used to detect the effect of the test drug on serum superoxide dismutase (SOD) and malondialdehyde (MDA). The Chandler in vitro method was used to detect thrombosis and blood viscosity in vitro of control group, Tianlong Tongxin tablets groups (4, 2, 1 g·kg-1), compound Danshen tablets group and aspirin group. The Born turbidimetric method was used to observe the platelet aggregation levels of control group, Tianlong Tongxin tablets groups (2, 1, 0.5 g·kg-1), compound Danshen tablets group and aspirin group. Result:Compared with the sham operation group, the myocardial infarction area, serum SOD and MDA in the model group were significantly increased (P<0.05). Compared with the model group, Tianlong Tongxin tablets groups, Hexinshuang group and compound Danshen tablets group could reduce the degree of myocardial infarction, and the percentage of infarction area in the ventricle and heart decreased significantly (P<0.05, P<0.01). Compared with the control group, Tianlong Tongxin tablets groups (4, 2 g·kg-1), compound Danshen tablets group and Aspirin tablets group could significantly shorten the length of thrombosis (P<0.05, P<0.01), and significantly reduce the wet weight and dry weight of thrombosis (P<0.05, P<0.01). In addition, the whole blood viscosities under 150, 60 and 5 s-1 shear rates were significantly reduced (P<0.05, P<0.01). Compared with the control group, the platelet aggregation rates induced by collagen, adenosine bisphosphate (ADP) and arachidonic acid (AA) in rabbits were significantly reduced in Tianlong Tongxin tablet group (2, 1 g·kg-1), compound Danshen tablet group and Aspirin tablet group (P<0.05, P<0.01). Conclusion:Tianlong Tongxin tablet can protect rat myocardial ischemia reperfusion injury, inhibit platelet aggregation and thrombosis, and reduce blood viscosity.
8.Effects of sleep deprivation induced blood stasis syndrome on platelet activation in rats.

Blood stasis syndrome is the pre-state of thrombotic disease. The model of blood stasis syndrome in rats was induced by sleep deprivation to study on effects of blood stasis syndrome on platelet activation. The weight, the color of tongue and hemorheology for the blood stasis syndrome of Chinese medicine were measured after modeling. The release of platelet granules and platelet activation factors in plasma were detected by ELISA kit related indicators to provide experimental basis for platelet function evaluation and related drug effects in syndrome research. The results showed that the weight of the model group rats was significantly lower than that of the normal group (<0.01). The tongue showed a dark purple blood stasis pattern, and the R, G and B values of the tongue surface in model group were significantly lower than those of the normal group (<0.01). The hemorheological parameters including high shear, middle shear and low shear viscosity in whole blood were significantly higher than those in the normal group (<0.01). But plasma viscosity did not change significantly. The release levels of platelet α particles (GMP-140, -TG, PF4) and dense particles (ADP, 5-HT) were significantly higher than those in the normal group (<0.01). The levels of TXB₂ and 6-keto-PGF₁α in plasma were significantly higher than those in the normal group (<0.01). The ratios of TXB₂ and 6-keto-PGF₂α were also significantly higher than those in the normal group (<0.01). The levels of PAF in plasma in model group were significantly higher than those in the normal group (<0.01). It was concluded that platelet functions could be changed induced by sleep deprivationin rats with blood stasis syndrome, and there might be inflammation and endothelial cell dysfunction.
9.Battlefield extremity injuries and time-effect treatment

Objective To propose time-effect standards and standard treatment measures for battlefield extremity injuries.Methods By conducting retrospective analysis of battlefield extremity injuries in the militariy in different countries using literature retrieval and comparative analysis,time-effect standards and standard technical measures of battlefield extremity injuries were outlined.Results In wars of conventional weapons,extremity injuries are the most prevalent.Such treatment should give top priority to the timing of rescue and be implemented in conjunction with the injury classification.Conclusion Time-effect standards and standard treatment measures for battlefield extremity injuries are proposed to strengthen the time-effect treatment capacity of battlefield extremity injuries.
10.Protective effect of Yinxing Mihuan oral solution on acute myocardial ischemia injury in rats

This study was designed to investigate the effect of Yinxing Mihuan oral solution on rats with myocardial ischemia injury. Left anterior descending (LAD) coronary artery was occluded in rats to establish the model. Yinxing Mihuan oral solution was given by intragastric administration daily for one week at dosage of 309 and 618 mg·kg-1. Cardiac ultrasound function, pathologic change and serum myocardial enzymes were determined to evaluate the effect of Yinxing Mihuan oral solution. The heart function was significantly reduced in the model group compared with sham operation group, and the pathologic damage was clear. The changes were significantly improved by Diltiazem hydrochloride tablets in heart function and ejection fraction (P < 0.01). The ejection fraction and stroke volume was improved by Yinxing Mihuan oral solution (618 mg·kg-1) (P < 0.05, P < 0.01). In addition, Yinxing Mihuan oral solution decreased the myocardial damage and inhibited inflammatory reaction, and inhibited platelet activation factor. Yinxing Mihuan oral solution can protect against myocardial ischemia injury, inflammation and platelet activation in the rat model.
