1.Efficacy and safety associated with the infusion speed of immunoglobulin for the treatment of Kawasaki disease

Objective To evaluate the efficacy and safety differences of immunoglobulin(IVIG)infusion rates in the treatment of Kawasaki disease(KD)in children.Methods KD children treated in the Department of Pediatrics of Anqing First People's Hospital were retrospectively selected as the research subjects.According to different infusion speeds of IVIG,KD children were divided into 12 h-group and 24 h-group.The main outcomes observed in this study included the duration of fever and the incidence of coronary artery lesions(CAL).The secondary observation outcomes included the proportion of IVIG non-response,total IVIG dose,proportion of third-line treatment,and laboratory examination indicators(white blood cell count,neutrophil percentage,platelet count,albumin,creatinine,aspartate aminotransferase,sodium ion,C-reactive protein,and immunoglobulin G level).Results A total of 60 children with KD were included in the study,with 41 cases in the 12 h-group and 19 cases in the 24 h-group.There were no significant differences in the duration of fever,incidence of CAL,proportion of IVIG non-response,total IVIG dose,and proportion of third-line treatment between the two groups(P>0.05).Compared with before treatment,the white blood cell count,neutrophil percentage,and C-reactive protein significantly decreased on the second day and the seventh day in both the 12 h-group and the 24 h-group(P<0.05);while platelet count,sodium ions,and immunoglobulin G significantly increased(P<0.05).Albumin showed a downward trend and then an upward trend(P<0.05),while creatinine and aspartate aminotransferase showed no significant changes(P>0.05).In addition,there were no significant statistical differences between different time points(P>0.05).Two patients in the 12 h-group experienced adverse events,while none in the 24 h-group experienced adverse events.Conclusion The efficacy and safety of IVIG administered over 12 h were similar to those administered over 24 h.
2.The prediction value of Field Assessment Stroke Triage for Emergency Destination score in acute large vessel occlusion stroke

Objective:To validate the predictive function of Field Assessment Stroke Triage for Emergency Destination (FAST-ED) score on large vessel occlusion (LVO) in Chinese population.Methods:The information about the patients who had the disease onset within 24 hours, were treated in the Emergency Department of Jinling Hospital, and diagnosed as ‘acute ischemic stroke’ was collected. Via the emergent brain computed tomography angiography or digital subtraction angiography, the patients were divided into LVO group and non-LVO group. The scores of FAST-ED were calculated according to the National Institutes of Health Stroke Scale (NIHSS) scores and compared with Rapid Arterial oCclusion Evaluation (RACE), 3-item Stroke Scale (3I-SS), Cincinnati Stroke Triage Assessment Tool (C-STAT), and Prehospital Acute Stroke Scale (PASS) scores. Moreover, the patients were further divided into anterior and posterior circulation lesion groups to explore whether the FAST-ED scale can differ the anterior or posterior circulation effectively.Results:Three hundred and eighty-one patients were eventually included, among whom 284 were diagnosed as LVO, and 97 were diagnosed as non-LVO. Receiver operating characteristic curves showed that cut-off value of 4 optimized the scale (sensitivity: 0.76, specificity: 0.69, area under the curve: 0.78). The area under the curve of FAST-ED score(0.78) showed no statistically significant difference with NIHSS (0.79), RACE (0.77), 3I-SS (0.78) and C-STAT scores (0.75), and exhibited statistically significant difference with PASS score (0.74; 95% CI 0.69-0.78, P=0.01). FAST-ED score showed no statistically significant difference in predicting anterior and posterior circulation lesions. Conclusions:FAST-ED score can predict LVO in a rather accurate manner. It can predict anterior and posterior circulation lesions with similar effectiveness. So FAST-ED is able to be a prehospital screening tool and make assistance to the prehospital treatment.

The morbidity and mortality of stroke caused by large vessel occlusion are high,and its outcome is closely associated with emergency treatment.In order to receive treatment within the time window,the effective prehospital assessment is very important.The prehospital stroke scale simplifies emergency screening and assessment of such patients.Although the predictive value is good,its role remains controversial.This article reviews some of the prehospital stroke scales used to identify large vessel occlusions and analyzed the characteristics of different scales.
4.The effect of noise exposure on insulin sensitivity in mice may be mediated by the JNK/IRS1 pathway.

BACKGROUND:
Epidemiological studies have suggested that noise exposure may increase the risk of type 2 diabetes mellitus (T2DM), and experimental studies have demonstrated that noise exposure can induce insulin resistance in rodents. The aim of the present study was to explore noise-induced processes underlying impaired insulin sensitivity in mice.
METHODS:
Male ICR mice were randomly divided into four groups: a control group without noise exposure and three noise groups exposed to white noise at a 95-dB sound pressure level for 4 h/day for 1, 10, or 20 days (N1D, N10D, and N20D, respectively). Systemic insulin sensitivity was evaluated at 1 day, 1 week, and 1 month post-noise exposure (1DPN, 1WPN, and 1MPN) via insulin tolerance tests (ITTs). Several insulin-related processes, including the phosphorylation of Akt, IRS1, and JNK in the animals' skeletal muscles, were examined using standard immunoblots. Biomarkers of inflammation (circulating levels of TNF-α and IL-6) and oxidative stress (SOD and CAT activities and MDA levels in skeletal muscles) were measured via chemical analyses.
RESULTS:
The data obtained in this study showed the following: (1) The impairment of systemic insulin sensitivity was transient in the N1D group but prolonged in the N10D and N20D groups. (2) Noise exposure led to enhanced JNK phosphorylation and IRS1 serine phosphorylation as well as reduced Akt phosphorylation in skeletal muscles in response to exogenous insulin stimulation. (3) Plasma levels of TNF-α and IL-6, CAT activity, and MDA concentrations in skeletal muscles were elevated after 20 days of noise exposure.
CONCLUSIONS
Impaired insulin sensitivity in noise-exposed mice might be mediated by an enhancement of the JNK/IRS1 pathway. Inflammation and oxidative stress might contribute to insulin resistance after chronic noise exposure.

5.Clinical effect of treating in lower extremity venous extensive ulcer of simple varicosity of great saphenous vein with full thickness skin grafting of plastic surgery

Objective:To observe the clinical effect of full thickness skin grafting of plastic surgery in the treatment of lower extremity venous extensive ulcer of simple varicosity of great saphenous vein.Methods:A total of 23 patients from January 2011 to December 2016 with lower extremity venous extensive ulcer of simple varicosity of great saphenous vein,who underwent great saphenous vein high ligation,stripping,varicose vein branch ablation and subfascial perforating vein surgery combined with full thickness skin grafting of plastic surgery.Results:The ulcers in 23 cases were all healed.There was no recurrence in six months to two years follow-up.Conclusions:For lower extremity venous extensive ulcer of simple varicosity of great saphenous vein,on the basis of the traditional operation,full thickness skin grafting of plastic surgery can improve the cure rate and decrease the recurrence rate.
6.Relationship between sex hormone binding globulin and metabolic syndrome in elderly males

To investigate the relationship between sex hormone binding globulin (SHBG) and metabolic syndrome in elderly males in Shanghai,all the subjects (≥ 60 years old,male) underwent measurements of weight,height,waist and hip circumferences,and blood pressures,serum levels of fasting plasma glucose,total cholesterol,triglyceride,high density lipoprotein-cholesterol (HDL-C),and low density lipoprotein-cholesterol were determined (Hitachi,7600),while the levels of serum insulin,total testosterone,and SHBG were determined by using chemiluminescence methods.Free testosterone was calculated by using the Vermeulen equation.Metabolic syndrome was defined according to the criteria of the Chinese Diabetes Society (CDS 2004).The SHBG level in the metabolic syndrome group was significantly lower than that in non-metabolic syndrome group [(40.50 ± 26.16) nmol/L vs (47.80± 20.34) nmol/L,P<0.01].With increasing number of metabolic syndrome components,the level of SHBG became lowered progressively.The subjects were divided into four subgroups according to SHBG Quartiles.From Quartile 1 to Quartile 4,body mass index,waist-hip ratio,diastolic blood pressure,fasting plasma glucose,triglyceride,fasting insulin,homeostasis model assessment for insulin resistance(HOMA-IR),free testosterone,free androgen index,and free testosterone percentage became progressively lowered,while age and HDL-C became raised (P<0.05).SHBG was correlated significantly with age,body mass index,waist-hip ratio,diastolic blood pressure,fasting plasma glucose,HDL-C,and triglyceride.Age,HDL-C,and body mass index remained independently associated with SHBG in the multivariate regression analysis.In a logistic regression taking metabolic syndrome as the dependent variable,SHBG and HOMA-IR were included in the final model with statistical significance.Lowered SHBG is a risk factor of metabolic syndrome in elderly males.SHBG may be an independent predictor of metabolic syndrome,but the mechanism of how SHBG is involved in the metabolic syndrome needs to be further studied.
7.Tacrolimus therapy in refractory lupus nephritis: a prospective study in a single center

ObjectiveThe purpose of this study was to assess the efficacy,safety and optimal dose of tacrolimus monotherapy in patients with refractory lupus nephritis(LN) who were resistant to cyclophosphamide(CYC).MethodsA total of 14 LN patients (2 men and 12 women) with persistent proteinuria who were resistant to CYC treatment more than 8 g for half a year were enrolled.Tacrolimus was initiated at 2 mg/d (patient weight<60 kg) or 3 mg/d(patient weight≥60 kg) which was administered in two divided doses.Prospective data on daily proteinuria,serum album level and serologic lupus activity were collected and followed for 6 months.ANOVA and Pearson correlation analysis were used for statistical analysis.Results Mean age at baseline was(30±9) years.Mean urinary protein decreased significantly from(6.2±5.1) g at baseline to (1.1±0.9) g at 6 months (F=16.21,P<0.01).Mean serum album level increased significantly from (27.9±9.7) g/L at baseline to(37.8±2.2) g/L at 6 months(F=16.71,P<0.01 ).Complete or partial response was observed in 86% of patients receiving tacrolimus therapy.The effective dosage in this study was 0.03-0.06mg·kg-1·d-1 of the patients who had complete response or partial response to tacrolimus.The tacrolimus level in partially and completely responding patients was less than 3 ng/ml.There was no significant difference among blood tacrolimus levels of complete,partial,and no response patients [(1.6-±0.4),(2.0±0.6) and (22±1.1) ng/nl],respectively).No definite correlation was found between efficacy and tacrolimus level.Tacrolimus was well tolerated at current dose,besides one with new onset hypertension and one with alopecia.ConclusionOur results suggest that tacrolimus at low dosage and serum level is potentially effective and safe for the treatment of patients with LN and persistent proteinuria resistant to CYC.The optimal dosage of tacrolimus for LN may be 0.03-0.06 mg·kg-1·d-1.
8.Differentiation characteristics of human periodontal ligament cell population in vitro.

OBJECTIVETo explore the multi-differentiated capability of human periodontal ligament cell population (hPDLP), and provide a theoretical basis for the periodontal regeneration by tissue engineering technique.
METHODShPDLP was cultured from periodontium of human tooth by the outgrowth method. STRO-1 and CD 146 expression were investigated by flow cytometry. hPDLP was induced to odontogenic/osteogenic-like and adipogenic-like cell. The multilineage differentiation capacities of hPDLP were evaluated by alizarin red stain, oil red O stain, anti-CD146 and STRO-1 immunocytochemistry, and reverse-transcriptase polymerase chain reaction analysis.
RESULTShPDLP was isolated from human periodontium and most of the cells retained their fibroblastic spindle shape. hPDLP can be induced into osteoblast-like cells and adipocyte-like cells, and calcium deposition and lipid droplets were detected perspectively. And the eighth generation of hPDLP had weaker potential into adipocyte-like cells than the first passage, however, there was no difference to the aspect of calcification ability between the two passages.
CONCLUSIONhPDLP cultured in vitro can differentiate into adipocytes and osteoblasts, and the first to third passage cells may have the predominance of differentiation potential.

9.The Renaturation and Activity Study of LexA From Pseudomonas aeruginosa

Objective:To optimize the renaturation procedure of denatured LexA,prepare the repressor LexA from Pseudomonas aeruginosa(PA),which have the satisfactory biologic activity.Methods:The LexA was renatured by the GSH/GSSG dilution method,and the renatured protein were purified by Ni2+ chelate affinity chromatography and gel filtration chromatography,following desalination by Sephadex G25 gel column.The renaturation result were detected by the native polyacrylamide gel electrophoresis and RPHPLC.The immunological activity of all LexA proteins,including the denatured,renatured protein and the renatured protein that was treated with the DTT,were determined by Western blot.Results:The renatured LexA appears both monomer and multimer,which is confirmed by the native polyacrylamide gel electrophoresis analysis and RPHPLC.Gel retardation experiments shows that the renatured LexA have satisfactory biologic activity.
10.A study of making use of computer-aided projection fringe system to measure breast dimension.

OBJECTIVETo measure breast basic dimension by using computer-aided projection fringe system.
METHODSA system has been developed for measuring breast basic dimension based on computer-aided projection fringe measurement and programming software. Plastic manikins breast's SN-N (sternal notch to nipple distance), N-ML (nipple to midline distance), N-N (internipple distance), MBW (base width of breast) and N-IMF (nipple to inframammary fold distance) are measured with this system. At the same time, these items are also measured with routine ruler.
RESULTSThis study indicate that the system has some merits: (1) non-touching measurement; (2) it is very rapid, the patient measured need hold his breath only 0.5 second, and all the time it takes is about 2.5 minutes; (3) the measurement's sensitivity is as high as to 0.6 mm, which meets the clinic requirement entirely; (4) the measurement's accuracy of the system is not significantly when comparing to the routine ruler's.
CONCLUSIONSComputer-aided projection fringe system for measuring breast basic dimension is feasible and advanced.
