1.Effect of Highly Expressed lysophosphatidyllecithin acyltransferase 4 on Proliferation of Pancreatic Cancer

		                        			ObjectiveTo investigate the expression level of lysophosphatidyllecithin acyltransferase 4 (LPCAT4) in pancreatic cancer and its effect on the proliferation of pancreatic cancer cells. MethodsIn this study, the differentially expressed genes of patients with KRAS mutant and wild-type pancreatic cancer were analyzed by online database LinkedOmics. The LPCAT4 expression in pancreatic cancer tissues was analyzed online by the University of Alabama at Birmingham Cancer Data Analysis (UALCAN), Sangerbox and gene expression profile interaction analysis 2 (GEPIA2). Kaplan-Meier Plotter database was used to explore the correlation between LPCAT4 and the prognosis of patients with pancreatic cancer. The expression of LPCAT4 in human pancreatic cancer cells were detected by quantitative real-time PCR and Western blot analysis. LPCAT4 was knocked down in the high-expressing SW1990 cell line and overexpressed in the low-expressing MIA PaCa-2 cell line. The effects of LPCAT4 expression on cell proliferation were assessed using CCK-8 and EdU assays. STRING and GEPIA2 databases were used to obtain LPCAT4 binding and coexpressed genes in tumors, which were then analyzed by GO and KEGG. ResultsAnalysis of the LinkedOmics online database revealed a significant upregulation of LPCAT4 in patients with KRAS mutant pancreatic cancer compared to patients with KRAS wild-type pancreatic cancer. The online analysis of GEPIA2, UALCAN and Sangerbox 3.0 showed that the expression of LPCAT4 was higher in pancreatic cancer than in normal tissues. Analysis of the Kaplan-Meier Plotter database revealed that high LPCAT4 expression was associated with poorer prognosis in pancreatic cancer patients.Western blot and qPCR results showed that expression of LPCAT4 in pancreatic cancer cell lines was significantly higher than in normal pancreatic ductal epithelial cells. Knockdown of LPCAT4 in SW1990 cells inhibited proliferation, while overexpression in MIA PaCa-2 cells promoted proliferation. Enrichment analysis indicated that LPCAT4 was closely related to sulfur metabolism. ConclusionsLPCAT4 is highly expressed in pancreatic cancer and is associated with poor prognosis of patients. It plays a significant regulatory role in the proliferation of pancreatic cancer cells, with its expression level closely correlated with cell proliferation capacity. These findings reveal the critical role of LPCAT4 in the malignant progression of pancreatic cancer and provide important evidence for its potential as a therapeutic target. 
		                        		
		                        		
		                        		
		                        	
2.Anti-infective treatment for a severe case of Legionella pneumonia:pharmaceutical care and literature analysis

		                        			OBJECTIVE To provide certain therapeutic ideas and references for the pharmaceutical care of severe Legionella pneumonia in anti-infection treatment. METHODS Clinical pharmacists participated in the entire treatment process of a patient with severe Legionella pneumonia, and assisted clinical physicians in evaluating the infecting pathogens using the WUH (Winthrop- University Hospital criteria) scoring system, based on the patient’s clinical symptoms, physical signs, and changes in pulmonary imaging. Leveraging their pharmaceutical expertise, clinical pharmacists recommended a combination of piperacillin sodium and tazobactam with moxifloxacin hydrochloride for anti-infection treatment, and closely monitored the patient’s clinical manifestations. They promptly identified delirium and abnormally elevated levels of lipase, amylase and liver enzymes, and successively suggested adjusting the treatment plan to a combination of piperacillin sodium and tazobactam with doxycycline or azithromycin for anti- infection after analyzing the causes, along with liver protection treatment, enteral nutrition, and parenteral nutrition. Additionally, clinical pharmacists closely monitor the patient’s medication adherence and provide her with medication education. RESULTS The clinical physicians accepted the recommendations of the clinical pharmacists, and the patient improved after treatment and was discharged. A follow-up examination one month later showed no recurrence. CONCLUSIONS Clinical pharmacists, when assisting clinicians in treating severe Legionella pneumonia, not only pay attention to changes in the patient’s clinical symptoms and physical signs, but also closely monitor the adverse reactions of fluoroquinolone, tetracycline, and macrolide antibiotics. They should promptly recognize adverse reactions and provide recommendations for adjusting treatment plans, as well as offer comprehensive pharmaceutical care throughout the patient’s treatment, to ensure the effectiveness and safety of clinical therapy.
		                        		
		                        		
		                        		
		                        	
3.Multiparametric quantitative MRI for diagnosis of thyroid-associated ophthalmopathy complicated with dysthyroid optic neuropathy

		                        			Objective To observe the value of multiparametric quantitative MRI for diagnosis of thyroid-associated ophthalmopathy(TAO)complicated with dysthyroid optic neuropathy(DON).Methods Fifty-five TAO patients with 109 affected eyes were retrospectively enrolled and divided into DON group(22 cases with 44 affected eyes)and non DON group(33 cases with 65 affected eyes)based on complicated with DON or not.Clinical data and multiparametric quantitative MRI indicators were compared between groups.The influencing factors of TAO complicated with DON were screened with logistic regression to establish a model,and the diagnostic efficacy of the model was observed.Results Significant differences of the course of disease,degree of eyeball protrusion,muscle index,as well as the number,thickness,T1 value,T2 value,fat fraction and orbital fat water fraction of thickened extraocular muscle were found between groups(all P<0.05).T1 value and orbital fat water fraction of thickened extraocular muscle were both independent influencing factors of TAO complicated with DON,with the area under the curve(AUC)for diagnosing TAO complicated with DON of 0.859 and 0.868,respectively,and AUC of the combined diagnosis of the two was 0.922,significantly higher than orbital fat water fraction alone(P=0.034)but not significantly different with that of T1 value alone(P=0.851).Conclusion T1 value and orbital fat water fraction of thickened extraocular muscle based on multiparametric quantitative MRI were helpful for diagnosing TAO complicated with DON.

5.Effects of intravenous thrombolysis combined with Xingnaojing injection on intracranial arterial hemodynamic indexes and neurological function in patients with cerebral infarction

		                        			Objective:To investigate the effects of intravenous thrombolysis combined with Xingnaojing injection on hemodynamic indexes and neurological function in patients with cerebral infarction. Methods:A total of 142 patients with cerebral infarction who were treated in Xing An Meng Hospital from April 2020 to May 2021 were included in this study. They were randomly divided into a control group ( n = 71, intravenous thrombolysis) and a Xingnaojing injection group ( n = 71, intravenous thrombolysis + Xingnaojing injection). Intracranial arterial hemodynamic indexes, National Institutes of Health Stroke Scale score, Fugl-Meyer Assessment Scale score, serum inflammatory factors, oxidative stress indexes, brain injury markers, and the incidence of adverse reactions were compared between the two groups. Results:After treatment, serum levels of interleukin-1β, interleukin-6, and tumor necrosis factor-α were significantly lower in the Xingnaojing injection group than the control group [interleukin-1β: (4.05 ± 0.83) ng/L vs. (6.85 ± 1.02) ng/L, interleukin-6: (43.61 ± 5.14) ng/L vs. (60.31 ± 7.04) ng/L, tumor necrosis factor-α: (35.93 ± 4.25) ng/L vs. (20.93 ± 3.11) ng/L, t = 17.94, 16.14, 15.37, all P < 0.001]. After treatment, the mean blood flow velocities of the anterior cerebral artery, middle cerebral artery, and posterior cerebral artery in the Xingnaojing injection group were significantly higher than those in the control group [anterior cerebral artery: (49.36 ± 5.28) cm/s vs. (41.15 ± 5.12) cm/s, middle cerebral artery: (61.27 ± 7.02) cm/s vs. (50.19 ± 6.08) cm/s, posterior cerebral artery: (44.92 ± 5.63) cm/s vs. (37.26 ± 4.93) cm/s, t = 9.40, 10.05, 8.62, all P < 0.001]. After treatment, the National Institutes of Health Stroke Scale score and Fugl-Meyer Assessment Scale score in the Xingnaojing injection group were superior to those in the control group [National Institutes of Health Stroke Scale score: (10.36 ± 1.52) points vs. (14.62 ± 2.05) points, Fugl-Meyer Assessment Scale score: (76.19 ± 8.08) points vs. (65.28 ± 7.14) points, t = 14.06, 8.52, both P < 0.05]. After treatment, the serum level of malondialdehyde in the Xingnaojing injection group was significantly higher than that in the control group [(6.35 ± 1.02) μmol/L vs. (10.05 ± 1.63) μmol/L), t = 16.21, P < 0.001]. The serum level of superoxide dismutase in the Xingnaojing injection group was significantly lower than that in the control group [(114.31 ± 13.69) U/L vs. (92.25 ± 10.16) U/L), t = 10.90, P < 0.001]. Serum levels of neuron-specific enolase and S100β in the Xingnaojing injection group were significantly lower than those in the control group [neuron-specific enolase: (24.01 ± 3.24) IU/L vs. (30.31 ± 4.02) IU/L, S100β: (0.73 ± 0.17) ng/L vs. (1.13 ± 0.22) ng/L, t = 10.28, 12.12, both P < 0.001). There was a significant difference in the incidence of adverse reactions between the two groups ( P > 0.05). Conclusion:Intravenous thrombolysis combined with Xingnaojing injection for the treatment of cerebral infarction can improve intracranial hemodynamics, reduce the inflammatory response and oxidative stress, and alleviate brain tissue injury. 		                        			1,2,3-trichloropropane (TCP) is an industrially synthesized aliphatic chlorinated hydrocarbon and an intermediate product in the industrial production of epichlorohydrin, which can be used as a precursor for the manufacture of soil fumigant and organic solvents. Due to its biological toxicity, environmental persistence and strong environmental migration ability, 1,2,3-TCP is listed as an emerging organochlorine pollutant in the environment and regulated by many international organizations. Currently, the degradation of 1,2,3-TCP and the remediation of 1,2,3-TCP-contaminated sites receive great attention, but the degradation mechanism of 1,2,3-TCP has not been summarized in depth. This article discussed the origin of 1,2,3-TCP, its environmental impact and ecological effects, and the physical and chemical degradation techniques. This was followed by summarizing the degradation mechanisms of 1,2,3-TCP (e.g., aerobic co-biodegradation, anaerobic biodegradation). Specially, the pathways and mechanisms of microbial biodegradation and transformation of 1,2,3-TCP in anoxic environments (e.g., groundwater) were thoroughly reviewed. The feasibility of using 1,2,3-TCP as an electron acceptor by organohalide-respiring bacteria under anoxic conditions was predicted based on thermodynamic analysis. It is also an immune organ and plays an important role in maintaining anti-infection, autoimmune stability, and anti-tumor. In particular, the liver has unique immunological advantages. Its immune cells can maintain the liver's immune homeostasis and participate in immunoregulation. A variety of immunotherapy is used in clinical trials for the treatment of difficult and critical liver diseases. 8.Effect of Cardiovascular Family-Care APP on Self-management Behavior of Patients with Coronary Heart Disease

		                        			Objective? To explore the effect of cardiovascular family-care APP on self-management behavior of patients with coronary heart disease. Methods? The hospitalized patients with coronary heart disease who were admitted in the department of cardiovascular medicine of a Class Ⅲ Grade A hospital from May 2016 to May 2018 were recruited by non-probability sampling method. All the research subjects were randomly divided into intervention group and control group by random number table. The intervention group consisted of 143 cases and control group consisted of 147 cases. The intervention lasted 12 months. The intervention group received health management of cardiovascular family-care APP, while the control group received routine discharged-guidance and outpatient clinic follow-up. Self-management behavior and risk factors of coronary heart disease were evaluated at baseline and 6 months and 12 months after intervention. Results? At 6 months after intervention, the total score of self-management behavior, medical management and emotional management in the intervention group were significantly higher than those in the control group, with statistical significance (P<0.05). At 12 months after intervention, the total score of self-management behavior and medical management in intervention group were significantly higher than those in control group, with statistical significance (P<0.05). After 6 months of intervention, BMI, alcohol consumption, blood pressure control of the intervention group were better than those of the control group, with statistical significance (P<0.05). After 12 months of intervention, the control of blood pressure in the intervention group was better than that in the control group (P< 0.05). Conclusions? Cardiovascular family-care APP can promote the establishment of good self-management behavior in patients with coronary heart disease. 9.The comparative study of nursing practice before and after Joint Commission International accreditation

		                        			Objective To investigate the influence of Joint Commission International (JCI) accreditation on nursing practice.Methods Totally 30 medical institutions which had passed the JCI accreditation were investigated to compare the scores of nursing practice before and after JCI accreditation.Results Before JCI accreditation,the average standardized score of governance,leadership and management dimension was 81.20,but the scores of other dimensions were below 80.After the JCI accreditation,the quality improvement and patient safety(QPS) dimension scored 81.10,and scores of other dimensions were all above 90.The differences of all dimensions were statistically significant (P<0.001) before and after the JCI accreditation,the scores were all much greater than those before the accreditation,and improvement of AOP(77.37%)and IPSG(69.86%)was most obvious with statistical significance (P<0.001).Conclusion JCI accreditation plays a certain role in promoting nursing practice,making nursing process more systematic and comprehensive and the safety of nursing guaranteed.
		                        		
		                        		
		                        		
		                        	
10.Correlation between microalbuminuria and short-term outcome in patients with acute ischemic stroke

		                        			ObjectiveTo investigate the relationship between microalbuminuria (MAU) and short-term outcome in patients with acute ischemic stroke.MethodsThe consecutive patients with acute ischemic stroke admitted to hospital were enrolled prospectively.The first urine specimen was taken on the following morning after admission for detecting urine albumin/creatinine ratio (UACR).UACR 30-300 mg/g was defined as MAU positive.Stroke severity was evaluated with the National Institutes of Health Stroke Scale (NIHSS) at admission and the modified Rankin Scale (mRS) was used to evaluate functional outcome at discharge, and good outcome was defined as mRS score of 0 to 2.ResultsA total of 244 patients with acute ischemic stroke were enrolled, including MAU positive in 53 patients (27.12%), and poor outcome in 67 patients (27.50%).Univariate analysis showed that age, baseline NIHSS score, systolic blood pressure, fasting blood glucose, globulin, D-dimer, white blood cell count, neutrophils, and the proportions of ischemic heart disease in patients of the MAU positive group were significantly higher than those of the MAU negative group (all P<0.05).Multivariate logistic regression analysis showed that MAU (odds ratio [OR] 1.520, 95% confidence interval [CI] 1.151-1.794;P=0.031), baseline NIHSS score (OR 1.570,95% CI 1.357-1.808;P<0.001) were the independent risk factors for short-term poor outcome in patients with acute ischemic stroke.ConclusionsThe incidence of MAU is high in patients with acute ischemic stroke.MAU positive can be used as one of the independent predictors of short-term poor outcome in patients with acute ischemic stroke.
