1.New di-spirocyclic labdane diterpenoids from the aerial parts of Leonurus japonicus.

Phytochemical investigation on the ethanol extract of a well-known medicinal herb Leonurus japonicus, led to the separation of 18 labdane type diterpenoids (1-18). Through comprehensive spectroscopic analyses and quantum chemical calculations, these compounds were structurally characterized as six new interesting 5,5,5-di-spirocyclic ones (1-6), two new (7 and 8) and six known (13-18) interesting 6,5,5-di-spirocyclic ones, a new rare 14,15-dinor derivative (9), and three new ones incorporating a γ-lactone unit (10-12). An in vitro neuroprotective assay in RSC96 cells revealed that compounds 7 and 12 exhibited neuroprotective activity in a concentration-dependent way, comparable to the reference drug N-acetylcysteine.

2.Study on Protective Mechanism of Dexmedetomidine on Cerebral Injury in Sepsis Model Rats Based on SIRT 1/ Akt/GSK3β/β-catenin Signaling Pathway

OBJECTIVE:To in vestigate the effects of dexmedetomidine (Dex)on SIRT 1/Akt/GSK3β/β-catenin signaling pathway in cerebral injury of sepsis model rats ,and explore the mechanism of its protecitve effect on cerebral injury. METHODS : A total of 80 male SD rats were randomly divided into sham operation group (Sham group ),sepsis group (CLP group ),CLP+Dex group(10 μg/kg Dex),CLP+Dex+Sirtinol group (10 μg/kg Dex+2 μL/100 g SIRT 1 inhibitor sirtinol ),with 20 mice in each group. Two hours before modeling ,CLP+Dex+Sirtinol group was injected with sirtinol via lateral ventricle. Sepsis model was induced by cecal ligation and perforation in each group (in sham group ,only operation was performed but no ligation was performed). At 0,3,6 h after modeling ,CLP+Dex group and CLP+Dex+Sirtinol group were given Dex (10 μg/kg) intraperitoneally,Sham group and CLP group were given constant volume of normal saline intraperitoneally. Cerebral tissue water content,Evans blue (EB)content,apoptosis in cerebral cortex ,the levels of IL- 1β and TNF-α in cerebral tissue as well as the protein expression of SIRT 1,p-Akt,p-GSK3β and β-catenin in hippocampus were detected 24 h after last medication. RESULTS : Compared with Sham group ,cerebral tissue water content ,EB content ,the number of apoptotic cells in cerebral cortex as well as the levels of IL- 1β and TNF-α in cerebral tissue were increased significantly(P<0.05),while the protein expression of SIRT 1, p-Akt,p-GSK3β and β-catenin in hippocampus were decreased significantly (P<0.05). Compared with CLP group ,cerebral tissue water content ,EB content ,the number of apoptotic cells in cerebral cortex as well as the levels of IL- 1β and TNF-α in cerebral tissue were decreased significantly in CLP+Dex group (P<0.05),while the protein expression of SIRT 1,p-Akt,p-GSK3β and β-catenin in hippocampus were increased significantly (P<0.05). Sirtinol could significantly reverse the above-mentioned cerebral protection and factor regulation effects of Dex (P<0.05). CONCLUSIONS :Dex can protect the cerebral tissue of sepsis model rats,which may play an anti-inflammatory and anti-apoptotic role by activating SIRT 1/Akt/GSK3β/β-catenin signaling pathway ,so as to reduce cerebral edema ,protect blood-brain barrier and reduce cerebral injury.
3. Analysis of adverse reaction induced by articaine hydrochloride and epinephrine tartrate injection in 75 cases

Objective:
To investigate the causes and clinical manifestation of adverse reaction of articaine hydrochloride and epinephrine tartrate injection.
Methods:
A retrospective analysis was conducted on the adverse drug reactions (ADR) of local anesthetic articaine hydrochloride and epinephrine tartrate injection.
Results:
In 75 cases of adverse reactions, there were 40 cases of female and 35 cases of male. Adverse reactions occured more frequently at the age of 3-10 [33% (25/75)] and 1-10 min and one day after injection, respectively accounting for 20% (15/75), and two days, accounting for 15% (15/75), 10-21 days accounting for 8% (6/75). The main manifestations were injection site ulcers, followed by skin reactions such as pain, swelling, necrosis and pruritus at the injection site.
Conclusions
The main adverse reactions of articaine hydrochloride and epinephrine tartrate injection are the injection site ulceration, followed by injection site pain, rash, pruritus and drowsiness, nausea and dizziness, palpitations, sweat and hypotension. Doctors should ask the medical history in detail and pay close attention to the patient's medication safety.
4.Effects of home enteral nutrition for post-operative patients of esophageal cancer

Objective To explore the method and efficacy of home enteral nutrition for patients undergoing radical resection of esophagus cancer. Methods A total of 85 patients who underwent radical surgery for esophageal cancer and jejunum fistula from January 2016 to March 2017 were enrolled and were divided into observation group (n=44) and control group (n=41) by random number table method. Patients of control group took normal family diet orally after nutrition assessment and jejunum fistula clipped while patients of observation group received extra special immune enteral nutrition preparations for tumor through fistula of jejunostomy under the guidance of a nutrition support team. After one month treatment, plasma nutrition indicators and European Screening for Nutrition Risk (NRS 2002) assessment were performed to analyze the difference of two groups to compare the nutrition status. Results All indicators did not differ when discharged for two groups (P>0.05). After one month intervation, level of hemoglobin, total serum protein, albumin, prealbumin and transferrin of the observation group were statistically higher than those of control group [(12.90±1.33) vs. (10.83±1.35)g/dl, (72.22±4.83) vs. (63.90±5.44)g/L, (41.63±2.67) vs. (36.98±3.69)g/L,(217.70±56.72) vs. (166.27±39.97)mg/L,(2.41±0.38) vs. (1.76±0.47)g/L; P< 0.01]. Percentage of patients with high nutritional risk (NRS 2002 ≥3) in the observation group was lower than that of the control group (45.5% vs. 68.3%; P<0.05). Conclusions Nutritional support at home after discharge could improve the nutritional status and reduce the risk of malnutrition for patients who underwent radical surgery for esophageal cancer.
5.Simultaneous Determination of 7 Active Constituents in Tangshen Qingdu Granule by HPLC-DAD

OBJECTIVE:To establish a method for the simultaneous determination of 7 active constituents in Tangshen qing-du granule. METHODS:HPLC was performed on the column of SHIMADZU Inert Sustain C18 with mobile phase of acetonitrile-0.1%phosphoric acid at a flow rate of 1.0 mL/min,detection wavelength was 327 nm for chlorogenic acid and caffeic acid,280 nm for baicalin,228 nm for arctiin and 276 nm for wogonoside,baicalein and wogonin,column temperature was 35℃,and injection volume was 10 μL. RESULTS:The linear range was 4.830-154.6 μg/mL for chlorogenic acid and(r=0.9998),0.750-24.1 μg/mL for caffeic acid(r=0.9997),22.859-731.5 μg/mL for baicalin(r=0.9997),8.491-271.7 μg/mL for arctiin(r=0.9993),2.471-79.0μg/mL for wogonoside(r=0.9996),6.656-213.0 μg/mL for baicalein(r=0.9994) and 2.756-88.2 μg/mL for wogonin (r=0.9998);RSDs of precision,stability and reproducibility tests were lower than 2.0%,recoveries were 96.86%-100.82%(RSD=1.46%,n=6),98.79%-101.09%(RSD=0.93%,n=6),97.57%-101.51%(RSD=1.37%,n=6),97.76%-99.63%(RSD=0.77%,n=6),97.99%-100.12%(RSD=0.76%,n=6),96.54%-101.07%(RSD=1.87%,n=6) and 96.60%-99.59%(RSD=1.14%,n=6). CONCLUSIONS:The method is simple with good precision,stability and reproducibilty,and can be used for the simultaneous determination of 7 active constituents in Tangshen qingdu granule.
6.Study on Water Extraction Technology of Yinju Jiedu Oral Liquid

OBJECTIVE:To optimize the water extraction technology of Yinju jiedu oral liquid,and provide reference for the industrial production of the preparation. METHODS:According to the investigation of extraction time-extraction rate curves of chlo-rohenic acid of Yinju jiedu formula and extraction rate of chlorohenic acid in Lonicera japonica and other combined medicinal mate-rials in the formula,decoction methods and time of L. japonica were determined. Using the comprehensive scores of linarin,harpa-goside,(R,S)-epigoitrin,psoralen+angelicin contents and dry extraction yield as indexes,L9(34)orthogonal test was designed to detect the effects of adding water amount,decoction time times and optimize the extraction technology of the residues and other me-dicinal materials. Verification test was conducted. RESULTS:The optimal technology was L. japonica decocted first for 30 min with 8-fold water;the residues and other medicinal materials were decocted with 8-fold water for 3 times,1 h each time;combin-ing all the syrups. In verification test,the average contents of chlorohenic acid,linarin,harpagoside,(R,S)-epigoitrin,psoralen+angelicin were respectively 34.51,10.31,1.97,0.21,9.79 mg/g(RSD=1.24%,1.19%,1.40%,1.71%,1.28%,n=3);aver-age dry extraction yield was 25.4%(RSD=1.64%,n=3);average extraction rate of chlorohenic acid was 78.95%(RSD=1.24%,n=3). CONCLUSIONS:In the optimized water extraction technology,both the extraction rate of chlorohenic acid and contents of other ingredients are relatively high. The technology is stable and feasible.
8.Effects of sleep deprivation on polysomnography and executive function in patients with depression.

BACKGROUNDSleep deprivation (SD) has been used in treatment of depression disorder, and could effectively improve the patients' depressive symptoms.The aim of the study was to explore the effects of SD on electroencephalographic (EEG) and executive function changes in patients with depression.
METHODSEighteen depression patients (DPs) and 21 healthy controls (HCs) were enrolled in the present study. The whole night polysomnography (PSG) was recorded by Neurofax-1518K (Nihon Kohden, Japan) system before and after 36 hours of SD. The level of subjects' depression state was assessed by Visual Analogue Scale (VAS), and the executive function was assessed by Wisconsin Card Sorting Test (WCST).
RESULTSSignificantly decreased sleep latency (SL; before SD: (31.8 ± 11.1) minutes, after SD: (8.8 ± 5.2) minutes, P < 0.01) and REM sleep latency (RL; before SD: (79.8 ± 13.5) minutes, after SD: (62.9 ± 10.2) minutes, P < 0.01) were found after SD PSG in depression patients. Decreased Stage 1 (S1; before SD: (11.7 ± 2.9)%, after SD: (7.3 ± 1.1)%, P < 0.01) and Stage 2 (S2, before SD: (53.8 ± 15.5)%, after SD: (42.3 ± 14.7)%, P < 0.05) of non-rapid eye movement (NREM) sleep, and increased Stage 3 (S3, before SD: (11.8 ± 5.5)%, after SD: (23.6 ± 5.8)%, P < 0.01) and Stage 4 (S4, before SD: (8.8 ± 3.3)%, after SD: (27.4 ± 4.8)%, P < 0.01) NREM sleep were also found. After SD, the depression level in patients decreased from 6.7 ± 2.1 to 2.9 ± 0.7 (P < 0.01). In WCST, the patients showed significantly decreased Response errors (Re, before SD: 22.3 ± 2.4, after SD: 18.3 ± 2.7, P < 0.01) and Response preservative errors (Rpe, before SD: 11.6 ± 3.6, after SD: 9.3 ± 2.9, P < 0.05). Depression patients' RE (t = 2.17, P < 0.05) and Rpe (t = 2.96, P < 0.01) also decreased significantly compared to healthy controls.
CONCLUSIONSD can improve depression symptom and executive function in depression patients.

Objective To observe the influence of simulated microgravity on rat islet. Methods Isolated islet were assigned to flask-culture or bioreactor-culture. Gross structure and ultrastructure of islet were observed by transmission electron microscopy (TEM) and scanning electron microscopy (SEM). Results Islets ultrastructure on 7th day in bioreactor closely resembled fresh islets,with well-formed secretory granules and abundant mitochondria. SEM showed under microgravity islets communicating each other with cavity-like areas. Conclusions The ultrastructure of islets cultured under microgravity closely resembled fresh islets.
