1.Relationship between screening myopia and physical fitness index in college freshmen without majoring in public safety administration

		                        			Objective:
		                        			To explore the relationship between visual acuity and physical fitness of university freshmen, so as to provide reference for myopia prevention and control for freshmen.
		                        		
		                        			Methods:
		                        			From October to November 2022, 2 160 college freshman without majoring in public safety administration, selected from Guangxi Police College in 2022 by using the stratified cluster random sampling method, were reviewed for the results of visual acuity test and physical fitness scores. The physical fitness indices were evaluated by using the  Z scores of physical fitness test scores, and the strength of association between the level of physical fitness index and myopia was analyzed by using Logistic regression model.
		                        		
		                        			Results:
		                        			Among 2 160 college freshman without majoring in public safety administration, 917 (42.5%) students were diagnosed screening myopia, including 66 (3.1%) cases of high myopia, 383 (17.7%) cases of moderate myopia and 468 (21.7%) cases of mild myopia. The differences in the distribution of visual acuity tests among students with different physical fitness indices, body mass index, and gender were statistically significant ( Z/H=54.50, 49.53, 15.51, P <0.01). Low level and low middle level physical fitness indices were associated with screening myopia among freshmen[ OR (95% CI )=2.81(1.93-4.08),1.87(1.38-2.54)], and low level physical fitness indexes were associated with high myopia [ OR (95% CI )=7.22(2.33-22.32)] ( P <0.01).
		                        		
		                        			Conclusions
		                        			Screening myopia among college freshman without majoring in public safety administration is related to physical fitness, and low level and low middle level physical fitness index are risk factors for myopia. 2.Clinical efficacy and safety of amlodipine besylate and benazepril hydrochloride tablets (II) in the treatment of primary hypertension

		                        			Objective:To investigate the clinical efficacy and safety of amlodipine besylate and benazepril hydrochloride tablets (II) in the treatment of primary hypertension.Methods:A total of 280 patients with primary hypertension who were treated at Shougang Shuigang Hospital between June 2022 and June 2023 were selected as study subjects. A clinical case-control study was conducted, and the RAND function method was utilized to randomly allocate the subjects into four groups, each receiving a different treatment: amlodipine besylate group (Group A, n = 70), benazepril hydrochloride group (Group B, n = 70), compound formulation amlodipine besylate and benazepril hydrochloride tablets group (Group C, n = 71), and amlodipine besylate plus benazepril hydrochloride group (Group D, n = 69). Relevant therapeutic indicators (blood pressure compliance rate, changes in blood pressure values) and safety indicators (adverse reactions, medication adherence) were observed. Results:The blood pressure compliance rates of Group C and Group D were 91.5% (65/71) and 89.9% (62/69), respectively. There was no statistically significant difference between the two groups ( χ2 = 1.24, P = 0.143), but both were higher than the rates of 77.1% (54/70) and 74.3% (52/70) in Group A and Group B, respectively ( χ2 = 5.68, 4.86, P = 0.004, 0.012). Before treatment, there was no statistically significant difference in systolic and diastolic blood pressure among the four groups of patients (all P > 0.05). After treatment, there was a statistically significant decrease in both systolic and diastolic blood pressure among the four groups compared with their pre-treatment levels (all P < 0.05). Specifically, Group C and Group D exhibited significant reductions in blood pressure following treatment ( t = 4.35, 5.12, 7.25, 5.86, all P < 0.05). Meanwhile, there was no statistically significant difference in systolic blood pressure between Group C and Group D after treatment ( P > 0.05), while diastolic blood pressure was lower in Group C than Group D after treatment ( t = 6.01, P < 0.05). There was a significant downward trend observed in total cholesterol, triglycerides, and low-density lipoprotein cholesterol levels (all P < 0.05). Notably, Group B and Group D reported higher incidences of dry cough, with 15 and 10 cases, respectively, compared with Group A and Group C, which had 1 and 3 cases, respectively. These differences were statistically significant ( χ2 = 4.25, 5.04, both P < 0.05). Furthermore, the treatment compliance rates for Group A, Group B, and Group C were 72.9% (51/70), 71.4% (50/70), and 74.6% (53/71), respectively, all exceeding the 46.4% (32/69) compliance rate of Group D. These differences were also statistically significant ( χ2 = 4.68, 5.24, 4.98, all P < 0.05). Conclusion:The clinical efficacy and safety of the compound formulation amlodipine besylate and benazepril hydrochloride tablets (II) in the treatment of primary hypertension are superior to those of single tablets and combination therapy.

		                        			Hepatocellular carcinoma(HCC),commonly known as primary liver cancer,is a major cause of malignant tumors and cancer-related deaths in China,accounting for approximately 85%of all cancer cases in the country.Several guidelines have been used to diagnose and treat liver cancer.However,these guidelines provide a broad definition for classifying advanced liver cancer,with an emphasis on a singular approach,without considering treatment options for individual patients.Therefore,it is necessary to establish a comprehensive and practical expert consensus,specifically for China,to enhance the diagnosis and treatment of HCC using the Delphi method.The classification criteria were refined for Chinese patients with HCC,and the corresponding optimal treatment regimen recommendations were developed.These recommendations took into account various factors,including tumor characteristics,vascular tumor thrombus grade,distant metastasis,liver function status,portal hypertension,and the hepatitis B virus replication status of patients with primary HCC,along with treatment prognosis.The findings and rec-ommendations provide detailed,scientific,and reasonable individualized diagnosis and treatment strategies for clinicians.
		                        		
		                        		
		                        		
		                        	
4.Potential value of liver macrophages and their plasticity in the treatment of ACLF

		                        			Acute-on-chronic liver failure(ACLF)is a group of clinical syndromes related to severe acute liver function damage and multiple organ failure caused by various acute inducing factors on the basis of chronic liver disease.Due to its serious condition,rapid progression and high mortality,it has attracted more and more attention.Recent studies have shown that the pathogenesis of ACLF mainly includes direct injury and immune injury.As the main immune cells in the liver,the immunoregulatory role of liver macrophages in ACLF has been increasingly recognized.Liver macrophages have excellent phenotype conversion function and plasticity characteristics under the influence of epigenetic reprogramming or local microenvironment.This adaptive expression ability can use key mediators to promote the early conversion of anti-inflammatory phenotype to alleviate liver injury.A large number of studies have shown that liver macrophages have a certain potential in reversing the process of ACLF.Therefore,from the perspective of the plasticity characteristics of liver macrophages,this paper expounds the role of liver macrophages in ACLF and the research on the intervention of ACLF disease process,and summarizes its potential significance in the treatment of ACLF.
		                        		
		                        		
		                        		
		                        	
5.Genetic characterization of Vibrio cholerae strains genome in Fuzhou City from 2018 to 2023

		                        			Abstract: Objectives To sequence the whole genome of Vibrio cholerae strains isolated from Fuzhou between 2018 and 2023, predict virulence genes, antimicrobial resistance genes, and sequence loci information, analyze the genetic relationships  among different strains, and provide evidence for cholera prevention and control. Methods Whole genome sequencing was performed on 60 strains of Vibrio cholerae, and bioinformatics software was used for quality control, gene assembly, and prediction of the sequencing data. PubMLST, ResFinder, and VFDB databases were used to predict the multilocus sequence typing, antimicrobial resistance genes, and virulence genes of different strains. Combined with the NCBI database, the phylogenetic tree was constructed by the maximum likelihood method through the core cgSNP phylogenetic analysis and de-recombination analysis. Results    Based on the typing of 7 housekeeping genes, 60 Vibrio cholerae strains can be categorized into 16 known STs and 38 newly assigned STs. The clinical isolate H339 of serogroup O1 was identified as ST75. Serogroup O1 food isolates H13, H363, and H381 were all ST175. H263 and H357, the NOVC isolates, were both ST1218. H293 and H306 were ST1700. H311, H314, and H316 were all ST1699, with the remaining isolates displaying diversity. A total of 29 antimicrobial resistance genes were predicted, including aminoglycosides, β-lactams, quinolones, and folate pathway antagonists, with the majority of the strains carrying quinolones antimicrobial resistance genes. According to the VFDB prediction, all isolates had the virulence factors rtx and hlyA, 96.7% (58/60) of the strains carried the tlh genes, all serogroup O1 isolates carried tcp, zot, and ace genes, and all serogroup O1 clinical isolates carried ctxA genes. Phylogenetic tree analysis of the whole genome divided all strains into 20 branches, indicating high genomic divergence. Conclusions Thirty-eight new STs were identified. Genetic correlations were found among serogroup O1 food isolates, whereas serogroup O1 clinical isolates and serogroup O1 food isolates, as well as between serogroup O1 and NOVC strains, show distant phylogenetic relationships. There was diversity among the isolates. 6.Etiological characteristics and molecular typing of food poisoning incidents by Vibrio parahaemolyticus in Fuqing City, Fujian Province from 2021 to 2023

		                        			Abstract: Objective    To analyze the etiological characteristics of Vibrio parahaemolyticus (VP) isolated from food-borne outbreaks in Fuqing City between 2021 and 2023, and to explore its distribution characteristics and epidemic types.   Methods    The serotypes, virulence genes (tlh, tdh, trh), and drug resistance profiles of 23 VP strains isolated from 4 food-borne disease outbreaks were tested by agglutination, multiple real-time PCR, and broth microdilution methods. Pulsed-field gel electrophoresis (PFGE) was used for molecular typing to analyze homology. Results From 2021 to 2023, four food-borne disease outbreaks occurred in Fuqing City, leading to the isolation and identification of 23 VP strains. Among these, 19 clinical strains were divided into two serotypes: O10:K4 type, comprising 13 strains (68.4%), and O4:KUT type, comprising 6 strains (31.6%); the tdh gene carrying rate was 100.0%, and no trh gene was detected. The serotypes of the four food isolates were O3∶K4, O10:K24, O11∶KUT, and O3∶K37, respectively, all differing from the clinical strains, with two strains coming from the same food item without tdh and trh genes detected. The resistance rate of all isolates to cefazolin was the highest (87.0%), with two strains (8.7%) from different sources showing multidrug resistance. The PFGE genotypes of 19 case isolates were clustered and correlated with their serotypes and drug resistance profiles; strains isolated from the same outbreak event showed high similarity, whereas food strains exhibited polymorphism in PFGE genotypes. Conclusions    The predominant serotype causing the four outbreaks was O10:K4, with some strains showing multidrug resistance; all clinical strains carried the tdh gene.  The high correlation between strains from the same outbreak event suggests they are homologous. Special attention should be paid to the risk of outbreaks and pandemics caused by the O10:K4 type strain again. Additionally, food safety risk monitoring and assessment should be strengthened to provide an effective basis for epidemic prevention and control and early identification and warning.
		                        		
		                        		
		                        		
		                        	
7.Shouwuwan Regulates Synaptic Plasticity of Hippocampal Neurons in Rat Model of D-galactose-induced Aging via mTOR Signaling Pathway

		                        			ObjectiveTo investigate the effect of Shouwuwan on the synaptic plasticity of hippocampal neurons in the rat model of D-galactose-induced aging via the mammalian target of rapamycin (mTOR) signaling pathway. MethodA total of 50 male SPF-grade SD rats were randomized into normal group, model group, vitamin E (0.018 g·kg-1) group, and low- and high-dose (1.08,2.16 g·kg-1, respectively) Shouwuwan groups. Except the normal group, the other four groups were treated with D-galactose (120 mg·kg-1) for the modeling of aging. The rats were simultaneously administrated with corresponding agents by gavage. After six weeks of modeling, Morris water maze test was carried out to examine the behavioral changes. The whole brain and hippocampus samples were collected. The expression of postsynaptic density protein-95 (PSD-95) and synaptophysin (SYN) in the hippocampus was detected by immunohistochemistry. Golgi staining was employed to observe the changes in the morphology and function of neurons. Western blot and Real-time fluorescence quantitative polymerase chain reaction (Real-time PCR) were respectively employed to determine the mRNA and protein levels of mTOR, phosphorylated (p)-mTOR, p70 ribosome protein S6 kinase (p70S6K), phosphorylated (p)-p70S6K, eukaryotic translation initiation factor 4E-binding protein 2 (4EBP2), and phosphorylated (p)-4EBP2 in the hippocampus. ResultCompared with the normal group, the model group showed slow swimming (P<0.01), extended total swimming distance (P<0.05), prolonged latency (P<0.01), and decreased crossing number (P<0.01). The modeling inhibited the expression of PSD-95 and SYN in the CA1 region of the hippocampus (P<0.01), with the weakest staining effect and the smallest region, decreased the intersections of hippocampal neuron dendrites with concentric circles at the concentric distance of 100, 140, 180, and 200 μm from the cell body (P<0.01), and reduced the length and density of dendritic spine (P<0.01). In addition, the modeling up-regulated the mRNA levels of mTOR and p70S6K and the protein levels of p-mTOR and p-p70S6K (P<0.01) and down-regulated the mRNA level of 4EBP2 and the protein levels of 4EBP2 and p-4EBP2 (P<0.01). Compared with the model group, low- and high-dose Shouwuwan increased the average swimming speed (P<0.01), shortened the latency (P<0.01), increased the crossing number (P<0.01), promoted the expression of PSD-95 and SYN in the hippocampal CA1 region (P<0.01), increased the intersections between hippocampal neuronal dendrites and concentric circles at the concentric distance of 100, 140, 180,200 μm from the cell body (P<0.01), and increased the number, length, and density of dendritic spine (P<0.01). Furthermore, Shouwuwan down-regulated the protein levels of p-mTOR and p-p70S6K (P<0.01), up-regulated the protein levels of 4EBP2 and p-4EBP2 (P<0.05,P<0.01), down-regulated the mRNA levels of mTOR and p70S6K (P<0.01), and up-regulated the mRNA level of 4EBP2 (P<0.01). ConclusionShouwuwan can improve the learning and memory ability of rats exposed to D-galactose, promote the expression of proteins associated with synaptic plasticity, improve the morphology of neurons, repair neural function, reduce neuronal apoptosis, and inhibit mTOR signaling pathway to delay brain aging. 
		                        		
		                        		
		                        		
		                        	
8.Effects of paternal age on pregnancy outcomes in frozen-thaw embryo transfer cycles

		                        			【Objective】 To observe the effects of paternal age on the pregnancy outcomes in frozen embryo transfer (FET) cycles. 【Methods】 The clinical data of two groups after propensity score matching (PSM) were retrospectively analyzed, including 738 cycles in the <40 year group and 387 cycles in the 40-60 year group. The differences in general information and pregnancy outcomes were compared between the two groups, and logistic regression analysis of pregnancy outcomes were conducted. 【Results】 There were no statistical differences in age, duration of infertility, female body mass index (BMI), infertility factors, fertilization method, endometrial preparation methods, endometrial thickness on the day of transformation, stage of embryo transfer, number of embryos transferred, and number of high-quality embryos between the two groups after PSM (P>0.05). The clinical pregnancy rate (52.2%vs. 67.2%) and live birth rate (41.1% vs. 57.2%) decreased in the 40-60 year group compared with those in the <40 year group (P<0.001), but there was no statistical difference in abortion rate (19.8% vs. 13.7%) (P>0.05). 【Conclusion】 Advanced paternal age decreases clinical pregnancy rate and live birth rate.
		                        		
		                        		
		                        		
		                        	
9.Key Genes and Regulatory Networks of Acute-on-chronic Liver Failure Based on Multiple Bioinformatics Analysis

		                        			Objective To screen key genes and pathways of acute on-chronic liver failure(ACLF)by multiple bioinformatics,and to provide theoretical basis for molecular biology studies and biomarker screening of ACLF.Methods ACLF transcriptome mRNA mi-croarray data set was downloaded from Gene Expression Omnibus(GEO),and limma package in R software was used to analyze the difference expression genes.The gene ontology(GO)function enrichment and Kyoto Encyclopedia of Genes and Genomes(KEGG)anal-ysis were analyzed differential genes through David database.Protein-protein interaction(PPI)network was analyzed using STRING da-tabase,the key differential genes were screened by Cytoscape software.Results A total of 329differentially expressed genes were screened,including 185 up-regulated genes and 144 down-regulated genes.GO functional enrichment analysis obtained 385 items,in-cluding immune receptor activity,cytokine receptor activity,T cell receptor binding and other biological functions(P<0.05).KEGG pathway enrichment analysis screened 36signaling pathways,among which the immune and inflammatory pathways including Th1 and Th2 cell differentiation,Th17 cell differentiation pathway,T cell receptor signaling pathway,primary immune deficiency,NF-κB signaling pathway and TNF signaling pathway.Among these key genes,CD3G,CD3D,IL7R,LCK,IL1R2,IL18R1,IL1R1 and MAPK14 related to ACLF were further obtained,which may become potential biomarkers and therapeutic targets of ACLF.Conclusion This study demon-strates that CD3G,CD3D,IL7R,LCK,IL1R2,IL18R1,IL1R1 and MAPK14may become the core genes related to the occurrence and development of ACLF and new therapeutic targets in the future.
		                        		
		                        		
		                        		
		                        	
10.Practice and thinking of standardized cultivation of master of public health

		                        			In recent years, in the face of the risks and challenges of public health emergencies, the demand and supply of public health talents are relatively tight, and the reform and exploration of the standardized training of public health professionals has become one of the most urgent and important topics. This study analyzed the problems of curriculum system setting, implementation of practical links and composition of tutors in the training process, and learned that Wenzhou Medical University has carried out practical exploration in strengthening theoretical study, highlighting the combination of research and study, deepening professional practice, etc., which has achieved the orderly promotion of public health training supported by supporting reform measures; Take the post competence as the guidance, and vigorously improve the practicing ability; Based on the process and result assessment, promote the reform of training mode and other preliminary results. However, there are also shortcomings and deficiencies in accuracy, supply and effectiveness.
