BackgroundThe occurrence rate of dangerous behaviors in patients with severe mental disorders is higher than that of the general population. In China, there is limited research on the prediction of dangerous behaviors in community-dwelling patients with severe mental disorders, particularly in terms of predicting models using data mining techniques other than traditional methods. ObjectiveTo explore the influencing factors of dangerous behaviors in community-dwelling patients with severe mental disorders and testing whether the classification decision tree model is superior to the Logistic regression model. MethodsA total of 11 484 community-dwelling patients with severe mental disorders who had complete follow-up records from 2013 to 2022 were selected on December 2023. The data were divided into a training set (n=9 186) and a testing set (n=2 298) in an 8∶2 ratio. Logistic regression and classification decision trees were separately used to establish predictive models in the training set. Model discrimination and calibration were evaluated in the testing set. ResultsDuring the follow-up period, 1 115 cases (9.71%) exhibited dangerous behaviors. Logistic regression results showed that urban residence, poverty, guardianship, intellectual disability, history of dangerous behaviors, impaired insight and positive symptoms were risk factors for dangerous behaviors (OR=1.778, 1.459, 2.719, 1.483, 3.890, 1.423, 2.528, 2.124, P<0.01). Being aged ≥60 years, educated, not requiring prescribed medication and having normal social functioning were protective factors for dangerous behaviors (OR=0.594, 0.824, 0.422, 0.719, P<0.05 or 0.01). The predictive effect in the testing set showed an area under curve (AUC) of 0.729 (95% CI: 0.692~0.766), accuracy of 70.97%, sensitivity of 59.71%, and specificity of 72.05%. The classification decision tree results showed that past dangerous situations, positive symptoms, overall social functioning score, economic status, insight, household registration, disability status and age were the influencing factors for dangerous behaviors. The predictive effect in the testing set showed an AUC of 0.721 (95% CI: 0.705~0.737), accuracy of 68.28%, sensitivity of 64.46%, and specificity of 68.60%. ConclusionThe classification decision tree does not have a greater advantage over the logistic regression model in predicting the risk of dangerous behaviors in patients with severe mental disorders in the community. Objective:To evaluate the recovery of daytime and nighttime urinary continence in patients with robotic-assisted radical cystectomy and ileal orthotopic neobladder from 1 to 60 months after surgery, and systematically analyze the influencing factors of daytime and nighttime urinary continence recovery. Methods:The convenient sampling method was used to select clinical data of 60 patients who underwent robotic-assisted radical cystectomy and ileal orthotopic neobladder by a single operator from December 2014 to January 2020 and they were followed up for daytime and nighttime use of urine pads and prognosis. A total of 44 patients were eligible for follow-up data. Satisfactory recovery of daytime and nighttime urinary continence was defined as the use of less than or equal to 1 pad, and complete recovery of daytime and nighttime urinary continence was defined as no urine leakage. The daytime and nighttime urinary continence recovery in patients with robotic-assisted radical cystectomy and ileal orthotopic neobladder was assessed at 1, 3, 6, 12, 24, 36, and 60 months. Cox regression was used to analyze the influencing factors of postoperative urinary continence recovery. Results:Cox regression multivariate analysis showed that preservation of neurovascular bundles was associated with satisfactory recovery of daytime urinary continence, satisfactory recovery of nighttime urinary continence and complete recovery of daytime urinary continence ( P<0.05) . Clevien grading of complications within 90 d could affect the satisfaction of daytime urinary control recovery. Preoperative hydronephrosis was an independent factor affecting the satisfaction of nighttime urinary continence recovery. Conclusions:Preserving neurovascular bundles is an influencing factor in promoting postoperative urinary continence recovery in patients with robotic-assisted radical cystectomy and ileal orthotopic neobladder. In addition, postoperative complications and preoperative hydronephrosis are independent factors affecting the satisfaction of daytime and nighttime urinary continence recovery, respectively, but it need to be further confirmed by multicenter prospective studies.

Objective:To explore the current situation of empathy for pain in medical students and analyze its influencing factors. Methods:This study was a cross-sectional study. From June 2018 to January 2019, convenience sampling was used to select 768 undergraduate students from 4 medical universities in Shanghai, Jiangsu Province and Zhejiang Province as the research subject. The General Information Questionnaire, the Chinese version of the Empathy for Pain Scale (EPS) , and the Chinese version of the Interpersonal Reactivity Index (IRI-C) were used for investigation. Multiple linear regression was used to analyze the influencing factors. Results:Among 768 medical students, the total score of EPS was (2.56±0.66) , of which the physical and mental discomfort reaction dimension score was (2.28±0.74) , and the empathy reaction dimension score was (3.39±0.79) . The results of multiple linear regression analysis showed that major, being the only child or not, often watching violent movies or not, and personal distress were the influencing factors of medical students' physical and mental discomfort reaction with a statistical difference ( P<0.05) , explaining 20.1% of the variation in total. Gender, grade, clinical learning experience, viewpoint selection, empathic attention and imagination were the influencing factors of the empathy reaction of medical students also with a statistical difference ( P<0.05) , explaining 19.7% of the variation in total. Conclusions:The empathy for pain of medical students is at a moderate level and is affected by many factors. Medical education should strengthen the cultivation of empathy behavior practice ability, and multi-dimensionally cultivate and evaluate students' empathy ability in combination with patients' evaluation.
