1.Long-term hypomethylating agents in patients with myelodysplastic syndromes: a multi-center retrospective study

		                        			Objective:To evaluate the efficacy and safety of hypomethylating agents (HMA) in patients with myelodysplastic syndromes (MDS) .Methods:A total of 409 MDS patients from 45 hospitals in Zhejiang province who received at least four consecutive cycles of HMA monotherapy as initial therapy were enrolled to evaluate the efficacy and safety of HMA. Mann-Whitney U or Chi-square tests were used to compare the differences in the clinical data. Logistic regression and Cox regression were used to analyze the factors affecting efficacy and survival. Kaplan-Meier was used for survival analysis. Results:Patients received HMA treatment for a median of 6 cycles (range, 4-25 cycles) . The complete remission (CR) rate was 33.98% and the overall response rate (ORR) was 77.02%. Multivariate analysis revealed that complex karyotype ( P=0.02, OR=0.39, 95% CI 0.18-0.84) was an independent favorable factor for CR rate. TP53 mutation ( P=0.02, OR=0.22, 95% CI 0.06-0.77) was a predictive factor for a higher ORR. The median OS for the HMA-treated patients was 25.67 (95% CI 21.14-30.19) months. HMA response ( P=0.036, HR=0.47, 95% CI 0.23-0.95) was an independent favorable prognostic factor, whereas complex karyotype ( P=0.024, HR=2.14, 95% CI 1.10-4.15) , leukemia transformation ( P<0.001, HR=2.839, 95% CI 1.64-4.92) , and TP53 mutation ( P=0.012, HR=2.19, 95% CI 1.19-4.07) were independent adverse prognostic factors. There was no significant difference in efficacy and survival between the reduced and standard doses of HMA. The CR rate and ORR of MDS patients treated with decitabine and azacitidine were not significantly different. The median OS of patients treated with decitabine was longer compared with that of patients treated with azacitidine (29.53 months vs 20.17 months, P=0.007) . The incidence of bone marrow suppression and pneumonia in the decitabine group was higher compared with that in the azacitidine group. Conclusion:Continuous and regular use of appropriate doses of hypomethylating agents may benefit MDS patients to the greatest extent if it is tolerated.
		                        		
		                        		
		                        		
		                        	
2.Clinical characteristics of primary pulmonary lymphoepithelioma-like carcinoma: analysis of 31 cases

		                        			Objective:To improve the understanding of primary pulmonary lymphoepithelioma-like carcinoma (PPLELC).Methods:A retrospective case series study was conducted. The clinical data of 31 patients with PPLELC who were admitted to the First Affiliated Hospital of Guangxi Medical University from January 2012 to June 2023 were retrospectively analyzed, and their clinical features were summarized. The correlations of organ metastasis, tumor stage, serum tumor markers, lactate dehydrogenase, and albumin with survival time were analyzed.Results:Among the 31 patients, 13 (41.9%) were male and 18 (58.1%) were female, aged (50±9) years old, with no smoking history in 24 cases (77.4%). The common clinical manifestations were cough(24 cases, 77.4%) and sputum (19 cases, 61.3%), and 7 patients (22.6%) were detected by physical examination; 24 cases (77.4%) had elevated levels of serum tumor markers, and the rest of the 7 cases (22.6%) had normal levels of various tumor markers. All of the patients had a single lesion, with a predominance of the right middle lung (8 cases, 25.8%), and 23 cases (74.2%) had lymph node metastasis. Immunohistochemical detection showed that the positive rate of CK was 67.7% (21/31), and the positive rates of squamous cell carcinoma markers CK5/6, p63 and p40 were 90.3% (28/31), 80.6% (25/31) and 77.4% (24/31), respectively. The positive rate of EBER in situ hybridization detection was 85.2% (23/27). Genetic testing showed 6 cases had epidermal growth factor receptor (EGFR) mutation. The median survival time [ M ( Q1, Q3)] of the groups without lymph node metastasis and with lymph node metastasis was 33.0 months (7.3 months, 9.3 months) and 19.0 months (7.0 months, 27.0 months), and the difference was statistically significant ( P < 0.001). The median survival time of patients with stage Ⅰ-Ⅱ and with stage Ⅲ-Ⅳ was 20.0 months (12.5 months, 42.0 months) and 18.5 months (6.5 months, 38.5 months), and the difference was statistically significant ( P = 0.002). One stage Ⅰ A patient was treated with surgery alone and survived at 92 months of follow-up. Ten cases were treated with immunotherapy and had a good outcome. Conclusions:PPLELC is prevalent in non-smokers, the lesions are mostly in the right middle lung, and it is easily misdiagnosed as squamous cell carcinoma. The positive EBER in situ hybridization detection can help the diagnosis; lymph node metastasis is common. Tumor stage, lymph node metastasis and CYFRA21-1 level may be correlated with the survival of patients. The patients can benefit from immunotherapy, and anti-angiogenic therapy combined with chemotherapy is an optional treatment regimen.
		                        		
		                        		
		                        		
		                        	
3.Application of the mandarin Chinese version of the Leicester cough questionnaire in the chronic disease management of adult patients with severe asthma

		                        			Objective:To explore the application of the mandarin Chinese version of the Leicester cough questionnaire (LCQ-MC) in the chronic disease management of adult patients with severe asthma.Methods:Thirty patients with severe asthma without a smoking history who received treatment and chronic disease management in the First Affiliated Hospital of Guangxi Medical University from December 2018 to December 2020 and met the inclusion criteria were selected as the research subjects. Their clinical symptoms, LCQ-MC, asthma control test (ACT), mini asthma quality of life questionnaire (MiniAQLQ), visual analogue scale (VAS) and lung function test were recorded. The reliability of LCQ-MC and its correlation with ACT, MiniAQLQ, VAS and lung function were analyzed.Results:Among the 30 patients, there were 14 males and 16 females, with an average age of (50.5±10.5) years old and a medical history of (38.3±15.5) years. The main manifestations were cough which was observed in 29 cases (96.7%), wheezing in 25 cases (83.3%), nasal symptoms in 14 cases (46.7%) and chest tightness in 11 cases (36.7%). All lung functions were consistent with the diagnosis of asthma. The total score of LCQ-MC was (16.1±4.2), in which the physiological, psychological and social dimensions were (5.0±1.2), (5.6±1.6), (5.5±1.7), respectively; and ACT (18.9+5.5), MiniAQLQ (4.9±1.4), VAS (33.5±32.4). The Cronbach′s α for the questionnaire was more than 0.70, and the results were positively correlated with ACT and MiniAQLQ ( r=0.553, 0.593, P=0.002, 0.001), negatively correlated with VAS ( r=-0.762, P<0.001). The correlations between LCQ-MC and these scores were consistent with the results after 6 months. Conclusion:Cough is an important symptom of severe asthma, and LCQ-MC can be effectively used for evaluation and chronic disease management of adult patients with severe asthma.

5.Clinical value of serum N-MID,total TP1NP,β-CTx detection combination with whole-body bone scintigraphy in early diagnosis of bone metastasis in patients with malignant tumor

		                        			Objective To investigate the clinical value of bone metabolism biochemical marker N-MID,TP1NP and beta-CTx combined with whole body bone scintigraphy in early diagnosis of bone metastasis of tumor.Methods The concentration of the 3 markers were measured by the electrochemical luminescence analysis method in 30 cases of healthy control group and 210 cases of patients with malignant tumor,which were divided into non bone metastasis group(45 cases) and bone metastasis group(165 cases).The bone metastasis group were divided into 4 grades(0-grade Ⅲ) by Soloway classification according to whole body bone imaging.Results The levels of serum N-MID,TP1NP and beta-CTx in 165 malignant tumor patients with bone metastasis were significantly higher than in 45 malignant tumor patients with bone metastasis and in 30 healthy control group,the difference was statistically significant(P<0.05).With the increase of the number of metastatic lesions in the bone metastasis group,the serum levels of N-MID,TP1NP,and beta-CTx were increased gradually,and they were positively correlated with the progression of the disease.According to the analysis of ROC curve,the cut-off value,sensitivity and specificity in the diagnosis of tumor bone metastasis were 17.59 ng/mL,70.3%,88.9% for serum N-MID,43.04 ng/mL,78.2%,95.6% for TP1NP,and 0.48 ng/mL,73.9%,93.3% for beta-CTx.Under the ROC curve(AUC) was 0.831 for serum N-MID,0.890 for TP1NP,and 0.869 for beta-CTx.The sensitivity and specificity of three bone metabolic markers in the diagnosis of bone metastasis of malignant tumor were significantly higher.Conclusion Bone metabolism biochemical markers:Serum N-MID,TP1NP and beta-CTx for diagnosis of bone metastasis of malignant tumor are sensitive,accurate and simple,which can significantly improve the efficiency of diagnosis of bone metastasis,and can be combined with whole-body bone scintigraphy in early diagnosis of bone metastasis with malignant tumor.
		                        		
		                        		
		                        		
		                        	
6.Diagnostic value of combined detection of autoantibodies against p53 and Bmi-1 in lung cancer

		                        			Objective To evaluate the diagnostic value of combined detection of serum autoantibodies against against p53 and Bmi-1 in lung cancer (LC).Methods Serum levels of autoantibodies against p53 and Bmi-1 were measured by enzyme-linked immunosorbent assay (ELISA) in 92 patients with LC and 80 normal controls.The combined diagnostic value was evaluated with the receiver operating characteristic (ROC) curve.Results The serum levels of autoantibodies against p53 and Bmi-1 were significantly higher in LC than in normal controls (0.438 ±0.705 vs 0.045 ±0.035,Z =-7.667,P <0.01;0.117±0.061 vs 0.068 ±0.031,Z =-7.179,P <0.01).The levels of autoantibodies against p53 and Bmi-1 were not related to age,gender,pathological classification,lymph node status or tumor-node-metastasis (TNM) stage (P > 0.05).The combined detection of two autoantibodies provided an enhanced sensitivity of 63.0%,a specificity of 91.2% and an area under curve (AUC) of 0.881,which showed better diagnostic efficiency compared to individual autoantibodies.Conclusions Combined detection of autoantibodies against p53 and Bmi-1 shows good diagnostic value,which may aid diagnosis of LC.
		                        		
		                        		
		                        		
		                        	
7.Investigation and analysis of psychological health condition in cervical cancer patients received radiation and chemotherapy

		                        			Objective To investigate the status of mental health and related factors in cervical cancer patients received radiotherapy and chemotherapy,and to implement individualized nursing intervention.Methods 60 cervical cancer patients with radiation and chemotherapy were enrolled in this study.The depression self rating scale(SDS),anxiety self-assessment scale(SAS) were evaluated,and the influence factors of self-designed questionnaires were investigated.During the same period,60 healthy women of 4 regions in Shantou were selected as control group.The SDS and SAS were compared in two groups.And combined with the influencing factors of the investigation group after the implementation of nursing intervention 7 d psychological assessment again.Results Cervical cancer patients with radiotherapy and chemotherapy,the SDS and SAS scores were (56.1±7.7)points,(45.2±7.5)points,which were significantly higher than those of healthy women [(45.0±6.3)points,(39.2±6.5)points,t=8.142,4.588,all P<0.01].After 7 days of nursing intervention,the SDS and SAS scores in cervical cancer patients decreased significantly[SDS(49.5±6.8)points,SAS(42.7±6.2)points,P<0.05],and the influence factors of patients' mental health were worry about the hospitalization expenses,do not understand the disease related knowledge,fear,the influence of family and society,and worry about the side effects of treatment on the prognosis of disease(88%,80%,80%,75%,68.3%,65%).Conclusion Cervical cancer patients received radiotherapy and chemotherapy have poor mental health status,in the treatment of diseases at the same time,the nursing staff should give psychological counseling and support,improve the patients' anxiety and depression,promote rehabilitation.
		                        		
		                        		
		                        		
		                        	
8.Research on improving medication compliance in lung transplant patients by nursing intervention

		                        			Objective To investigate impact of nursing intervention on medication compliance in lung transplant patients.Methods A total of 46 lung transplant patients from April 2011 to December 2013 were divided into the experimental group(n =23) and the control group (n =23) by operation order.The control group received routine care,while on the basis of it,the experimental group received nursing intervention based on the result of blood concentration.Results The medication adherence after 6 months of the operation between the two groups had no significant differences (P >0.05).After 12 months of intervention,21 patients (91.30%) in the experimental group complied with the medication,18 patients (78.26%) in the control group complied with the medication.There was significant difference between the two groups (x2 =8.641,P < 0.05).The blood concentrations after 6 months of the operation between the two groups had no significant differences (P > 0.05).After 12 months,the blood concentrations of the experimental group was (13.15 ± 3.14) ng/L,which was significantly higher than that of the control group (t =7.658,P < 0.05).Conclusions Nursing intervention can effectively improve medication compliance of lung transplant patients.Establishing a good nurse-patient relationship,strengthening health education,doing repeated training and condition changes of patients is the key point to improve medication compliance of lung transplant patients.
		                        		
		                        		
		                        		
		                        	
9.Community-based intensive health management for type Ⅱ diabetics

		                        			Objective To discuss the effectiveness of community-based intensified health management on type Ⅱ diabetes patients.Metbods From June 2010 to December 2011,1 024 type Ⅱ diabetes patients in Mishi Lane community were enrolled and assigned to the intervention group (n=501,obtaining intensive health management) and the control group (n=523,obtaining routine community health management).Fastingbloodglucose (FBG),2-hour postprandial blood glucose (PBG2h),HbA1c,blood pressure (BP),triglyceride (TG) and body mass index (BMI) were tested and recorded before and after the intervention.Student's t test or Chi-square test were used for data analysis.Results Differences of age (t=0.124,P>0.05) and gender (x2=2.0,P>0.05) were not statistically significant (both P>0.05).Following the intensive health management,the control rates of HbA1c,BP,TG and BMI were increased to 75.4%,30.4%,85.6% and 49.0%,respectively (x2 values were 4.54,4.71,6.54 and 3.67; all P<0.05).TG,FBG and PBG2h of the intervention group were (1.5 ± 0.9),(6.2 ± 1.3) and (8.3 ± 2.4) mmol/L,respectively,which were significantly lower than those of the control group (all P<0.05).Conclusion Community-based intensive health management could be effective for diabetes patients.
		                        		
		                        		
		                        		
		                        	
10.Knowledge-attitude-practice(KAP)-based health management of type 2 diabetes among community residents

		                        			Objective To discuss the effectiveness of knowledge-attitude-practice(KAP)-based health management on type 2 diabetes in community residents.Methods From June 2010 to December 2011,930 type 2 diabetes patients living in Mishi Lane Community were enrolled and assigned to the intervention group(n=494) and the control group(n=436).The KAP mode was used for the intervention group,and the routine management mode was applied to the control group.Fasting blood glucose(FBG),PBG2 h,HbAlc,blood pressure(BP),triglyceride(TG) and body mass index(BMI) were tested and recorded.Student's t test or Chi-square test was used for data analysis.Results Age(t=0.124,P>0.05),gender(x2=2.0,P>0.05) and other demographics showed no statistically significant difference between the two groups(all P>0.05).After one-year KAP-based health management,HbA 1 c of the intervention group reduced to(6.1 ± 1.2)%,FBG and PBG2 h reduced by 1.7 and 3.23 mmol/L,respectively(t=4.926,P<0.05; t=4.306,P<0.05; t=4.523,P< 0.05).Diet,physical exercises,medication,regular inspection,unhealthy habits and questionare test awaveness of the intervention group were improved(86.03%,82.19%,85.63%,70.45% 76.11% and 88.46%,respectively.Drinking,dietary structure,overweight and physical exercises of the intervention group were also improved(14.17%,15.79%,70.65% and 68.83%,respectively; all P<0.001).Conclusion The KAPbased health management is proved to be effective in the control of community diabetic patients.
