1.Expression of lncRNA SNHG8 in placenta accreta and its effect on trophoblast invasion and migration

Objective To investigate the expression of lncRNA SNHG8 in placenta accrete(PA)and its effect on trophoblast invasion and migration.Methods qRT-PCR was used to detect the expression of lncRNA SNHG8 in placenta tissue of 30 cases in PA group and 30 cases in control group,and the correlation between lncRNA SNHG8 expression and prenatal ultrasound score of 30 cases in PA group was analyzed.Transwell and scratch assay were used to detect the effect of lncRNA SNHG8 interference on the invasion and migration of human chorionic trophoblast cells(HTR8/SVneo cells),and western blot was used to detect the expression of MMP-2 and MMP-9.The downstream targets of lncRNA SNHG8 were predicted by StarBase software,and the expression of lncRNA SNHG8 was detected in placental tissues of the two groups.Dual luciferase reporter assay was used to detect the targeting relationship between lncRNA SNHG8 and miR-542-3p.Results Compared with that of the control group,the expression of lncRNA SNHG8 was up-regulated in the placenta tissue of the PA group(P<0.05),and it was positively correlated with prenatal ultrasound score.Interference with lncRNA SNHG8 inhibited the invasion and migration of trophoblast cells(P<0.05);the protein expression of MMP-9 and MMP-2 also decreased signifi-cantly(P<0.05).Biological prediction indicates that miR-542-3p had a binding site with lncRNA SNHG8,and miR-542-3p expression was down-regulated in PA placental tissue(P<0.05).Dual luciferase reporter assay confirmed that lncRNA SNHG8 could target miR-542-3p.Compared with si-SNHG8+inhibitor-NC,co-transfection of si-SNHG8 and miR-542-3p inhibitor enhanced the invasion and migration ability of trophoblast cells(P<0.05).Conclusion lncRNA SNHG8 is highly expressed in PA and is related to the severity of PA.LncRNA SNHG8 promotes the invasion and migration of trophoblast by regulating the level of miR-542-3p.The study suggests that lncRNA SNHG8 plays an important role in the invasion and migration of PA trophoblast cells,which is expected to be a clinical diagnostic biomarker and therapeutic target.
2.Ultrasound microvascular flow imaging combined with vascular endothelial growth factor for diagnosis of fetal growth restriction

Objective To observe the value of ultrasound microvascular flow imaging(MV-Flow)combined with maternal serum vascular endothelial growth factor(VEGF)expression level for diagnosis of fetal growth restriction(FGR).Methods Totally 87 pregnant women with FGR(FGR group,including 43 cases of gestational week<28 weeks[<28 weeks subgroup]and 44 cases of ≥28 weeks[≥28 weeks subgroup])and 112 normal pregnant women with normal fetuses(normal control group,55 with gestational week<28 weeks[NC 1 subgroup]and 57 with ≥28 weeks[NC 2 subgroup])were prospectively enrolled.MV-Flow technology was used to measure placental microvascular index(MVI),and the placental microvascular circulation was evaluated.The expression level of maternal serum VEGF was detected simultaneously,also of placental maternal surface immediately after delivery.The receiver operating characteristic curves were drawn to explore the value of placental MVI,maternal serum VEGF and the combination of placental MVI,maternal serum VEGF for diagnosing FGR.Results The levels of placental MVI and maternal serum VEGF in 2 subgroups of FGR group were both lower than those in control group(all P<0.01).Placental VEGF expression level in FGR group was significantly lower than that in control group(P<0.01).The area under the curve(AUC)of placental MVI,maternal serum VEGF and their combination for diagnosing FGR<28 weeks was 0.981,0.870 and 0.997,respectively,while for diagnosing FGR≥28 weeks was 0.991,0.867 and 0.993,respectively.AUC of maternal serum VEGF alone for diagnosing in 2 subgroups of FGR were both lower than that of placental MVI and combination of placental MVI and maternal serum VEGF(all P<0.05),while no significant difference of AUC was found between placental MVI and combination of maternal serum VEGF and placental MVI(both P>0.05).Conclusion Both placental MVI and maternal serum VEGF level could be used to screen FGR,and the former was more valuable.
3.Research progress on the mechanisms of carcinogenic correlation,heterogeneity,and drug resistance of extrachromosomal DNA in small cell lung cancer

With the progression of research on extrachromosomal DNA(ecDNA),it has been shown that ecDNA exists mainly in tumor cells and plays a crucial role in tumor heterogeneity and drug resistance.ecDNA is observed in several cancer types,but rarely in normal cells.Due to their strong oncogene amplification and dynamic alteration capabilities,patients with ecDNA-containing tumor cells often have negative clinical prognoses.Research has confirmed the presence of ecDNA in the cancer cells of patients with small cell lung cancer(SCLC).This re-view provides a comprehensive summary of the formation mechanism of ecDNA,the processes through which it is amplified in cancer cells,the mechanisms through which ecDNA promotes tumor growth,recurrence,and metastasis,and its relationship with high drug resistance in SCLC.Finally,we generalize the treatment direction for ecDNA-enriched SCLCs,thereby guiding future research.
4.Health information needs of inpatients with chronic diseases and utilization of community health resources in the comprehensive hospitals in Shanghai

ObjectiveTo understand the health information needs of chronic disease inpatients and the current utilization of community health resources, and to analyze the influencing factors, so as to provide basis for personalized and high-quality health education for chronic disease inpatients and to guide them to make full use of community health resources. MethodsFrom November 2020 to February 2021, we conducted a face-to-face multi-center clinical epidemiological survey with paper questionnaire in three general hospitals of Shanghai. The study included 404 inpatients with diabetes, hypertension, coronary heart disease, chronic respiratory diseases, cancer and other chronic diseases. Results94.01% of the 404 respondents had different needs for disease or health related information, and there was no significant difference between patients of different ages, genders and educational backgrounds in their needs for health knowledge. Among these patients, only 39.11% of them participated in the establishment of a card for chronic disease management in the community or signed for a family doctor. The participation rate of male patients was lower than that of female patients (P<0.05). ConclusionPatients with chronic diseases have a high demand for health related information, and we need to provide health information in multiple ways. We should increase investment in community health resources, improve service quality, and guide residents with chronic diseases to make full use of community health resources.
5.Study on the bioequivalence of Palbociclib capsules in human

OBJECTIVE To evaluate the bioequivalence of a single oral administration of two palbociclib preparations in healthy subjects under fasting and fed conditions. METHODS Twenty-four healthy subjects (fasting test) and twenty healthy subjects (fed test) were enrolled and divided into two groups. A single-center, open-label, single-dose, two-formulation, two- period, two-sequence and crossover trial was designed. The subjects in the two groups were given the test preparation (domestic Palbociclib capsules) or the reference preparation (original Palbociclib capsules) orally under fasting or fed conditions respectively followed by a 14-day washout period. The blood samples were collected at different time points before and after treatment. After pretreatment, the mass concentration of palbociclib in vivo was determined by high-performance liquid chromatography-tandem mass spectrometry with palbociclib-d8 as the internal standard. SAS V9.4 software was used to calculate the pharmacokinetic parameters and evaluate the bioequivalence. RESULTS Under fasting condition, the cmax of the test preparation and the reference preparation were (71.4±18.1) and (73.8±19.0) ng/mL; AUC0-t were (1 754±412) and (1 793±448) h·ng/mL; AUC0-∞ were (1 851±456) and (1 887±478) h·ng/mL, respectively. Under fed condition, the cmax of the test preparation and the reference preparation were (78.4±18.3) and (81.9±21.7) ng/mL; AUC0-t were (1 905±375) and (1 932±318) h·ng/mL; AUC0-∞ were (2 027±411) and (2 050±342) h·ng/mL, respectively. The 90%CI of the geometric mean ratio of the above parameters was within the acceptable range (80.00%-125.00%). Under fasting and fed conditions, there were 20 and 16 adverse events in 9 and 8 subjects, respectively, but no serious adverse event was observed. CONCLUSIONS Under the fasting and fed conditions, the test preparation and the reference preparation of Pibociclib capsules are bioequivalent and have comparable safety.
6.SpyGlass-guided laser lithotripsy versus laparoscopic common bile duct exploration for large common bile duct stones: a non-inferiority trial

Objective:To evaluate the clinical efficacy and safety of SpyGlass-guided laser lithotripsy for large common bile duct (CBD) stones with diameter>2 cm.Methods:From August 2015 to August 2018, a total of 157 patients with large CBD stones at the First Affiliated Hospital of Shandong First Medical University who met the inclusion criteria were randomly divided into SpyGlass group ( n=78, underwent SpyGlass-guided laser lithotripsy) and laparoscopic common bile duct exploration (LCBDE) group ( n=79, underwent LCBDE) by using random numbers. Non-inferiority test was used for rates of one-time stone removal and total stone removal, and the non-inferiority margin was set to 10%. The transform rate, incidence of short-term complications, hospital stay, and quality of life (assessed by the gastrointestinal quality of life index) were compared between the two groups. Results:The total success rates of stone clearance were 92.3% (72/78) and 96.2% (76/79) in the SpyGlass group and LCBDE group, respectively ( P=0.023), with valid non-inferiority hypothesis. The one-time stone removal rates were 83.3% (65/78) and 96.2% (76/79), respectively ( P=0.124), with invalid non-inferiority hypothesis. There were no significant differences in the incidence of transform [7.7% (6/78) VS 3.8% (3/79), P=0.294] or short-term complications [5.1% (4/78) VS 10.1% (8/79), P=0.246] between the two groups. Compared with the LCBDE group, the SpyGlass group had a shorter hospital stay (5.65±0.94 d VS 8.84±1.54 d, P=0.001) and higher scores of gastrointestinal quality of life index (1 month after operation: 99.85±4.36 VS 91.51±5.47, P=0.001; 3 months after operation: 131.24±3.32 VS 112.32±7.77, P=0.001). Conclusion:For large CBD stones, the efficacy of SpyGlass-guided laser lithotripsy is not inferior to LCBDE, and it is less invasive. In the future, SpyGlass-guided laser lithotripsy could be an important option for the treatment of large CBD stones.
7.Safety and efficacy of endoscopic resection for early primary non-ampullary duodenal cancer

Objective:To evaluate the safety and efficacy of endoscopic resection in the treatment of patients with primary non-ampullary duodenal early cancer.Methods:A total of 20 cases with primary non-ampullary duodenal early cancer receiving endoscopic resection were collected from Jan 2015 to Dec 2019 at the Department of Endoscopy, the First Affiliated Hospital of Shandong First Medical University.Results:The size of lesions ranged from 0.3-2.5 cm (0.9±0.5)cm.The size of removed membrane samples ranged from 1.5-3.5 cm (2.5±0.7)cm. The edges were all negatiue pathologically. Duodenal perforation occurred in 2 cases, and all were successfully clipped by endoscopy. The follow-up time was from 4-42 months (20.4±11.4)m and no recurrence was found.Conclusion:Endoscopic resection is a safe and effective method for primary non-ampullary duodenal early cancer.
8.Effects of mobile health management system on dietary behavior of patients with coronary heart disease

Objective:To explore the effect of mobile health (mHealth) management system on dietary behavior of patients with coronary heart disease (CHD) .Methods:Using the convenient sampling method, CHD patients who were hospitalized in Department of Cardiology of Beijing Anzhen Hospital and Beijing Chaoyang Hospital of Capital Medical University were selected as the research objects from June 2019 to January 2020. The research objects were randomly divided into the intervention group and the control group using SAS software. Finally, 68 cases in the intervention group and 66 cases in the control group completed the study. The intervention group received mhealth management, while the control group received routine management, and they were followed up to 1 month after discharge. The proportion of patients with the low-salt and low-oil diet, the intake of salt, oil, staple food, vegetables, fruits, fish, shrimp, poultry and eggs, BMI and blood pressure were compared between the two groups.Results:At 1 month follow-up, the intervention group was better than the control group in terms of low salt and low oil diet, salt intake, oil intake and vegetable intake compliance, and the differences were statistically significant ( P<0.05) . At 1 month follow-up, there were no statistically significant differences in BMI and blood pressure between two groups ( P>0.05) . Conclusions:The mobile health system can effectively increase the proportion of patients with a low-salt and low-oil diet, and improve the intake compliance of salt, edible oil and vegetables in patients with CHD. In future studies, the intervention time should be extended to further explore the long-term intervention effect of mobile health and give full play to the role of mobile health in continuous behavior management of patients with CHD after discharge.
9.Effect of acupoint catgut embedding therapy combined with Ziwu-Liuzhu acupuncture on the sleep quality of the patients with depression

Objective To investigate the effect of acupoint catgut embedding combined with Ziwu-Liuzhu theory based acupuncture on the clinical efficacy and sleep quality of depressive patients. Methods A total of 64 patients who met the inclusion criteria were randomly divided into two groups, 32 in each group. The control group was treated with routine electro-acupuncture, while the observation group was treated with acupoint catgut embedding combined with Ziwu-Liuzhu theory based acupuncture. Both groups were treated for 8 weeks. Sleep quality was assessed by Pittsburgh Sleep Quality Index (PSQI), and depression was assessed by Hamilton Depression Scale (HAMD) to evaluate the clinical efficacy. Results After treatment, the HAMD score (11.3 ± 2.4 vs. 14.5 ± 2.6, t=4.986) and PSQI index (9.6 ± 2.3 vs. 11.8 ± 2.5, t=3.307) in the observation group were significantly lower than those in the control group (P<0.01). The quality of life (12.9 ± 2.3 vs. 11.3 ± 2.5, t=2.598) and mental health score (13.5 ± 2.4 vs. 11.3 ± 2.6, t=3.413) of WHOQOL-BREF scale in the observation group were significantly higher than those in the control group (P<0.01). The total effective rate was 93.8% (30/32) in the observation group and 75.0% (24/32) in the control group. There were significant differences between the two groups (χ2=4.246, P=0.038). Conclusions Acupoint catgut embedding combined with Ziwu-Liuzhu theory based acupuncture can effectively improve depressive state, sleep quality and quality of life in patients with depression, and the curative effect is better than conventional electro- acupuncture treatment.
10.Application of arsenicals in the treatment of relapsed and refractory multiple myeloma

Objective To investigate the efficacy and adverse reactions of arsenicals in the treatment of refractory or relapsed multiple myeloma. Methods The data of 21 patients with refractory or relapsed multiple myeloma were retrospectively analyzed, including 10 patients with arsenicals (the arsenious acid group) and 11 cases treated with new regimens or treatments containing Bortezomib (the control group). The arsenious acid group comprised 5 males and 5 females, with the average age of (62±5) years old. The control group comprised 7 males and 4 females, with the average age of (59±9) years old. The total effective rate and overall survival (OS) in both groups were observed after 2 treatment courses. Results The median effective time and the total effective cases in the arsenious acid group and the control group was 8.00 mouths (6.00-9.00 mouths) and 3.50 mouths (1.00-24.00 mouths), 3 (3/10) and 6 (6/11) respectively. There was a statistically significant difference in the total effective rate (P= 0.245). There was no significant difference in the adverse reactions rate between the two groups (P> 0.05). The median OS time was 10.00 months (8.00-24.00 months) and 6.00 months (1.00-69.00 months) in the arsenious acid group and the control group respectively (P=0.900). Conclusion The arsenious acid in the treatment of refractory or relapsed multiple myeloma can not increase the incidence of adverse reactions with a favorable cost performance.
