1.Efficacy and safety of endoscopic retrograde cholangiopancreatography combined with oral cholangiopancreatography in the treatment of duodenal papilla cholecystectomy

ObjectiveTo investigate the feasibility and safety of endoscopic retrograde cholangiopancreatography (ERCP) combined with oral cholangiopancreatography in the treatment of major duodenal papilla gallbladder polyps. MethodsA retrospective analysis was performed for the clinical data of eight patients with choledocholithiasis and gallbladder polyps who underwent ERCP and combined with oral cholangiopancreatography for major duodenal papilla cholecystectomy in Center of Digestive Endoscopy, Jilin People’s Hospital, from May 2022 to June 2024, and related data were collected, including the success rate of surgery, the technical success rate of gallbladder polyp removal, the superselective method of cystic duct, the time of operation, the time of gallbladder polyp removal, and surgical complications. ResultsBoth the success rate of surgery and the technical success rate of gallbladder polyp removal reached 100%, and of all eight patients, three patients used guide wire to enter the gallbladder under direct view, while five patients received oral cholangiopancreatography to directly enter the gallbladder. The time of operation was 51.88±12.34 minutes, and the time of gallbladder polyp removal was 23.13±10.94 minutes. The diameter of gallbladder polyp was 2 — 8 mm, and pathological examination showed inflammatory polyps in three patients, adenomatous polyps in one patient, and cholesterol polyps in four patients. There were no complications during or after surgery. The patients were followed up for 2 — 27 months after surgery, and no recurrence of gallbladder polyp was observed. ConclusionOral cholangiopancreatography is technically safe and feasible in endoscopic major duodenal papilla cholecystectomy.

Objective:To study the current status of longitudinal extrauterine growth restriction (EUGR) in extremely preterm infants (EPIs) and to develop a prediction model based on clinical data from multiple NICUs.Methods:From January 2017 to December 2018, EPIs admitted to 32 NICUs in North China were retrospectively studied. Their general conditions, nutritional support, complications during hospitalization and weight changes were reviewed. Weight loss between birth and discharge > 1SD was defined as longitudinal EUGR. The EPIs were assigned into longitudinal EUGR group and non-EUGR group and their nutritional support and weight changes were compared. The EPIs were randomly assigned into the training dataset and the validation dataset with a ratio of 7∶3. Univariate Cox regression analysis and multiple regression analysis were used in the training dataset to select the independent predictive factors. The best-fitting Nomogram model predicting longitudinal EUGR was established based on Akaike Information Criterion. The model was evaluated for discrimination efficacy, calibration and clinical decision curve analysis.Results:A total of 436 EPIs were included in this study, with a mean gestational age of (26.9±0.9) weeks and a birth weight of (989±171) g. The incidence of longitudinal EUGR was 82.3%(359/436). Seven variables (birth weight Z-score, weight loss, weight growth velocity, the proportion of breast milk ≥75% within 3 d before discharge, invasive mechanical ventilation ≥7 d, maternal antenatal corticosteroids use and bronchopulmonary dysplasia) were selected to establish the prediction model. The area under the receiver operating characteristic curve of the training dataset and the validation dataset were 0.870 (95% CI 0.820-0.920) and 0.879 (95% CI 0.815-0.942), suggesting good discrimination efficacy. The calibration curve indicated a good fit of the model ( P>0.05). The decision curve analysis showed positive net benefits at all thresholds. Conclusions:Currently, EPIs have a high incidence of longitudinal EUGR. The prediction model is helpful for early identification and intervention for EPIs with higher risks of longitudinal EUGR. It is necessary to expand the sample size and conduct prospective studies to optimize and validate the prediction model in the future.
3.Bacterial culture and its clinical analysis of patients undergoing transnasal skull base surgery

Objective To analyze the status of the bacteria in the operative area of transnasal skull base surgery and its correlation with postoperative intracranial infection.Methods The procedure of transnasal skull base surgery was divided into three stages:nasal passage preparation(stage 1),tumor resection(stage 2),and skull base reconstruction(stage 3).Bacterial sampling was taken from the mucosa of the anterior wall of sphenoid sinus or clival recess of sellar floor at the beginning of each stage;and the positive rate of bacterial culture in different stages of operation and its correlation with postoperative intracranial infection were analyzed.Results A total of 105 patients were enrolled in this study,and 315 samples were taken.The average time point of sampling in the three stages was 20.3,45.1 and 131.3 min after the beginning of operation,respectively.The positive results were 9 cases(2.9%)in the stage 1,8 cases(2.5%)in the stage 2,and 23 cases(7.3%)in the stage 3,which were 24 cases of Staphylococcus epidermidis,7 cases of Staphylococcus aureus,3 cases of hemolytic streptococcus,2 cases of Klebsiella pneumonia,and 4 cases of Escherichia coli.There was no significant difference in the positive cases between stage 1 and stage 2(P=0.955),but there were significant differences between stage 1 or 2 and stage 3(P=0.013;P=0.007).There were 36(11.4%)patients with at least one positive result in the three stages,17(16.2%)with cerebrospinal fluid leakage,and 12(11.4%)with intracranial infection.The risk of intracranial infection was 3.1 times higher in patients with positive bacterial culture than patients with negative bacterial culture(OR=3.1,95%CI:0.9-10.6),which was not statistically significant;patients with CSF leakage were 61.4 times higher than those without CSF leakage(OR=61.4,95%CI:11.2-337.1),which was statistically significant(P<0.001).The consistency rate of bacteria in nasal cavity and postoperative cerebrospinal fluid culture was 57.1%.Conclusion The positive rate of bacterial culture in the operative area of transnasal skull base surgery increases significantly with the extension of operation time,which is a potential risk index of postoperative intracranial infection.

Familial glucocorticoid deficiency type 4(FGD4) is a rare autosomal recessive disorder caused by mutations in the nicotinamide nucleotide transhydrogenase(NNT) gene. The article presented clinical data, laboratory results, and genetic mutation findings of a child with FGD4. Additionally, a retrospective analysis of the clinical data of FGD4 patients reported domestically and internationally was conducted, summarizing the types of gene mutations and clinical characteristics. This case identifies novel mutation sites in the NNT gene, providing a basis for the early diagnosis and treatment of FGD4 patients.

6.Effects of intravenous thrombolysis combined with Xingnaojing injection on intracranial arterial hemodynamic indexes and neurological function in patients with cerebral infarction

Objective:To investigate the effects of intravenous thrombolysis combined with Xingnaojing injection on hemodynamic indexes and neurological function in patients with cerebral infarction. Methods:A total of 142 patients with cerebral infarction who were treated in Xing An Meng Hospital from April 2020 to May 2021 were included in this study. They were randomly divided into a control group ( n = 71, intravenous thrombolysis) and a Xingnaojing injection group ( n = 71, intravenous thrombolysis + Xingnaojing injection). Intracranial arterial hemodynamic indexes, National Institutes of Health Stroke Scale score, Fugl-Meyer Assessment Scale score, serum inflammatory factors, oxidative stress indexes, brain injury markers, and the incidence of adverse reactions were compared between the two groups. Results:After treatment, serum levels of interleukin-1β, interleukin-6, and tumor necrosis factor-α were significantly lower in the Xingnaojing injection group than the control group [interleukin-1β: (4.05 ± 0.83) ng/L vs. (6.85 ± 1.02) ng/L, interleukin-6: (43.61 ± 5.14) ng/L vs. (60.31 ± 7.04) ng/L, tumor necrosis factor-α: (35.93 ± 4.25) ng/L vs. (20.93 ± 3.11) ng/L, t = 17.94, 16.14, 15.37, all P < 0.001]. After treatment, the mean blood flow velocities of the anterior cerebral artery, middle cerebral artery, and posterior cerebral artery in the Xingnaojing injection group were significantly higher than those in the control group [anterior cerebral artery: (49.36 ± 5.28) cm/s vs. (41.15 ± 5.12) cm/s, middle cerebral artery: (61.27 ± 7.02) cm/s vs. (50.19 ± 6.08) cm/s, posterior cerebral artery: (44.92 ± 5.63) cm/s vs. (37.26 ± 4.93) cm/s, t = 9.40, 10.05, 8.62, all P < 0.001]. After treatment, the National Institutes of Health Stroke Scale score and Fugl-Meyer Assessment Scale score in the Xingnaojing injection group were superior to those in the control group [National Institutes of Health Stroke Scale score: (10.36 ± 1.52) points vs. (14.62 ± 2.05) points, Fugl-Meyer Assessment Scale score: (76.19 ± 8.08) points vs. (65.28 ± 7.14) points, t = 14.06, 8.52, both P < 0.05]. After treatment, the serum level of malondialdehyde in the Xingnaojing injection group was significantly higher than that in the control group [(6.35 ± 1.02) μmol/L vs. (10.05 ± 1.63) μmol/L), t = 16.21, P < 0.001]. The serum level of superoxide dismutase in the Xingnaojing injection group was significantly lower than that in the control group [(114.31 ± 13.69) U/L vs. (92.25 ± 10.16) U/L), t = 10.90, P < 0.001]. Serum levels of neuron-specific enolase and S100β in the Xingnaojing injection group were significantly lower than those in the control group [neuron-specific enolase: (24.01 ± 3.24) IU/L vs. (30.31 ± 4.02) IU/L, S100β: (0.73 ± 0.17) ng/L vs. (1.13 ± 0.22) ng/L, t = 10.28, 12.12, both P < 0.001). There was a significant difference in the incidence of adverse reactions between the two groups ( P > 0.05). Conclusion:Intravenous thrombolysis combined with Xingnaojing injection for the treatment of cerebral infarction can improve intracranial hemodynamics, reduce the inflammatory response and oxidative stress, and alleviate brain tissue injury. The combined therapy is beneficial to protect the neurological function of patients with cerebral infarction and is highly safe.
7.Analysis of the rectum and bladder changes in prostate precise radiotherapy under the bowel and bladder preparation

Objective:To analyze the clinically acceptable and reproducible bladder and rectum volumes of prostate cancer patients during radiotherapy under bladder and bowel preparation, aiming to provide quantitative indicators for bowel and bladder preparation before and after radiotherapy.Methods:Clinical data of 275 prostate cancer patients with strict bladder and bowel preparation and completion of whole course radical radiotherapy at Sun Yat-sen University Cancer Center from April 2015 to December 2020 were retrospectively analyzed. Patients were scanned with cone beam CT (CBCT) before each treatment and the setup error was recorded. Sixty-six patients were selected by simple random sampling and the bladder and rectum on daily CBCT was outlined using MIM software. The relationship between the ratio of daily bladder or rectum volume to the planned bladder or rectum volume (relative value of volume) and setup error was analyzed. Quantitative data were expressed as mean±SD. Normally distributed data were analyzed by paired t-test while non-normally distributed data were assessed by Kruskal-Wallis test.Results:The bladder and rectum volume on planning CT were (370.87±110.04) ml and (59.94±25.07) ml of 275 patients. The bladder and rectum volumes on planning CT were (357.51±107.38) ml and (65.28±35.37) ml respectively of the 66 selected patients with 1611 sets of CBCT images. And the bladder and rectum volumes on daily CBCT were (258.96±120.23) ml and (59.95 ± 30.40) ml. The bladder volume of patients was decreased by 3.59 ml per day on average during the treatment and 0.37 ml for the rectum volume. According to the bladder volume on planning CT, all patients were divided into three groups: <250 ml, 250-450 ml and >450 ml groups. The relative value of volume in the 250-450 ml group during the course of radiotherapy was the smallest. And the setup error in the superior and inferior (SI) direction was (0.28±0.24) cm and (0.19±0.17) cm in the left and right (LR) direction, significantly lower than those in the other two groups (both P≤0.027). According to the rectum volume on planning CT, all patients were divided into four groups: <50 ml, 50-<80 ml, 80-120 ml and >120 ml groups. The <50 ml group had the smallest relative value of volume during radiotherapy, and the setup error in the SI direction was (0.26±0.22) cm and (0.24±0.22) cm in the anterior and posterior (AP) direction, significantly smaller than those in the other groups (both P≤0.003). The setup errors in the SI, LR, AP directions of the enrolled 66 patients were (0.30±0.25) cm, (0.20±0.18) cm and (0.28±0.27) cm, respectively. Among them, the relative value of bladder volume in the AP direction was (0.73±0.37) in the setup error <0.3 cm group, which was statistically different from those in the setup error 0.3-0.5 cm and >0.5 cm groups (both P<0.05). Conclusion:Under the bladder and bowel preparation before planning CT, the appropriate bladder and rectum volumes are in the range of 250-450 ml and <50 ml, which yields higher reproducibility and smaller setup error.
8.Anticoagulant effectiveness and safety evaluation of a modified heparin anticoagulant method in children treated with hemoperfusion

Objective To examine the anticoagulant effectiveness and safety of a modified heparin anticoagulant method with those of the standard hepair anticoagulant method in children treated with hemoperfusion,and to provide a basis for optimizing the anticoagulant protocol of hemoperfusion treatment.Methods Twenty-three children treated with a total of 41 sessions of hemoperfusion from June 2021 to February 2022 in Department of Pediatric Nephrology of West China Second Hospital of Sichuan University were selected as research objects by convenient sampling method.The participants were divided into a standard anticoagulation group and an improved anticoagulation method group according to the different heparin-anticoagulation schemes.A total of 11 children in the standard anticoagulation group received 19 sessions of hemoperfusion treatment using the standard heparin anticoagulation scheme,i.e,the first dose of 0.5-1.0 mg/kg heparin was given intravenously,5-10 minates before hemoperfusion,followed by continuous intravenous infusion of 0.2-0.5 mg·kg-1·h-1 heparin which was expected to be stopped 30 minutes before the ending of hemoperfusion.A total of 12 patients in the improved anticoagulation method group underwent 22 sessions of hemoperfusion treatment using an improved heparin anticoagulant method,i.e,intravenous injection 1 mg/kg of heparin before the start of treatment without additional administrations of heparin thereafter.The changes in activated partial thromboplastin time(APTT)at 30,60,90 minutes,and the end of treatment in two groups of children,as well as the coagulation status of the two groups of pipelines and filters,together with the bleeding situation of the children,were monitored.The differences in platelet count(PLT)and recovery of APTT 1 hour after treatment between the two groups of children were compared.Results Compared with the standard anticoagulation method,the improved anticoagulation method showed 68.8%lower APTT value exceeding 300 seconds[β =-1.166,odds ratio(OR)= 0.312,95%confidence interval(95%CI)was 0.125-0.775,P = 0.012]was safer than standard method,and both of them showed good anticoagulation effects at 30,60,90 minutes,and at the end of treatment.The anticoagulation effect of heparin was not influenced by sex(P = 0.179)as well as age(P = 0.821).The improved anticoagulation method group showed better APTT recovery 1 hour after treatment than the standard anticoagulation group(11/22 vs.3/19,P<0.05).Both anticoagulation methods did not show any coagulation in the pipeline or filter during the treatment process,and the patient did not experience active bleeding within 24 hours after the end of treatment.Conclusion The improved anticoagulant method can achieve anticoagulant effectiveness comparable to that of standard anticoagulation method,is safer than standard method,and at the same time,simplifies the operational process,thereby could reduce the risk of blood borne infections.
9.Current situation and countermeasures of the construction of training bases for gerontological nurse specialists in China

Objective:To understand the current situation of the construction of provincial level and above gerontological nurse specialists (GNSs) training bases in China, including the establishment of bases, training program settings, admission criteria for GNSs, and requirements for training teachers, so as to provide reference for optimizing the training of GNSs.Methods:This study is a cross-sectional survey. From August to October 2021, a self-designed questionnaire was used to investigate the geriatric nursing committees of nursing associations at provincial level and above. A total of 32 electronic questionnaires were distributed to nursing associations in 31 provinces/autonomous regions/municipalities directly under the Central Government and the Geriatric Nursing Committee of the Chinese Nursing Association, and 32 valid questionnaires were recovered, with an effective recovery rate of 100.0% (32/32) .Results:In China, 25 nursing associations at or above the provincial level set up training bases for GNSs, and about 3 400 GNSs were trained. 80.0% (20/25) of training bases required a minimum education level of tertiary education and a minimum professional title of junior nurse for GNS trainees. 64.0% (16/25) of training bases required a minimum of 5 working years for GNS trainees. More than half of the training bases required training teachers with a bachelor's degree or above, and a professional title of supervisor nurse or above.Conclusions:The training base for GNSs in China is in a stage of exploration and rapid development. The training system, student admission criteria, and teacher requirements for GNSs among various training bases have not yet formed a unified standard, and further standardization are needed.
10.Effects of LEARNS health education model in outpatient colonoscopy patients

Objective:To explore the effectiveness of listen, establish, adopt, reinforce, name, strengthen (LEARNS) health education model in outpatient colonoscopy patients.Methods:From March to August 2022, 90 outpatient patients who underwent colonoscopy examination at the Endoscopy Center of Peking University First Hospital were selected as the study subject using convenience sampling. The patients were divided into a control group and an experimental group using a random number table method, with 45 patients in each group. The control group adopted conventional health education, while the experimental group adopted the LEARNS health education model on the basis of the control group. We compared the scores of bowel preparation knowledge, bowel preparation status, and satisfaction with health education between two groups of patients.Results:After intervention, the bowel preparation knowledge and satisfaction with health education in the experimental group were higher than those in the control group, with statistically significant differences ( P<0.01). There was no statistically significant difference in bowel preparation ( P>0.05) . Conclusions:The LEARNS health education model can effectively improve the knowledge of bowel preparation and satisfaction with health education of outpatient colonoscopy patients.
