1.Efficacy and Safety of Acupuncture Combined with Levodopa in the Treatment of Parkinson's Disease: A Randomized Controlled Trial

		                        			ObjectiveTo evaluate the clinical efficacy and safety of acupuncture combined with levodopa in the treatment of Parkinson's disease(PD). MethodsA total of 60 patients with PD were enrolled and randomly assigned to test group or control group, with 30 patients in each group. The control group received levodopa only, starting at 100 mg per dose, three times daily, with gradual increases not exceeding a maximum daily dose of 800 mg. The test group received acupuncture three times per week in addition to levodopa. Both groups were treated for 12 weeks. Assessments were conducted before treatment, after 6 and 12 weeks treatment, using the Unified Parkinson's Disease Rating Scale(UPDRS), Wearing-Off Questionnaire-9(WOQ-9), Montreal Cognitive Assessment(MoCA), Mini-Mental State Examination(MMSE), Depression Rating Scale(DRS), Hamilton Depression Scale(HAMD), Hamilton Anxiety Scale(HAMA), PD Questionnaire-39(PDQ-39), and Pittsburgh Sleep Quality Index(PSQI). Repeated measures ANOVA was utilized to evaluate the effects of time, group, and their interaction on each index. Spearman correlation analysis was conducted to examine the relationships between combined treatment and outcome scores. Adverse events in both groups were recorded throughout the study. ResultsBoth groups showed significant improvements after 6 and 12 weeks treatment, with decreases in UPDRS total score, WOQ-9 total score, DRS score, HAMD score, HAMA score, PDQ-39 score, and PSQI score, and increases in MoCA and MMSE scores(P<0.05). Compared with the control group, the test group demonstrated significantly greater improvements in all the above indicators after 6 and 12 weeks (P<0.05). Repeated measures ANOVA showed significant time main effects, group main effects, and their interaction across all outcome measures(P<0.01). Spearman correlation analysis revealed that combined therapy was significantly negatively correlated with UPDRS, WOQ-9, DRS, HAMD, HAMA, PDQ-39, and PSQI scores, while positively correlated with MoCA and MMSE scores after 12 weeks of treatment(P<0.05). Both groups did not experience any serious adverse events and did not affect treatment. ConclusionAcupuncture combined with levodopa is more effective than levodopa alone in improving motor function, non-motor symptoms, cognitive function, depression and anxiety, quality of life, and sleep quality in patients with PD, with good safety.

		                        			Based on the theory of toxin in The Inner Canon of Yellow Emperor (《黄帝内经》), this paper explores the concept and connotation of toxin from several aspects, including medicinal toxicity, pathogenic toxicity, pathoge-nesis and treatment of toxin pathogen. It explores the mechanism and significance of using toxin substances in curing and inducing diseases, sorts out the etiological and pathological evolution of toxic pathogens, clarifies the derivation and differentiation process from external toxins to internal toxins, and elucidates the interactions between toxin and other pathogenic factors such as phlegm, stasis, dampness, and fire. It further distinguishes the logical hierarchy of toxin caused by metabolic disorders such as sugar, fat, and drowning, clarifies that the essence of the etiology of "hidden toxin", and determines the corresponding treatment principles and methods for toxic pathogens. 3.Effects and mechanism of osthol on skin wound healing and angiogenesis in rats

		                        			OBJECTIVE To investigate the effects of osthole (OST) on skin wound healing and angiogenesis in rats by regulating the sonic hedgehog (SHH) signaling pathway. METHODS Full-layer skin defect wound model rats were established and then randomly separated into Model group, OST low-dose, medium-dose and high-dose groups (OST-L group, OST-M group, OST-H group, 20, 30, 40 mg/kg OST), high-dose OST+SHH inhibitor cyclopamide group (OST-H+cyclopamide group, 40 mg/kg OST+10 mg/kg cyclopamide), with 12 rats in each group. Another 12 rats were selected as the control group. The wound healing of rats on 1, 7 and 14 days of administration was observed, and the wound healing rate of rats in each group was measured. The pathological changes and collagen deposition in rat wound tissue were observed; the levels of angiopoietin-1 (Ang-1) and basic fibroblast growth factor (bFGF) in wound tissue of rats were detected; the relative expressions of vascular endothelial growth factor A (VEGFA) and vascular endothelial growth factor receptor-2 (VEGFR-2) mRNA were also detected in wound tissue of rats; the protein expressions of VEGFA, VEGFR-2, SHH and glioma-associated oncogene homolog-1 (GLI1) were determined in wound tissue of rats. RESULTS Compared with Model group, the healing rate of skin wound, relative expression of collagen protein, the levels of Ang-1 and bFGF, the mRNA and protein expressions of VEGFA and VEGFR-2, and the protein expressions of SHH and GLI1 were all significantly increased in OST-M and OST-H groups (P<0.05). The wound tissue underwent significant re- epithelialization, with reduced inflammatory cell infiltration and granulation tissue edema, and an increase in the number of new blood vessels. SHH inhibitor cycloparamide weakened the promoting effects of OST on skin wound healing and angiogenesis in rats. 4.Inhibition of Oxidative Stress by Wuzi Yanzongwan to Ameliorate Idiopathic Oligoasthenospermia: A Review

		                        			Idiopathic oligoasthenospermia (IO) has been increasingly emphasized in the diagnosis and treatment of male infertility. Oxidative stress damage directly affects sperm quality and spermatogenesis, constituting a major causative factor of IO. Firstly, due to its high content of polyunsaturated fatty acids, the sperm plasma membrane is highly sensitive to reactive oxygen species (ROS), leading to lipid peroxidation accumulation and even inducing ferroptosis. Secondly, deficient downstream key proteins in the base excision repair pathway render sperm unable to repair extensive DNA oxidative damage under oxidative stress. Simultaneously, under oxidative stress, the apoptotic pathway of sperm is cascade-activated, causing rapid loss of motility. ROS further disrupts the hypothalamic-pituitary-gonadal axis, inhibiting testosterone production and ultimately affecting spermatogenesis. Wuzi Yanzongwan,in line with traditional Chinese medicine theory of treating IO through "nourishing kidney essence and harmonizing Yin and Yang", clinically demonstrates its ability to improve sperm morphology, count, and motility, thereby enhancing male fertility. The research on the pharmacological constituents of Wuzi Yanzongwan primarily involves establishing a characteristic spectrum of Chinese medicine to achieve quality control and exploring the pharmacology of effective components. Studies have found that its main active ingredients consist of flavonoids and phenylpropanoids. Specifically, compounds such as hyperin, acteoside, kaempferol, and schisandrin A are identified as the primary active substances and quality control components. These compounds exhibit strong antioxidant activity and have been partly applied in research related to reproductive endocrine disorders. Tripterygium glycoside is primarily used for modeling of oxidative stress-induced IO. It leads to the accumulation of various lipid peroxides in testicular tissues and concurrently compromises the body's antioxidant capacity. Mechanistic studies have found that Wuzi Yanzongwan can inhibit elevated ROS levels in IO models and enhance the body's antioxidant capacity, thereby ameliorating inflammation, suppressing cell apoptosis, promoting testosterone production, and ultimately alleviating the decline in sperm quality and spermatogenesis caused by oxidative stress. 
		                        		
		                        		
		                        		
		                        	
5.Dilution conditions and standardization study for semen samples using computer-aided sperm analysis

		                        			Objective To explore the dilution conditions and standards in detecting the semen samples with high sperm concentration using computer-aided sperm analysis(CASA)systems.Methods CASA systems with 10 μm depth disposable counting chambers were used for the examination.The samples were divided into undiluted group(Group 1∶sperm concentration<50×106/mL)and diluted groups(Group 2∶50×106/mL≤ sperm concentration<100× 106/mL;Group 3∶sperm concentration≥100× 106/mL).When sperm concentration<50×106/mL,no dilution was performed.When sperm concentration≥50× 106/mL,the samples were diluted with saline at 1∶n/(50×106)ratio(n=sperm concentration,n/[50×106]rounded down)to<50×106/mL of sperm concentration.The sperm concentration,progressive motility(PR),non-progressive motility(NP),total motility(PR+NP)and immotile sperm percent-age(IM)were analyzed before and after dilution.The consistency of results pre-and post-dilution was compared.ROC curve was used to analyze the optimal dilution threshold.Results The differences in the parameters pre-and post-dilution gradually rosed with the increased sperm concentration.ROC curve analysis showed optimal dilution thresholds were 133.05 × 106/mL,101.75 × 106/mL,118.60×106/mL,90.90×106/mL,111.83×106/mL for the sperm concentration,PR+NP,PR,NP and IM respectively.Considering sperm concentration and NP were most affected the undiluted high concentration samples,the optimal comprehensive dilution threshold was determined as 125.08× 106/mL.Conclusion When sperm concentration exceeds 125×106/mL,it is recommended to dilute semen sample with normal saline.
		                        		
		                        		
		                        		
		                        	
6.Current situation and improvement strategies of semen testing in hospitals without developing assisted reproductive technolo-gy in Hunan province

		                        			Objective To investigate the current status of semen testing in the hospitals without assisted reproductive technology in Hu-nan Province,propose improvement strategies to enhance the diagnosis of male infertility and promote reproductive health services in these hospitals.Methods Questionnaire surveys and on-site investigations were conducted to examine the semen testing status in 67 hospitals without assisted reproductive technology in Hunan Province.The existing problems were summarized,and improvement strate-gies were suggested.Results Among the 67 hospitals,59.7%(40/67)performed semen testing.Of these,45%(18/40)possessed dedicated personnel,60%(24/40)possessed staff received relevant training,and 22.5%(9/40)held certificates.Only 30%(12/40)conducted sperm morphology testing,among them 20%(8/40)able to stain and interpret following WHO standards.The hospitals of 37.5%(15/40)used computer-assisted sperm analysis(CASA)systems,and 22.5%(9/40)were equipped with phase-contrast microscopes.30%(12/40)had dedicated examination areas,and 32.5%(13/40)had independent ejaculation rooms,17.5%(7/40)used disposable slides to observe sperm concentration.In 2022,the hospitals of 57.5%(23/40)had an average daily sample volume<1.The hospital of 67.5%(27/40)performed standard operating procedures,40%(16/40)followed the WHO 5th la-boratory manual for routine testing,15%(6/40)conducted internal quality control,and 12.5%(5/40)participated in external quali-ty assessment(EQA).Another 55%(22/40)wished to participate in EQA.Conclusion The semen testing capacity in the hospitals,in which assisted reproductive technology is not yet carried out currently,urgently requiring multifaceted improvements.The proposed strategies include emphasizing semen testing,establishing reproductive medicine consortia for comprehensive support,establishing a provincial quality control center for EQA,founding a professional committee for a learning platform,offering training to enhance staff expertise,and including the pre-pregnancy semen testing in public health programs.
		                        		
		                        		
		                        		
		                        	
7.Visualization analysis on research literature about Linggui Zhugan Decoction based on CiteSpace

		                        			Objective:To investigate the research status and hotspots of Linggui Zhugan Decoction.Methods:The research literature about Linggui Zhugan Decoction was retrieved from CNKI, VIP, Wanfang Data and CBM databases from January 1, 2000 to July 11, 2023. NoteExpress 3.8 software was used to merge and remove the weight, and CiteSpace 6.2.R4 software was used to analyze the author, research institution, key words and draw the knowledge map.Results:A total of 1 521 articles were included, and the number of published papers showed a fluctuating upward trend. The main source journals were Chinese Journal of Urban and Rural Enterprise Hygiene, Acta Chinese Medicine, Information of Traditional Chinese Medicine, etc. The main research institutions were Tianjin University of Traditional Chinese Medicine, Beijing University of Chinese Medicine, Shandong University of Traditional Chinese Medicine, etc. Among the most published papers were Huang Jinling from Anhui University of Chinese Medicine (22 articles), Wang Liang from the Institute of Integrated Chinese and Western Medicine of Anhui Academy of Chinese Medicine (14 articles) and Shi Hui from Anhui University of Chinese Medicine (10 articles); high-frequency keywords included chronic heart failure, vertigo, clinical efficacy and so on. Conclusions:The research on Linggui Zhugan Decoction focuses on the clinical research of circulatory system, respiratory system, digestive system and urinary system. 8.Clinical Observation on the Wenyang Huayin Prescription in the Treatment of Chronic Heart Failure in Elderly Patients with Fluid Retention due to Yang Deficiency

		                        			Objective To observe the clinical efficacy of Wenyang Huayin Prescription(cooked Aconiti Lateralis Radix Praeparata,Cinnamomi Ramulus,Codonopsis Radix,Atractylodis Macrocephalae Rhizoma,Poria,Descurainiae Semen Lepidii Semen,etc.)combined with conventional western medicine in the treatment of elderly patients with chronic heart failure(CHF).Methods A total of 120 elderly CHF patients with fluid retention due to yang deficiency syndrome who were treated in People's Hospital of Chenghai District from December 2021 to December 2022 were selected as the research objects.They were randomly divided into control group and observation group,with 60 cases in each group.The control group was given conventional western medicine treatment,and the observation group was treated with Wenyang Huayin Prescription on the basis of the treatment of the control group.The course of treatment was four weeks.The indexes of the two groups were observed before and after treatment:(1)TCM syndrome scores;(2)serum levels of N-terminal pro-B-type natriuretic peptide(NT-proBNP),high-sensitivity C-reactive protein(hs-CRP),tumor necrosis factor-α(TNF-α)and interleukin-6(IL-6);(3)left ventricular ejection fraction(LVEF),left ventricular end-diastolic diameter(LVEDD);(4)6-minute walking distance.The clinical efficacy and safety of the two groups were compared.Results After treatment,(1)the scores of TCM syndromes in the two groups were significantly lower than those before treatment(P<0.05),and the scores in the observation group were significantly lower than those in the control group(P<0.05).(2)The levels of serum hs-CRP,TNF-α,IL-6 and NT-proBNP in the two groups were significantly lower than those before treatment(P<0.05),and the levels in the observation group were significantly lower than those in the control group(P<0.05).(3)The levels of LVEF in the two groups were significantly higher than those before treatment(P<0.05),and the levels of LVEDD were significantly lower than those before treatment(P<0.05).The level of LVEF in the observation group was significantly higher than that in the control group(P<0.05),and the level of LVEDD was significantly lower than that in the control group(P<0.05).(4)The 6-minute walking distance in the two groups was significantly increased when compared with that before treatment(P<0.05),and the increase in the observation group was significantly greater than that in the control group(P<0.05).(5)The total effective rate of the observation group(96.67%)was significantly higher than that of the control group(86.67%)(P<0.05);the total incidence of adverse reactions in the observation group(1.67%)was significantly lower than that in the control group(11.67%)(P<0.05).Conclusion Wenyang Huayin Prescription combined with conventional western medicine treatment can effectively reduce the TCM syndrome score of elderly CHF patients with fluid retention due to yang deficiency syndrome,improve clinical symptoms and cardiac function,and reduce the body's inflammatory response.The clinical efficacy is superior to that of western medicine alone,and the safety is relatively high.
		                        		
		                        		
		                        		
		                        	
9.Visualization Analysis on Research Literature about TCM for the Treatment of Attention Deficit Hyperactivity Disorder

		                        			Objective To analyze the research status,hotspots and trends of TCM in the treatment of attention deficit hyperactivity disorder(ADHD).Methods The literature on the treatment of ADHD by TCM were was retrieved from CNKI,VIP,Wanfang Data,and CBM from the establishment of the databases to 7th,Sep.2023.NoteExpress 3.9 was used to manage and remove weight;Excel 2019 was used to draw a line trend chart for the number of published literature.CiteSpace 6.1R.6 software was used to perform co-occurrence and clustering analysis on authors,institutions and keywords,and a visual graph was drawn.Results A total of 1215 articles were included after screening.800 authors were involved,forming research teams with Han Xinmin,Wang Junhong,Ma Rong and Li Yirui as the cores respectively;Nanjing University of Chinese Medicine,Guangzhou University of Chinese Medicine,Beijing University of Chinese Medicine and so on published more papers.High-frequency keywords included clinical efficacy,acupuncture and moxibustion treatment,clinical experience,Chinese materia medica and so on;research frontiers included clinical efficacy,acupuncture and moxibustion treatment,clinical experience,data mining and attention.Conclusion The main research on the treatment of ADHD by TCM includes clinical efficacy,clinical experience,animal experiments and data mining,and relatively stable research teams have been formed,but there is less cooperation between teams and institutions.
		                        		
		                        		
		                        		
		                        	
10.Predictive value of inflammatory cells and clinical features in prognosis for non-small cell lung cancer immunotherapy

		                        			Objective To investigate the predictive value of inflammatory cells and clinical features in the prognosis of immune checkpoint inhibitors (ICIs) treating non-small cell lung cancer (NSCLC).Methods The data of 163 cases of stage Ⅲ and Ⅳ NSCLC patients treated with the ICIs in this hospital from January 1,2017 to December 31,2022 were collected.The CT examination was conducted after 6-8 weeks treatment.The pa-tients were divided into the objective remission group[complete remission (CR)+partial remission (PR)]and non-objective remission group[stable disease (SD)+progressed disease (PD)],disease control group (CR+PR+SD) and non-disease control group (PD),persistent clinical benefit group (DCB) and non-DCB group.The differences in clinical features and inflammatory cells indicators were compared among the differ-ent groups.The receiver operating characteristic (ROC) curve was adopted to evaluate the predictive efficiency of the inflammatory cells indicators for DCB.The influencing factors analysis of progression free survival (PFS) time and overall survival (OS) time adopted the Cox regression analysis.Results The lymphocyte count (ALC) in the disease control group was higher than that in the non-disease control group.The neutro-phil to lymphocyte ratio (NLR),platelet-lymphocyte ratio (PLR) and mononuclear lymphocyte ratio (MLR) were lower than those in the non-disease control group.The proportions of squamous cell carcinoma,stage Ⅲ,ECOG score 0-1 point,adverse reactions in the DCB group were higher than those in the non-DCB group (P<0.05),the PLT count,NLR,PLR and MLR were lower than those in the non-DCB group (P<0.05). The ROC curve analysis results showed that PLT,NLR,PLR and MLR could serve as the indicators for pre-dicting DCB,the area under of ROC curve (AUC) was 0.633,0.602,0.635 and 0.604 respectively,the opti-mal cut off values were 187×109/L (P=0.004),5.0 (P=0.026),235 (P=0.003) and 0.35 (P=0.024) re-spectively.The multivariate Cox regression analysis showed that non-squamous carcinoma including adenocar-cinoma (HR=1.565,95%CI:1.057-2.316) and other pathologic types (HR=2.285,95%CI:1.326-3.936),ECOG score 2-3 points (HR=2.375,95%CI:1.652-3.415),AMC≥0.65×109/L (HR=1.847,95%CI:1.160-2.938) and PLR≥235 (HR=1.557,95%CI:1.016-2.386) were the independent risk factors for short PFS.The ECOG score 2-3 points (HR=4.615,95%CI:2.882-7.391),AMC≥0.65×109/L (HR=5.161,95%CI:2.984-8.925) and PLR ≥235 (HR=1.732,95%CI:1.059-2.833) were the independent risk fac-tors for short OS (P<0.05),and having adverse reactions (HR=0.472,95%CI:0.294-0.757) was the independ-ent protective factor for short OS (P<0.05).Conclusion Lower PLT,AMC,NLR,MLR and PLR,higher ALC,squamous cell carcinoma,TNM stage Ⅲ,ECOG score 0-1 point and immunotherapy related adverse reactions could prompt that the prognosis is good in ICIs treating advanced NSCLC.PLT,NLR,PLR and MLR could serve as the indicators for predicting DCB.
