1.Construction of an infectious disease risk assessment system for childcare institutions in Shanghai

		                        			ObjectiveTo explore the construction of a risk assessment indicator system for common infectious diseases in Shanghai’s childcare institutions, and to provide a reference standard for the prevention and control of infectious diseases, staff training and system construction in childcare institutions. MethodsBy combining the Delphi method with the literature review and expert consultation, the hierarchical dimensions and items at all levels of the risk assessment indicator system for common infectious diseases in Shanghai’s childcare institutions were constructed, and the weighting coefficients were determined by analytic hierarchy process. ResultsA total of 14 experts from the field of childcare institutions, infectious disease control, child healthcare and health supervision participated in the Delphi consultation. The system consisted of four core dimensions: organizational management, team building, hardware equipment, and infectious disease surveillance and disposal, with the weighting coefficients of 0.285 9, 0.261 6, 0.204 3 and 0.248 2, respectively. The evaluation indicator system consisted of 4 primary indicators, 15 secondary indicators and 45 tertiary items. The positivity coefficients of the two rounds of Delphi consultation were 0.93 and 1.00, the authority coefficients were both 0.81, and the Kendall’s coefficient of concordance were 0.44 and 0.49, respectively (P<0.01). ConclusionThe high expert engagement and coordination indicate that organizational management and team building remain the critical priorities for infectious disease prevention and control in Shanghai’s childcare institutions. It is recommended to strengthen financial investment, improve institutional mechanisms, and enhance personnel reserves and capacity building for healthcare teachers, thereby systematically upgrading the infectious disease control capabilities of childcare institutions. 
		                        		
		                        		
		                        		
		                        	
2.Research on establishing pioneer branches with distinctive features basing on the inheritance and devel-opment of traditional Chinese medicine cultural essence

		                        			This article aims to explore the significant influence of the Communist Party of China(CPC)on traditional Chinese medicine(TCM).The article reviews the historical development of TCM in modern times and the impact of the CPC,combined with the achievements of the TCM department of comprehensive hospitals,to explore the approach of building pioneer branches with distinctive TCM national cultural characteristics,and to consider how to inherit and innovate the development of TCM cultural essence.The article emphasizes the significant contribution of the CPC to the modernization of TCM and the cause of human health.Through this research,it will contribute to a better understanding of the important role of the CPC in the protec-tion and inheritance of TCM culture and explore cultivation programs for party branches with TCM characteristics to better inherit,innovate and develop traditional Chinese medicine.
		                        		
		                        		
		                        		
		                        	
3.Establishment and application of drug use evaluation criteria for roxadustat

		                        			Objective To establish the drug use evaluation(DUE)standards of roxadustat,and to evaluate its clinical application to promote its rational use.Methods Based on the drug labels,referring to relevant guidelines and expert consensus,the DUE criteria for roxadustat were established through the Delphi method,including items such as drug indications,drug use process and the results of medication.A retrospective study was conducted to evaluate the rationality of cases which included inpatients who used roxadustat for the first time from January 1,2020 to December 31,2022,with a medication period of more than one month in Fujian Provincial Hospital.Results A total of 175 medical records were included,of which 14 records were fully met the clinical application evaluation criteria,and the medication reasonable rate was 8.0%.The unreasonable situation were mainly manifested in the inappropriate clinical outcomes(92.0%),the inappropriate use dosages(52.6%),inappropriate monitoring of adverse reactions(34.9%),inappropriate timing of administration(13.1%),drug interactions(8.7%),inappropriate drug conversion(5.7%),inappropriate efficacy monitoring(4.0%).Conclusion The established DUE standards for roxadustat are scientific,practical and feasible,and the evaluation results show a rate of irrationality in the use of roxadustat in the hospital,and it is necessary to standardize the use of roxadustat in terms of dosage,adverse reaction monitoring,and timing of administration.
		                        		
		                        		
		                        		
		                        	
4.Iodine nutritional status and influencing factors of pregnant women in Hangzhou City

		                        			Objective:To investigate the iodine nutritional status of pregnant women in Hangzhou City and analyze its influencing factors, in order to provide a basis for guiding pregnant women in Hangzhou City to supplement iodine scientifically.Methods:A stratified random sampling method was used to conduct a questionnaire survey on 1 400 pregnant women in Hangzhou City from March to October 2018. Random urine samples and household salt samples from pregnant women were collected, and the levels of urine iodine and salt iodine were measured using arsenic cerium catalytic spectrophotometry and direct titration, respectively. The iodine nutritional status of pregnant women with different basic characteristics and dietary levels were analyzed and compared (Kruskal-Wallis test), and the main influencing factors affecting the urinary iodine concentration level of pregnant women were identified (the ordinal multiple classification logistic regression analysis).Results:The median urinary iodine of pregnant women was 135.00 μg/L, and the qualified iodized salt consumption rate of pregnant women was 83.36% (1 167/1 400). In terms of basic characteristics, there were statistically significant differences in the distribution level of urine iodine among pregnant women with different pregnancy frequency, delivery frequency, and natural abortion frequency ( P < 0.05). In terms of diet, the frequency of consuming milk, yogurt, meat, and whether pregnant women consumed cabbage and cauliflower showed statistically significant differences in urinary iodine distribution levels ( P < 0.05). Ordinal multiple classification logistic regression analysis showed that the urine iodine levels of pregnant women who were pregnant twice were higher than those who were pregnant ≥3 times ( OR = 1.64, P = 0.003). Pregnant women who never or occasionally consumed yogurt had lower urine iodine levels than or equal to those who consumed 2 bottles of yogurt per day ( OR = 0.53, P = 0.044). Pregnant women who never or occasionally consumed meat and those who consumed meat once a week had higher urinary iodine levels than that who consumed meat ≥2 times per week ( OR = 1.40, 1.47, P < 0.05). Conclusions:The overall iodine nutrition of pregnant women in Hangzhou City is at an deficiency level. Pregnancy experience and dietary level are influencing factors on iodine nutrition of pregnant women. It is necessary to carry out in-depth health education for pregnant women, improve the dietary structure during pregnancy, and improve the abnormal iodine nutrition of pregnant women.
		                        		
		                        		
		                        		
		                        	
5.The epidemiology and risk factors of delayed diagnosis in newly reported HIV/AIDS cases in Xuhui District, Shanghai, 2018‒2022

		                        			ObjectiveTo investigate the trends and significant determinants of delayed HIV diagnosis (DHD) among newly reported HIV/AIDS cases in Xuhui District, Shanghai between 2018 and 2022. MethodsIn the newly reported HIV/AIDS cases, patients died within one year without accident, HIV/AIDS cases with CD4 cell count <200 cells·μL-1, and AIDS cases with a CD4 cell count between 200 to 499 cells·μL-1 were defined as delayed diagnosis. Univariate and multivariate logistic analysis were employed to explore the influencing factors of DHD. ResultsAmong the 862 newly reported HIV/AIDS cases, The DHD rate was 39.79% without statistically significant difference by year(χ2=4.508, P=0.342). Patients with CD4 cell count <200 cells·μL-1 contributed the largest proportion of DHD. During 2018‒2022, the DHD rate declined among HIV/AIDS patients who were younger than 35 years old or 45‒65 years old, never married, original diagnosis from tertiary specialized hospitals. Patients who were 65 years or older, married or divorced, with heterosexual transmitted HIV/AIDS, and original diagnosis from other types of testing and tertiary metropolitan hospital, had sustainably higher DHD rates. The number of HIV screening and diagnosed from voluntary counseling and testing (VCT) decreased during the COVID-19 epidemic, while the DHD rate increased sharply. Multivariate logistic regression analysis suggested the DHD rates were higher among older age, other types of testing(OR=3.805, 95%CI: 2.260‒6.406)and pre-operative testing(OR=2.411, 95%CI: 1.424‒4.081). Patients who received CD4 test in 15 days had a higher DHD rate compared to the cases received CD4 test exceeding 90 days (OR=0.336, 95%CI: 0.216‒0.522). ConclusionThere is no significant decrease of delayed HIV diagnosis rate in Xuhui District in recent years, and the number of HIV tests has decreased in 2022. Monitoring of newly reported HIV/AIDS should be conducted continuously. Expansion of HIV antibody screening should be conducted in non-infectious departments and inpatient departments in healthcare institutions, particularly metropolitan hospitals. Assistance should be provided to clinicians and elderly patients for improving their ability to recognize and perceive the risk of HIV/AIDS, in order to enhance early diagnosis and subsequent treatment. 
		                        		
		                        		
		                        		
		                        	
6.Effect and mechanism of matrine on steatosis Chang Liver cells induced by oleic acid

		                        			OBJECTIVE To investigate the effects of matrine (MT) on steatosis Chang Liver cell model induced by oleic acid (OA) and its possible mechanism. METHODS Chang Liver cells were divided into blank group, model group and MT low-dose, medium-dose group and high-dose groups (0.1, 0.5, 1.0 mmol/L). Except for blank group, the other groups were treated with 1.0 mmol/L OA for 24 h to establish steatosis model, and MT groups were given corresponding concentrations of drugs for 24 h. The activities of steatosis Chang Liver cells were observed; the morphologies of intracellular lipid droplets were observed and lipid content was also determined. The contents of liver function indexes [alanine transaminase (ALT), aspartate transaminase (AST), total bilirubin (TBIL), alkaline phosphatase (ALP)], as well as mRNA and protein expressions of farnesoid X receptor (FXR), cytochrome P450 7A1 (CYP7A1) and fibroblast growth factor 19 (FGF19) were all detected. RESULTS OA and MT had no significant effect on the activity of Chang Liver cells. After OA treatment, orange lipid droplets formed in cytoplasm; compared with blank group, relative lipid content and the levels of liver function indexes were increased significantly, while the mRNA and protein expressions of FXR, CYP7A1 and FGF19 were down-regulated significantly (P<0.05). After treatment of low, medium and high concentrations of MT, above indexes were all reversed significantly (P<0.05). CONCLUSIONS MT could significantly improve the lipid content and liver function indexes of steatosis Chang Liver cells induced by OA though regulating FXR/CYP7A1/ FGF19 signaling pathway.
		                        		
		                        		
		                        		
		                        	
7.Research and Design of Calibration Device for Medical Test Lung.

		                        			In order to solve the problems of quality control and traceability of medical test lung for meeting the calibration conditions of JJF 1234-2018 Calibration Specification for Ventilators, the calibration device and method are researched for compliance and airway resistance of medical test lung in this paper. A calibration device for medical test lung is designed using constant volume active piston technology to simulate human breathing. Through comparison experiment, the deviation between this device and the similar foreign device can be found. The deviation is lower than 0.4% for lung compliance and lower than 0.7% for airway resistance. The calibration of lung compliance and airway resistance can be completed by this device. This device has a clear and complete traceability path to ensure quality control from the source. The calibration of ventilator is improved. 8.Efficacy and safety of intravitreal ranibizumab alone and combined laser therapy for the treatment of DME: a meta-analysis

		                        			Objective:To evaluate the efficacy and safety of intravitreal ranibizumab combined with laser (IVR+ Laser) and the intravitreal ranibizumab (IVR) monotherapy for the treatment of diabetic macular edema (DME).Methods:A meta-analysis was conducted on randomized controlled trial (RCT) literature related to IVR+ Laser therapy and IVR alone for DME.Databases including Cochrane Library, PubMed, EMbase, Web of Science, SinoMed, CNKI, VIP and WanFang Data were searched from their inception to April 2022.Literature screening, data extraction, quality evaluation and cross-checking were conducted independently by two researchers according to inclusion and exclusion criteria.Then a meta-analysis was conducted using RevMan 5.4.1 software.The two therapies were compared in terms of best corrected visual acuity (BCVA), central macular thickness (CMT), mean number of injections and adverse events.Results:Twelve RCTs involving 1 695 eyes were included in the study.Meta-analysis showed that at the end of follow-up, IVR+ Laser demonstrated better improvement in BCVA and CMT than IVR alone, and there were significant differences in the changes in BCVA and CMT between the two groups (weighted mean difference[WMD]=-0.66, 95% confidence interval[ CI]: -1.11--0.21, P<0.01; WMD=-5.05, 95% CI: -9.21--0.89, P=0.02).IVR+ Laser required significantly fewer injections than IVR alone (WMD=-1.16, 95% CI: -2.07--0.25, P=0.01).There were no significant differences in the adverse events incidence between the two therapies (all at P>0.05). 		                        			The acute combination fractures of the atlas and axis in adults have a higher rate of neurological injury and early death compared with atlas or axial fractures alone. Currently, the diagnosis and treatment choices of acute combination fractures of the atlas and axis in adults are controversial because of the lack of standards for implementation. Non-operative treatments have a high incidence of bone nonunion and complications, while surgeries may easily lead to the injury of the vertebral artery, spinal cord and nerve root. At present, there are no evidence-based Chinese guidelines for the diagnosis and treatment of acute combination fractures of the atlas and axis in adults. To provide orthopedic surgeons with the most up-to-date and effective information in treating acute combination fractures of the atlas and axis in adults, the Spinal Trauma Group of Orthopedic Branch of Chinese Medical Doctor Association organized experts in the field of spinal trauma to develop the Evidence-based guideline for clinical diagnosis and treatment of acute combination fractures of the atlas and axis in adults ( version 2023) by referring to the "Management of acute combination fractures of the atlas and axis in adults" published by American Association of Neurological Surgeons (AANS)/Congress of Neurological Surgeons (CNS) in 2013 and the relevant Chinese and English literatures. Ten recommendations were made concerning the radiological diagnosis, stability judgment, treatment rules, treatment options and complications based on medical evidence, aiming to provide a reference for the diagnosis and treatment of acute combination fractures of the atlas and axis in adults.
		                        		
		                        		
		                        		
		                        	
10.Investigation on nutrition and health knowledge among adult residents in Hangzhou City

		                        			Objective:
		                        			To investigate the awareness of nutrition and health knowledge among adult residents in Hangzhou City, so as to provide insights into improving human nutrition and health.
		                        		
		                        			Methods:
		                        			The permanent residents at ages of 18 to 64 years were sampled using a multi-stage stratified random sampling method from 13 counties (districts) of Hangzhou City from June to July, 2021. Participants' demographics and awareness of nutrition and health knowledge were collected using the Questionnaire on Nutrition and Health Knowledge among Chinese Adult Residents, and the awareness of nutrition and health knowledge was analyzed among participants with different demographic features.
		                        		
		                        			Results:
		                        			A total of 5 106 questionnaires were allocated, and 4 920 valid questionnaires were recovered, with an effective recovery rate of 96.36%. The respondents included 2 575 men (52.34%) and 2 345 women (47.66%), and had a mean age of (39.89±12.85) years. The median score of nutrition and health knowledge was 67.50 (interquartile range, 16.50) points, and the awareness of nutrition and health knowledge was 24.49%, while the awareness of dietary recommendation, food characteristics, nutrition and disease, food choice and food safety was 15.89%, 16.77%, 54.17%, 26.36% and 60.69%, respectively. The awareness of nutrition and health knowledge was significantly lower among men than among women (21.01% vs. 28.32%, P<0.05), and among urban residents than among rural residents (22.94% vs. 27.34%, P<0.05). There were significant differences in the awareness of nutrition and health knowledge among residents in terms of age, occupation and educational level (P<0.05). 
		                        		
		                        			Conclusions
		                        			Age, gender, region, occupation and educational level may affect the awareness of nutrition and health knowledge among adult residents in Hangzhou City. Health education about nutrition and health knowledge requires to be reinforced with adaptations to characteristics of residents.
