1.Value of brain 18F-FDG PET/CT in the preoperative localization of epileptogenic foci in refractory epilepsy patients with negative MRI

Objective:To evaluate the value of 18F-FDG PET/CT for preoperative localization of epileptogenic foci in refractory epilepsy patients with negative MRI. Methods:Clinical data (550 lobes) of 55 epilepsy patients (38 males, 17 females, age (20.0±8.1) years) with negative MRI who underwent preoperative 18F-FDG PET/CT-MRI between January 2014 and June 2020 at the First Affiliated Hospital of Jinan University were retrospectively analyzed. The sensitivity, specificity, accuracy, positive predictive value (PPV) and negative predictive value (NPV) of 18F-FDG PET/CT, video electroencephalogram (VEEG), PET/CT+ VEEG and PET/CT-VEEG for localizing epileptogenic foci were calculated using stereoelectroencephalography (SEEG) and the outcomes of at least 1 year of postoperative follow-up as reference standards. χ2 test was used to compare the efficiencies of different examination modalities for unilobar, multilobar and all patients. Results:The correct lateralization rate of epileptogenic foci was 92.6%(25/27) using PET/CT. The sensitivity, specificity, accuracy, PPV and NPV of PET/CT for localization of epileptogenic foci were 65.1%(54/83), 77.9%(364/467), 76.0%(418/550), 34.4%(54/157) and 92.6%(364/393), respectively. The sensitivities of PET/CT-VEEG for localization of epileptogenic foci in all patients and patients with multilobar epilepsy were higher than those of VEEG alone (75.9%(63/83) vs 45.8%(38/83), 68.6%(35/51) vs 31.4%(16/51); χ2 values: 15.80, 14.16, both P<0.001). The specificities of PET/CT+ VEEG for localization of epileptogenic foci in all patients and patients with unilobar epilepsy were higher than those of VEEG alone (97.6%(456/467) vs 94.6%(442/467), 97.9%(282/288) vs 94.1%(271/288); χ2 values: 5.66, 5.48; P values: 0.017, 0.019). The sensitivity of PET/CT-VEEG (PET/CT and VEEG concordance) for localization of epileptogenic foci was higher than that of PET/CT+ VEEG (PET/CT and VEEG discordance) (8/9 vs 28.4%(21/74); χ2=10.40, P=0.001), and its specificity and accuracy were higher than those of PET/CT-VEEG (PET/CT and VEEG discordance) (93.4%(57/61) vs 71.7%(291/406), 92.9%(65/70) vs 72.1%(346/480); χ2 values: 13.23, 13.96; both P<0.001). Conclusions:18F-FDG PET/CT can localize and lateralize epileptogenic foci in patients with negative MRI. The combination of 18F-FDG PET/CT and VEEG improves the sensitivity, specificity, and accuracy for epileptogenic foci detection. 18F-FDG PET/CT is more accurate in detecting epileptogenic foci when it is concordant with VEEG.
2.Comprehensive evaluation of research outputs from the Capital′s Funds for Health Improvement and Research

Objective:The Capital′s Funds for Health Improvement and Research (referred to as ″CFH″), established to address clinical medical issues, have been operating for over a decade. This study aims to comprehensively evaluate the achievements of this fund and provide empirical support and recommendations for optimizing the operational model of the clinical research fund.Methods:An online questionnaire was used to investigate the following aspects of CFH: subject area, the person in charge, derived projects, theses, patents, transformation of results, and popularization and promotion.Results:A total of 745 projects were collected through the online survey, and after rechecking for outliers, 720 completed projects were ultimately included. The top three disciplinary domains, in terms of the number of completed projects, were oncology, cardiovascular diseases, and neurological disorders. The age distribution of project leaders exhibited a left-skewed pattern, with a median age of 45 years. 319 (44%) received support from other related projects subsequently. 95% of the projects resulted in paper publication. 211 (29%) projects applied for patents and copyrights, with 141 projects being granted patent authorization. 78 (11%) projects successfully achieved technology transfer, with transfer and licensing being the most common modes. 156 (22%) projects disseminated their outputs, often targeting secondary hospitals and community health institutions.Conclusions:CFH aligns with local needs, and the outputs are substantial. In the future, consideration can be given to establishing an evaluation mechanism, increasing efforts to promote high-quality outputs, strengthening the organizational model set by the project guidelines, and further improving the rolling support mechanism.
3.Quantification of the iduronate-2-sulfatase activity in prenatal diagnosis of mucopolysaccharidosis type Ⅱ

Objective:To evaluate the activity of iduronate-2-sulfatase (IDS) in fetal villi and peripheral blood plasma of pregnant women at high risk of mucopolysaccharidosis type Ⅱ (MPS Ⅱ), and to discuss the application of gene analysis in prenatal diagnosis of MPS Ⅱ.Methods:The enzymatic testing and gene analysis results of 23 pregnant women at high risk of MPS Ⅱ, who underwent prenatal diagnosis in Guangzhou Women and Children′s Medical Center from February 2013 to December 2020, were analyzed retrospectively.The IDS activity in fetal villi (30 cases) and plasma (28 cases) was detected by artificial substrate fluorescence.The IDS activity in fetal villi (28 cases) and plasma (34 cases) of normal pregnant women was taken as control.Meanwhile, the fetal villi of both pregnant women at high risk of MPS Ⅱ and normal pregnant women were also analyzed by gene testing and for fetal sex identification.Data were compared between groups by the independent samples t test. Results:The normal reference values of the IDS activity in fetal villi and plasma of normal pregnant women were(71.2±23.4) nmol/(mg·4 h) and (611.1±114.5) nmol/(mL·4 h), respectively.Among the 30 cases of high-risk fetal villi, the IDS activity in fetal villi of 8 affected male fetuses was (1.7±0.3) nmol/(mg·4 h), which was significantly lower than that of 11 unaffected male fetuses (83.2±6.3) nmol/(mg·4 h) and that of 9 non-carrier female fetuses (80.0±7.5) nmol/(mg·4 h) ( t=10.8, 8.8; all P<0.01). Meanwhile, the IDS activity was measured in the maternal peripheral plasma of 28 pregnant women at high risk of MPS Ⅱ.Among them, the IDS activity in 8 affected male fetuses was(225.4±20.5) nmol/(mL·4 h), which was significantly lower than that in non-affected male fetuses[(451.0±15.1) nmol/(mL·4 h)] and that in non-carrier female fetuses[(467.7±45.3)nmol/(mL·4 h)]. Eight known pathogenic mutations were found in 30 cases at high risk of MPS Ⅱ of fetal villi, and the mutation types were c. 1048A>C, c.212G>A, c.514C>T, c.257C>T, c.425C>T, and c. 998C>T.Of the 8 cases, 6 affected male fetuses had significantly reduced IDS activities, and the other 2 female carriers had normal IDS enzyme activities. Conclusions:The IDS activity in fetal villi and peripheral plasma of pregnant woman is consistent with the gene analysis results.The IDS activity has an important reference value for the prenatal diagnosis of MPS Ⅱ in the first trimester.When no genetic mutations are found in the probands or the pathogenicity of the new mutation remains unclear, the IDS activity in fetal villi can be detected separately for the prenatal diagnosis of MPS Ⅱ.
4.Epidemiological characteristics of new pneumoconiosis in different industries in Tianjin

Objective:To analyze the distribution characteristics of new pneumoconiosis in different industries in Tianjin from 2009 to 2018, in order to provide a theoretical basis for the prevention and treatment of pneumoconiosis.Methods:In November 2019, the data of new pneumoconiosis cases in Tianjin from 2009 to 2018 were collected and classified according to difference industries. The epidemiological characteristics of new pneumoconiosis in different industries were analyzed based on time, type of pneumoconiosis, type of work, age, and working age.Results:A total of 4657 new cases of pneumoconiosis were reported in Tianjin from 2009 to 2018, 4640 cases (99.63%) in the first stage, 13 cases (0.28%) in the second stage, and 4 case (0.09%) in the third stage. The number of new cases increased with time and then decreased. Among them, there were 3482 males (74.77%) and 1175 females (25.23%) . The proportion of women with new pneumoconiosis in the non-metallic mineral products industry was the highest, and the differences were statistically significant ( P<0.05) . There are differences in the distribution of new pneumoconiosis species in different industries (χ 2=4920.11, P<0.05) . Silicosis is mainly distributed in non-metallic mineral products industry, metal smelting and rolling processing industry, petroleum processing coking and nuclear fuel processing industries. Foundry worker′s pneumoconiosis is mainly distributed in the general equipment manufacturing industry and special equipment manufacturing industry, and cement pneumoconiosis is mainly distributed in the non-metallic mineral products industry. The types of work are mainly distributed in smelting, casting, raw materials and forming workers, and the sum of the three accounts for 36.46% (1689/4657) . The M ( P25, P75) of diagnosis age was 56.0 (50.0, 63.0) years, and the M ( P25, P75) of working age was 19.0 (16.0, 26.0) years. The working age of new pneumoconiosis patients in the petroleum processing, coking and nuclear fuel processing industries is 19.0 (16.0, 26.0) years, which is shorter than that of other industries ( P<0.05) ; The diagnostic age of the general equipment manufacturing industry is 54.0 (49.0, 59.0) years, which is less than that of other industries ( P<0.05) . New pneumoconiosis is mainly distributed in large and medium-sized enterprises. New cases of pneumoconiosis in non-metallic mineral products industry, metal smelting, calendar processing industry and general equipment manufacturing industry are mainly distributed in large and medium-sized enterprises. New cases of pneumoconiosis in the metal products industry, special equipment manufacturing industry and petroleum processing, coking and nuclear fuel processing industries are mainly distributed in large enterprises. Conclusion:The work of preventing and controlling pneumoconiosis in Tianjin has a long way to go. We should implement targeted measures according to the characteristics of pneumoconiosis industry, strengthen the supervision of hazard industries, and effectively control the occurrence of pneumoconiosis.

6.Stability evaluation of different internal fixation methods for zygomaticomaxillary complex fractures by finite element biomechanical analysis

Objective:To analyze the stability of different internal fixation methods for zygomaticomaxillary complex fracture using finite element biomechanical analysis and to provide a quantitative basis for the option of optimal internal fixation methods.Methods:One patient with zygomaticomaxillary complex ZMC fracture (Zingg B type) was enrolled from the Second Hospital of Dalian Medical University in October 2016.The zygomaticofrontal suture and the inferior orbital rim were fixed during the surgery, and the postoperative function was well recovered with no mouth opening restriction, diplopia or implant displacement, and had a symmetrical facial appearance.The preoperative orbital CT images were collected.The normal craniofacial bones finite element model (FEM/intact) was reconstructed through Mimics, Geomagic, Solidworks and Abaqus softwares based on the non-fractured side and verified.Based on the verified model, the segmentation and assembling was performed according to the fracture location, and the internal fixation models were established according to the methods of surgery, including fixed zygomaticofrontal suture model (FEM/ZFS), fixed inferior orbital rim model(FEM/IOR), fixed zygomaticofrontal suture and inferior orbital rim model (FEM/ZFS+ IOR), fixed ZFS and IOR and zygomaticomaxillary suture model (FEM/ZFS+ IOR+ ZMS). The masseter muscle strength was applied to the model.The stress and displacement were analyzed and the rotation angle of zygoma was calculated to compare the stability of different operative methods.The postoperative model (FEM/post) was established according to the actual operative method and was compared with FEM/ZFS+ IOR.This study protocol was approved by the Ethic Committee of The Second Hospital of Dalian Medical University (2020-33), and written informed consent was obtained from the subject before entering the study.Results:The established FEM/intact had a realistic appearance and good geometric similarity, and the validity and accuracy of model was verified.In the FEM/ZFS+ IOR and FEM/ZFS+ IOR+ ZMS, the maximal stress of the titanium plate was 396 MPa and 426 MPa, respectively, which was lower than the yield strength 483 MPa of pure titanium, and the maximal displacement of the fracture line was 0.10 mm and 0.06 mm, respectively, which was both≤0.1 mm, and the rotation angle of zygoma was both<2°.In the FEM/ZFS and FEM/IOR, the maximal stress of the titanium plate was 730 MPa and 501 MPa, respectively, which was higher than the yield strength of pure titanium; the maximal displacement of fracture line was 0.27 mm and 0.15 mm, respectively, which was >0.1 mm, and the rotation angle of zygoma was <2°.The results of FEM/post were consistent with those of FEM/ZFS+ IOR.Conclusions:The finite element analysis can perform digital analysis and evaluation of the stability of different internal fixation methods before surgery, which is available for the selecting of the optimal fixation methods.Finite element analysis can provide an objective quantitative basis for the precise treatment of zygomaticomaxillary complex fractures.
7.Study of No Observed Adverse Effect Level of Nickel and Its Preliminary Evaluation Biocompatibility.

The purpose of this study was to investigate the NOAEL of the nickel ion and provide with basic data for the biological evaluation of those medical devices containing nickel. Five groups SD rats were repeatedly exposed during 14 d respectively to nickel at first stage doses of 4.9, 3.7, 2.5 mg/(kg.d), and the second stage doses of 1.2, 0.25 mg/(kg.d) by the intravenous route. The results showed that the NOAEL of nickel ion is 0.25 mg/(kg.d) for SD rats, and the result was verified by subchronic systemic toxicity test of nickel alloy. The threshold of toxicological concern (TTC) of nickel is 150 μg/d (based on application of 100-fold uncertainty factor and a body weight of 60 kg)deduced by these data.

8.Effects of occupational stress and related factors on insomnia occurrence of workers in chip manufacturing industry

Objective:To investigate the prevenlence of insomnia on workers and related influcecing factors in chip manufacturing industry.Methods:By using cluster sampling method, 2 251 workers in chip manufacturing industry were selected as study subjects. The simple Job Demand-Control model Questionnnaire (JDC) , Effort-Reword Imbalance (ERI) Questionnnaire and Insomnia Symptoms Scale were used to evaluate JDC occupational stress, ERI occupational stress and insomnia symptoms respectively.Results:22.6% (509/2251) workers were found high level of insomnia. The risk factors of insomnia symptoms were high score of effort and overcommitment ( P< 0.05) . The risk of insomnia symptoms in high effort was 1.689 times that of low score (95% CI: 1.334-2.138) . The risk of insomnia symptoms in high overcommitment was 1.835 times that of low score (95% CI: 1.461-2.305) . The protective factors for insomnia symptoms were women, monthly income of more than 3000 yuan, college dregree or above, high work control and high reward ( P<0.05) . Conclusion:The high scores of effort and overcommitment are the risk factors for insomnia symptoms of employees in manufacture electronic devices. Enterprises should take comprehensive measures to pay attention to the occurrence of insomnia symptoms among employees with high score of effort and overcommitment.
9.Effects of Occupational stress and related factors on life satisfaction level of workers in electronic manufacturing industry

Objective:To investigate the influence of life satisfaction level and occupational stress related influencing factors on workers in chip manufacturing industry in order to provide guidance for psychological intervention.Methods:In June to December 2016, by using sampling method, 2251workers in electronic manufacturing industry were selected as study subjects. A self-made questionnaire was used to investigate the demographic and occupational data of workers, Job Demand-Control Model (JDC) 、Effort-reword Imbalance Model (ERI) 、Well-being Index (WHO-5) were used to investigate and analyze the status of life satisfaction respctively. To analyze the influence of occupational stress factors on life satisfaction.Results:There were 773 workers with low life satifaction, the detect rate was 34.5% (773/2251) . Single factor analysis showed that migrant workers, type of employment contract, week working time (h) , working system, contact risk factors, chronic disease and all occupational stress related factors were the influcecing factors of life satisfaction among workers ( P<0.05) . The results of multiple factor logistic regression analysis showed that contact risk factors, high effort in work and high effort inner work were the risk factors of low life satisfaction in electronic device manufacturers ( OR=1.544, 1.376, 1.788, P<0.05) . And non-migrant workers, high return in work, high autonomy and high social support were the protective factors of low life satisfaction in electronic device workers ( OR=0.606, 0.390, 0.615, 0.622, P<0.05) . Conclusion:Enterprises should strengthen the protection education of hazard factors for employees, those who contact hazard factors and high effort in work, improve worker's returns and autonomy, improve their social support, improve workers' life satisfaction, and build healthy enterprises.
